Clinical applications of stem cell-derived exosomes

Although stem cell-based therapy has demonstrated considerable potential to manage certain diseases more successfully than conventional surgery,it nevertheless comes with inescapable drawbacks that might limit its clinical translation.Compared to stem cells,stem cell-derived exosomes possess numerous advantages,such as non-immunogenicity,non-infusion toxicity,easy access,effortless preservation,and freedom from tumorigenic potential and ethical issues.Exosomes can inherit similar therapeutic effects from their parental cells such as embryonic stem cells and adult stem cells through vertical delivery of their pluripotency or multipotency.After a thorough search and meticulous dissection of relevant literature from the last five years,we present this comprehensive,up-to-date,specialty-specific and disease-oriented review to highlight the surgical application and potential of stem cell-derived exosomes.Exosomes derived from stem cells(e.g.,embryonic,induced pluripotent,hematopoietic,mesenchymal,neural,and endothelial stem cells)are capable of treating numerous diseases encountered in orthopedic surgery,neurosurgery,plastic surgery,general surgery,cardiothoracic surgery,urology,head and neck surgery,ophthalmology,and obstetrics and gynecology.The diverse therapeutic effects of stem cells-derived exosomes are a hierarchical translation through tissue-specific responses,and cell-specific molecular signaling pathways.In this review,we highlight stem cell-derived exosomes as a viable and potent alternative to stem cell-based therapy in managing various surgical conditions.We recommend that future research combines wisdoms from surgeons,nanomedicine practitioners,and stem cell researchers in this relevant and intriguing research area.

Early pregnancy loss,defined as a nonviable intrauterine pregnancy occurring before 12 weeks of gestation,is estimated to affect 25%of clinically diagnosed pregnancies[1].Several demographic,lifestyle,and environmental risk factors have been reported to associate with the risk of early pregnancy loss,yet the causes of most early pregnancy losses remain elusive;thus,further studies are needed.Perfluorinated compounds(PFCs)are a class of widespread environmental pollutants that can adversely affect human reproductive health.Humans are inevitably exposed to PFCs through diet,water,air and dust.A wealth of epidemiological and toxicological research using a variety of animal models has warned of the persistence,bioaccumulation,and toxicity of PFCs[2].

Summary What is already known about this topic?Red and processed meat consumption has been positively related to an increased risk of diabetes in Western populations.However,the results remain inconclusive within Asian populations.What is added by this report?This dose-response meta-analysis of prospective cohort studies conducted in East Asian populations reveals a positive relation between the consumption of processed meat and increased risk of diabetes.Furthermore,a Ushaped association was identified between the consumption of unprocessed red meat and the risk of diabetes.What are the implications for public health practice?This research presents substantive evidence advocating for the reduction of processed and unprocessed red meat consumption as a viable strategy for mitigating the risk of diabetes in East Asian populations.

To the Editor:Total pancreatectomy with islet cell autotransplantation(TPIAT)is a viable treatment option upon failed endoscopic and medical therapy for patients with chronic pancreatitis.This procedure involves surgical removal of the entire pancreas,isolation of islet cells and reinfusion of these cells into the liver via portal vein[1,2].The risk of contamination to the final islet cell product can occur at several stages of the isolation procedure[3].In order to ensure the sterility of the islet cell product,multiple samples from the preservation and cannulation solution,and the final islet cell product are sent for bacterial cultures.Prior studies have found variable clinical consequences of these cultures on infectious com-plications or graft function[3-9].Herein we aimed to determine the incidence of infection in 60 days post-TPIAT and its association with the culture data.

Flow cytometry is a versatile technique for analyzing stroke-induced changes in the immune system.Unlike other methods of cell identification such as immunohistochemistry,the technique is rapid,highly sensitive,and capable of quantifying multiple markers in cell suspensions.Utilizing cell sorters,flow cytometers can also produce highly enriched populations of viable cells for functional studies.The aim of this perspective is to appraise current flow cytometry methods in the field of stroke,provide guidance on best practices,and outline some of the future applications of the technique in pre-clinical and clinical stroke research.

Dear Editor,Targeting senescence therapy is a promising anticancer strategy by arresting the cell-cycle and inducing cellular senescence.However,senescent cells can promote malignancy and drug resistance.1,2 Thus,with the induction of senescence emerged as a viable therapeutic concept,it is important to consider how to eliminate those senescent cancer cells.

Dear Editor,For patientswith epidermal growth factor receptor(EGFR)mutant non-small cell lung cancer(NSCLC),EGFR tyrosine kinase inhibitors(TKIs)are used as the first-line treatment[1,2].Despite initial therapeutic responses,patients invariably experience disease progression due to acquired drug resistance[3].Resistance arises,in part,because a subset of cancer cells undergoes epithelial-mesenchymal transition(EMT)and remains viable despite exposure to EGFR TKI concentrations that eliminate the bulk population[4].The surviving cells can be re-sensitized to treatment by prolonged culture in the absence of EGFR TKIs,indicating a transient.

The immune-modulating effects of viable Weissella cibaria JW15 on RAW 264.7 macrophage cells

The objective of this study is to investigate the immune-enhancing ability of viable and heat-killed Weissella cibaria JW15(JW15)isolated from Kimchi in RAW 264.7 macrophages.The immune effects were evaluated by measuring the production of NO,cytokines,inflammatory enzyme,and activation of NF-κB.viable JW15 executed higher activity on stimulating the release of TNF-αas well as activating NF-κB compared to that of heatkilled JW15.Additionally,viable and heat-killed JW15 significantly increased the production of NO,IL-6 and TNF-αmore than that of Lactobacillus rhamnosus GG(LGG).Furthermore,viable JW15 induced higher production of i NOS compared with that of viable LGG.Collectively,our finding indicates that viable JW15 had similar,if not more,immune-enhancing activities as heat-killed JW15.In addition,viable JW15 had higher immune-enhancing activity than commercial strain LGG.Therefore,viable JW15 has the potential to be used as a functional food to improve the host immune response.
