Pharmacokinetics of sufentanil administered by target-controlled infusion in Chinese surgical patients

Background Target-controlled infusion （TCI） has been recently developed and successfully implemented in clinical practice. This study was conducted to determine the pharmacokinetics of TCI administered sufentanil in Chinese surgical patients. Methods The pharmacokinetics of sufentanil was investigated in 12 adult patients, aged 23-76 years, scheduled for prolonged surgery under general anesthesia. Anesthetic induction was carried out with propofol, rocuronium and TCI administered sufentanil aiming for target effect-site concentration of sufentanil 4 or 6 ng/ml. sufentanil TCI lasted for 30 minutes. Frequent arterial blood samples （1.5 ml） were drawn during and up to 24 hours after sufentanil TCI. Plasma sufentanil concentrations were measured by liquid chromatography-tandem mass spectrometry; limit of sensitivity of mass spectrometry was 5 pg/ml. The data were analyzed with the nonlinear mixed-effect model program. Results The pharmacokinetics of TCI administered sufentanil were optimally described by a three-compartment model with the following parameters： the central volume of distribution （V1） = 5.4 L, the volume of distribution at steady-state （Vdss） = 195.4 L, systemic clearance （CI1） = 1.10 L/min, and elimination half-life （t1/2 Y） = 271.8 minutes. Both age and gender affected the pharmacokinetic parameters. The rapid distribution clearance （012） was negatively correlated with patient age, and the volume of slowly equilibrating compartment （V3） was positively correlated with age. The Cl2 and the volume of rapidly equilibrating compartment （V2） were influenced by gender with male patients showing higher values of Cl2 and V2 than female patients. There was no relationship of body weight, lean body mass, plasma albumin, or target effect-site concentration of sufentanil with any of the pharmacokinetic parameters studied. Objective:To investigate the effects of propofol combined wit h s u f e n t a n i l a n d dexmedetomidine in cosmetic ***:The clinical data of 40 plastic surgery patients admitted to the hospital from June to November 2019 were retrospectively *** to the different anesthesia methods during surgery,they were divided into control group(propofol combined with sufentanil and normal saline,20 cases)and was compared with the observation group(propofol combined with sufentanil and dexmedetomidine,20 cases).The anesthetic effect,total dosage of propofol,spontaneous breathing recovery time,and adverse reactions were compared between the two ***:The total dosage of propofol in the observation group was less than that in the control group,and the spontaneous breathing recovery time was shorter than that in the control *** difference was statistically significant(P0.05).Conclusion:The application of propofol in combination with sufentanil and dexmedetomidine in cosmetic anesthesia can reduce the dosage of propofol,speed up the anesthesia recovery,and have better anesthetic effect and safety.

Effects of dexmedetomidine and sufentanil on hemorheology,stress response and inflammatory factors in patients undergoing laparoscopic repair of gastrointestinal perforation

Objective: To observe the clinical application of dexmedetomidine combined with sufentanil anesthesia in patients undergoing laparoscopic repair of gastrointestinal perforation, and the changes of hemodynamics, stress response and inflammatory factors were analyzed before and after operation. Methods: A total of 102 patients with laparoscopic gastrointestinal perforation were randomly divided into control group (n=51) and observation group (n=51) according to the lottery method. The control group was given anesthesia with sufentanil, and the observation group was given dexmedetomidine combined with sufentanil anesthesia, and the other anesthesia drugs were the same. The hemodynamics before anesthesia induction (T1), 10 min after medication (T2), pneumoperitoneum (T3), and tube drawing (T4), serum stress response and inflammatory factor levels before operation and after 24 h of operation were measured in all subjects. Results: At T1 and T2, there was no significant difference in hemodynamics between the two groups. At T3and T4, HR, SBP, and DBP in observation group were significantly lower than those in the control group at corresponding period, and compared with the observation group at T, there were no significant difference. At T3, and T4, the HR, SBP and DBP of the control group were significantly higher than those of T1. There was no significant difference in serum stress hormone between the two groups before surgery. After surgery 24 h, two groups of serum ACTH, Cor and ALD levels were higher than those of preoperative, and the observation group was significantly lower than those of the control group. There was no significant difference in serum inflammatory cytokines between the two groups before surgery. After surgery 24 h, the levels of serum IL-10 and TNF-毩 in the two groups were higher than those before operation, and the observation group was significantly lower than those of the control group. Effects of Ropivacaine-sufentanil Epidural Analgesia on Labor and Maternal and Neonatal Outcomes

[Objectives]This study aimed to investigate the effects of ropivacaine-sufentanil epidural analgesia on labor and maternal and neonatal outcomes.[Methods]A total of 180 primiparas in full-term pregnancy were *** were randomly divided into treatment group(n=90)and control group(n=90).The primiparas in the treatment group were injected epidurally with ropivacaine and sufentanil for analgesia,and the primiparas in the control group were subjected to vaginal *** VAS scores at 5,10,30 and 60 min of analgesia were *** vaginal bleeding amount,total labor duration,neonatal Apgar score and vaginal delivery rate of the two groups were compared.[Results]Compared with the control group,the VAS score in the treatment group differed insignificantly after 5 min of analgesia(P>0.05),and decreased significantly after 10,30 and 60 min of analgesia(P0.05).In the treatment group,the vaginal delivery rate increased(P0.05).[Conclusions]Epidural analgesia with ropivacaine and sufentanil has a good analgesic effect and good safety,and is worthy of clinical promotion.

Effects of sufentanil/fentanyl combined with propofol on the anesthesia of minimally invasive drainage for hypertensive cerebral hemorrhage

Objective: To compare the effects of sufentanil/fentanyl combined with propofol on the anesthesia of minimally invasive drainage for hypertensive cerebral hemorrhage. Methods:Patients with hypertensive cerebral hemorrhage who received minimally invasive drainage in Huanggang Cerebrovascular Hospital between June 2014 and February 2017 were selected and randomly divided into two groups, observation group received sufentanil combined with propofol anesthesia, and control group received fentanyl combined with propofol anesthesia. The serum contents of nerve injury, stress response, oxidation reaction and inflammatory response markers were measured during surgery and 12 h after surgery. Results: During surgery and 12 h after surgery, serum TF, NSE, GFAP, GLU, NE, E, ACTH, Cor, Ins, MDA, AOPP, 8-OHdG, NO, ICAM-1, TNF-α, IL-6, IL-17 and IL-23 levels of observation group were significantly lower than those of control group. Conclusion: sufentanil combined with propofol for minimally invasive drainage of hypertensive cerebral hemorrhage is more effective than fentanyl combined with propofol to reduce the brain damage and inhibit the inflammatory response, stress response and oxidation reaction.

分析小剂量罗哌卡因复合sufentanil麻醉措施实施后,剖宫产蛛网膜下腔麻醉效果,评估该措施的实际应用价值。方法:2020年1月-2020年12月,选择我院120例剖宫产产妇,所有入选者均给予随机分组法(两组:各60例)。组别设置:研究组、对照组,上述组别分别给予小剂量罗哌卡因复合sufentanil麻醉、罗哌卡因药物麻醉。对比上述措施的临床效果。结果:与研究组相比,对照组不良反应发生率较高,Psufentanil麻醉安全性及可行性较高,可以显著提升麻醉效果,对不良反应抑制具有积极意义。
