Current trends in three-dimensional visualization and real-time navigation as well as robot-assisted technologies in hepatobiliary surgery

With the continuous development of digital medicine,minimally invasive precision and safety have become the primary development trends in hepatobiliary surgery.Due to the specificity and complexity of hepatobiliary surgery,traditional preoperative imaging techniques such as computed tomography and magnetic resonance imaging cannot meet the need for identification of fine anatomical regions.Imaging-based three-dimensional(3D)reconstruction,virtual simulation of surgery and 3D printing optimize the surgical plan through preoperative assessment,improving the controllability and safety of intraoperative operations,and in difficult-to-reach areas of the posterior and superior liver,assistive robots reproduce the surgeon’s natural movements with stable cameras,reducing natural vibrations.Electromagnetic navigation in abdominal surgery solves the problem of conventional surgery still relying on direct visual observation or preoperative image assessment.We summarize and compare these recent trends in digital medical solutions for the future development and refinement of digital medicine in hepatobiliary surgery.

A three dimensional visualized physical simulation for natural gas charging in the micro-nano pore system

A micro-nano pore three-dimensional visualized real-time physical simulation of natural gas charging, in-situ pore-scale computation, pore network modelling, and apparent permeability evaluation theory were used to investigate laws of gas and water flow and their distribution, and controlling factors during the gas charging process in low-permeability(tight) sandstone reservoir. By describing features of gas-water flow and distribution and their variations in the micro-nano pore system, it is found that the gas charging in the low permeability(tight) sandstone can be divided into two stages, expansion stage and stable stage. In the expansion stage, the gas flows continuously first into large-sized pores then small-sized pores, and first into centers of the pores then edges of pores;pore-throats greater than 20 μm in radius make up the major pathway for gas charging. With the increase of charging pressure, movable water in the edges of large-sized pores and in the centers of small pores is displaced out successively. Pore-throats of 20-50 μm in radius and pore-throats less than 20 μm in radius dominate the expansion of gas charging channels at different stages of charging in turn, leading to reductions in pore-throat radius, throat length and coordination number of the pathway, which is the main increase stage of gas permeability and gas saturation. Among which, pore-throats 30-50 μm in radius control the increase pattern of gas saturation. In the stable stage, gas charging pathways have expanded to the maximum, so the pathways keep stable in pore-throat radius, throat length, and coordination number, and irreducible water remains in the pore system, the gas phase is in concentrated clusters, while the water phase is in the form of dispersed thin film, and the gas saturation and gas permeability tend stable. Connected pore-throats less than 20 μm in radius control the expansion limit of the charging pathways, the formation of stable gas-water distribution, and the maximum gas saturation. ＜正＞The software for three-dimensional visualization of Chinese Continental Scientific Drilling （CCSD） well hole is studied and realized. The main function and the key technology of this software is introduced, especially the key technology data preprocessing, designing of the main data structure, computing the coordinate of each level of the well, graphic display, database supporting and so on. In the end, giving the results of CCSD well hole is based on the true data. The software is a general one that can display the view of any other wells with their own data.

Astrocytes,the multi-functional glial cells with the most abundant population in the brain,integrate information across their territories to regulate neuronal synaptic and cerebrovascular activities.Astrocytic calcium(Ca^(2+))signaling is the major readout of cellular functional state of astrocytes.The conventional two-photon in vivo imaging usually focuses on a single horizontal focal plane to capture the astrocytic Ca^(2+)signals,which leaves>80%spatial information undetected.To fully probe the Ca^(2+)activity across the whole astrocytic territory,we developed a pipeline for imaging and visualizing volumetric astrocytic Ca^(2+)time-lapse images.With the pipeline,we discovered a new signal distribution pattern from three-dimensional(3D)astrocytic Ca^(2+)imaging data of mice under isoflurane anesthetic states.The tools developed in this study enable a better understanding of the spatiotemporal patterns of astrocytic activity in 3D space.

Volumetric comparative analysis of anatomy through far-lateral approach:surgical space and exposed tissues

Background:The three-dimensional(3D)visualization model has ability to quantify the surgical anatomy of farlateral approach.This study was designed to disclose the relationship between surgical space and exposed tissues in the far-lateral approach by the volumetric analysis of 3D model.Methods:The 3D skull base models were constructed using MRI and CT data of 15 patients(30 sides)with trigeminal neuralgia.Surgical corridors of the far-lateral approach were simulated by triangular pyramids to represent two surgical spaces exposing bony and neurovascular tissues.Volumetric comparison of surgical anatomy was performed using pair t test.Results:The morphometric results were almost the same in the two surgical spaces except the vagus nerve(CN X)exposed only in one corridor,whereas the volumetric comparison represented the statistical significant differences of surgical space and bony and neurovascular tissues involved in the two corridors(P<0.001).The differences of bony and neurovascular tissues failed to equal the difference of surgical space.Conclusions:For far-lateral approach,the increase of exposure for the bony and neurovascular tissues is not necessarily matched with the increase of surgical space.The volumetric comparative analysis is helpful to provide more detailed anatomical information in the surgical design.

Topological approach of liver segmentation based on 3D visualization technology in surgical planning for split liver transplantation

BACKGROUND Split liver transplantation(SLT)is a complex procedure.The left-lateral and right tri-segment splits are the most common surgical approaches and are based on the Couinaud liver segmentation theory.Notably,the liver surface following right trisegment splits may exhibit different degrees of ischemic changes related to the destruction of the local portal vein blood flow topology.There is currently no consensus on preoperative evaluation and predictive strategy for hepatic segmental necrosis after SLT.AIM To investigate the application of the topological approach in liver segmentation based on 3D visualization technology in the surgical planning of SLT.METHODS Clinical data of 10 recipients and 5 donors who underwent SLT at Shenzhen Third People’s Hospital from January 2020 to January 2021 were retrospectively analyzed.Before surgery,all the donors were subjected to 3D modeling and evaluation.Based on the 3D-reconstructed models,the liver splitting procedure was simulated using the liver segmentation system described by Couinaud and a blood flow topology liver segmentation(BFTLS)method.In addition,the volume of the liver was also quantified.Statistical indexes mainly included the hepatic vasculature and expected volume of split grafts evaluated by 3D models,the actual liver volume,and the ischemia state of the hepatic segments during the actual surgery.RESULTS Among the 5 cases of split liver surgery,the liver was split into a left-lateral segment and right trisegment in 4 cases,while 1 case was split using the left and right half liver splitting.All operations were successfully implemented according to the preoperative plan.According to Couinaud liver segmentation system and BFTLS methods,the volume of the left lateral segment was 359.00±101.57 mL and 367.75±99.73 mL,respectively,while that measured during the actual surgery was 397.50±37.97 mL.The volume of segment IV(the portion of ischemic liver lobes)allocated to the right tri-segment was 136.31±86.10 mL,as determined using the topological approach to

Reconstructing the portal vein through a posterior pancreatic tunnel:New choice for portal vein thrombosis during liver transplantation

BACKGROUND Thrombectomy and anatomical anastomosis(TAA)has long been considered the optimal approach to portal vein thrombosis(PVT)in liver transplantation(LT).However,TAA and the current approach for non-physiological portal reconstructions are associated with a higher rate of complications and mortality in some cases.AIM To describe a new choice for reconstructing the portal vein through a posterior pancreatic tunnel(RPVPPT)to address cases of unresectable PVT.METHODS Between August 2019 and August 2021,245 adult LTs were performed.Forty-five(18.4%)patients were confirmed to have PVT before surgery,among which seven underwent PV reconstruction via the RPVPPT approach.We retrospectively analyzed the surgical procedure and postoperative complications of these seven recipients that underwent PV reconstruction due to PVT.RESULTS During the procedure,PVT was found in all the seven cases with significant adhesion to the vascular wall and could not be dissected.The portal vein proximal to the superior mesenteric vein was damaged in one case when attempting thrombolectomy,resulting in massive bleeding.LT was successfully performed in all patients with a mean duration of 585 min(range 491-756 min)and mean intraoperative blood loss of 800 mL(range 500-3000 mL).Postoperative complications consisted of chylous leakage(n=3),insufficient portal venous flow to the graft(n=1),intra-abdominal hemorrhage(n=1),pulmonary infection(n=1),and perioperative death(n=1).The remaining six patients survived at 12-17 mo follow-up.CONCLUSION The RPVPPT technique might be a safe and effective surgical procedure during LT for complex PVT.However,follow-up studies with large samples are still warranted due to the relatively small number of cases.
