Progress on Prevention and Treatment of Cerebral Small Vascular Disease Using Integrative Medicine

Cerebral small vessel disease(CSVD)is a senile brain lesion caused by the abnormal structure and function of arterioles,venules and capillaries in the aging brain.The etiology of CsvD is complex,and disease is often asymptomatic in its early stages.However,as CsvD develops,brain disorders may occur,such as stroke,cognitive dysfunction,dyskinesia and mood disorders,and heart,kidney,eye and systemic disorders.As the population continues to age,the burden of CsvD is increasing.Moreover,there is an urgent need for better screening methods and diagnostic markers for CsvD,in addition to preventive and asymptomatic-and mild-stage treatments.Integrative medicine(IM),which combines the holistic concepts and syndrome differentiations of Chinese medicine with modern medical perspectives,has unique advantages for the prevention and treatment of CsvD.In this review,we summarize the biological markers,ultrasound and imaging features,disease-related genes and risk factors relevant to CsvD diagnosis and screening.Furthermore,we discuss IM-based csvD prevention and treatment strategies to stimulate further research in this field.

Ischemic stroke often elicits profound white matter lesions,which results in poor neurological outcomes and impairing the recovery in post-stroke.However,few studies have focused on white matter lesions caused by cerebral ischemia.The present study was aimed to investigate the effects of cornel iridoid glycoside(CIG),a main active component extracted from Comus officinalis,on the white matter injury induced by ischemic stroke.CIG(60 and 120 mg·kg) were administered intragastrically 6 h after middle cerebral artery occlusion reperfusion(MCAO) surgery once a day for 7 d.A series of behavioral tests were carried out to evaluate the neurological function of MCAO rats.white matter structure was detected by luxol fast blue staining and transmission electron microscopy.Immunohistochemical staining was used to assess myelin loss,oligodendrocytes maturation and glial activation.Results showed that CIG remarkably decreased neurological deficit score,accelerated the recovery of somatosensory and motor functions,and ameliorated the memory deficit in MCAO rats.CIG alleviated white matter lesions and demyelination,increased myelin basic protein expression and the number of mature oligodendrocytes in the corpus callosum of MCAO rats.Besides,CIG inhibited the activation of microglia and astrocytes.Further data obtained by western blot analysis indicated that CIG increased the expression of brain-derived neurotrophic factor(BDNF)/p-TrkB,neuregulin-1/ErbB,and PI3 K p110α/p-Akt/p-mTOR in the corpus callosum of MCAO rats.Our findings demonstrated that CIG protected against white matter lesions induced by cerebral ischemia and the mechanisms were partially contributed to increasing BDNF and activating neuregulin-1/ErbB signaling and its downstream PI3 K/Akt/mTOR pathway in white matter.

Objective Leukoaraiosis（LA）,named as white matter lesions（WML）,is a common finding on magnetic resonance imaging（MRI） of the brain in elderly people with prevalence from 50 to 100%worldwide,mainly characterized by patchy demyelination,loss of oligodendroglial cells,axonal loss and denudation of the ependyma in regions of white matter hyperintensity.However,the mechanisms leading to the pathophysiology and neuroimaging abnormalities of the cerebral white matter have yet been poorly understood to date.Our approach is to identify an epigenetic basis for this complicated illness and further to decipher the pathogenic genes and its associated molecular mechanisms from the occurrence to the progression of LA through a deep going case-control study of LA.Methods All subjects were recruited in this study based on our new MRI-pathoimaging categorization of LA（Normal,Type Ⅰ LA,Type Ⅱ LA）.We conducted genome-scale methylation analysis among those three groups of subjects.Significantly methylation-associated genetic difference in both occurrence and progression of LA,were verified by pyrosequencing and further ontologically predicted into pathogenic mechanism/pathway.Results（1） As many as 152 and 198 genes were identified to show meaningful dissimilarity in the occurrence and progression of LA,respectively.Likewise,32genes were shared in between these two pathological processes.（2） Ontology analysis highlighted the epigenetic disruption in major mechanism of immune response（T-cell-,cytokine- and interferon-mediated signaling pathways）,neuron behaviors（development and interactive synaptic transmission/assembling） and cell performance（adhesion and apoptosis）.（3） Most meaningful methylation difference were validated by independent LA population（n=54）.Conclusion For the very first time,aberrant DNA methylation were reported,which are allied with both occurrence and progression of LA from Chinese population.Given that,most of LA-associated methylated genes and pathways were strongly associated with inflammatory development to fir
