Titanium mesh fusion device in the treatment of thoracolumbar burst fracture

thoracolumbar burst fracture, a common condition in clinic, often leads to severe spinal instability and neurologic deficit. In most cases, the compression that caused by backward protrusion of the fracture fragments to the spinal cord, makes complete decompression difficult through a posterior approach. Here, we reviewed the clinical records of 22 patients with thoracolumbar burst fracture treated by anterior corpectomy, decompression of the spinal cord, and implantation of titanium mesh cage.

KumaFix fixation for thoracolumbar burst fractures： a prospective study on selective consecutive patients

Background Short-segment U-shaped pedicle screw fixation has been widely used to treat thoracolumbar burst *** studies have reported the disadvantages of traditional U-shaped pedicle screw,which included a relatively high rate of adjacent segment degeneration and screw failure,including screw pullout and *** purpose of this study was to assess the efficacy of open reduction and fixation using KumaFix fixation system in treatment of thoracolumbar burst *** From June 2011 to June 2012,45 consecutive patients with thoracolumbar burst fractures were *** were randomly assigned to treatment with KumaFix （the treatment group,n=23） or traditional U-shaped pedicle screw （the control group,n=22）.The patients were followed up postoperatively and were assessed with regard to radiologic and clinical *** outcomes were assessed mainly on the basis of Cobb angle and vertebral wedge *** outcomes were evaluated mainly with use of Visual Analog Scale （VAS） for pain and the Oswestry Disability Index （ODI） *** All patients were followed up from 9 to 22 *** were no significant differences between the two groups with regard to preoperative *** operation time in the treatment group was significantly lower than that in the control *** Cobb angles and vertebral wedge angles in two groups were significantly decreased after surgery,and these have been well maintained at the last follow-up with mild correction *** results of clinical outcome showed lower VAS and ODI scores in two groups compared with those preoperative,and the treatment group had greater improvement on the ODI compared with the control group at the last *** Compared with traditional U-shaped pedicle screw,KumaFix fixation system can achieve gradual,controlled reduction,provide enough space for bone implantation,and avoid acceleration of adjacent segment *** is an effective

Background:The impact of percutaneous internal fixation as a supplement to percutaneous kyphoplasty(PKP) for the management of the thoracolumbar burst fractures of the aged patients is *** conducted a clinical controlled trial to define the effect of percutaneous internal fixation with PKP on thoracolumbar fractures and patientrelated functional outcomes. Method:This study included 43 patients with thoracolumbar burst fractures without nerve injuries who were elder than 65 years *** were randomly assigned to treatment with simple PKP(control group,n=22) and percutaneous short-segment pedicle screw internal fixation with PKP(treatment group,n=21).The patients were followed for at least two years postoperatively and were assessed with regard to clinical and radiological outcomes. Clinical outcomes were evaluated mainly with use of visual pain analog scale(VAS) and questionnaire score of Oswestry disability index(ODI).Radiological outcomes were assessed mainly on the basis of Cobb kyphosis angle and loss of kyphosis correction. Results:There was no significant difference between the two groups with regard to the preoperative indexes. The treatment group had better VAS values and had more postoperative improvements in ODI results as compared with the control group(P fractures in the injured vertebra and one patient had refracture in the adjacent *** operation complications need managements were noted postoperatively in both groups. Conclusions:Compared with simple PKP operation,percutaneous internal fixation with PKP is one kind of more ideal surgical technique for the treatment of selected elderly thoracolumbar burst fractures.

PURPOSE: Surgical treatment in the case of thoracolumbar fracture is very *** fixation is most frequently used compared to the anterior fixation;however, thenumber of levels to be instrumented still remains a matter of debate. We want to compare thesurgical outcomes of short segment and intermediate segment posterior fixation forthoracolumbar burst fracture in order to provide a theoretical basis for the better treatment ofthoracolumbar burst fracture.MATERIALS and METHODS: A total of36patients who had a single level Type A-3(AO classification) burst fracture between T11to L2were selected and were managedusing posterior instrumentation and fusion, no laminectomy or discectomy procedure wasdone. They were divided into two groups as follows;Group I (n=16) included patients whowere operated by intermediate segment posterior fixation (ISPF) that is two vertebrae aboveand one vertebrae below the fracture vertebrae or one vertebrae above and two vertebraebelow the fracture level, Group II (n=20) included patients who were operated by shortsegment posterior fixation (SSPF) that is one vertebrae above and one vertebrae below thefracture vertebrae. The mean follow up period was11months ranging from6months to15months. The outcomes were analyzed in terms of Cobb’s Angle (CA), Sagittal Index (SI)and Anterior Body Height (ABH).RESULT: Both short segment and intermediate segment posterior transpedicular fixationhave good outcomes in the treatment of thoracolumbar fractures in terms of correction ofCobb’s angle, Sagittal index and Anterior body height. A statistically significant differencebetween the two study groups was noted for the immediate post op of the Cobb’s angle(p=0.031), sagittal index (p=0.048) as well as on the Cobb’s angle (p=0.024) and anteriorbody height (p=0.045) on the final follow up.CONCLUSION: Both short segment and intermediate segment posterior transpedicularfixation are able for correcting the cobb angle, sagittal index, and
