paired Coagulation Status in the Crohn＇s Disease Patients Complicated with Intestinal Fistula

Background： Intestinal fistula is one of the common complications of Crohn＇s disease （CD） that might require surgical treatment. The clinical characteristics and outcomes of CD with intestinal fistula are much different from CD alone. This study was to investigate whether the coagulation status of CD is changed by intestinal fistula. Methods： Data were retrospectively analyzed for 1.0 patients with a definitive diagnosis of CD who were registered at the Jinting Hospital between January 201. and September 201.. Baseline clinical characteristics and laboratory indices of initial admission and 7 days after intestinal fistula resections were collected. Student＇s t-test and the Wilcoxon rank-sum test were used to compare differences between the two groups. Results： Compared with CD patients without intestinal fistula, prothrombin time （PT） in patients with intestinal fistula was significantly longer （1..1. ± 1.27 s vs. 1..1.± 1.51.s,P〈0.001.in overall cohort： 1..56± 1.21.svs. 1..61.±0.73 s,P 0.001.in females： and 1..51. 1.1. s vs. 1..37± 1.66 s, P - 0.003 in males）. Platelet （PLT） count was much lower in intestinal fistula group than in nonintestinal fistula group （262.53 ± 94.36 × 1.9/L vs. 31..36 ± 1.1.91. 1.9/L, P = 0.009）. Multivariate logistic regression showed that intestinal fistula was significantly associated with a prolonged PT （odds ratio [OR] 1.900, P 〈 0.001.）, a reduced amount of PLT （OR = 0.996, P = 0.（1.4）, and an increased operation history （OR = 5.408, P 〈 0.001.）. Among 65 CD patients receiving intestinal fistula resections, PT was obviously shorter after operation than baseline （ 1..28 ± 1.1. s vs. 1..02 ± 1.64 s, P = 0.006）. Conclusions： Intestinal fistula was significantly associated with impaired coagulation status in patients complicated with CD. Coagulation status could be improved after intestinal fistula resections.

Microbial processes and factors controlling their activities in alkaline lakes of the Mongolian plateau

A striking feature of the Mongolian plateau is the wide range of air temperatures during a year, -30 to 30~C. High summer temperatures, atmospheric weathering and the arid climate lead to formation of numerous alkaline soda lakes that are covered by ice during 6-7 months per year. During the study period, the lakes had pH values between 8.1.to 1..4 and salinity between 1.8 and 360 g/L. According to chemical composition, the lakes belong to sodium carbonate, sodium chloride-carbonate and sodium sulfate-carbonate types. This paper presents the data on the water chemical composition, results of the determination of the rates of microbial processes in microbial mats and sediments in the lakes studied, and the results of a Principal Component Analysis of environmental variables and microbial activity data. Temperature was the most important factor that influenced both chemical composition and microbial activity, pH and salinity are also important factors for the microbial processes. Dark CO2 fixation is impacted mostly by salinity and the chemical composition of the lake water. Total photosynthesis and sulfate-reduction are impacted mostly by pH. Photosynthesis is the dominant process of primary production, but the highest rate （386 mg C/（L.d）） determined in the lakes studied were 2-3 times lower than in microbial mats of lakes located in tropical zones. This can be explained by the relatively short warm period that lasts only 3-4 months per year. The highest measured rate of dark CO2 assimilation （59.8 mg C/（L·d）） was much lower than photosynthesis. The highest rate of sulfate reduction was 60 mg S/（L·d）, while that of methanogenesis was 75.6 μL CH4/（L·d） in the alkaline lakes of Mongolian plateau. The rate of organic matter consumption during sulfate reduction was 3-4 orders of magnitude higher than that associated with methanogenesis.

Liver resection for early hepatocellular cancer: Comparison of centers in 3 different countries

AIMTo compare patients who underwent resection of early stage hepatocellular cancer (HCC) in three different countries. METHODSThis retrospective study characterizes 573 stage I/II HCC patients treated with liver resection in 3 tertiary-referral centers: Tokyo (n = 250), Honolulu (n = 1.6) and Shanghai (n = 1.7). RESULTSShanghai patients were younger, predominantly male, hepatitis-B seropositive (94%) and cirrhotic (93%). Tokyo patients were older and more likely to have hepatitis-C (67%), smaller tumors, low albumin, and normal alpha-fetoprotein. The Honolulu cohort had the largest tumors and 30% had no viral hepatitis. Age-adjusted mortality at 1.and 5-years were lower in the Tokyo cohort compared to Honolulu and there was no difference in mortality between Shanghai and Honolulu cohorts. Elevated alpha-fetoprotein, low albumin and tumor > 5 cm were associated with increased 1.year mortality. These factors and cirrhosis were independently associated with increased 5-year mortality. Independent risk factors of survival varied when examined separately by center. CONCLUSIONThe profile of early-stage HCC patients is strikingly different across countries and likely contributes to survival differences. Underlying differences in patient populations including risk factors/comorbidities influencing disease progression may also account for variation in outcomes.

Influence of La^(3+) and Dy^(3+) on the properties of the long afterglow phosphor CaAl_2O_4: Eu^(2+), Nd^(3+)

The long afterglow phosphor CaAl2O4： Eu^2＋, Nd^3＋ was prepared by the high temperature solid-state reaction method, and the influence of La^3＋ and Dy^3＋ on the properties of the long afterglow phosphor was studied by X-ray diffiaction （XRD）, photoluminescence （PL）, and thermoluminescence （TL）. The XRD pattem shows the host phase of CaAl2O4 is produced and no impurity phase appears. The peak wavelength of the phosphor does not vary with La^3＋ and Dy^3＋ doping. It implies that the crystal field, which affects the 5d electron states of Eu^2＋, is not changed dramatically after doping of La^3＋ and Dy^3＋. The TL spectra indicate that the phosphor doped with La^3＋ or Dy^3＋ produces different depths of trap energy level. In the mechanism of long afterglow luminescence, it is considered that La^3＋ or Dy^3＋ works as trap energy level. The decay time lies on the number of electrons in the trap energy level and the rate of the electrons returning to the excitation level.

BACKGROUND Anastomotic leak constitutes a major problem in abdominal *** insufficiency,topical or systemic factors contribute to disrupted healing of the performed bowel anastomosis and result in anastomosis leakage,with detrimental effects on patient postoperative *** the investigation of several factors and the invention of protective materials,the ideal agent to prevent anastomotic leaks is yet to be *** To study the effect of platelet rich plasma(PRP)on the healing of bowel *** A systematic literature search was performed in PubMed,EMBASE,and Scopus databases to identify studies investigating the effect of PRP application on bowel *** Eighteen studies were eligible with a total population of 71. animals including rats(1. studies),rabbits(2 studies)and pigs(2 studies).No postoperative complications were reported following PRP *** out of 1. studies reported a statistically significant higher anastomosis bursting pressure in PRP groups compared to control either in healthy animals or animal models with underlying condition or intervention,such as intraperitoneal chemotherapy or *** results were reported by ten studies in terms of tissue hydroxyproline *** study reported significant increase in collagen deposition in PRP *** application resulted in significantly decreased inflammatory cell infiltration in the presence of peritonitis or intraperitoneal chemotherapy(6 studies).CONCLUSION The application of PRP is associated with improved bowel anastomosis outcomes,especially in animal models having an underlying condition affecting the normal healing *** application seems to augment the normal healing process under these ***,further studies are needed to investigate the potential role of PRP on bowel anastomosis healing,especially in clinical settings.

Primary leiomyosarcoma of the thyroid gland with prior malignancy and radiotherapy:A case report and review of literature

BACKGROUND Leiomyosarcoma(LMS) of the thyroid gland is a rarely presented tumor that offers poor prognosis. To the best of the authors' knowledge, there currently exist only 28 known cases described in the literature(limited to English).CASE SUMMARY Herein a case is reported of a 60-year-old female patient who had an LMS of the thyroid, which was accompanied by periodic dysphonia and breathing disorder as well as the feeling of pressure in the chest and neck. At the time the disease was diagnosed, no metastases were detected. Prior to the diagnosis, the patient experienced a uterine adenocarcinoma that had been treated by surgical procedure and radiotherapy. For the LMS, a total thyroidectomy was performed,followed by radiotherapy. Since metastases were also discovered in the lungs,sternum, and femur, chemotherapy was administered as ***, the tumor cells in the thyroid indicated positively for alpha smooth muscle actin, calponin, and H-caldesmon, but were negative for CD34, p63, estrogen receptor, progesterone receptor, and Epstein-Barr *** Although the etiology of the LMS is as of yet unknown, prior malignancy and radiation should be considered as risk factors.

Robotic lobectomy-the future of minimally invasive lobectomy?

It has been almost 20 years since the first reports of minimally invasive lobectomies appeared. Despite the tremendous amounts of research performed on VATS lobectomy showing its benefit over open thoracotomy, a mere 32% of all lobectomies are performed via this technique in the Society for Thoracic Surgeons database and only 6% in the Nationwide Inpatient Sample （1.. So, why is it that in a recent review of clinical stage I lung cancers over 70% were still completed using open thoracotomy？（2）. Advocates of an open approach still cite the ability to sample and perform a ＂more thorough＂ lymphadenectomy, the instability of the VATS platform and the lack of precision with the fissure-less-dissection VATS technique as reasons to maintain the status quo.

Laparoscopic adjustable gastric banding:a report of 228 cases

Objective:To evaluate the surgical outcomes and complications after laparoscopic adjustable gastric banding(LAGB)in obese ***:This retrospective study included 228 patients(73 males and 1.5 females,mean age,32.5±1..3 years)who underwent LAGB at the Changhai Hospital of the Second Military Medical University from June 2003 to June *** body weight and postoperative complications were followed ***:The pre-operative mean body mass index(BMI)was 39.5±6.3 kg/m^(2).Except in one case of inadequate exposure of the stomach,all laparoscopic procedures were successfully accomplished,with no conversion to open *** mean operation time was 65.0±20.3 *** mean hospital stay was 2.7±0.9 *** postoperative complications(30 days)occurred in 75 cases(32.9%),including 56 cases(24.6%)with band-associated *** percentage of excess weight loss(EWL%)at 1.3 and 5 years was 40.5±30.5%,59.5±41.5%and 58.9±46.4%,*** percentages of patients with EWL%>25%,>50%and>75%were,respectively,60%,33%and 0%at 1.year follow-up,43%,39%,and 1.%at 3 years follow-up and 40%,34%and 1.%at 5 years *** Although LAGB has low peri-operative mortality and morbidity rates,it is associated with a high late complication rate and unsatisfactory weight *** may be optional,but not the first choice,for the treatment of obesity.
