Epidemiological Approach to Understand Rheumatic Heart Valve Disease: Case of Thoracic Cardiovascular Clinic of Fann Hospital (Dakar)

Objective: Rheumatic heart disease is the major cause of heart valve disease in developing countries. In Senegal, cardiac pathology is dominated by rheumatic pathology whose hospital prevalence was estimated at 0.15% [1]. Epidemiological data on rheumatic heart valve disease (RHVD) are unfortunately imprecise due to the lack of good quality data collection in a large number of countries, especially in sub-Saharan areas and in Central Asia. The objective of this epidemiological study is to collect data about rheumatic heart valve disease in our clinic which is the only surgical center of Senegal. Patients and Methods: Our study conducted between 2014 and 2017 involved a population of one hundred and eighty-three patients with complete medical records, all followed at the Thoracic and Cardiovascular Surgery Clinic at Fann Hospital for rheumatic valve disease. This population was the subject of an epidemiological survey based on a questionnaire covering clinical parameters as consanguinity, clinical signs, diagnosis and surgical intervention. Results: Our work is an epidemiological study involving one hundred and eighty-three (183) patients with various types of rheumatic valve disease, followed at the Thoracic and Cardiovascular Surgery Clinic at Fann Hospital, and who responded to a questionnaire about clinical aspects of this disease. With the formation of the Center for Compact and Efficient Fluid Power （CCEFP） in 2006, there has been a resurgence of academic fluid power research in the USA. The centre’s vision is to make fluid power the technology of choice for power generation, transmission, storage, and motion control. To address fluid power’s key technical barriers, the CCEFP research strategy supports and coordinates pre-competitive research in three thrust areas： efficiency, compactness and effectiveness, where effectiveness means making fluid power safer, easier to use, leak free and quiet. This paper reviews some of the most important results from the first decade of CCEFP research.

Does Gender Affect the Outcomes of Multiple Valve Heart Surgery?

Background: Multiple valve surgery exposes patients to major morbidity and mortality. Little is known about the effect of gender on the outcomes of multiple valve surgery. Methods: In 69 patients who had multiple valve surgery for rheumatic valvular heart disease, 51 patients had mitral and aortic valve replacement, 9 patients had mitral and aortic valve replacement and tricuspid valve repair, 4 patients had mitral valve replacement and tricuspid valve repair, 4 patients had mitral and tricuspid valve repair, and 1 patient had mitral and tricuspid valve replacement. Outcomes were evaluated with univariate analysis. Results: Women had significantly smaller body surface area and smaller left ventricular end-systolic area than men. Women and men had similar left ventricular ejection fraction and New York Heart Association functional class. Univariate analysis showed that in women (but not men), older age, atrial fibrillation, lower left ventricular ejection fraction, and New York Heart Association functional class II and III were associated with longer hospital and intensive care unit stay. In men (but not women), longer cardiopulmonary bypass time was associated with increased left ventricular end-systolic area at 12 months after surgery. Longer aortic cross-clamp time was associated with increased left ventricular end-systolic area in men but only weakly in women. Conclusion: We concluded that gender is an independent predictor of outcomes after multiple valve heart surgery.

Now 100 years have passed, since Arthur Wehnelt published his famous paper entitled: "On the emission of negative ions from glowing metal compounds and related-phenomena". This paper was the start of the oxide cathode, which has become the work horse not only in cathode ray tubes with a production of about 700 million oxide cathodes in 2000, but also in low pressure gas discharge lamps with about 7 billion pieces produced in the same year. A more systematic and detailed investigation of the physical processes in the oxide cathode began about 1920. The studies of Koller and Becker proved, that the emission of the cathode is due to an excess of metallic Ba in the oxide, which is produced during cathode activation. This excess Ba was thought to be present in the form of a monoatomic Ba film, which also exhibits a much lower Ba evaporation rate than bulk Ba. Reimann 1930 and others in the next years found, that there is a close relationship between the electrical conductivity of the coating and the emission. Regarding the theoretical model and basic investigations, new insights have been obtained and some of these was presented at this conference. Thus the oxide cathode still remains a modern and challenging subject despite its 100 years history.

Abstract: The design of reciprocating compressor is very complicated and time-consuming since there are a lot of detailed calculations involved. On the other hand, designers are required by more and more compressor users to design the machine according to their specific applications. Some computer programs have been developed to address this situation cluring the past few years, but all of them run under DOS. These programs have no user-friendly GUI, and also become very inconvenienl because nearly all the computers run with Windows nowadays. Reported in this paper is the development of a software package called RCCAD for the design of reciprocating compressors. This software is based on the latest Windows technique and has an excellent user-friendly GUI. Main programs integrated in the package are thermodynamic calculation, dynamic evaluation, valves design, pressure pulsation analysis and gas property calculation. Several powerfid post-processing programs are also included in the package to show the calculation results. With this software package, almost all the design work can be carried out accurately and efficiently. Some examples are also presented in the paper.

Eustachian Valve Endocarditis—First <i>Staphylococcus epidermidis</i>Case Description

Eustachian Valve Endocarditis (EVE) has been considered an extremely rare clinical entity and only 26 adult cases have been reported in literature. Eustachian valve (EV) is an embryological remnant of the sinus venosus, redirecting oxy-genated fetal blood from inferior vena cava across foramen ovale and left atrium. In adults it is considered a benign rudimentary structure and its persistence is uncommon. Only 4% of the population presents a visible and redundant EV by echocardiography. Up to 10% of infective endocarditis (IE) are right sided and occurs predominantly in patients with predisposing factors as intravenous drug abusers, permanent pacemaker wires, central venous lines, or immunologic disorders, predominantly in tricuspid valve, but with a reported incidence up to 3.3% of EVE. Vast majority of EVE are due to S. aureus in 53%-90% cases. We strongly recommend analysing the EV when echocardiography is performed in a patient suspected of having IE, irrespective of whether vegetation in another valve is found. In this case report we describe the first EVE related to Staphylococcus epidermidis in a 35-year-old male with permanent Shaldon-catheter.
