Etoricoxib versus indometacin in the treatment of Chinese patients with acute gouty arthritis： a randomized double-blind trial

Background Acute gout is an intensely painful, inflammatory arthritis. Although the non-steroidal anti-inflammatory drugs （NSAIDs） are widely used for this condition, the efficacy is based on only a few studies, particularly in China. We tried to assess the safety and efficacy of etoricoxib in the treatment of acute gouty arthritis in China. Methods A randomized, double-blind, active comparator study was conducted at 10 sites in China. Patients （n=178; 〉18 years of age） with acute gouty attack （〈48 hours） were treated for 5 days with etoricoxib （12. mg/d; n=89） or indometacin （75 mg twice daily; n=89）. the primary efficacy end point was self-assessed pain in the affected joint （0-4 point Likert scale） from days 2.5. Secondary end points included investigator assessments of tenderness and swelling, patient/ investigator global assessments of response to therapy, and patients discontinuing treatment. Safety was assessed by adverse events （AEs）. Results Etoricoxib and indometacin had comparable primary and secondary end points. Mean change difference from baseline from days 2.5 was 0.03 （95% confidence interval （CI） -0.19 to 0.2.; P=0.6364）, which fell within the prespecifled comparative bounds of -0.5 to 0.5. No severe AEs were associated with etoricoxib use. Non-severe AEs were mainly digestive and general, and most （73.7%） were mild, although they caused withdrawal of two subjects in the etoricoxib group, due to bilateral renal calculi and uronephrosis of the left kidney （unrelated to etoricoxib） and fever and chills （potentially etoricoxib-related）. Overall, AEs were similar, although the absolute number of AEs in the etoricoxib group （n=31） was less than the indometacin group （n=34）. Conclusions Etoricoxib （12. mg once daily） is effective in treating acute gout, is generally safe and well-tolerated, and is comparable in efficacy to indometacin （75 mg twice daily）.

New evidence for microstructural evolution model of acid-activated montmorillonite

Ca-montmorillonite samples from Choushan treated at various acid concentrations were studied using chemical analysis, XRD, 2.Si and 2.A1 MAS NMR, to investigate the microstructure of the activated montmorillonites. With the increase of acid concentration and dissolution of cations of montmorillonite, the intensity of d(001) decreased obviously and significant changes of the microstructure of Si and Al occurred. there were two new types of structural units of Si atoms formed: (SiO)3SiOH units and Q4(0A1) units. For Al atoms, in the course of activation, the removal of one of a pair of octahedral aluminium atoms from montmorillonite removed two hydroxyl groups and left the other aluminum of the pair in the four-fold coordination. the 2.A1 signal at 5 54.0 corresponding to AlIV arose from the four-coordinated Al in the octahedral sheet. this study first confirmed, by 2.Si and 2.A1 MAS NMR spectra, that the model of microstructural evolution for activated montmorillonite postulated by thomas et al. (1950)

Premenstrual dysphoric disorder(PMDD) affects nearly 5% of women of reproductive age. Symptomatic heterogeneity, together with largely unknown genetics, has greatly hindered its effective treatment. In the present study, analysis of genomic sequencing-based copy number variations(CNVs) called from 100 kb white blood cell DNA sequence windows by means of semisupervized clustering led to the segregation of patient genomes into the D and V groups, which correlated with the depression and invasion clinical types,respectively, with 89.0% consistency. Application of diagnostic CNV features selected using the correlation-based machine learning method enabled the classification of the CNVs obtained into the D group, V group, total patient group, and control group with an average accuracy of 83.0%. the power of the diagnostic CNV features was 0.98 on average, suggesting that these CNV features could be used for the molecular diagnosis of the major clinical types of PMDD. this demonstrated concordance between the CNV profiles and clinical types of PMDD supported the validity of symptom-based diagnosis of PMDD for differentiating between its two major clinical types, as well as the predominantly genetic nature of PMDD with a host of overlaps between multiple susceptibility genes/pathways and the diagnostic CNV features as indicators of involvement in PMDD etiology.

Dear Editor,Uncommon mutations in exons 18-2. of the epidermal growth factor receptor(EGFR)gene account for 10%–15%of all EGFR mutations when considered as a whole group[1,2..However,each variant confers heterogeneous clinical outcomes to different generations of EGFR tyrosine kinase inhibitors(TKIs)with G719X,L861Q,and/or S768I showing adequate sensitivity to EGFR inhibition[1–3].Osimertinib,based on its superior survival outcomes,has become the preferred first-line treatment for patients diagnosed with advanced non-small cell lung cancer(NSCLC)harboring common EGFR mutations[4];however,its efficacy in patients harboring G719X,S768I,and/or L861Q mutations was comparable or even inferior to Afatinib[5].Afatinib,a second-generation EGFR-TKI,has received approval for extended clinical indication in treating previously untreated patients with metastatic NSCLC harboring G719X,L861Q,and/or S768I based on the findings from the pooled analysis of three clinical trials(LUX-Lung 2.3/6)[2..the real-world clinical efficacy of Afatinib for treating this patient subset has been consistently demonstrated by two large retrospective studies[6,7].In China,chemotherapy remains a standard first-line treatment for this patient subset,with Afatinib available only as an off-label treatment option.

the design criteria for metal-organic frameworks(MOFs)have been established by evaluating the rela-tionship between their key characteristics and magnesium-ion conductivity based on three types of sec-ondary building blocks(Zn_(4)O(CO_(2.)_(6):MOF-5 and MOF-177;Cu_(2.(CO_(2.)4:MOF-199,MOF-143,MOF-14,and MOF-399;Cu_(2.O_(2.(CO_(2.)2.Cu-MOF-74)to achieve pseudo-solid-state magnesium-ion *** found that open-metal sites and channel layouts play a pivotal role in promoting magnesium-ion transport dy-namics at reduced activation energy,transforming MOF scaffolds into ionic-channel analogs.X-ray ab-sorption spectroscopy combined with Raman and Fourier-transform infrared spectroscopy predicted the chemical environment,solvents,and anions that occupied coordinatively unsaturated metal *** chemical compositions of electrolytes determined by^(1)H-NMR(nuclear magnetic resonance)and organic elemental analysis confirmed that isoreticular expansion increases the molar percentage of charge carri-ers,providing high *** current research systematically reveals the impacts of different MOF characteristics on ionic conduction performance,paving the way for the construction of a new class of fast and selective multivalent-ion pseudo-solid electrolytes.

Excess crude glycerol derived as a by-product from biodiesel industry prompts the need to valorise glycerol to value-added *** this context,catalytic steam reforming of glycerol(SRG) was proposed as a promising and sustainable alternative for producing renewable hydrogen(H2..Herein,the development of nickel(Ni) supported on ceria-modified mesoporous γ-alumina(γ-Al2.O3) catalysts and their applications in catalytic SRG(at550-750℃ atmospheric pressure and weight hourly space velocity,WHSV,of 44,12. ml·g^-1·h^-1(STP)) is *** of the developed catalysts were characterised using many technique *** findings show that ceria modification improved Ni dispersion on γ-Al2.O3 catalyst support with highly active small Ni particles,which led to a remarkable catalytic performance with the total glycerol conversion(ca.99%),glycerol conversion into gaseous products(ca.77%) and H2.yield(ca.62.).the formation rate for H2.production(14.4 ×10^(-5)mol·s^-1·g^-1, TOF(H2.=3412.s^-1) was significantly improved with the Ni@12.Ce-Al2.O3 catalyst,representing nearly a 2.fold increase compared with that of the conventional Ni@AI2.O3 *** addition,the developed catalyst also exhibited comparatively high stability(for 12.h) and coke resistance ability.

Background:Existing reporting guidelines pay insufficient attention to the detail and comprehensiveness reporting of surgical *** Surgical techniqUe rePorting chEcklist and standaRds(SUPER)aims to address this gap by defining reporting standards for surgical *** SUPER guideline intends to apply to articles that encompass surgical technique in any study design,surgical discipline,and stage of surgical ***:Following the EQUATOR(Enhancing the QUAlity and Transparency Of health Research)Network approach,16 surgeons,journal editors,and methodologists reviewed existing reporting guidelines relating to surgical technique,reviewed papers from 15 top journals,and brainstormed to draft initial items for the *** initial items were revised through a three-round Delphi survey from 2. multidisciplinary Delphi panel experts from 13 countries and *** final SUPER items were formed after an online consensus meeting to resolve disagreements and a three-round wording refinement by all 16 SUPER working group members and five SUPER ***:the SUPER reporting guideline includes 2. items that are considered essential for good and informative surgical technique *** items are divided into six sections:background,rationale,and objectives(items 1 to 5);preoperative preparations and requirements(items 6 to 9);surgical technique details(items 10 to 15);postoperative considerations and tasks(items 16 to 19);summary and prospect(items 2. and 2.);and other information(item 2.).Conclusions:the SUPER reporting guideline has the potential to guide detailed,comprehensive,and transparent surgical technique reporting for *** may also assist journal editors,peer reviewers,systematic reviewers,and guideline developers in the evaluation of surgical technique papers and help practitioners to better understand and reproduce surgical technique.
