Evaluation of Ventricular-vascular Coupling in patients with Type 2 Diabetes Mellitus Using 2-Dimensional speckle Tracking Imaging

The elastic and functional coupling of heart and vessels makes the stroke work （sW） of the heart optimal. speckle tracking imaging （sTI） can evaluate the myocardial strain and function. We studied ventricular-vascular coupling in 80 diabetic patients with different systolic function using sTI. The patients were divided into two groups according to ejection fraction （EF）： the diabetes mel- litus with normal EF （DMN） group and the diabetes mellitus with abnormal EF （DMA） group. Forty-two volunteers served as control group. The relative wall thickness （RWT）, left ventricular mass index （LVMI）, stroke volume （sV）, sW, rate-pressure product （Rpp）, systemic vascular resis- tance index （sVRI）, left ventricular end-systolic elastance （Ees）, effective arterial elasticity （Ea） and ventricular-vascular coupling index （VVI） were measured and calculated by conventional echocardi- ography. The longitudinal strain （Ls） at basement （LsBA）, papillary muscle （LsvM） and cardiac apex （LsAv） was assessed with sTI. It was found：（A） compared with control group, in DMN and DMA groups, LsBA, LsvM and LsAp decreased, and they were lower in DMA group. （B） VVI, Rpp and sVRI increased, and they were higher in DMN group; Ees decreased, and it was lower in DMA group. （C） LsBA, LspM, and LsAv had negative correlation with VVI. LsAp, RWT, LVMI and sW were independent predictors for VVI. The area under the receiver operating characteristic （ROC） curves was used for identification of DMA and DMN with LsBA, LspM, and LsAp, and the area under the ROC of LsAp was the largest. This study supports that myocardial Ls could reflect the ventricu- lar-vascular coupling. Different segments had an order to “respond to” the state of the coupling, and the cardiac apex might be the earliest.

A series of novel organic-inorganic hybrid proton-conducting electrolyte membranes with silane-crosslinked sulfonated poly(aryl ether ketone)(sC-spAEK) networks was prepared via a simple procedure that includes solution casting and acid treatment. The organosilicon pendants of the silane-grafted spAEK, which were expected to serve as coupling and crosslinking agents, were found to play a key role in the homogenous dispersion of inorganic particles and improved the performance of hybrid membranes. The hybrid membranes exhibited enhanced proton conductivity, and sC-spAEK/TiO2-4 showed an extremely high proton conductivity of 0.1472 s/cm at 100℃. The crosslinked hybrid membranes also demonstrated good chemical resistance, oxidative stability, and mechanical properties. The crosslinked hybrid membranes with excellent comprehensive performance may be a promising material for proton exchange membrane fuel cells.

Carbon fiber reinforced plastic(CFRp)has been applied in aeronautics,aerospace,automotive and medical industries due to its superior mechanical ***,due to its difficult-to-cut characteristic,various damages in twist drilling and chip removal clog in core drilling could happen,inevitably reducing hole quality and hole-manufacturing *** paper proposes the wave-motion milling(WMM)method for CFRp hole-manufacturing to improve hole *** paper presents a motion path model based on the kinematics of the WMM *** wave-motion cutting mode in WMM was analyzed ***,comparison experiments on WMM and conventional helical milling(CHM)of CFRp were carried out under dry *** results showed that the hole surface quality of the CFRp significantly improved with a decrease of 18.1%–36%of Ra value in WMM compared to *** exerted a significantly weaker thrust force than that of CHM with a reduction of 12.0%–24.9%and 3%–7.7%for different axial feed per tooth and tangential feed per tooth,***,the hole exit damages significantly decreased in *** average tear length at the hole exit in WMM was reduced by 3.5%–29.5%and 35.5%–44.7%at different axial feed per tooth and tangential feed per tooth,***,WMM significantly alleviated tool *** experimental results suggest that WMM is an effective and promising strategy for CFRp hole-manufacturing.

The anti-amyloid-j(anti-Aβ)fibrils and soluble oligomers antibody aducanumab were approved to effectively slow down the progression of A lzheimer's disease(AD)at higher doses in 2019,reaffirming the therapeutic effects of targeting the core pathology of AD.A timely and accurate diagnosis in the prodromal or pre-dementia stage of AD is essential for patient recruitment,stratifcation,and monitoring of treat ment effects.A D core biomarkers amyloid-B(Aβ1-42),total tau(t-tau),and phosphorylated tau(p-tau)have been clinically validated to reflect AD-type pathological changes through cerebrospinal fuid(CsF)measurement or positron-emission to-mography(pET)and found to have high diagnostic performance for AD identification in the stage of mild cognitive *** development of ultrasensitive immunoassay technology enables AD pathological proteins such as tau and neurofilament light(NFL)to be measured in blood ***,combined biomarker detection or targeting multiple biomarkers in immunoassays will increase detection sensitivity and specifcity and improve diagnostic accuracy..This review summarizes and analyzes the performance of current detection methods for early diagnosis of AD,and provides a concept of detection method based on multiple biomarkers instead of a single target,which may become a potential tool for early diagnosis of AD in the future.

Intention of healthcare workers to accept COVID-19 vaccination and related factors:A systematic review and meta-analysis

Considering medical and economic burden of coronavirus disease 2019(COVID-19), a high COVID-19 vaccination coverage among healthcare workers(HCWs) is an urgent need. The aim of this systematic review and meta-analysis was to evaluate the intention of HCWs to accept COVID-19 vaccination and to identify related factors. We searched pub Med, MEDLINE, scopus, Web of science, pro Quest, CINAHL and med Rxiv until July 14, 2021. The heterogeneity between results was very high;thus, we applied a random effects model to estimate pooled effects. We performed subgroup and meta-regression analysis to identify possible resources of heterogeneity. Twenty four studies, including 50 940 HCWs, met the inclusion criteria. The overall proportion of HCWs that intend to accept COVID-19 vaccination was 63.5%(95% confidence interval: 56.5%-70.2%) with a wide range among studies from 27.7% to 90.1%. The following factors were associated with increased HCWs’ willingness to get vaccinated against COVID-19: male gender, older age, white HCWs, physician profession, higher education level, comorbidity among HCWs, vaccination against flu during previous season, stronger vaccine confidence, positive attitude towards a COVID-19 vaccine, fear about COVID-19, individual perceived risk about COVID-19, and contact with suspected or confirmed COVID-19 patients. The reluctance of HCWs to vaccinate against COVID-19 could diminish the trust of individuals and trigger a ripple effect in the general public. since vaccination is a complex behavior, understanding the way that HCWs take the decision to accept or refuse COVID-19 vaccination will give us the opportunity to develop the appropriate interventions to increase COVID-19 vaccination uptake.
