BACKGROUND:Statins reduce the incidence of strokes among patients at increased risk for cardiovascular disease;whether they reduce the risk of stroke after a recent stroke or transient ischemic attack(TIA)remains to be ***:We randomly assigned 4731 patients who had had a stroke or TIA within one to six months before study entry,had low-density lipoprotein(LDL)cholesterol levels of 100 to 190 mg per deciliter(2.6 to 4.9 mmol per liter),and had no known coronary heart disease to double-blind treatment with 80 mg of atorvastatin per day or *** primary end point was a first nonfatal or fatal ***:The mean LDL cholesterol level during the trial was 73 mg per deciliter(1.9 mmol per liter)among patients receiving atorvastatin and 129 mg per deciliter(3.3 mmol per liter)among patients receiving *** a median follow-up of 4.9 years,265 patients(11.2 percent)receiving atorvastatin and 311 patients(13.1 percent)receiving placebo had a fatal or nonfatal stroke(5-year absolute reduction in risk,2.2 percent;adjusted hazard ratio,0.84;95 percent confidence interval,0.71 to 0.99;P = 0.03;unadjusted P = 0.05).The atorvastatin group had 218 ischemic strokes and 55 hemorrhagic strokes,whereas the placebo group had 274 ischemic strokes and 33 hemorrhagic *** five-year absolute reduction in the risk of major cardiovascular events was 3.5 percent(hazard ratio,0.80;95 percent confidence interval,0.69 to 0.92;P = 0.002).The overall mortality rate was similar,with 216 deaths in the atorvastatin group and 211 deaths in the placebo group(P = 0.98),as were the rates of serious adverse *** liver enzyme values were more common in patients taking ***:In patients with recent stroke or TIA and without known coronary heart disease,80 mg of atorvastatin per day reduced the overall incidence of strokes and of cardiovascular events,despite a small increase in the incidence of hemorrhagic stroke.

Newborn screening(NBS)refers to a maternal and newborn healthcare technology,in which special examinations of congenital and genetic diseases that could seriously impact the health of newborns,are implemented during the neonatal period to provide early diagnosis and treatment[1].With a history of more than 60 years,NBS has advanced greatly due to technological progress resulting in significant improvement in the number of diseases covered by NBS and in screening efficiency[2-7].

OBJECTIVE: To compare sexual functioning and health-related quality-of-li fe outcomes of total abdominal hysterectomy (TAH) and supracervical hysterectomy ( SCH) among women with symptomatic uterine leiomyomata or abnormal uterine bleedi ng refractory to hormonal management. METHODS: We randomly assigned 135 women sc heduled to undergo abdominal hysterectomy in 4 U.S. clinical centers to either a total or supracervical procedure. The primary outcome was sexual functioning at 2 years, as assessed by the Medical Outcomes Study Sexual Problems Scale. Secon dary outcomes included specific aspects of sexual functioning and health-relate d quality-of-life at 6 months and 2 years. RESULTS: Sexual problems improved d ramatically in both randomized groups during the first 6 months and plateaued by 1 year. Health-related quality-of-life scores also improved in both groups. At 2 years, both groups reported few problems with sexual functioning (mean scor e on the Sexual Problems Scale for SCH group 82, TAH group 80, on a 0-to-100 s cale with 100 indicating an absence of problems; difference = +2,95%confidence interval -8 to +11), and there were no significant differences between groups . CONCLUSION: Supracervical and total abdominal hysterectomy result in similar s exual functioning and health-related quality of life during 2 years of follow- up. This information can help guide physicians as they discuss surgical options with their patients.

The maximum internal spanning tree(MIST) problem is utilized to determine a spanning tree in a graph G,with the maximum number of possible internal *** incremental maximum internal spanning tree(IMIST) problem is the incremental version of MIST whose feasible solutions are edge-sequences e,e,…,esuch that the first k edges form trees for all k ∈ [n-1].A solution’s quality is measured using maxopt(G,k)/|In(T)| with lower being ***,opt(G,k) denotes the number of internal vertices in a tree with k edges in G,which has the largest possible number of internal vertices,and |In(T)| is the number of internal vertices in the tree comprising the solution’s first k *** first obtained an IMIST algorithm with a competitive ratio of 2,followed by a 12/7-competitive algorithm based on an approximation algorithm for MIST.

Synthesis and Conformation Analysis of cis-1,2-Disubstituted Cyclododecanes

cis-1,2-Disubstituted cyclododecanes 2 were synthesized by sodium borohydride reduction of 2-monosubstituted cyclododecanones and their structures were confirmed by 1HNMR, ^13C NMR and elemental analysis. The higher cis-selectivity of NaBH4 reduction of 2- monosubstituted cyclododecanones was rationalized by the mode of ＂corner position carbonyl participation＂. Crystal data for 2c： Mr = 263.21, monoclinic, space group P21/c, a = 1.11140（7）, b = 2.62590（17）, c = 0.91360（6） nm, β= 106.1840（10）°, V = 2.5606（3） nm^3, Dc = 1.366 g/cm^3, Z = 8, F（000） = 1104, μ（MoKα= 3.182 mm^-1, S = 0.837, the final R = 0.0460 and wR = 0.1033. Crystal X-ray diffraction analysis for 2c showed that its ring skeleton adopts [3333] conformation, in which the OH group presents at the side-exo position and the other one at the corner carbon. The 1H NMR data of 2 showed that 1-corner-R-2-side-exo-OH [3333] and 1-corner-OH-2-side-exo-R [3333] conformations coexist in dynamic equilibrium in the solution, but only the former presents in the crystal.

Inhomogeneous dealloying kinetics along grain boundaries during liquid metal dealloying

In this study, the inhomogeneous dealloying phenomenon during the liquid metal dealloying(LMD) was investigated using FeNi+Mg and(FeCo)Ni+Mg systems. For the FeNi+Mg system, the inhomogeneous dealloying and wetting of Mg melt occurred along triple junction(TJ) and grain boundary(GB).Temperature increase enhances the inhomogeneous dealloying kinetics and leads to the formation of the plate-shaped abnormal ligaments at the GB region. The energy banlance between a GB energy(γGB)and solid-liquid interface energies(γsl) is the key factor governing the inhomogeneous dealloying and wetting. Particularly, the low-energy twin boundaries were unaffected by the inhomogeneous ***, precursor microstructure is an important factor determining the final morphology of dealloyed material as well as its physical properties. In the case of the(FeCo)Niprecursor, all TJ and GB were stable against the preferred penetration of Mg melt from 600 ℃ to 800 ℃. It was concluded that a minor addition of alloying elements(VorCr) changes GB characteristics as well as γslof the precursor ***, this significantly influences dealloying mechanisms and final morphology of the dealloyed material. The current findings demonstrate the importance of GB engineering in the precursor materials for the technological application of liquid metal dealloying for the synthesis of advanced structural and functional materials.

Conformation of α,α,α′-Trisubstituted Cyclododecanone

Ten α,α,α′-trisubstituted cyclododecanones were synthesized and characterized by elemental analyses, infrared, 1^H NMR and 13^C NMR spectra, and X-ray diffraction. NMR data could not give conformational information clearly, but some of their ring skeleton conformations of cyclododecanone moiety were showed to remain the unchanged [3333]-2-one conformation with little distortion, while the others were changed to the [3324]-2-one conformation in their crystal structures. These are consistent with the results of molecular mechanics calculation with Sybyl 6.9 software and Tripos force field, and semi-empirical quantum calculation with AM 1 method in Gaussian 98 software. Two geminal substituting groups are located at α-corner carbon atom, and the third group is at α-side-exo carbon atom in both conformations. Both [3333]-2-one and [3324]-2-one conformations are present in a dynamic equilibrium in the solution, but only one preferred conformation exists in the crystal solid.

Braking PSR J1734–3333 with a possible fall-back disk

The very small braking index of PSR J1734-3333, n = 0.9 ± 0.2, chal- lenges the current theories of braking mechanisms in pulsars. We present a possible interpretation that this pulsar is surrounded by a fall-hack disk and braked by it. A modified braking torque is proposed based on the competition between the magnetic energy density of the pulsar and the kinetic energy density of the fall-back disk. With this torque, a self-similar disk can fit all the observed parameters of PSR J1734-3333 with natural initial values of parameters. In this regime, the star will evolve to the re- gion having anomalous X-ray pulsars and soft gamma repeaters in the P -/5 diagram in about 20 000 years and stay there for a very long time. The mass of the disk around PSR J1734-3333 in our model is about 10M similar to the observed mass of the disk around AXP 4U 0142＋61.
