Objectives: Medically unexplained symptoms (MUS) are common in all medical settings. These conditions remain controversial, aetiology remains poorly understood and treatments have been slow to develop. This study aimed to examine the beliefs held by psychologists and other professionals about MUS, which may impact upon clinical practice. Design: 375 clinical psychology trainees from 23UKtraining courses, 12 neurologists and 19 psychiatrists in training completed a weblink survey designed to elicit a range of beliefs about MUS cause and treatment. Results: All three groups viewed MUS as a common clinical problem. Use of terminology differed between groups. All three groups held a view that sexual abuse was a medium to high risk factor for developing MUS. Only a minority of psychologists and psychiatrists doubted that the human mind is capable of massive repression for past distressing events;and few psychologists, and no psychiatrists, doubted the traditional psychodynamic causal model of MUS. Neurologists were generally more skeptical. Only a minority of all three groups disagreed that hypnosis was a helpful way to uncover memories that people can not access. Around one third of each group believed that traumatic memories recovered in therapy were reliable. Dualistic thinking was prevalent among all three groups, but more so among psychiatrists. Conclusions: The data show that many professsionals hold beliefs about MUS for which, empirical support is lacking. These beliefs may impact on clinical practice. Whether such beliefs are deemed to be correct or incorrect, they should be acknowledged.

Worldwide, the leading cause of death is ischemic heart disease. Other than medical and surgical management, alternative therapy such as relaxing music has been identified as having an impact on reducing morbidity in ischemic heart disease. Although several studies have been conducted to find out the impact of music on pain, anxiety, heart rate and stress in myocardial ischaemia, literature on the long term impact of music on severity of symptoms associated with stable angina is very sparse. Therefore, the whole purpose of this study was to determine the long term effects of Indian music on severity of symptoms in patients with stable angina. Methodology: A single blind randomized clinical trial was conducted on 60 patients of 45 to 65 years of age with stable angina. Intervention group (n = 30) listened to a music based on Indian classical system at home twice a day complementary to their regular treatment for a period of one month. Control group (n = 30) was only on their usual treatment. Both groups were assessed prior and one month after the study period for severity of symptoms based on Canadian classification of angina guidelines and their treatment. Results: Severities of symptoms (timing of the chest pain, chest pain during walking and climbing a staircase, the effect of chest pain in day to day physical activities, frequency and the number of GTN used per week and frequency of consultation a doctor for chest pain) were significantly improved in the study group (p 0.05). Conclusion: Systematically, regular listening of music based on Indian classical system significantly improves the severity of the stable angina symptoms. Hence music has a potential benefit in considering for use as complementary to angina treatment in reducing morbidity.

After the age of 50, the prostate begins to increase in size. This is known as benign prostatic hypertrophy (BPH). Compression of urethra by enlarged prostate causes dribbling reduced force of the urinary stream, pain and occasional bleeding or infection. This causes difficulty in urinating and requires many men to get up several times during the night to urinate. The aim of our study was to evaluate an influence of new fermented whey based product (FWP) to several biochemical parameters and lower urinary tract symptoms (LUTS). Patients from the outpatient department of Andrology Center were randomized. This study involved the patients with minor to moderate LUTS, International Prostate symptoms Score (IPSS) range 3 - 19 but not prostatitis (NIH-Chronic Prostatitis Symptom Index (NIH-CPSI) 4 and 2-glass test negative for prostate inflammation and infection). Consumption of the FWP decreased the IPSS score in patients with moderate LUTS/BPH (p 0.001) whereas irritative and obstructive symptoms changed in parallel. There was a correlation between LUTS symptoms change and level of inflammation- and oxidative stress-related indices (blood high-sensitive C-reactive protein, hsCRP;glycated haemoglobin, HbA1c;oxidized low density lipoprotein, oxLDL;interleukine-10, IL-10 and 8-isoprostanes in the urine). Statistically significant changes in mentioned parameters occurred only in study group. Compression of urethra by enlarged prostate explains LUTS in BPH patients. Elevated oxidative stress (OxS) intensifies peroxidation of cell membrane phospholipids. This generates 8-isoprostanes (8-EPI), the prostaglandin-like compounds that can exaggerate LUTS. 8-isoprostanes may cause constriction of bladder and urethra in nanomolar concentrations. Consuming the whey-based product fermented by special lactobacilli strains may improve LUTS as well as OxS and diminish LUTS-related inflammatory response.

Alzheimer disease(AD) has now become the most common brain disorder among the older population. In addition, the currently existing therapeutics only offer temporary symptomatic relieves. Therefore, further research and development of more efficacious and disease-modifying agents for the prevention, treatment and restoration of AD will have tremendous value from both scientific, and economic standpoints. Over the past few years, our series of studies have identified several highly promising anti-AD dimeric leads, with disease-modifying potentials. In this presentation, the latest progress on the neuroprotective and disease modifying effects and the underlying mechanisms of those candidates will be comprehensively illustrated and discussed.

Background: Several recently published studies suggest that screening for symptoms could improve the early diagnosis of ovarian cancer. This report describes the development of a simple and reliable method of collecting symptom information in a primary care clinic. Methods: 1200 women, ages 40 - 87, completed several versions of a draft symptom index (SI) assessment form during their visits to a primary care clinic. Factors associated with a positive SI result were examined. Providers were surveyed about acceptability of the symptom screening procedures. Findings: Variation in the instructions provided to women influenced the rate at which women indicated having symptoms indicative of a positive SI, 5% had positive results when written instructions emphasized listing only current symptoms. Women coming to the clinic because of a current medical concern or problem did have higher rates of positive SI results, as did non-white women (p symptoms potentially indicative of ovarian cancer. Use of such a form for ovarian cancer screening purposes is acceptable to most women and providers in a primary care clinic setting.

Background: Maternal mortality in developing countries is unacceptably high with eclampsia being consistently among the top causes. As yet, primary prevention of this complication is not possible since causes of preeclampsia are largely unknown and bio-chemical, hematological and radiological markers have proved unsuitable for routine prediction of eclamptic fits. Although headache, visual disturbance, abdominal pain, nausea, and vomiting are routinely elicited when managing pre-eclampsia and have been reported to predict eclamptic fits, the literature attempting to characterize them is scanty. We sought to establish characteristics of the prodromal symptoms of eclampsia and compare them with similar symptoms as experienced by normotensive pregnant women at Muhimbili National Hospital (MNH) in Tanzania. Methods: This study was conducted at MNH in 2010 by enrolling 123 eclamptic and 123 normotensive women. Women in the two groups were interviewed about their experiences and characteristics of headache, visual disturbances, abdominal pain, nausea and vomiting using a semi structured questionnaire. The severity, nature and other characteristics of the symptoms were assessed using standard scale/methods and data compared among the two groups. Results: Prodromal symptoms of eclampsia were present in 90% of eclamptic women. Headache was more frequent among eclamptic women (88%) than the normotensive (43%), p < 0.001). The symptom was also more perceived as severe among eclamptic (46.3%) than the normotensive (5.7%), p < 0.001. The most frequent location for headache was frontal in 65.7% of eclamptic women compared to frontal (41.5%) or generalized (39.6%) for the normotensive. Likewise, visual problems were significantly more frequent among eclamptic women (39%) compared to the normotensive (3%), p < 0.001. Upper abdominal pain was significantly more reported by eclamptic (36%) than normotensive women (0.9%), p = 0.001. Background: Herbal dentifrices appear to have become an attractive alternative for some consumers and its use has gained appreciable acceptance in Nigeria. This could be partly due to the perception that herbal toothpastes like other herbal products are “natural”, devoid of chemicals and therefore superior to regular toothpastes. Aggressive advertisement of the products may also contribute to this new trend. Mucosal symptoms such as glossitis and intolerance to spicy foods seen in patients attending the oral diagnosis clinic are usually associated with anaemia and nutritional deficiency states. The disturbing trend of an increasing number of patients presenting with such oral mucosal symptoms associated with the use of herbal dentifrices prompted this study. This trend was observed in two isolated oral medicine clinics. Objective: To sensitize oral health, allied professionals and consumers on the possible adverse effects of regular use of some herbal dentifrices sold in Nigeria. Materials and Methods: Routine patients of two oral medicine clinics in Lagos, namely the Randle General Hospital and the Lagos University Teaching Hospital who complained of some adverse mucosal signs and symptoms following the regular use of two identified herbal toothpastes made up the study population. The study was conducted between April 2010-April 2011. A detailed history and examination was carried out on these patients and clinical photographs of oral signs observed was taken in consenting patients. Data analysis was done using the Epi-info 6 software. P values ≤ 0.05 was considered statistically significant and data was presented in table format. Results: A total of 45 patients, 9 (20%) males and 36 (80%) fe-males were seen. The age range of patients was 14 - 78 years;mean age 45.5 ± 14.9 years. The duration of use of herbal dentifrices ranged from 2 weeks - 84 months (mean 24 ± 11.5 months). Objective: To examine the efficacy of gabapentin for the treatment of behavioral and psychological symptoms of dementia (BPSD). Design: A retrospective chart review. Settings: Tertiary care geriatric psychiatry inpatient unit. Participants: 230 patients with BPSD. Measurements: The socio-demographic information, type of behaviors, co-morbid psychiatric and medical diagnoses, daily doses of medications and side-effects were recorded. Results: Of the 230 patients, 22 were treated with gabapentin. Twenty of these patients were on a combination of gabapentin and an antipsychotic medication while two patients were treated with gabapentin monotherapy. Eighteen of the 20 patients in the combination group tolerated the treatments with little or no side effects as did the two patients in the monotherapy group. Conclusions: Gabapentin may be a safe option for the treatment of BPSD in combination with antipsychotic medications. Gabapentin may also be effective as monotherapy in certain patients with BPSD.

Insomnia symptoms and Sleep Duration Are Associated with Impaired Cardiac Autonomic Modulation in Children

Limited population-based data from children are available to investigate the associations between insomnia and sleep duration in relationship to impaired cardiac autonomic modulation (CAM). Therefore, the objective of this study is to examine the cross-sectional associations between insomnia symptoms, sleep duration, and CAM in a population-based sample of children. All 616 children were randomly selected from Central Pennsylvania to participate in a one-night polysomnography (PSG) study in a sleep laboratory. The Pediatric Behavior Scale was used to assess parent-reported insomnia. Sleep duration was assessed objectively by PSG. CAM was measured by heart rate variability (HRV) analysis of 9-hour-beat-to-beat RR intervals. Multivariate linear regression models were used to assess the association between insomnia, sleep duration and HRV. The mean (SD) age was 9.2 (1.7) years, with 25.5% non-white and 48.9% male. After adjusting for age, race, gender, BMI percentile, %REM sleep, apnea-hypopnea-index, sleep latency, and sleep efficiency, the means(SE) of HRV indices were lower in children with insomnia than in children without: 6.56 (0.07) vs. 6.78 (0.04) m2 on logHF, respectively;and 6.47 (0.06) vs. 6.61 (0.03) m2 on logLF, respectively (all P symptoms. There is a similar pattern of association between shorter objective sleep duration and HRV. Insomnia symptoms and shorter objective sleep duration are associated with lower HRV and higher HR and LF/HF ratio, indicative of disturbance of CAM towards more sympathetic and lower parasympathetic modulation. These data provide supporting evidence of physiological activation associated with insomnia and short sleep duration even in very young children.
