susceptibility-weighted imaging in Thrombolytic Therapy of Acute Ischemic Stroke

Objective： To provide a comprehensive and latest overview of susceptibility-weighted imaging （SWI） in the application of thrombolysis in acute ischemic stroke, and to update the decision-making effect and clinical value of SWI on identifying stroke patients suitable for thrombolytic therapy and possible benefits and risks followed. Data Sources： Literatures referred to this review were collected from PubMed, Medline, and EMBASE published till May 2017, using the search terms including susceptibility-weighted imaging, gradient-echo, T2＊, thrombolysis, recombinant tissue plasminogen activator （rt-PA）, thrombolytic therapy, and stroke. Study Selection： Papers in EngLish or with available English abstracts were considered, with no limitation of study design. References were also identified from the bibliographies of identified articles and the authors＇ files. Results： SW1 is of guiding significance for thrombolytic therapy in stroke patients, it can predict the location and length of thrombus and ischemic penumbra. It is worthy of noting that susceptibility vessel sign （SVS） on SWI can be used to predict recanalization after thrombolytic therapy and whether it is better to implement endovascular thrombolectonqy in combination or alone. SW1 is sensitive in detecting cerebral microbleed （CMB）, and CMB might not be a contraindication for thrombolytic therapy, yet CMBs in multiple loci could possibly be related to intracranial hemorrhage （ICH） after thrombolysis. SVS and CMB on SWI sequence are of instructive value in performing antiplatelet therapy after thrombolytic therapy. Cerebral venous change on SWI is related to lower recanalization rate and poor outcome after thrombolysis. Conclusions： It seems that SWI can be applied to guide individualized thrombolytic therapies and assist clinicians in making better decisions by weighing benefits and risks. However, there still exist controversies about the relationship between signs on SWI and thrombolytic therapy.

Utility of susceptibility-weighted imaging in Parkinson’s disease and atypical Parkinsonian disorders

In the clinic,the diagnosis of Parkinson’s disease(PD)largely depends on clinicians’*** the diagnosis is made,approximately 80%of dopaminergic cells in the substantia nigra(SN)have been ***,it is rather challenging to differentiate PD from atypical parkinsonian disorders(APD).Clinially-available 3T conventional MRI contributes little to solve these *** pathologic alterations of parkinsonism show abnormal brain iron deposition,and therefore susceptibility-weighted imaging(SWI),which is sensitive to iron concentration,has been applied to find iron-related lesions for the diagnosis and differentiation of PD in recent *** now,the majority of research has revealed that in SWI the signal intensity changes in deep brain nuclei,such as the SN,the putamen(PUT),the globus pallidus(GP),the thalamus(TH),the red nucleus(RN)and the caudate nucleus(CN),thereby raising the possibility of early diagnosis and ***,the signal changes in SN,PUT and TH sub-regions may settle the issues with higher *** this article,we review the brain iron deposition of PD,MSA-P and PSP in SWI in the hope of exhibiting a profile of SWI features in PD,MSA and PSP and its clinical values.

Recurrent Transient Ischemic Attacks Revealing Cerebral Amyloid Angiopathy: A Comprehensive Case

This case report investigates the manifestation of cerebral amyloid angiopathy (CAA) through recurrent Transient Ischemic Attacks (TIAs) in an 82-year-old patient. Despite initial diagnostic complexities, cerebral angiography-MRI revealed features indicative of CAA. Symptomatic treatment resulted in improvement, but the patient later developed a fatal hematoma. The discussion navigates the intricate therapeutic landscape of repetitive TIAs in the elderly with cardiovascular risk factors, emphasizing the pivotal role of cerebral MRI and meticulous bleeding risk management. The conclusion stresses the importance of incorporating SWI sequences, specifically when suspecting a cardioembolic TIA, as a diagnostic measure to explore and exclude CAA in the differential diagnosis. This case report provides valuable insights into these challenges, highlighting the need to consider CAA in relevant cases.

Determination of brain iron content in patients with Parkinson’ s disease using magnetic susceptibility imaging

Objective To compare the phase radians in several cerebral regions between patients with Parkinson’s disease （PD） and control subjects, and to evaluate whether iron deposition quantified by susceptibility-weighted imaging （SWI） is related to the severity of motor symptoms of PD. Methods SWI consisted of both magnitude and phase images from a fully flow-compensated, 3-dimensional and gradient-echo （GRE） sequence. Magnitude and phase data were collected at GE HD 1.5T. The regions evaluated included frontal white matter, grey matter, cerebrospinal fluid, putamen, caudate nucleus （CN）, sub- stantia nigra pars compacta （SNc）, substantia nigra pars reticulata （SNr）, and red nucleus （RN）. A total number of 42 patients （12 patients without cognitive dysfunction, and 30 with cognitive dysfunction from mild to moderate degrees） and 30 control subjects were employed in the present study. Results The phase radians of SNc, CN and RN in PD patients were lower than those in control subjects （P〈0.05）. Conclusion The phase radians can be used to estimate the brain iron deposition in PD patients, which may be helpful in the diagnosis and longitudinal monitoring of PD.

Detecting prostate cancer and prostatic calcifications using advanced magnetic resonance imaging

Prostate cancer and prostatic calcifications have a high incidence in elderly men. We aimed to investigate the diagnostic capabilities of susceptibility-weighted imaging in detecting prostate cancer and prostatic calcifications. A total number of 156 men, including 34 with prostate cancer and 122 with benign prostate were enrolled in this study. Computed tomography, conventional magnetic resonance imaging, diffusion-weighted imaging, and susceptibility-weighted imaging were performed on all the patients. One hundred and twelve prostatic calcifications were detected in 87 patients. The sensitivities and specificities of the conventional magnetic resonance imaging, apparent diffusion coefficient, and susceptibility-filtered phase images in detecting prostate cancer and prostatic calcifications were calculated. McNemar＇s Chi-square test was used to compare the differences in sensitivities and specificities between the techniques. The results showed that the sensitivity and specificity of susceptibility-filtered phase images in detecting prostatic cancer were greater than that of conventional magnetic resonance imaging and apparent diffusion coefficient （P 〈 0.05）. In addition, the sensitivity and specificity of susceptibility-filtered phase images in detecting prostatic calcifications were comparable to that of computed tomography and greater than that of conventional magnetic resonance imaging and apparent diffusion coefficient （P 〈 0.05）. Given the high incidence of susceptibility-weighted imaging （SWl） abnormality in prostate cancer, we conclude that susceptibility-weighted imaging is more sensitive and specific than conventional magnetic resonance imaging, diffusion-weighted imaging, and computed tomography in detecting prostate cancer. Furthermore, susceptibility-weighted imaging can identify prostatic calcifications similar to computed tomography, and it is much better than conventional magnetic resonance imaging and diffusion-weighted imaging.

Comparison of Diagnostic Effects of T2-weighted imaging,DWI,SWI,and DTI in Acute Cerebral Infarction

Objective:To achieve precision medicine,the use of imaging methods to help the clinical detection of cerebral infarction is conducive to the clinical development of a treatment plan and increase of the cure rate and improvement of the prognosis of ***:In this work,T2-weighted imaging(T2WI),diffusion-weighted imaging(DWI),susceptibility-weighted imaging(SWI),and diffusion tensor imaging(DTI)examinations were performed on 34 patients with clinically diagnosed cerebral infarction to measure the difference in signal intensity between the lesion and its mirror area and make a comparative analysis by means of the Student-Newman-Keuls ***:The detection rate of T2WI was 79%(27/34),the detection rate of DWI was 97%(33/34),the detection rate of SWI was 88%(30/34),and the detection rate of DTI was 94%(32/34).Conclusion:The imaging performance was in the order DWI>DTI>SWI>T2WI for the diagnosis of cerebral infarction,and combined imaging is better than single imaging.

Dural arteriovenous fistulas(DAVFs)include a wide range of pathological conditions that are associated with intracranial vessel *** some types of DAVFs present with typical neuroimaging characteristics,others share overlapping pathological and neuroimaging features that can hinder accurate ***,misclassification of the various types of DAVFs is *** knowledge of DAVF imaging findings is essential to avoid such *** digital subtraction angiography(DSA)is considered the gold standard for diagnosing and evaluating ***,angiography cannot detect changes in a patient’s brain *** magnetic resonance imaging(MRI)sequences,including MR angiography(MRA),allow the evaluation of DAVFs without ionizing radiation or *** MRI techniques,such as susceptibility-weighted imaging(SWI)and dynamic contrast-enhanced MRA,provide added value to real-time physio-pathological data regarding the hemodynamics of *** these techniques,new insights using high-resolution vascular wall MRI are incorporated for the noninvasive evaluation of *** article reviews the pathophysiology of DAVFs,focusing on the specifics of MRI findings that facilitate their *** role of conventional and advanced MRI sequences for DAVFs was assessed using insights derived from the data provided by structured reports of multimodal MRIs to evaluate DAVFs.
