Relationships between selfreflectiveness and clinical symptoms in individuals during pre-morbid and early clinical stages of psychosis

Background Self-reflectiveness,one dimension of cognitive insight,plays a protective role in an individual’s mental state.Both high and low levels of self-reflectiveness have been reported in patients with schizophrenia and individuals at clinical high risk for the illness.Aims This study aimed to explore the relationship patterns between self-reflectiveness and clinical symptoms in individuals during the pre-morbid and early clinical stages of psychosis.Methods A total of 181 subjects,including individuals with attenuated positive symptoms(APS,n=122)and patients with first-episode psychosis(FEP,n=59),completed the Beck Cognitive Insight Scale and were evaluated using the Schedule of Assessment of Insight and Positive and Negative Syndrome Scale.All subjects were classified into three groups according to their level of selfreflectiveness:low level(LSR,n=59),medium level(MSR,n=67)and high level(HSR,n=55).Both linear and nonlinear relationships between self-reflectiveness and clinical symptoms were explored.Results More individuals with APS were classified into the MSR group,while more patients with FEP were classified into the LSR group.The LSR group demonstrated less awareness of illness than the MSR and HSR groups,more stereotyped thinking and poorer impulse control but less anxiety than the MSR group,and lower levels of blunted affect and guilt feelings than the HSR group.The MSR group demonstrated lower stereotyped thinking than the HSR group.Compared to the LSR group,the MSR group had increased selfreflectiveness,improved awareness of illness,decreased stereotyped thinking,and better impulse control,but increased feelings of guilt.The HSR group showed increased stereotyped thinking when compared to the MSR group,but the other variables did not change significantly between these two groups.Overall,self-reflectiveness demonstrated an approximately inverse S-shaped relationship with the awareness of illness,a U-shaped relationship with stereotyped thinking and poor impulse control,and an almost linear relationsh

Role of depressive symptoms in cardiometabolic diseases and subsequent transitions to all-cause mortality:an application of multistate models in a prospective cohort study

Background and purpose The role of depression in the development and outcome of cardiometabolic diseases remains to be clarified.We aimed to examine the extent to which depressive symptoms affect the transitions from healthy to diabetes,stroke,heart disease and subsequent all-cause mortality in a middle-aged and elderly European population.Methods A total of 78212 individuals aged≥50 years from the Survey of Health Ageing and Retirement in Europe were included.Participants with any baseline cardiometabolic diseases including diabetes,stroke and heart disease were excluded.Depressive symptoms were measured by the Euro-Depression scale at baseline.Participants were followed up to determine the occurrence of cardiometabolic diseases and all-cause mortality.We used multistate models to estimate the transition-specific HRs and 95%CIs after adjustment of confounders.Results During 500711 person-years of follow-up,4742 participants developed diabetes,2173 had stroke,5487 developed heart disease and 7182 died.Depressive symptoms were significantly associated with transitions from healthy to diabetes(HR:1.12,95%CI:1.05 to 1.20),stroke(HR:1.31,95%CI:1.18 to 1.44),heart disease(HR:1.26,95%CI:1.18 to 1.34)and all-cause mortality(HR:1.41,95%CI:1.34 to 1.49).After cardiometabolic diseases,depressive symptoms were associated with the increased risk of all-cause mortality in patients with diabetes(HR:1.54,95%CI:1.25 to 1.89),patients who had stroke(HR:1.29,95%CI:1.03 to 1.61)and patients with heart disease(HR:1.21,95%CI:1.02 to 1.44).Conclusions Depressive symptoms increase the risk of diabetes,stroke and heart disease,and affect the risk of mortality after the onset of these cardiometabolic conditions.Screening and treatment of depressive symptoms may have profound implications for the prevention and prognosis of cardiometabolic diseases.

To the Editor:Alterations of circadian rhythms seem to be the casual contribution to sleep disturbances,depression,and other non-motor symptoms in Parkinson's disease(PD).[1,2]By restoring the circadian rhythm,bright light therapy(BLT)might be a potentially new treatment option for PD.However,no studies have conclusively demonstrated the effects of BLT on the non-motor symptoms in PD.

symptoms of dryness discomfort in soft contact lens wearers frequently lead to discontinuation from wear.The negative influence of pre-fitting tear dysfunctions appears likely to be exacerbated by the challenges to tear homeostasis caused by contact lenses.The corneal mechanisms for symptoms in contact lens wearers are different to those for dry eye disease because the cornea is insulated by the lens from ambient conditions as well as from lid wiper friction during blinking.symptoms of dryness discomfort might be the consequence of increased lid wiper friction during blinking when the lens front surface becomes soiled and dry and exhibits very rapid tear break up.It is possible that some cases of contact lens intolerance and discontinuation could be a function of lid wiper neuropathy.In relation to the possibility of corneal neuropathy,a stagnant post-lens tear pool with the possibility of increased concentrations of metabolic by-products,cellular debris,and bacterial exotoxins,might have the potential to disturb the corneal epithelial and sub-basal nerves.Contributions by contact lens-induced inflammation to any neuropathic changes may partly depend on the degree to which inflammatory mediators are concentrated in a stagnant post-lens tear pool.It does not appear to be known if corneal neuropathic changes could develop under these conditions.The chances of neuropathic involvement may be greater if discomfort develops after a significant period of successful wear and there is a history of comorbid pain conditions.Esthesiometry and in vivo confocal microscopy in discontinued contact lens wearers may support a diagnosis of contact lens-related corneal neuralgia.

Trajectories of mental health symptoms predict long-term quality of life among Wenchuan adolescent survivors:A 10-year follow-up study

Purpose:Previous studies usually examine the associations between psychological distresses and quality of life(QOL)with a variable-centred approach,while little is known about the effect of the individual variance in time-varying changes of psychological distresses on QOL.Therefore,this study aimed to examine whether individual variance in psychological distresses during the early phases post-earthquake would develop different QOL’s levels among adolescent survivors 10-year after the Wenchuan earthquake.Methods:Data were extracted from the Wenchuan Earthquake Adolescent Health Cohort Study.The current study included 744 adolescent survivors who effectively completed surveys at 6 months,24 months,and 10 years after the earthquake.Self-report questionnaires were administered to collect information on socio-demographic characteristics,earthquake exposure,life events,anxiety symptoms,depressive symptoms,posttraumatic stress symptoms(PTSS),and QOL.Data were analysed using hierarchical multiple regression.Results:Trajectories of psychological distresses were classified as follow:resistance(anxiety 40.73%;depression 54.70%;PTSS 74.46%),recovery(anxiety 17.20%;depression 9.27%;PTSS 10.35%),delayed dysfunction(anxiety 10.35%;depression 18.15%;PTSS 6.18%),and chronicity(anxiety 31.72%;depression 17.88%;PTSS 9.01%).After controlling covariates,hierarchical multiple regression only revealed that the anxiety trajectory with delayed dysfunction remained significantly predictive for four domains of QOL(physical health,psychological health,social relationships,and environment).Conclusion:The current study highlights the importance of focusing on the variations in trajectories of anxiety symptoms among disaster survivors and providing individualized mental health services to improve survivors’QOL.

To the Editor:Quantitative magnetic resonance imaging(MRI)objectively evaluates the degeneration of cartilage in knee osteoarthritis(KOA)patients by measuring changes in proteoglycans,collagen fibers,and water content,thereby facilitating early clinical diagnosis and treatment.[1]Western Ontario and MacMaster Universities(WOMAC)osteoarthritis index score is used to evaluate the symptoms of KOA patients.[2]Presently,few studies have evaluated the correlation between quantitative MRI findings and clinical symptoms(eg.,study performed by Zarins et al[3]).This study explored the relationship between quantitative MRI in T1rho and T2-mapping sequence and WOMAC osteoarthritis index scores of KOA patients.

Development and validation of a clinical nomogram predicting bladder outlet obstruction via routine clinical parameters in men with refractory nonneurogenic lower urinary tract symptoms

We aimed to develop and validate a clinical nomogram predicting bladder outlet obstruction(BOO)solely using routine clinical parameters in men with refractory nonneurogenic lower urinary tract symptoms(LUTS).A total of 750 eligible patient ≥50 years of age who had previously not responded(International Prostate Symptom Score[IPSS]improvement2 cmH20 ml^-1 s^-1.A stepwise multivariable logistic regression analysis was conducted to determine the predictors of BOO,and^-coefficients of the final model were selected to create a clinical nomogram.The final multivariable logistic regression model showed that age,IPSS,maximum urinary flow rate,postvoid residual volume,total prostate volume,and transitional zone index were significant for predicting BOO;these candidates were used to develop the final nomogram.The discrimination performance of the nomogram was 88.3%(95%Cl:82.7%-93.0%,P<0.001),and the nomogram was reasonably we 11-fitted to the ideal line of the calibration plot.Independe nt split-sample validation revealed 80.9%(95%Cl:75.5%-84.4%,P<0.001)accuracy.The proposed BOO nomogram based solely on routine clinical parameters was accurate and validated properly.This nomogram may be useful in determining further treatment,primarily focused on prostatic surgery for BOO,without impeding the detection of possible BOO in men with LUTS that is refractory to empirical medications.

Cimicifuga heracleifolia is therapeutically similar to black cohosh in relieving menopausal symptoms: evidence from pharmacological and metabolomics studies

In the market of botanical dietary supplements, Cimicifuga heracleifolia(CH) has always been considered as an adulterated species of Cimicifuga racemosa(CR), a conventional American herb with promising benefits to counteract troubles arising from the menopause. However, the detailed comparison of their therapeutic effects is lacking. In present study, the pharmacological and metabolomics studies were comparatively conducted between CH and CR in ovariectomized(OVX) female rats. Specifically, estrogen-like, anti-hyperlipidemia and anti-osteoporosis effects were evaluated through measuring serum biochemical parameters, histopathological examination and micro computed tomography(Micro-CT) scanning. At the same time, a gas chromatography-mass spectrometry(GC-MS)-based serum metabolomics method was employed to profile the metabolite compositional changes. As a result, both CR and CH displayed anti-osteoporosis and anti-hyperlipemia on menopause syndrome. Meanwhile, their potentials in improving the OVX-induced metabolic disorders were discovered. In conclusion, these results demonstrated that CH is therapeutically similar to CR in relieving menopausal symptoms and CH could be considered as a promising alternative to CR instead of an adulterant in the market of botanical dietary supplements.
