BACKGROUND Few studies have compared the efficacy and safety profile of a tailored eradication(TR)strategy based on the presence of a 23S ribosomal RNA point mutation with those of empirical bismuth-based quadruple therapy(EBQT)for first-line eradication of Helicobacter pylori(***)in Korean *** To compare the efficacy and safety of a TR strategy and those of EBQT regimen as first-line eradication therapy for *** This is an open-label,comparative study in which we prospectively enrolled patients over 18 years of age with *** infection and retrospectively reviewed their ***-positive patients diagnosed by rapid urease test,Giemsa staining,or dual priming oligonucleotide polymerase chain reaction(DPO-PCR)were enrolled from May 2016 to September 2018 at Gil Medical *** with *** infection received either a TR regimen or the EBQT *** the tailored therapy group that underwent DPO-PCR testing,patients with A2142G and/or A2143G point mutations were treated with a bismuth-containing quadruple *** eradication rate,patient-reported side effect rate,and *** eradication success rate were evaluated and compared between the *** A total of 150 patients were assigned to the TR(n=50)or EBQT group(n=100).The first-line eradication rate of *** did not differ between the groups(96.0%vs 95.7%,P=0.9).The rate of eradication-related side effects for TR was 12.0%,which differed significantly from that of EBQT(43.0%)for first-line treatment(P<0.001).CONCLUSION DPO-PCR-based TR for *** eradication may be equally efficacious,with less treatment-related complications,compared to EBQT in Korea,where clarithromycin resistance is high.

Sleeve pattern design of men’ s tailored - suit is one of the most difficult problems of clothing pattern design. Based on the experimental studies of armhole girth, the difference of armhole width and crown height, crown angle, under sleeve curve’s diagonal line length, crown ease and its distribution rule, this paper mainly deals with the sleeve pattern design of tailored - suit for Chinese men. The influences of these factors are discussed and furthermore, some qualitative, quantifiable conclusions are drawn to design perfect pattern of men’ s tailored - suit sleeve.

The tailored pulse loading method (TPLM) has been systematically studied. The mechanism ofinitiation of multiple fractures around a borehole produced by pulse loading has been explained. Itis shown that the number of created cracks is mainly dependent on the rise time of the pulse loadingrather than the loading rate. The relation among crack number, rise time of pulse loading, diameterof borehole and physical and mechanical properties of the rock about borehole has been set up. Theanalysis and the relation formulac have been varified with the numerical modeling and a searies ofexperiments.

Postoperative adjuvant chemotherapy(ACT)confers superior gastric cancer(GC)survival in the Eastern ***,is the current standard of ACT already excessive,or is it still necessary to increase its intensity for specific subgroups?tailored ACT strategies for GC depend on gradual exploration by clinical trials in selected ***,understanding the implications of previous and current research can help us respond wisely and design effective,rational trials,save medical resources and make better decisions in clinical *** reviewing and analyzing studies on ACT for GC patients undergoing curative resection,we found that research strategies for conducting"addition""ACT for specific stages of the disease have achieved great progress in making ACT more tailored and personalized in consideration of pathology ***,trials indicate that"addition"ACT strategies for GC patient subgroups based on histological characteristics might be helpful to move toward a more specific tailored and personalized management *** ACT research focused on different node statuses should also be conducted according to the biological specificity of lymph node(LN)***,future trials designed to determine tailored treatment based on histological and biological characteristics for specific subgroups are urgently needed and conducted as the theme of the 2019 American Society of Clinical Oncology(ASCO):Caring for Every Patient,Learning from Every Patient.

Reflective Ladder Topology Network Based on White Light Fiber-Optic Mach-Zehnder Interferometer

In order to improve the multiplexing capability of the optical sensors based on the lower interferential optic fiber sensing technology and the white light fiber-optic Mach-Zehnder interferometer,reflective ladder topology network ( RLT) with tailored formula was proposed. The topology network consists of 6 rungs sensing elements linked by 5 couplers. Two cases with different choices of couplers were contrasted: one is equal coupling ratio,and the other is tailored coupling ratio. Through the simulation of these two cases,the detailed multiplexing capability was analyzed,and accordingly the experiments were also carried out. The simulation results showed that,the tailored formula enhances the multiplexing capability of the structure. In the first case, the maximum number of sensors which can be multiplexed is 8,and in the other case is 12 fiber optic sensors. The experimental results have a good agreement with numerical simulation results. Thus,it is considered expedient to incorporate RLT into large-scale building,grounds,bridges,dams,tunnels,highways and perimeter security.

NaBH4 sodalites were obtained by two new modified methods of crystallization: (1) autothermal synthesis and (2) crystallization with crossover from gel to melt flow in NaOH flux. Syntheses results were presented according to XRD, SEM and FTIR. Besides important features of both synthesis procedures product properties like crystal size and morphology were investigated. Spherical agglomerates of microcrystalline sodalite of composition Na7[AlSiO4]6BH4(H2O)2 were already observed after 4 h without any external heating by the autothermal procedure. Sodalites of the same average composition but in form of agglomerated nanoparticles are crystallized after very short times (2 h 30’) by the crossover reaction from gel to melt flow. Hydrogen release by heating was further studied for two selected samples with comparable composition from each synthesis procedure. Total hydrogen release by hydrolysis reaction with the internal cage water was found during heating of the autothermal product in synthetic air up to 550°C. In contrast hydrogen release from the nanocrystalline sample of crossover synthesis was not completed when heated under the same conditions. These differences were discussed in terms of crystal size and an earlier loss of the internal water from the nanocrystals of the crossover

Transient neonatal diabetes mellitus 1(TNDM1) is a rare genetic disorder representing with severe neonatal hyperglycaemia followed by remission within one and a half year and adolescent relapse with type 2 diabetes in half of the patients. Genetic defects in TNDM1 comprise uniparental isodisomy of chromosome 6, duplication of the minimal TNDM1 locus at 6q24, or relaxation of genomically imprinted ZAC1 /HYMAI. Whereas the function of HYMAI, a non-coding m RNA, is still unidentified, biochemical and molecular studies show that zinc finger protein 1 regulating apoptosis and cell cycle arrest(ZAC1) behaves as a factor with versatile transcriptional functions dependent on binding to specific GC-rich DNA motives and interconnected regulation of recruited coactivator activities. Genome-wide expression profiling enabled the isolation of a number of Zac1 target genes known to regulate different aspects of β-cell function and peripheral insulin sensitivity. Among these, upregulation of Pparγ and Tcf4 impairs insulinsecretion and β-cell proliferation. Similarly, Zac1-mediated upregulation of Socs3 may attenuate β-cell proliferation and survival by inhibition of growth factor signalling. Additionally, Zac1 directly represses Pac1 and Rasgrf1 with roles in insulin secretion and β-cell proliferation. Collectively, concerted dysregulation of these target genes could contribute to the onset and course of TNDM1. Interestingly, Zac1 overexpression in β-cells spares the effects of stimulatory G-protein signaling on insulin secretion and raises the prospect for tailored treatments in relapsed TNDM1 patients. Overall, these results suggest that progress on the molecular and cellular foundations of monogenetic forms of diabetes can advance personalized therapy in addition to deepening the understanding of insulin and glucose metabolism in general.
