Active surveillance of adverse drug reactions in children in five Italian paediatric wards

Adverse drug reactions (ADRs) are an important clinical problem and contribute significantly to mortality and morbidity. Scant data on the safety of drug use in children are usually available at the time of marketing authorization, due to the limited number of trials performed in the paediatric population. Few studies monitored the incidence of ADRs in Italian hospitalized children, that cannot be compared for methodological reasons. A 6-month prospective observational study was, therefore, conducted on the paediatric wards of five hospitals in the Campania Region, Italy. Data were collected on all patients admitted to the wards during the study period through a structured questionnaire administered to the mothers and through a hospital chart review. Of the 752 patients enrolled, 86.2% were exposed to one or more drugs during hospitalization. The therapeutic class most prescribed was systemic antibacterial agents (47%). Six ADRs occurred during hospitalization (incidence 0.9%;95% CI 0.2% - 1.7%). In addition, one child was admitted to a hospital for an ADR. Five out of seven ADRs occurred in girls. The skin was the most affected organ. The medications implicated were amoxicillin, acyclovir, ibuprofen, ceftriaxone, paracetamol, and ranitidine. According to the Naranjo probability criteria, six ADRs were probably, and one possibly, related to the suspected drug. In conclusion, this study reveals that ADRs may be under-reported in children hospitalized in the Campania Region. Consequently, healthcare personnel should be alert to the possibility of ADRs. More accurate reporting of ADRs in children would result in safer use of drugs in such patients.

BACKGROUND The debate regarding diagnostic classification systems in psychiatry(categorial vs dimensional systems)has essential implications for the diagnosis,prevention and treatment of stress *** previously found a unique pattern of stress reaction in a study executed during the coronavirus disease 2019 pandemic using large representative samples in two countries,and termed it the Complex Stress Reaction Syndrome(CSRS).AIM To investigate CSRS,Type A(psychiatric symptoms,spanning anxiety,depression,stress symptoms,and posttraumatic stress disorder(PTSD)),with or without long-coronavirus disease(COVID)residuals(CSRS,Type B,neuropsychiatric symptoms spanning cognitive deficits and fatigue,excluding systemic symptoms).Our two-tailed hypothesis was that CSRS is a condition related to an unrecognized type of stress reaction in daily life in the general population(Type A)or that it is related to the severe acute respiratory syndrome coronavirus 2 infection and its long-COVID residuals(Type B).METHODS 977 individuals in four continents(North America,Europe,Australia and the Middle East)completed the online study questionnaire in six languages using the Qualtrics *** study was managed by six teams in six countries that promoted the study on social *** questionnaire assessed anxiety,depression,stress symptoms and PTSD(CSRS,Type A),cognitive deficits and fatigue(CSRS,Type B).The data were analyzed using Proportion Analyses,Multivariate Analysis of Co-Variance(MANCOVA),linear regression analyses and validated clinical cutoff *** The results of the Proportion Analyses showed that the prevalence of 4 symptoms spanning anxiety,depression,stress symptoms,and PTSD was significantly higher than the most prevalent combinations of fewer symptoms across 4 continents,age groups,and *** supports the transdiagnostic argument embedded in the CSRS(Type A).The same pattern of results was found in infected/recovered *** prevalence of the

Lithium(Li)metal is considered as the candidate for the next generation of Li metal battery(LMB)anodes due to its high capacity and the lowest potential,which is expected to meet the requirements of energy storage ***,the uncontrollable growth of Li dendrites during the charge/discharge process,as well as the resulting problems of poor cycling stability,low coulomb efficiency and safety risk,has restricted the commercialization of Li ***,an in-situ interfacial film containing three-dimensional(3D)rod-like micron-structure silver(Ag)is constructed on the surface of the Li *** to the 3D rod-like micron-structure used to homogenize the distribution of current density,achieving uniform nucleation and growth of electrodeposited Li,the produced Li-Ag alloy was employed to restrain the formation of“dead”Li and the in-situ formed lino_(3) was utilized to facilitate the stability of solid-electrolyte interface(SEI)film,so the growth of dendritic Li is suppressed via the synergistic effect of structure and surface chemistry *** obtained Li anode can achieve cycling stability at a high current density of 10 mA/cm^(2).This work considers multiaspect factors inducing uniform Li electrodeposition,and provides new insights for the commercialization of LMB.

Final results of a 2:1 control-case observational study using interferon beta and interleukin-2,in addition to first-line hormone therapy,in estrogen receptor-positive,endocrine-responsive metastatic breast cancer patients

Aim:We conducted a pilot study that combines immunotherapy(cyclic interleukin-2 interferon-beta sequence)and hormone therapy(HT)to overcome endocrine resistance in metastatic breast ***:The final results of a 2:1 control-case retrospective observational study are here shown following 22 additional months of postoperative follow-up and 6 further *** were 95 controls and 42 cases in *** 95 controls were ER+/HER2-metastatic breast cancer patients who underwent first-line HT with aromatase inhibitors(AIs)or ***-eight of them(28.9%)also received biological drugs including cyclin kinase inhibitors(CKIs).The 42 cases were ER+metastatic breast cancer patients who received interferon beta-interleukin-2 immunotherapy in addition to first-line *** estrogen receptor modulators/down-regulators(SERMs/SERDs)were used for HT in 39(92.9%)of them and AIs in the remaining ***:Median progression-free survival(PFS)and overall survival(OS)were significantly longer in the 42 studied patients who received hormone immunotherapy(HIT)than in the 95 controls(median time 33 vs.18 months,P=0.002,and 81 vs.62 months,P=0.019).In the analysis adjusted for disease-free interval(DFI),hormone receptor,HER2 status,visceral involvement,AIs,and biological therapy,the PFS and OS hazard ratio(HR)further increased in favor of the 42 cases(P=0.004 and P=0.044 respectively).In the same ER+/HER2-metastatic breast cancer patients treated with both AIs and CKIs,a median PFS ranging from 25.3 to 28.18 months and a median OS of 37.5 months were ***:This study strongly suggests multi-center randomized clinical trials should be performed to enter our proposed immunotherapy into clinical practice.

We report on the design and first results from experiments looking at the formation of radiative shocks on the Shenguang-Ⅱ(SG-Ⅱ) laser at the Shanghai Institute of Optics and Fine Mechanics in ***-heating of a two-layer CH/CH-Br foil drives a～40 km/s shock inside a gas cell filled with argon at an initial pressure of 1 *** use of gas-cell targets with large(several millimetres) lateral and axial extent allows the shock to propagate freely without any wall interactions,and permits a large field of view to image single and colliding counter-propagating shocks with time-resolved,pointprojection X-ray backlighting(～20 μm source size,4.3 keV photon energy).Single shocks were imaged up to 100 ns after the onset of the laser drive,allowing to probe the growth of spatial nonuniformities in the shock *** results are compared with experiments looking at counter-propagating shocks,showing a symmetric drive that leads to a collision and stagnation from～40 ns *** present a preliminary comparison with numerical simulations with the radiation hydrodynamics code ARWEN,which provides expected plasma parameters for the design of future experiments in this facility.
