Trifocal toric intraocular lenses in eyes with low amount of corneal astigmatism

AIM:To evaluate the refractive and visual outcomes following cataract surgery and implantation of a trifocal toric intraocular lens(IOL)in eyes with low degrees of corneal astigmatism.METHODS:Twenty-six eyes of 22 patients who underwent implantation a trifocal toric IOL(FineVision PODFT,PhysIOL s.a.,Liege,Belgium)were enrolled.Phacoemulsification with femtosecond laser,capsular tension ring insertion and intraoperative aberrometry were performed in all cases.All IOLs used showed a cylinder power of 1.00 D.Main outcome measures were refractive error and corrected-distance visual acuity(CDVA)and uncorrected-distance visual acuity(UDVA)values.Eyes were evaluated at 4 mo post-surgery.RESULTS:Totally 50%of eyes showed a spherical equivalent(SE)within±0.13 D and all of them within±0.50 D.The mean SE and refractive cylinder were-0.02±0.23 and-0.16±0.22 D,respectively.Vector analysis revealed that 100%of eyes were within±0.50 D for the SE and cylindrical components(J0 and J45).Refractive changes were not correlated with keratometric changes(P>0.05)showing that the reduction in astigmatism comes from the trifocal toric IOL.Of 81%and 96%of eyes showed UDVA and CDVA of 20/20,respectively.The postoperative mean values of monocular distance Snellen decimal UDVA and CDVA were 0.97±0.05 and 0.99±0.02(about 20/20),respectively.CONCLUSION:Our study suggests that the use of this trifocal toric IOL in patients with low amount of astigmatism provides accurate refractive outcomes and enables them to achieve excellent visual acuity.

Accuracy of corneal astigmatism correction with two Barrett toric calculation methods

AIM: To compare the prediction error between Barrett toric calculator and the new online AcrySof toric calculator which incorporated Barrett astigmatism algorithm in Chinese cataract eyes with normal axial length and anterior chamber depth(ACD). METHODS: Prospective case-control study. All the cases had axial length(21-26 mm) with ACD no less than 2.4 mm.Keratometric values were measured by LenSTAR 900. The Barrett toric calculator was used in group 1. In group 2, SRK-T formula was used to determine the spherical power of the toric lens, and subsequent calculation of the cylinder type was performed using the new online Alcon toric calculator. At 1 and 3 mo after surgery, a comprehensive subjective optometry was performed. The predicted residual astigmatism calculated by the two calculators was compared with that obtained by postoperative refraction,and the difference was defined as the astigmatism correction error [error of refractive astigmatism(ERA)].The error magnitude(EM) refers to the algebraic deviation of ERA, and the error vector(EV) indicates the vector deviation of ERA. The influence of the two calculation methods on the correction accuracy of toric IOL was quantitatively analyzed. RESULTS: The |EM| obtained at 1 mo after surgery were0.21±0.12 D, 0.22±0.18 D in group 1 and group 2 respectively,and correspondingly turned to be 0.19±0.13 D, 0.20±0.19 D at3 mo after surgery, with no statistical difference(P=0.633,P=0.877). The vector analysis showed that |EV| values in two groups at 1 mo after surgery were 0.29±0.14@105(D@angle) and 0.35±0.20@113(D@angle), respectively,whereas |EV| values 3 mo after surgery were 0.27±0.16@86(D@angle) and 0.32±0.23@102(D@angle), respectively. The differences between the groups were not statistically significant(P=0.119, P=0.261). CONCLUSION: The clinical effect of Barrett toric calculator has a much more accurate tendency than that of new online AcrySof toric calculator, but is not evident in cases with normal axial length and normal anterior posterior ratio.

In this paper, we introduce the concept of P-difference varieties and study the properties of toric P-difference varieties. toric P-difference varieties are analogues of toric varieties in difference algebraic geometry. The category of affine toric P-difference varieties with toric morphisms is shown to be antiequivalent to the category of affine P [x]-semimodules with P [x]-semimodule morphisms. Moreover, there is a one-to-one correspondence between the irreducible invariant P-difference subvarieties of an affine toric P-difference variety and the faces of the corresponding affine P [x]-semimodule. We also define abstract toric P-difference varieties by gluing affine toric P-difference varieties. The irreducible invariant P-difference subvariety-face correspondence is generalized to abstract toric P-difference varieties. By virtue of this correspondence, a divisor theory for abstract toric P-difference varieties is developed.

Evaluation of axis alignment and refractive results of toric phakic IOL using image-guided system

AIM: To evaluate accuracy of axis alignment and refractive results of toric phakic intraocular lens(IOL) implantation using a digital imaging system. METHODS: This retrospective study investigated toric implantable collamer lens(ICL) implantation in 30 eyes of 21 patients with myopic astigmatism more than 2.0 D guided with digital imaging system. Data were collected during the first week after phakic IOL implantation.RESULTS: Thirty eyes of 21 patients were included in our study. Patients includes 9 males and 12 females. The mean age of the patients was 26.5±7.1(range 21-44)y. The mean preoperative manifest astigmatism was 3.2±1.7(range from 2.25 to 4.75) D. The mean postoperative uncorrected distance visual acuity(UCDVA) were 0.07±0.07(range from 0.1 to 0.0) log MAR. The mean postoperative residual refractive cylinder was 0.25±0.29(range 0-0.75) D. Eyes with postoperative residual refractive cylinder of 0.5 D or less represented 80%(24 eyes). The mean postoperative toric IOL misalignment measured by the OPD scan III was 1.9°±1.45°(range from 0 to 5°). CONCLUSION: Image guided system allows accurate alignment of toric ICL. This is associated with good postoperative visual acuity and low residual refractive astigmatism which correlates with the precision of toric phakic IOL alignment.

Outcomes and possible risk factors associated with axis alignment and rotational stability after implantation of the toric implantable collamer lens for high myopic astigmatism

AIM: To assess the visual outcomes and possible risk factors associated with axis alignment and rotational stability after implantation of toric implantable collamer lens (TICL) for the correction of high myopic astigmatism. METHODS: In this prospective, nonrandomized clinical study, 54 consecutive eyes of 29 patients with high myopic astigmatism received TICL implantation. To evaluate postoperative axis deviation from the intended axis, a digital anterior segment photograph was taken. The ultrasound biomicroscopy(UBM) was used to observe footplate-position. RESULTS: After mean follow-up of 8.6 months, mean manifest refractive cylinder (MRC) decreased 79.3% from (-1.88 +/- 1.49)D preoperatively to (0.39 +/- 0.61)D postoperatively. MRC within 1.00D occurred in 68.5% (37/54) of eyes, whereas 48.1% (26/54) had MRC within 0.50D. Mean manifest refraction spherical equivalent (MRSE) changed from (-1208 +/- 422)D preoperatively to (-0.41 +/- 0.61)D postoperatively. Uncorrected binocular vision of 20/20 or better occurred in 72.2% (39/54) of patients compared with binocular best-corrected visual acuity (BCVA) of 20/20 or better in 44.4% (24/54) preoperatively. The mean difference between intended and achieved TICL axes was (6.96 +/- 8.37)degrees. Footplates of TICLs were in the ciliary sulcus in 22 eyes (46.3%), below the ciliary sulcus in 32 eyes (53.7%). The angle of TICL rotation had significant correlation with the footplates-position (t = 2.127; P=0.045) and the postoperative TICL vaulting (r=-0.516; P=0.000). CONCLUSION: The results of our study further support the safety, efficacy and predictability of TICL for the correct high myopic astigmatism. The footplate-position of TICL and vault value should be taken into consideration as two possible risks factors for TICL rotation.

Combination of toric and multifocal intraocular lens implantation in bilateral cataract patients with unilateral astigmatism

AIM： To assess the binocular visual function in bilateral cataract patients with unilateral astigmatism after combined implantations of toric with multifocal intraocular lens （IOL）, and to compare with that of toric and monofocal IOL implantation.METHODS： All the 30 patients with unilateral astigmatism suffered bilateral cataract were randomly divided into two groups： toric plus multifocal IOL group and toric plus monofocal IOL group. Uncorrected and corrected visual acuity at distance （5.0 m）, intermediate distance （0.6 m）, and near （0.33 m）, contrast sensitivity, and stereopsis were assessed 6mo after surgery. Patients were also surveyed for visual disturbances and spectacle dependence.RESULTS： Binocular uncorrected visual acuity （LogMAR） of toric/multifocal IOL eyes at distance, intermediate, near were 0.05±0.05, 0.24±0.10, and 0.14±0.06 respectively. The values of toric plus monofocal IOL eyes were 0.06±0.07, 0.26±0.08, and 0.37±0.10 respectively. These values did not indicate significant differences between two groups with exception of near visual acuity. In the photopic condition （with or without glare）, the contrast sensitivity of multifocal IOL eyes was significant lower than the monofocal IOL eyes in 18 cpd. In the mesopic condition, the contrast sensitivity of multifocal group was significant lower than monofocal group in 12 cpd, and in mesopic glare condition, this significant difference was found both in 6 cpd and 12 cpd. The stereopsis of toric/multifocal IOL eyes decreased slightly （100±80 seconds of arc, t=2.222, P=0.136）. Mean near vision for patient satisfaction was statistically significantly higher in toric/multifocal IOL group patients versus than that in toric/monofocal IOL group （80% vs 25.5%, P=0.000）. Visual disturbance was not noticed in either group.CONCLUSION： Although the combination of toric and multifocal IOL implantation results in compromising stereoacuity, it can still provide patients with high levels of spectacle freedom and good overall binocular visual acuity.

目的:评价2.4mm透明角膜切口联合toric IOL植入治疗白内障合并角膜散光患者的临床疗效。方法:收集2021年9月至2022年11月间于吉林大学白求恩第二医院行2.4mm透明角膜切口联合TECNIS toric IOL植入治疗白内障合并角膜散光≥0.75D患者36例47眼的临床资料进行回顾性分析。收集患者术前资料:性别、年龄、眼别、前房深度、眼轴长度、眼压、泪液分泌试验(Schirmer试验)、晶体球镜度数、角膜曲率、全眼和角膜散光度数、预计残余散光、UCVA(裸眼视力)、BCVA(矫正视力),全眼和角膜高阶像差,IOL散光轴位、客观视觉质量,主观视觉质量等;于术后1w,1m,3m进行随访并收集以下资料:术后UCVA、BCVA、全眼和角膜散光度数、OPD-Scan III视觉质量分析仪测量IOL旋转轴位并计算旋转度数、全眼和角膜高阶像差、客观视觉质量相关参数,采用问卷调查的形式评估患者术后主观视觉质量、满意度。采用SPSS 25.0软件进行统计分析。结果:1.术后1w,1m,3m的UCVA及BCVA较术前均显著提高,差异有统计学意义(P0.05)。2.散光:患者术后各时间段的全眼散光较术前均显著降低,差异有统计学意义(P0.05),术后1m,3m的角膜散光较术后1w降低,差异有统计学意义(P0.05)。5.客观视觉质量:OPD结果显示术后PSF点扩散函数光斑面积明显减小,术后3m的SR值(0.17±0.02)较术前的SR值(0.05±0.04)显著提高,差异有统计学意义(t=15.818,Ptoric IOL植入安全、有效、可预测性强,提高术后裸眼、矫正远视力和降低术前角膜散光的临床效果显著,并且角膜切口可在术后早期恢复。2.2.4mm透明角膜切口联合toric IOL植入可以有效降低全眼高阶像差,为患者提供术后满意的主观和客观视觉质量。3.2.4mm透明角膜切口联合toric IOL植入术后IOL旋转稳定性良好,脱镜率高,满意度好,提高了患者术后生活质量。

toric Heaps, Cyclic Reducibility, and Conjugacy in Coxeter Groups

In 1986, G.X. Viennot introduced the theory of heaps of pieces as a visualization of Cartier and Foata’s “partially commutative monoids”. These are essentially labeled posets satisfying a few additional properties, and one natural setting where they arise is as models of reduced words in Coxeter groups. In this paper, we introduce a cyclic version of a heap, which loosely speaking, can be thought of as taking a heap and wrapping it into a cylinder. We call this object a toric heap, because we formalize it as a labeled toric poset, which is a cyclic version of an ordinary poset. Defining the category of toric heaps leads to the notion of certain morphisms such as toric extensions. We study toric heaps in Coxeter theory, because a cyclic shift of a reduced word is simply a conjugate by an initial or terminal generator. As such, we formalize and study a framework that we call cyclic reducibility in Coxeter theory, which is closely related to conjugacy. We introduce what it means for elements to be torically reduced, which is a stronger condition than simply being cyclically reduced. Along the way, we encounter a new class of elements that we call torically fully commutative (TFC), which are those that have a unique cyclic commutativity class, and comprise a strictly bigger class than the cyclically fully commutative (CFC) elements. We prove several cyclic analogues of results on fully commutative (FC) elements due to Stembridge. We conclude with how this framework fits into recent work in Coxeter groups, and we correct a minor flaw in a few recently published theorems.

目的评价Acrysof toric 人工晶状体的临床效果和旋转稳定性,以及角膜曲率测量方法对其矫正效果的影响.方法收集浙江大学医学院附属第一医院眼科植入toric IOL 的白内障患者20例23只眼.随机根据自动验光仪、角膜地形图和IOL Master测量的角膜曲率值计算植入相应的Acrysof toric IOL,观察术后3个月裸眼视力（UCVA）、最佳矫正视力（BCVA）、角膜散光、总散光、人工晶状体旋转度,计算术源性散光（SIA）,比较术前三种角膜曲率测量方法对散光矫正的影响.结果术后患眼的UCVA为0.69±0.16,BCVA为0.83±0.15.患眼的总散光从术前的3.23D降至0.59D,下降了约82%.而术前后角膜曲率、角膜散光值差异无统计学意义.对术后总散光与目标残余散光的差值,三种角膜曲率测量方法之间差异无统计学意义.结论 Acrysof toric 人工晶状体具有良好的散光矫正效果和旋转稳定性.自动角膜曲率测量方法对其散光矫正效果的影响不大.

toric Intraocular Lens Malposition Corrected by Lens Repositioning to Manifest Refractive Cylinder Axis in Patient with Irregular Astigmatism Due to Corneal Scar

A case is presented of a patient with an unexpected poor visual result and subsequent correction following cataract removal surgery via phacoemulsification and intraocular lens implantation using a toric intraocular lens implant (IOL). The initial operation resulted in an uncorrected vision of 20/100 (0.70 logMAR). Retrospective analysis of the patient’s corneal topography revealed irregular astigmatism secondary to remote trauma to the cornea. The cylinder axis on manifest refraction (MR) was significantly different from measured keratometry, so a second procedure was performed to align the cylinder axis of the IOL with the steep axis on MR. This repositioning procedure improved visual outcome to a final uncorrected vision of 20/25 (0.10 logMAR) and best corrected acuity of 20/20 (0.0 logMAR).
