HCV-RNA positivity in peripheral blood mononuclear cells of patients with chronic HCV-infection: does it really mean viral replication?

AIM: To analyze the association of HCV-RNA with peripheral blood mononuclear cells (PBMC) and to answer the question whether HCV-RNA positivity in PBMC is due to viral replication. MeTHODS: HCV-RNA was monitored in serum and PBMC preparations from 15 patients with chronic HCV infection before, during and after an IFN-alpha therapy using a nested RT/PCR technique. In a second approach, PBMC from healthy donors were incubated in HCV positive plasma. ReSULTS: In the IFN-alpha responding patients,HCV-RNA disappeared first from total RNA preparations of PBMC and then from serum. In contrast, in relapsing patients, HCV-RNA reappeared first in serum and then in PBMC. A quantitative analysis of the HCV-RNA concentration in serum was performed before and after transition from detectable to non detectable HCV-RNA in PBMC-RNA and vice versa. When HCV-RNA was detectable in PBMC preparations, the HCV concentration in serum was significantly higher than the serum HCV-RNA concentration when HCV-RNA in PBMC was not detectable. Furthermore, at no time during the observation period was HCV specific RNA observed in PBMC, if HCV-RNA in serum was under the detection limit. Incubation of PBMC from healthy donors with several dilutions of HCV positive plasma for two hours showed a concentration dependent PCR positivity for HCV-RNA in reisolated PBMC. CONCLUSION: The detectability of HCV-RNA in total RNA from PBMC seems to depend on the HCV concentration in serum. Contamination or passive adsorption by circulating virus could be the reason for detection of HCV-RNA in PBMC preparations of chronically infected patients.

Background emerging evidence suggests that stem cells and progenitor ce lls der ived from bone marrow can be used to improve cardiac function in patients after acute myocardial infarction. In this randomised trial, we aimed to assess whethe r intracoronary transfer of autologous bone-marrow cells could improve global l eft-ventricular ejection fraction (LVeF) at 6 months’follow-up. Methods After successful percutaneous coronary intervention (PCI) for acute ST-segment eleva tion myocardial infarction, 60 patients were randomly assigned to either a contr ol group (n=30) that received optimum postinfarction medical treatment, or a bon e-marrow-cell group (n=30) that received optimum medical treatment and intraco ronary transfer of autologous bone-marrow cells 4.8 days (SD 1.3) after PCI. Pr imary endpoint was global left-ventricular ejection fraction (LVeF) change from baseline to 6 months’follow-up, as determined by cardiac MRI. Image analyses were done by two investigators blinded for treatment assignment. Analysis was pe r protocol. Findings Global LVeF at baseline (determined 3.5 days > after PCI) was 51.3 (9.3%) in controls and 50.0(10.0%) in the bone-marrow cell group (p=0.59). After 6 months, mean global LVeF had increased by 0.7 percentage points in the c ontrol group and 6.7 percentage points in the bone-marrow-cell group (p=0.0026 ). Transfer of bone-marrow cells enhanced left-ventricular systolic function p rimarily in myocardial segments adjacent to the infarcted area. Cell transfer di d not increase the risk of adverse clinical events, in-stent restenosis, or pro arrhythmic effects. Interpretation Intracoronary transfer of autologous bone-ma rrow-cells promotes improvement of left-ventricular systolic function in patie nts after acute myocardial infarction.

Die Prim rstabilit t von Pedikelschrauben im Knochen h ngt in hohem Masse von der Knochendichte ab. Die Augmentation mit Zement ist ein klinisch einsetzbares Verfahren, um die Schrauben Verankerung im osteoporotischen Knochen zu *** dieser Studie wurde eine neuartiges resorbierbares Knochenersatzmaterial (α BSM TM ) für diese Augmentation benutzt. Biomechanische Testungen wurden in vitro bei 16 Lendwirbelk rpern (L3-L5) aus 6 Individuen(75,2±13,7 Jahre) durchgeführt. Vor der biomechanischen Testung wurde bei allen Pr paraten die trabekul re Knochendichte mittels pQCT gemessen und beide Pedikel mit USS Pedikelschrauben (5,0 mm × 45 mm) besetzt, von welchen eine mit α BSM TM augmentiert war. Beim axialen Auszugstest wurden die maximale axiale Auszugskraft (F max) sowie die energieaufnahme bestimmt. Der Medianwert der F max stieg beim Auszugstest durch die Zementierung mit α BSM TM um 80 % von 370 N (ohne Zement) auf 665 N (mit Zement). Die energieaufnahme bis zum erreichen der F max (e F max) und bei Dislokation bis 2,0 mm (e 2 mm) steigerte sich ebenfalls um 83 % und 68 %. Die Unterschiede waren signifikant. (Wilcoxon′s Test, P e Ausreisskrafte F max (ohne oder mit Zement) korrelierten eng mit der Knochendichte ( r =0,9056 und r =0,9585). Unsere Resultate zeigen, da eine Augmentation mit dem α BSM TM die primare Stabilitat von Pedikelschrauben verbessern kann. Der effekt scheint auf einer Optimierung der Kontaktfl che und einer Aussteifung der schraubennahen Spongiosa zu beruhen. Das Material k nnte geeignet sein, die Verankerung von Pedikelschrauben bei osteoporotischen Patieten zu verbessern.

Factors shaping community structure of endophytic fungi–evidence from the Pinus-Viscum-system

AIM: To compare the value of endoscopic retrograde cholangiography (eRC) and standard T2-weighted magnetic resonance cholangiography (MRC) in the evaluation process as adult-to-adult right lobe living donor liver transplantation (LDLTx) demands a successful outcome, and exact knowledge of the biliary tree is implicated to avoid biliary complications, ***: After starting the LDLTx program, 18 liver transplant candidates were selected for LDLTx by a stepwise evaluation process. eRC and standard T2-weighted MRC were performed to evaluate the biliary system of the donor liver. The anatomical findings of eRC and MRC mapping were compared using the Ohkubo classification. ReSULTS: eRC allowed mapping of the whole biliary system in 15/15 (100%) cases, including 14/15 (93.3%) with biliary variants while routine MRC was only accurate in 2/13 (15.4%) cases. MRC was limited in depicting the biliary system proximal of the hepatic bifurcation. Postoperative biliary complications occurred in 2 donors and 8 recipients. Biliary complications were associated with Ohkubo type C, e or G in 6/8 recipients, and 2/3 recipients with biliary leak received a graft with multiple (≥2) bile ducts. CONCLUSION: Pretransplant eRC is safe and superior over standard MRC for detection of biliary variations that occur with a high frequency. However, precise knowledge of biliary variants did not reduce the incidence of postoperative biliary complications.

Metasediments from the Tethyan Himalaya (TH) were sampled for paleomagnetic studies in several areas. In this paper, we will present the first results from Carboniferous and early Triassic marly limestones from Hidden Valley (Central Nepal).. The paleomagnetic directions reflect a Tertiary overprint probably synchronous with the metamorphism. In this area, the metamorphic conditions reached during Tertiary are poorly constrained. Temperatures are probably in between 300 and 400℃. The age of the thermal event is still debated. No geochronological data is available in this area. Previously published geochronological data from the northern part of TH metasediments in India ranges from 47 to 42Ma (Ar/Ar Illite) after Weissman et al. (1999) and Bonhomme and Garzanti (1991). While in the southern part (close to HHC), biotite Ar/Ar data ranges from 30 to 26Ma in Marsyandi Valley (Coleman and Hodges, 1998) and muscovite Ar/Ar ranges from 18 to 12Ma in the upper Kali Gandaki Valley (Godin et al., 1998).. In this context, the age of the magnetization can′t be defined with precision.

The family of voltage-gated （Shaker-like） potassium channels in plants includes both inward-rectifying （Kin） channels that allow plant cells to accumulate K＋ and outward-rectifying （Kout） channels that mediate K＋ efflux. Despite their dose structural similarities, Kin and Kout channels differ in their gating sensitivity towards voltage and the extracellular K＋ concentration. We have carried out a systematic program of domain swapping between the Kout channel SKOR and the Kin channel KAT1 to examine the impacts on gating of the pore regions, the S4, S5, and the S6 helices. We found that, in particular, the N-terminal part of the S5 played a critical role in KAT1 and SKOR gating. Our findings were supported by molecular dynamics of KAT1 and SKOR homology models. In silico analysis revealed that during channel opening and closing, displacement of certain residues, especially in the S5 and S6 segments, is more pronounced in KAT1 than in SKOR. From our analysis of the S4-S6 region, we conclude that gating （and K＋-sensing in SKOR） depend on a number of structural elements that are dispersed over this -145-residue sequence and that these place additional constraints on configurational rearrangement of the channels during gating.
