Dear Editor,The pandemic of coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus 2(SARS-Co V-2)has become a global public health threat.Here we use a TRACE-seq-based metatranscriptomic analysis to compare host responses and vaginal microbiome of postmenopausal female patients with underlying severe COVID-19 disease with those of healthy females,thereby providing insights into the changes in the microenvironment of women's reproductive system.

To the Editor:Primary vaginal carcinoma is a rare malignant disease,which represents only approximately 1%to 2%of female genital malignancies.vaginal adenosis is defined by the presence of metaplastic cervical or endometrial epithelium within the vaginal wall,and is associated with a high risk of vaginal carcinoma.In women prenatally exposed to diethylstilboestrol(DES),vaginal adenosis may arise in up to 90%,and these are often associated with vaginal clear cell adenocarcinoma.[1]DES is an orally active non-steroidal estrogen.The Food and Drug Administration began a clinical trial in 1943,and concluded that DES administration is beneficial for preventing premature deliveries and miscarriages.[2]In addition to primary vaginal clear cell carcinoma(PVCCC),in-uterus exposure to DES can also lead to T-shaped uterus anomaly.[3]In the Chinese population,PVCCC is an uncommon malignancy,and there are no reports on the relationship between PVCCC and DES exposure.[4]Given the rarity of PVCCC,it is significant to analyze relevant cases,and provide some valuable information for future clinical and basic studies.Therefore,the present study described ten cases of PVCCC,and summarized the characteristics of the diagnosis,management,and prognosis of these patients.

The vaginal and fecal microbiomes are related to pregnancy status in beef heifers

Background:The greatest impact on profitability of a commercial beef operation is reproduction.However,in beef heifers,little is known about the vaginal and fecal microbiota with respect to their relationship with fertility.To this end,we followed heifers through gestation to examine the dynamics of vaginal and fecal microbial composition throughout pregnancy.Results:Heifers were exposed to an estrus synchronization protocol,observed over a 12-day period,artificially inseminated 12 h to 18 h after observed estrus,and subsequently exposed to bulls for a 50-day breeding season.vaginal samples were taken at pre-breeding(n=72),during the first(n=72),and second trimester(n=72)for all individuals,and third trimester for individuals with confirmed pregnancies(n=56).Fecal samples were taken at prebreeding(n=32)and during the first trimester(n=32),including bred and open individuals.Next generation sequencing of the V4 region of the 16 S rRNA gene via the Illumina Mi Seq platform was applied to all samples.Shannon indices and the number of observed bacterial features were the same in fecal samples.However,significant differences in vaginal microbiome diversity between gestation stages were observed.No differences in beta-diversity were detected in vaginal or fecal samples regarding pregnancy status,but such differences were seen with fecal microbiome over time.Random Forest was developed to identify predictors of pregnancy status in vaginal(e.g.,Histophilus,Clostridiaceae,Campylobacter)and fecal(e.g.,Bacteroidales,Dorea)samples.Conclusions:Our study shows that bovine vaginal and fecal microbiome could be used as biomarkers of bovine reproduction.Further experiments are needed to validate these biomarkers and to examine their roles in a female’s ability to establish pregnancy.

Risk factors and maternal-fetal outcomes of pregnancies complicated by pre-eclampsia, following cesarean section after a trial vaginal birth

To the Editor:Pre-eclampsia is a leading cause of maternal morbidity and mortality.Although the etiology and progression of pre-eclampsia are not fully understood,it has been demonstrated that delivery is the definitive treatment.The clinician is then faced with the decision of when,and how,to proceed with the delivery.According to the guidelines,pre-eclampsia is not an indication of cesarean section,as expected with some serious complications.This means that pregnant women with pre-eclampsia can have a vaginal trial.However,most pregnant women with pre-eclampsia select to have a direct cesarean section at present.Whether having a cesarean section after trialing a vaginal delivery affects the outcomes of mother and child,and its related risk factors,are still unclear.The purpose of this study was to investigate the risk factors and maternal-neonatal outcomes in severe pre-eclampsia patients who underwent a transfer-cesarean section during vaginal labor.

Uncommon cause of voiding dysfunction in a female patient-vaginal abscess: A case report

BACKGROUND vaginal abscess is a treatable disease and should be considered in female patients with voiding difficulties and perineal tenderness. There are no reported cases of vaginal abscess causing voiding dysfunction in the absence of a previous surgery. Early diagnosis and drainage of vaginal abscesses may lead to excellent outcomes.CASE SUMMARY We presented a case of vaginal abscess that caused voiding dysfunction without surgery history. A 64-yearold woman had a past history of type 2 diabetes mellitus. She came to our clinic following urinary difficulty with perineal tenderness. Bladder ultrasonography revealed a pelvic cystic lesion with a mass effect on the bladder. The presence of a vaginal abscess was suspected following pelvic examination and transvaginal ultrasound.After transvaginal drainage of the vaginal abscess and a full course of antibiotic treatment, she recovered well without any urination symptoms. CONCLUSION Voiding dysfunction caused by vaginal abscess is rare but should be considered in female patients with perineal tenderness.

Diversity scaling of human vaginal microbial communities

DEAR EDITOR,The composition and diversity of the human vaginal microbial community have been investigated intensively due to the diversity-stability relationship (DSR)-based hypothesis for bacterial vaginosis (BV) etiology, which was first proposed in the 1990s and has received renewed interest in recent years. Nevertheless, diversity changes (scaling) across individuals in a cohort or population have not yet been addressed, which is significant both theoretically and practically. Theoretically, biodiversity scaling is the core of biogeography, and practically, inter-subject heterogeneity is critical for understanding the etiology and epidemiology of human microbiome-associated diseases such as BV. Here we applied the diversity-area relationship (DAR), a recent extension to the classic species-area relationship (SAR), to study diversity scaling of the vaginal microbiome by reanalyzing reported data collected from 1 107 postpartum women. The model used here characterized the power-law (or its extension) relationships between accrued diversity and areas (numbers of individuals), upon which four biogeographic profiles were thus defined. Specifically, we established the DAR profile (relationship between diversity scaling parameter and so-termed diversity order (q)), similarly pair-wise diversity overlap (PDO) profile, maximal accrual diversity (MAD) profile, and ratio of individual-level to population-level diversity (RIP) profile. These four profiles offer valuable tools to assess and predict diversity scaling (changes) in the human vaginal microbiome across individuals, as well as to understand the dynamics of vaginal microbiomes in healthy women.

Application of predictive model for vaginal birth after caesarean delivery

Medical practitioners are concerned with the selection of delivery mode after caesarean delivery.Several researchers have developed numerous models for predicting vaginal birth after caesarean delivery.This study selected seven widely used and representative advanced models,such as those of Grobman,Troyer and Parisi,Schoorel,Flamm,Gonen,Weinstain and Smith et al.,analysed the constitutions and clinical applications of the models and identified the factors associated with patients to provide midwives a scientific reference for vaginal delivery evaluation of pregnant women after caesarean delivery.

Adjuncts to case assessment of vaginal discharge syndrome in pregnant women

Objective:To investigate the aetiology of abnormal vaginal discharge,using a non-culture based method,among pregnant women presenting at the University of Calabar Teaching Hospital, Calabar,Nigeria.Methods:Two hundred consecutive antenatal patients,aged 18 to 38 years,with complaints of abnormal vaginal discharge between 1st April and 31st July 2004 were investigated clinically for the characteristics of the vaginal discharge.High vaginal swabs taken from the vaginal fornices were examined using a non-culture based method to determine the possible aetiology of the discharge.The possibility of integrating non-culture based laboratory methods in the syndromic case management of abnormal vaginal discharge in an antenatal clinic setting is discussed.Results: The commonest form of abnormal discharge was curdy white in 66%of cases.Ten(5%) women had malodourous vaginal discharged,92%had vulval itching;and superficial dyspareunia was seen in 29%of cases.Microscopic studies of vaginal discharge revealed the following findings: lactobacilli(96%),polymorphs(96%),’clue’ cells(4%);positive Whiff test(5%),and pH > 4.5 (7%).The clinical and laboratory assessment of each patient lasted between 35 and 45 minutes.The procedures used were acceptable to 78%of women.Conclusion:The use of non-culture based laboratory methods in the initial assessment of abnormal vaginal discharge can be a useful adjunct in the syndromic case management of abnormal vaginal discharge in pregnant women.

Antimicrobial resistance of abnormal vaginal discharges microorganisms in Ouagadougou,Burkina Faso

Objective:To assess the prevalence of bacterial strains and fungal strains infecting the vaginal tract and test their sensitivity to antibiotics in women attending Saint Camille Medical Centre in Ouagadougou.Methods:From January 2008 to December 2009,a total of 2000 vaginal swabs were cultivated for bacterial and fungal identification and isolation.Furthermore,bacterial strains were tested for their susceptibility to several antibiotics used in routine in the centre. Results:The results revealed that microbial isolation and identification was attempted for 1536/2000 sample,a positivity rate of 76.80%.Candida albicans(48.76%),followed by Escherichia coli(16.67%),Streptococcus agalactiae(8-14%) and Staphylococcus aureus(7.55%) were the major agents of genital tract infections in patients.Mycoplasma hominis and Ureaplasma urealyticum combined accounted for less than 7%.Trichomonas vaginalis was identified in 1.04%coses.The antimicrobial tests revealed that the microorganisms developed resistance to several antibiotics including beta lactams.However,antibiotics such as cefamenzol,ciprofloxacin and norfloxacin were still active on these bacteria.Conclusions:The results reveal that many sexually active women are infected by one or more microbial pathogens,probably because of the lock of hygiene or the adoption of some risky behaviors,such as not using condoms or having multiple sexual partners.Efforts should be made to address these points in the country.

Clinical diagnosis and surgical approaches of vaginal hyperplasia in bitches

Objective:To describe the breed predisposition, clinical diagnosis, pathological findings and the surgical approach through excision of the hyperplasic mass from the vagina.Methods:Twenty five bitches of different breeds suffering from a protruded mass from the vulva were examined clinically, blood samples were collected to ensure the phase of estrus that were determined by evaluating the exfoliative vaginal epithelium, and a histopathological examination of the hyperplastic mass was done after its surgical excision.Results:The current work revealed that the maximum value of estradiol 17-B was in Alabai breed while the maximum value of progesterone was in Pit bull breed. And Pit bull breed showed cornification and stratification of the vaginal mucosa with abundant eosinophilic cytoplasm and regular round nuclei.Conclusions:As the vaginal hyperplasia is a crucial gynaecological problem that affects different breeds of bitches, the current work provides a comprehensive diagnosis of the case and illustrates the surgical interference for its excision.
