Diagnostic accuracy of cystoscopic biopsy for tumour grade in outpatients with urothelial carcinoma of the bladder and the risk factors of upgrading

Objective:To assess the concordance of tumour grade in specimens obtained from diagnostic cystoscopic biopsy and transurethral resection of bladder tumour(TURBT)and explore the risk factors of upgrading.Methods:The medical records of 205 outpatients who underwent diagnostic cystoscopic biopsy before initial TURBT were retrospectively reviewed.Comparative analysis of the tumour grade of biopsy and operation specimens was performed.Tumour grade changing from low-grade to high-grade with or without variant histology was defined as upgrading.Logistic regression an-alyses were performed to identify the risk factors of upgrading.Results:For the 205 patients,the concordance of tumour grade between specimens obtained from biopsy and operation was 0.639.The concordance for patients who were preoperatively diagnosed with low-grade and high-grade was 0.504 and 0.912,respectively.Univariate and multivariate logistic regression analyses showed that older age,tumour multifocality,high neutrophil-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),and low lymphocyte-to-monocyte ratio(LMR)were significantly associated with upgrading(odds ratio ranging from 0.412 to 4.364).The area under the curve of the different multivariate models was improved from 0.752 to 0.821,and decision curve analysis demonstrated a high net benefit when NLR,LMR,and PLR were added.Conclusion:Diagnostic cystoscopic biopsy may not accurately represent the true grade of primary bladder cancer,especially for outpatients with low-grade bladder cancer.Moreover,older age,tumour multifocality,high NLR,PLR,and low LMR are risk factors of upgrading,and systemic inflammatory markers improve the predictive ability.

Clinicopathological factors associated with pathological upgrading from biopsy to prostatectomy in patients with ISUP grade group≤2 prostate cancer

We performed this study to investigate pathological upgrading from biopsy to prostatectomy and clinicopathological factors associated with grade group(GG)upgrading in patients with International Society of Urological Pathology(ISUP)GG 1 and 2 prostate cancer(PCa)in a Chinese cohort.We included patients diagnosed with PCa with ISUP GG 1 and 2 at biopsy,who underwent RP at our institution.Pre-and postoperative clinical variables were examined.Univariate and multivariate logistic regression analyses were conducted to identify independent factors associated with GG upgrading.Patients in GG upgraded group had higher total prostatespecific antigen(tPSA;median:14.43 ng ml^(−1) vs 10.52 ng ml^(−1),P=0.001)and PSA density(PSAD;median:0.45 ng ml^(−2) vs 0.27 ng ml^(−2),P3(86.4%vs 67.9%,Pupgrading after RP than those with GG 2(71 vs 54,P=0.016).Independent preoperative factors predicting GG upgrading were PI-RADS score>3(odds ratio[OR]:2.471,95%confidence interval[CI]:1.132–5.393;P=0.023),higher PSAD(P=0.001),and GG in biopsy(OR:0.241,95%CI:0.123–0.471;Pupgrading.Our results revealed that GG in the biopsy,PSAD,PI-RADS score>3,and PNI were independent factors of GG upgrading.These factors should be considered for patients with ISUP grade≤2 PCa.

Prostate Health Index(phi)and its derivatives predict Gleason score upgrading after radical prostatectomy among patients with low-risk prostate cancer

To analyze the performance of the Prostate Health Index(phi)and its derivatives for predicting Gleason score(GS)upgrading between prostate biopsy and radical prostatectomy(RP)in the Chinese population,an observational,prospective RP cohort consisting of 351 patients from two medical centers was established from January 2017 to September 2020.Pathological reclassification was determined by the Gleason Grade Group(GG).The area under the receiver operating characteristic curve(AUC)and logistic regression(LR)models were used to evaluate the predictive performance of predictors.In clinically low-risk patients with biopsy GG≤2,phi(odds ratio[OR]=1.80,95%confidence interval[95%CI]:1.14-2.82,P=0.01)and its derivative phi density(PHID;OR=2.34,95%CI:1.30-4.20,P=0.005)were significantly associated with upgrading to GG≥3 after RP,and the results were confirmed by multivariable analysis.Similar results were observed in patients with biopsy GG of 1 for the prediction of upgrading to RP GG≥2.Compared to the base model(AUC=0.59),addition of the phi or PHID could provide additional predictive value for GS upgrading in low-risk patients(AUC=0.69 and 0.71,respectively,both Pupgrading after RP in clinically low-risk patients.

upgrading of derived pyrolysis vapors for the production of biofuels from corncobs

A bubbling fluidized bed pyrolyzer was integrated with an in-situ honeycomb as a catalytic upgrading zone for the conversion of biomass to liquid fuels. In the upgrading zone, zeolite coated ceramic honeycomb （ZCCH） catalysts consisting of ZSM-5 （Si/ Al = 25） were stacked and N2 or recycled non-condensable gas was used as a carrier gas. Ground corncob particles were fast pyrolyzed in the bubbling bed using fine sand particles as a heat carrier and the resulting pyrolysis vapors were passed on-line over the catalytic upgrading zone. The influence of carrier gas, temperature, and weight hourly space velocity （WHSV） of catalyst on the oil product properties, distribution and mass balance were studied. Using ZCCH effectively increased the hydrocarbon yield and the heating value of the dry oil, especially in the presence of the recycled noncondensable gas. Even a low usage of zeolite catalyst at WSHV of 180 h^-1 was effective in upgrading the pyrolysis oil and other light olefins. The highest hydrocarbon （≥C2） and liquid aromatics yields reached to 14.23 and 4.17 wt-%, respectively. The undesirable products including light oxygenates, furans dramatically decreased in the presence of the ZCCH catalyst.

upgrading Pathways of Intelligent Manufacturing in China:Transitioning across Technological Paradigms

Intelligent technologies are leading to the next wave of industrial revolution in manufacturing.In developed economies,firms are embracing these advanced technologies following a sequential upgrading strategy-from digital manufacturing to smart manufacturing(digital-networked),and then to newgeneration intelligent manufacturing paradigms.However,Chinese firms face a different scenario.On the one hand,they have diverse technological bases that vary from low-end electrified machinery to leading-edge digital-network technologies;thus,they may not follow an identical upgrading pathway.On the other hand,Chinese firms aim to rapidly catch up and transition from technology followers to probable frontrunners;thus,the turbulences in the transitioning phase may trigger a precious opportunity for leapfrogging,if Chinese manufacturers can swiftly acquire domain expertise through the adoption of intelligent manufacturing technologies.This study addresses the following question by conducting multiple case studies:Can Chinese firms upgrade intelligent manufacturing through different pathways than the sequential one followed in developed economies?The data sources include semistructured interviews and archival data.This study finds that Chinese manufacturing firms have a variety of pathways to transition across the three technological paradigms of intelligent manufacturing in nonconsecutive ways.This finding implies that Chinese firms may strategize their own upgrading pathways toward intelligent manufacturing according to their capabilities and industrial specifics;furthermore,this finding can be extended to other catching-up economies.This paper provides a strategic roadmap as an explanatory guide to manufacturing firms,policymakers,and investors.

upgrading process of palm empty fruit bunches as alternative solid fuel: a review

A total of 37.5 million tons of palm empty fruit bunches(EFBs)produced in Indonesia in 2018 have the potential to be used as an alternative eco-friendly solid fuel.However,a pretreatment process is necessary to increase the heating value and reduce the po-tassium content of EFBs.Several methods can be employed to improve the characteristics of EFBs as a solid fuel,such as drying and torrefaction to increase the heating value and the leaching process for reducing the potassium content of EFBs.The main concept of increasing the heating value is to increase the fuel content,which is carbon.Through drying,the carbon ratio can be increased by reducing the moisture content and,through torrefaction,the carbon ratio could be increased due to the decomposition of ligno-cellulose.A simple way to reduce the potassium content of EFBs is the leaching process.Two types of leaching treatment have been studied before:soaking and stirring treatment.This study reviews those methods to make EFBs more suitable as a solid fuel with a high heating value and less potassium content.Increasing the carbon content in EFBs is crucial for enhancing their heating value as a fuel.Drying and torrefaction decrease the moisture content and modify the chemical structure,resulting in higher carbon ratios.The leaching process effectively reduces the potassium content of EFBs and stirring treatment is more effective than immersion treat-ment.Torrefaction positively affects leaching by decreasing the potassium content,while hydrophobicity may hinder leaching by re-pelling water.The high moisture content of leached EFBs requires additional energy for evaporation during torrefaction,resulting in a lower energy density yield compared with raw EFBs.Adjustments may also be required to account for the decreased mineral content,which functions as a torrefaction catalyst in EFBs that have not been leached.

Production of renewable fuels by blending bio-oil with alcohols and upgrading under supercritical conditions

The work studied a non-catalytic upgrading of fast pyrolysis bio-oil by blending under supercritical conditions using methanol,ethanol and isopropanol as solvent and hydrogen donor.Characterisation of the bio-oil and the upgraded bio-oils was carried out including moisture content,elemental content,pH,heating value,gas chromatography-mass spectrometry(GCMS),Fourier transform infrared radiation,13C nuclear magnetic resonance spectroscopy,and thermogravimetric analysis to evaluate the effects of blending and supercritical reactions.The GCMS analysis indicated that the supercritical methanol reaction removed the acids in the bio-oil consequently the pH increased from 2.39 in the crude bio-oil to 4.04 after the supercritical methanol reaction.The ester contents increased by 87.49% after the supercritical methanol reaction indicating ester formation could be the major deacidification mechanism for reducing the acidity of the bio-oil and improving its pH value.Simply blending crude bio-oil with isopropanol was effective in increasing the C and H content,reducing the O content and increasing the heating value to 27.55 from 17.51 MJ·kg^-1 in the crude bio-oil.After the supercritical isopropanol reaction,the heating value o f the liquid product slightly further increased to 28.85 MJ·kg^-1.

This review article summarizes the key published research on the topic of bio-oil upgrading using catalytic and non-catalytic supercritical fluid(SCF)conditions.The precious metal catalysts Pd,Ru and Pt on various supports are frequently chosen for catalytic bio-oil upgrading in SCFs.This is reportedly due to their favourable catalytic activity during the process including hydrotreating,hydrocracking,and esterification,which leads to improvements in liquid yield,heating value,and pH of the upgraded bio-oil.Due to the costs associated with precious metal catalysts,some researchers have opted for non-precious metal catalysts such as acidic HZSM-5 which can promote esterification in supercritical ethanol.On the other hand,SCFs have been effectively used to upgrade crude bio-oil without a catalyst.Supercritical methanol,ethanol,and water are most commonly used and demonstrate catalyst like activities such as facilitating esterification reactions and reducing solid yield by alcoholysis and hydrolysis,respectively.

Physical upgrading plan for slum riverside settlement in traditional area:A case study in Kuin Utara,Banjarmasin,Indonesia

This study aims to draw basic and global concepts for the physical improvement of slum riverside settlements as a fundamental step toward formalizing and securing tenure.Moreover,this work specifies attempts to arrange an upgrading plan derived from the basic strategy for our target location in the traditional settlement of Kuin Utara without failing to recognize its uniqueness and cultural activities.This paper discloses that the basic concept for physical upgrading in a slum settlement consists of three elements:(1)arranging street networks as fundamental systems that structure the settlement,(2)constructing public utility systems that are essential for livelihoods,and(3)providing common space and amenities as"external organs”that form identities and reveal the physical beauty of the location.The proposed infrastructures shall be directed to protect the local cultures and sociocultural activities of the inhabitants.The plan should also be adjusted with respect to the economic constraints in developing countries.Moreover,the improvement strategy for poor housings shall not orientate to modernization merely for beautification without considering the basic aspect:maintaining the dweller’s livelihood.

Mineralogy,Chemical Characteristics and upgrading of Beach Ilmenite of the Top Meter of Black Sand Deposits of the Kafr Al-Sheikh Governorate,Northern Egypt

Egyptian beach ilmenite occurs in a relatively high content in the naturally highly concentrated superficial black sand deposits at specific beach zones in the northern parts of the Nile Delta at Rosetta. Microscopic study shows that the ilmenite occurs as fresh homogeneous black or heterogeneous multicoloured altered grains and exhibits three types （homogeneous, exsolved and altered） of ilmenite varieties. XRD data of ilmenite indicates their association with minor hematite and quartz, whereas leucoxene shows its association with Nb-rutUe, pseudorutile and hematite. Grain size distribution suggests a very fine sand size of 〉89% and 80% and a fine sand size of 10.5% and 18.3% for fresh and altered ilmenites, respectively. The density of fresh, altered ilmenite and leucoxene concentrates varies from 2.70, 2.50 to 2.40 ton/m^3, suggesting a gradual decrease from high grade fresh to leucoxene and consistent with variation in magnetic susceptibility as a consequence of the leaching of iron. Mass magnetic susceptibility reveals 97.6% of ilmenite and 92% of the altered form are obtained at 0.20 and 0.48 ampere. Fresh iimenite exhibits variable TiO2 （47.18%） and Fe2O3^T （46.10%） with minor MnO, MgO and Cr2O3 （1.22, 1.10 and 0.51%）. The altered ilmenite is higher in TiO2 （76.16%） and SiO2 （4.68%） and lower in Fe2O3^T （14.45%）, MnO, MgO and Cr2O3 （0.39, 0.52 and 0.11%） compared with the fresh form. Three concentrates of ilmenites （G1, G2 and G3） were prepared from crude ore using a Reading cross belt magnetic separator under different conditions, revealing a gradual increase of TiO2, SiO2, Al2O3 and CaO accompanied by a decrease of Fe2O3^T, MgO and Cr2O3 with repetition of the separation processes. Several ore dressing techniques were carried out to upgrade the iimenite concentrate.
