Comparison of the clinical effects of dual-modality endoscopy and traditional laparotomy for the treatment of intra-and extrahepatic bile duct stones

BACKGROUND Bile duct stones(BDSs)may cause patients to develop liver cirrhosis or even liver ***,the success rate of surgical treatment for intrahepatic and extrahepatic BDSs is not satisfactory,and there is a risk of postoperative *** To compare the clinical effects of dual-modality endoscopy(duodenoscopy and laparoscopy)with those of traditional laparotomy in the treatment of intra-and extrahepatic *** Ninety-five patients with intra-and extrahepatic BDSs who sought medical services at Wuhan No.1 Hospital between August 2019 and May 2023 were selected;45 patients in the control group were treated by traditional laparotomy,and 50 patients in the research group were treated by dual-modality *** following factors were collected for analysis:curative effects,safety(incision infection,biliary fistula,lung infection,hemobilia),surgical factors[surgery time,intraoperative blood loss(IBL)volume,gastrointestinal function recovery time,and length of hospital stay],serum inflammatory markers[tumor necrosis factor(TNF)-α,interleukin(IL)-6,and IL-8],and oxidative stress[glutathione peroxidase(GSH-Px),superoxide dismutase(SOD),malondialdehyde(MDA),and advanced protein oxidation products(AOPPs)].RESULTS The analysis revealed markedly better efficacy(an obviously higher total effective rate)in the research group than in the control *** addition,an evidently lower postoperative complication rate,shorter surgical duration,gastrointestinal function recovery time and hospital stay,and lower IBL volume were observed in the research ***,the posttreatment serum inflammatory marker(TNF-α,IL-6,and IL-8)levels were significantly lower in the research group than in the control *** with those in the control group,the posttreatment GSH-Px,SOD,MDA and AOPPs in the research group were equivalent to the pretreatment levels;for example,the GSH-Px and SOD levels were significantly higher,while the MDA and AOPP levels were

Liquefaction and post-liquefaction behaviors of sands affected by immersion-induced degradation of crushed mudstone

A series of undrained triaxial tests was conducted to investigate the effect of crushed mudstone with the immersion-induced degradation on the liquefaction and post-liquefaction properties,and the undrained shearing behavior without precedent cyclic-loading histories of sands containing crushed *** tested materials with a main particle diameter of 2-0.85 mm were prepared by mixing sands and crushed mudstone to reach the prescribed mudstone content defined by dry mass ranging from 0% to 50%.The mixtures were subjected to immersion under a certain stress level and were subsequently *** addition,one-dimensional compression tests were also supplementally performed to visually observe the immersion-induced degradation of crushed *** test results mainly showed that: (1) the liquefaction resistance,the post-liquefaction undrained strength,and the undrained strength without a precedent cyclic-loading history decreased significantly with increasing mudstone content,M c ,up to 20%;(2) even a small amount of crushed mudstone affected these strengths;(3) the above-mentioned large reductions in the strengths were attributed to the immersion-induced degradation of crushed mudstone;(4) at M_(c) >20%,the liquefaction resistance increased while the significant increase in the undrained static strengths with and without precedent cyclic-loading histories was not observed;and (5) the increase in the liquefaction resistance at M_(c) >20% may have been attributed to both the gradual increase in the plasticity and the formation of the soil aggregates among deteriorated crushed mudstone,while the increase in the specimen density did not play an important role in such behavior.

We have newly designed an electrostatic sensor,called an electric field mill(EFM),to simplify the estimation of the charge position and charge amount transferred by lightning *** is necessary for this remote estimation of the transferred charge to measure electric field changes caused by charge loss at the time of a lightning strike at multiple *** multiple-station measurement of electric field changes,not only speed but also phase for exposure and shielding of the sensing plates inside each EFM of the array should be synchronized to maintain the sensitivities of the deployed ***,there is no such EFM with specified speed and phase control performance of the rotary ***,we developed a new EFM in which the rotary mechanism was controlled consistently to within 3%error by a GPS *** EFMs had been distributed in the Hokuriku area of Japan during the winter season of 2022-2023 for a test *** we describe the design and a simple calibration method for our new EFM *** analysis method based on the assumption of a simple monopole charge structure is also *** validation,locations of assumed point charges were compared with three-dimensional lightning mapping data estimated by radio observations in the MF-HF *** results indicated the validity to estimate transferred charge amounts and positions of winter cloud-to-ground lightning discharges with our new EFM array.

Predicting cardiovascular events in out-of-hospital patients presenting with atypical chest pain and complete left bundle branch block: role of CTA and echocardiographic Global Longitudinal Strain

Background Out-of-hospital patients presenting with atypical chest pain and complete left bundle branch block(LBBB)have to be stratified for the presence of coronary artery disease and the risk of developing heart failure(HF).We investigated the pro-gnostic role of coronary CT-angiography(CTA)and echocardiographic global longitudinal strain(GLS)in those patients in a mid-term *** Out-of-hospital patients with LBBB underwent echocardiography and a 64-slice CT angiography were evaluated *** of HF or a cardiovascular death were the events *** Seventy-eight patients(32 female;mean age:66.0±10.4 years were *** a follow-up of 33 months(IQR:17-77),one patient(1.5%)experienced a cardiovascular death,14 patients(17.9%)required urgent outpatient visits due to acute de-compensated HF(12 hospitalizations).Echocardiography showed a slightly reduced left ventricular ejection fraction(LVEF)(50.0%±9.8%)and GLS within the normal range(-16.2%±4.1%).CTA analysis showed coronary stenosis>50%in 28 patients(35.9%).A high Agatston score(>100)was observed in 29.5%.Notably,25 patients(32.1%)were diagnosed with left main coron-ary artery disease and 15 patients(16.7%)underwent revascularization during the follow *** associations were ob-served between events and LVEF(P=0.001),diastolic dysfunction grade≥2(P=0.02),GLS(P1),multiple coronary stenos-is(P=0.04)and Agatston score(P=0.05).Multivariate analysis confirmed the relationships with LVEF(R2=0.89,P1),dia-stolic dysfunction(R2=3.30,P=0.04),GLS(R2=1.43,P1),and Agatston score(R2=1.01,P=0.05).Conclusions In patients with complete LBBB,CTA and GLS identified those at a high risk of development HF.

BACKGROUND Morgagni hernia(MH)is a form of congenital diaphragmatic hernia(CDH)characterized by an incomplete formation of diaphragm,resulting in the protru-sion of abdominal organs into the thoracic *** estimated incidence of CDH is between 1 in 2000 and 1 in 5000 live births,although the true incidence is *** typically presents in childhood and can be diagnosed either pre-natally or ***,it can also be asymptomatic and carry the risk of developing into a life-threatening condition in *** SUMMARY A 76-year-old female with no history of prior abdominal surgeries presented for an elective colonoscopy for polyp *** the procedure,when approaching the hepatic flexure,the scope could not be advanced further despite multiple *** patient experienced mild abdominal discomfort,leading to the abortion of the *** in the recovery area,she developed increa-sing abdominal pains and *** abdominal imaging revealed her-niation of the proximal transverse colon through a MH into the chest with evi-dence of *** patient underwent laparoscopic urgent colonic resection and primary hernia repair and was discharged uneventfully 2 d *** A MH is a rare condition in adults that can present as a life-threatening compli-cation of colonoscopy,even in patients with a history of uneventful *** case highlights the importance of considering congenital and internal hernias when faced with sudden and unexplained difficulties during *** there is a suspicion of MH,the endoscopist should halt the procedure and immediately obtain abdominal imaging to confirm the diagnosis.

Biomass Control via UV Lamp Application and Nutrient Supply Restriction in the Biofiltration of Gaseous VOCs

Variation characteristics of acid rain in Zhuzhou,Central China over the period 2011-2020

Zhuzhou was one of the most polluted cities in China with the serious acid *** to the implementation of air pollution control measures from 2016 to 2018,the acid rain pollution in this city has *** order to understand the recent situation,a comprehensive study on the acid rain was carried out from January 2011 to December *** pH values during the study period varied from 3.3 to 7.5,with a volume-weighted mean value of *** predominant acidic components of the precipitation were SO_(4)^(2-)and NO_(3)^(-),accounting for 89.3% of the total *** ratio of non-sea-salt SO_(4)^(2-)to NO_(3)^(-)showed a decreasing trend,revealing that the pollution type of acid rain changed from sulfuric acid type to sulfuric acid and nitric acid compound *** correlation analysis(p<0.05)showed that SO_(4)^(2-)was positively correlated with NH_(4)^(+),Ca^(2+),and Mg^(2+);hence,it predominated in precipitation as(NH_(4))_(2)SO_(4),NH_(4)HSO_(4),CaSO_(4),and MgSO_(4).Significant positive correlation of Ca^(2+)with Mg^(2+)shows that they may originated mainly from *** positive correlation between SO_(4)^(2-)and F^(-)and Cl^(-)indicate that their source may be related to the non-ferrous metal smelting industry in *** correlation analysis shows that emissions from the non-ferrous metal smelting industry in the area have a large significant on SO_(4)^(2-)and F^(-)in precipitation,while Cl^(-)may still be emitted from other anthropogenic sources.
