Background:transcatheter closure(TCC)has emerged as the preferred treatment for selected congenital heart disease(CHD).While TCC offers benefits for patients with postoperative residual shunts,understanding its mid-and long-term efficacy and safety remains ***:This study aims to assess the mid-and long-term safety and efficacy of TCC for patients with residual atrial or ventricular septal shunts following CHD ***:In this consecutive retrospective study,we enrolled 35 patients with residual shunt who underwent TCC or surgical repair of CHD between June 2011 to October *** candidacy was determined based on established *** and electrocardiogram were conducted during the perioperative period and continued as part of long-term ***:Among the patients,5(14.3%)exhibited interatrial shunt-ing,while 30(85.7%)had interventricular *** was successfully implemented in 33 of 35 patients,with exceptions in two cases of post-ventricular septal defect repair due to anatomical challenges involving the shape and aortic *** resulted in a TCC success rate of 94.3%.Trace residual shunt was detected in two interventricular shunting cases and a mild residual shunt in one interventricular shunting case;all resolved by the three-month follow-up after *** complications included one hematoma at the puncture site and one transient junctional rhythm during the perioperative *** a median follow-up of 73 months,there were no instances of residual shunt,device embolization,occluder displacement,valve insufficiency,malignant arrhythmia,infective endocarditis,death,or other serious ***:TCC is an effective and safe therapy for patients with residual atrial or ventricular septal shunts following CHD *** support the consideration of TCC as the preferred treatment option for appropriate patient populations.

Objective:To evaluate the efficacy of Amplatzer duct occluder II(ADO II)in the treatment of perimembranous ventricular septal defect(pmVSD)in ***:Between June 2017 and June 2020,13 patients with pmVSD had attempted transcatheter closure using ADO II,seven of patients were used antegrade approach and six of them were used retrograde ***:There were 8 males and 5 females,age from 1 to 7 years,weight from 10.5 to 31.0 kg,and VSD size from 2.0 to 4.0 *** was successful in all cases with the outer diameter of the occluders ranging from 4 to 6 *** aortic,tricuspid regurgitation or residual shunt was found in the immediate ultrasound *** arrhythmia was observed in the Holter monitoring 3 days after the *** echocardiography indicated complete shunt *** evidence of occluder prolapse,malignant arrhythmia,or intensed valve regurgitation was seen on a median follow-up of 18 months(range,6 to 36 months).Conclusions:Based on our experience,ADO II showed good efficacy in the early and middle stages of pediatric pmVSD closures.

To evaluate the feasibility and short term results of transcatheter closure of secundum atrial septal defects (TCSASDs) using Amplatzer occluder device A new self centering device, the Amplatzer atrial septal occluder, has recently been evaluated in an animal model with excellent results This paper reports our clinical research with this device Methods 30 patients underwent an attempt at TCSASDs at an average age of 35 6±15 9?y (range, 5-62?y) and average weight of 63 6±14 5?kg (range, 17-78?kg) using the Amplatzer atrial septal occluder The average ASD diameter measured by transesophageal echocardiography (TEE) was 23 6±4 0?mm (range, 16-31?mm), and the average atrial septal defects (ASD) balloon stretched diameter was 25 2±5 8?mm (range, 9-34?mm) All patients had right atrial and ventricular volume overload with a Qp/Qs of 3 4±1 2 (1 4-5 2) A F8 12 catheter was used for delivery of the device in all patients Results The device was placed correctly in all patients There was immediate and complete closure in 30 patients The average device waist size used to close the defect was 25 6±5 9?mm (range, 9-34?mm) There was no episode of device embolization or any other complication Follow up was performed using transthoracic echocardiography (TTE) 1 day, 1 month, 3 months, 6 months and 1 year after the procedure The first day after operation; there was complete closure of the ASD in all patients; there have been no episodes of endocarditis, thromboembolism, or wire fracture Conclusion TCSASDs using the Amplatzer occluder device was an efficient nonsurgical method which had a very high technical success rate and satisfactoy short term and follow up results

Background Coronary artery fistula （CAF） is a rare *** CAF closure has been introduced using various materials,but only few data are available on the Guglielmi detachable coil （GDC）.The advantage of using GDC for transcatheter CAF closure is more controllable,therefore much safer when compared to other *** report is about our experience in transcatheter closure of CAF using fibered GDC in our *** ＆ Results From 2002 to 2007,there were 10 patients with CAFs （age range：28 to 56 year-old,7 males） who underwent transcatheter CAF *** were a total of 19 CAFs which originated from right coronary （n =5）,left circumflex （n =3）,left anterior descending artery （n =10） and left main trunk （n =1）.Median number of coil deployment for each fistula was 3 （range：1 to 6）.The pulmonary artery was the most common site of the distal communication of CAFs （n =14）,followed by right atrium （n =3）,left atrium （n =1） and left ventricle （n =1）.Immediate coronary angiography after GDC deployment revealed no residual shunt in 12 （63.2％） CAFs,significant reduction of the flow in 5 （26.3％）,while 2 （10.5％） could not be closed due to small *** （90％） patients underwent a repeated angiography within 3 to 8 *** 12 CAFs that were occluded immediately post-deployment,there were 2 CAFs with insignificant residual *** 6 CAFs with significantly decreased flow immediately post-deployment,2 were occluded totally in the follow-up *** total,12 （70.5％） CAFs were occluded completely and 5 （29.5％） CAFs still had insignificant residual flow,which did not need any additional coil *** a mean follow up of 4.3 ± 0.7 year,all patients remained symptom and complication *** The fibered GDC is a safe and effective method for percutaneous closure of the CAFs.

Effectiveness and Safety of transcatheter closure of Various Ventricular Septal Defects Using Second-Generation Amplatzer Duct Occluders

Objective:This study was designed to determine the long-term safety and efficacy of using the Amplatzer Duct Occluder II(ADO II)for the closure of various ventricular septal defects(VSDs).Methods:From January 2011 to December 2019,selected VSD patients were treated through transcatheter intervention using ADO II *** closure results and complications from 188 patients,involving 167 perimembranous ventricular septal defects(pmVSDs),9 intracristal VSDs,11 post surgery residual shunts and 1 post closure residual shunt with the mean outlet diameter3.1±0.8 mm under angiography,were enrolled in this ***:The success rate was 98.9%for all *** the median 77-month follow-up period,no cases of complete atrioventricular block(cAVB),infective endocarditis or death *** major adverse event(0.5%)was recorded:cerebrovascular accident occurred 1 day after the procedure in one patient who was transferred to the neurology *** residual shunt rate was 44.6%,which was the most common minor adverse *** cardiac conduction block rate was 4.3%.Specifically,one pmVSD patient developed intermittent LBBB during the 28-month *** were 3 patients(1.6%)with new-onset mild tricuspid insufficiency,and the insufficiency degree was stable during *** was no new-onset aortic insufficiency that ***:transcatheter closure of pmVSDs,some intracristal VSDs,some postsurgery or postclosure residual shunts using ADO II occluders were both safe and effective and yielded excellent long-term results in selected patients.

Outcomes of transcatheter closure in Outlet-Type Ventricular Septal Defect after 1 Year

Background:Ventricular septal defect(VSD)is the most common congenital heart *** VSD closure is an effective treatment for patients with muscular and perimembranous ***,there is a limit data for outlet VSD,especially impact to the aortic valve leaflet after transcatheter *** study aims to assess the outcomes of transcatheter closure of the outlet-type ventricular septal defect(OVSD)after 1 postoperative ***:A retrospective study was performed including 50 patients who underwent transcatheter(n=25)and surgical(n=25)OVSD closure during the exact time frame at two medical ***:The median age and body weight of patients in the transcatheter group were significantly higher than those of patients in the surgical group(7.0 vs.2.8 years;27.0 vs.11.4 kg;ptranscatheter group(5.0 vs.3.0 mm;ptranscatheter closure(100%)as effective as surgical *** than small residual shunt was present 20%and 8%immediately after the procedure in the transcatheter and surgical groups(p=0.50),which decreased to 12%and 4%at the 1-year follow-up(p=0.61),*** incidence of complete atrioventricular block and other complications was observed in both groups,and no significant differences were noted in the new onset or worsening of the aortic regurgitation in both groups(p=1.0).Conclusions:transcatheter treatment could be effectively and safely achieved for OVSD closure at 1-year follow-up.

A congenital coronary artery fistula(CCAF)combined with giant coronary aneurysm(CAA)is a rare congenital cardiac *** reported an 8-year-old patient who underwent transcatheter closure of both inlet and outlet of a proximal left coronary artery(LCA)-to-left ventricular(LV)fistula with CAA of 41 mm×28 mm in diameter,during which acute occlusion of left anterior descending coronary artery(LAD)occurred immediately after device implantation at the inlet of *** managed to prevent the patient from major adverse cardiac events by conservative therapy with dual antiplatelet agents instead of surgical removal of the *** patient recovered well and had been follow-up for 2 years with no late complications reported.
