squamous cell carcinoma(SCC)of the scalp is the second most prevalent skin cancer,following basal cell carcinoma.Notably,it has the capability to infiltrate the skull,dura mater,and even brain tissue.The cornerstone of treatment is the surgical removal of the lesion,with a particular focus on the depth of invasion,which is directly correlated with recurrence rates.Post-surgical strategies may involve immediate or delayed cranial bone reconstruction and repair of scalp defects using either artificial dermis or skin grafts.In the case presented,a substantial defect necessitated more than a single flap for primary repair.Hence,a single pedicle double-island flap was designed for reconstructing the occipital area.Due to increased tension on the flap following cranial bone repair,the bone repair was temporarily deferred.Postoperative care included adjuvant chemotherapy and radiotherapy to mitigate the risk of SCC recurrence.

BACKGROUND Endometriosis is a common benign gynecological disease that causes dysmenorrhea in women of childbearing age.Malignant tumors derived from endometriosis are rarely reported and are found in only 1%of all patients with endometriosis.Here,we report a well-differentiated squamous cell carcinoma(SCC)caused by squamous metaplasia of endometriosis that co-occurred in the uterus and ovaries.CASE SUMMARY A 57-year-old postmenopausal woman had a 6-month history of irregular uterine bleeding.The uterus and adnexa were examined by computed tomography,and there were two solid cystic masses in the pelvis and right adnexa.Histological findings of surgical specimens showed well-differentiated SCC arising from squamous metaplasia of ectopic endometrial glands in the uterus and ovaries.The patient received chemotherapy after surgery and was followed up for 3 mo without metastasis.CONCLUSION The continuity between ectopic endometrial glands and SCC supports that SCC originates from ectopic endometrial glands with metaplasia towards squamous epithelium.

BACKGROUND Adenocarcinoma has the highest incidence among malignant tumors of the small intestine(SI).squamous cell carcinoma(SCC)often occurs in organs covered with squamous epithelium.Primary or metastatic SCC originating from the SI is very rare,with very few cases reported in the literature.CASE SUMMARY This case report involves a 69-year-old man who developed abdominal pain after lunch.After admission,an abdominal computed tomography scan revealed perforation of the alimentary canal and multiple abnormal low-density lesions in the liver.During laparotomy,an approximately 4 cm×3 cm-sized solid tumor was found in the jejunum,located 30 cm from the Treitz ligament,with a perforation.An intestinal segment of approximately 15 cm was removed,including the perforated portion.The pathological result was SCC.In combination with liver imaging,a diagnosis of SI SCC with multiple liver metastases was considered.The patient died from hepatic failure 1 mo after the operation.CONCLUSION SI tumors are very rare compared to those originating in other digestive organs.Due to its insidious onset,the diagnosis of this disease is usually delayed.Clinicians must pay close attention to digestive symptoms such as persistent abdominal pain and melena.

BACKGROUND Cancer detection is a global research focus,and novel,rapid,and label-free techniques are being developed for routine clinical practice.This has led to the development of new tools and techniques from the bench side to routine clinical practice.In this study,we present a method that uses Raman spectroscopy(RS)to detect cancer in unstained formalin-fixed,resected specimens of the esophagus and stomach.Our method can record a clear Raman-scattered light spectrum in these specimens,confirming that the Raman-scattered light spectrum changes because of the histological differences in the mucosal tissue.AIM To evaluate the use of Raman-scattered light spectrum for detecting endoscopically resected specimens of esophageal squamous cell carcinoma(SCC)and gastric adenocarcinoma(AC).METHODS We created a Raman device that is suitable for observing living tissues,and attempted to acquire Raman-scattered light spectra in endoscopically resected specimens of six esophageal tissues and 12 gastric tissues.We evaluated formalin-fixed tissues using this technique and captured shifts at multiple locations based on feasibility,ranging from six to 19 locations 200 microns apart in the vertical and horizontal directions.Furthermore,a correlation between the obtained Raman scattered light spectra and histopathological diagnosis was performed.RESULTS We successfully obtained Raman scattered light spectra from all six esophageal and 12 gastric specimens.After data capture,the tissue specimens were sent for histopathological analysis for further processing because RS is a label-free methodology that does not cause tissue destruction or alterations.Based on data analysis of molecular-level substrates,we established cut-off values for the diagnosis of esophageal SCC and gastric AC.By analyzing specific Raman shifts,we developed an algorithm to identify the range of esophageal SCC and gastric AC with an accuracy close to that of histopathological diagnoses.CONCLUSION Our technique provides qualitative information for real-time mor

BACKGROUND The biological behavior of carcinoma of the esophagogastric junction(CEGJ)is different from that of gastric or esophageal cancer.Differentiating squamous cell carcinoma of the esophagogastric junction(SCCEG)from adenocarcinoma of the esophagogastric junction(AEG)can indicate Siewert stage and whether the surgical route for patients with CEGJ is transthoracic or transabdominal,as well as aid in determining the extent of lymph node dissection.With the development of neoadjuvant therapy,preoperative determination of pathological type can help in the selection of neoadjuvant radiotherapy and chemotherapy regimens.AIM To establish and evaluate computed tomography(CT)-based multiscale and multiphase radiomics models to distinguish SCCEG and AEG preoperatively.METHODS We retrospectively analyzed the preoperative contrasted-enhanced CT imaging data of single-center patients with pathologically confirmed SCCEG(n=130)and AEG(n=130).The data were divided into either a training(n=182)or a test group(n=78)at a ratio of 7:3.A total of 1409 radiomics features were separately extracted from two dimensional(2D)or three dimensional(3D)regions of interest in arterial and venous phases.Intra-/inter-observer consistency analysis,correlation analysis,univariate analysis,least absolute shrinkage and selection operator regression,and backward stepwise logical regression were applied for feature selection.Totally,six logistic regression models were established based on 2D and 3D multi-phase features.The receiver operating characteristic curve analysis,the continuous net reclassification improvement(NRI),and the integrated discrimination improvement(IDI)were used for assessing model discrimination performance.Calibration and decision curves were used to assess the calibration and clinical usefulness of the model,respectively.RESULTS The 2D-venous model(5 features,AUC:0.849)performed better than 2D-arterial(5 features,AUC:0.808).The 2D-arterial-venous combined model could further enhance the performance(AUC:0.869).The 3D-venous

Background:Cutaneous squamous cell carcinoma (CSCC) on the auricle is believed to carry a higher risk of metastatic spread.The rates of lymphatic metastasis reported in the literature have varied widely.There are no established prognostic criteria to determine which of these tumors are higher risk and warrant prophylactic treatment of the associated lymphatic basins.Objective:To retrospectively evaluate outcomes after surgical treatment of auricular CSCC in our department,examining excision completeness,tumor recurrence,and lymphatic metastasis.Secondarily,to identify factors associated with lymphatic metastasis.Methods:One hundred and thirty‐eight consecutive cases of auricular SCC were excised from 126 patients in our department over a 7‐year period (January 2012–December 2018).Data were retrospectively collected on patient characteristics,tumor histology,surgical procedures,and follow‐up.Results:Incomplete initial excision occurred in 17 cases (12.32%).Six patients (4.76%) had a local recurrence.Lymphatic metastasis occurred in eight patients (6.35%),on average within 10.25 months after primary excision.Six patients with metastasis died during follow‐up,with a mean survival of 10.2 months.Older age was associated with lymphatic metastasis (P = 0.0267).Other factors,including tumor recurrence,size,grade,cartilage invasion,and positive margins,were evaluated and not significantly associated with metastasis.Conclusion:In this study,the metastatic rate of auricular SCC was 6.35%,which is within the previously reported ranges.No histological prognostic factors were identified in this study,which may be due to our limited sample size.In the absence of established prognostic criteria,decisions regarding prophylactic treatment should be made on an individual basis with multidisciplinary support.

BACKGROUND Esophageal cancer is a common cause of cancer-related death worldwide.Cutaneous metastasis of esophageal squamous cell carcinoma is rare,particularly in diffuse skin metastasis.CASE SUMMARY In this case report,we describe an 82-year-old male who was diagnosed with esophageal squamous cell carcinoma.The tumor was staged as T4N3M1(Stage IVB).The pathological findings revealed poorly differentiated squamous cell carcinoma of the esophagus.Four months after diagnosis,the patient began chemotherapy,and symptoms were relieved after four cycles of chemotherapy.After that,the patient returned home without a systematic physical examination.One year after diagnosis,the patient realized that the skin of the abdominal wall was hard and rough without pain,and the color became darker than normal skin.Thirteen months after diagnosis,a biopsy of the patient’s abdominal lesion revealed that the skin metastasis was derived from the esophagus.Then the patient received two cycles of apatinib combined with docetaxel,but the abdominal lesion worsened.Two cycles of nivolumab were administered,but the patient eventually died of multiple organ failure.CONCLUSION This report highlights cutaneous metastasis as a late and untreatable metastasis of esophageal cancer.

BACKGROUND squamous cell carcinoma(SCC)of the liver is rare,and is more commonly found in the skin,rectum,cervical or inguinal lymph nodes.CASE SUMMARY A 73-year-old man had been experiencing right upper quadrant discomfort for some weeks.He had a 50-year history of smoking and drinking.On average,he smoked 20 cigarettes and consumed 200 galcoholdaily.He didn’t have a history of hepatitis or surgery.Fever,vomiting,jaundice,dysuria,chills,and abdominal distention were not observed at the time of admission.Tenderness in the right upper quadrant was found on physical examination,but there was no palpable abdominal mass.No obvious abnormalities in laboratory tests and tumor markers were found.The plasma retention rate of indocyanine green(ICG)at 15 min was 1.35%.Subsequent abdominal ultrasonography showed a mixed echoic mass approximately 3.8 cm diameter in the left caudate lobe of the liver.Abdominal computed tomography confirmed a 3.0 cm×3.5 cm irregular mass with inhomogeneous density and moderate delayed enhancement in the left caudate lobe of the liver.Laparoscopic left caudate lobectomy was performed to remove the liver mass.Intra-operative findings confirmed a non-cirrhotic liver,with a 3 cm×3.5 cm white tumor mass in the left caudate lobe with no tumor rupture and no hemoperitoneum.The resection margin was 1.0 cm in width.CONCLUSION We describe the first case of SCC in the left caudate lobe of the liver,which was successfully treated by surgical resection and postoperative immunotherapy.No tumor recurrence was observed during the 8-mo follow-up.

BACKGROUND Primary squamous cell carcinoma(SCC)with sarcomatoid differentiation of the kidney was rarely reported.This disease is usually related to renal stones,and due to a lack of symptoms and radiological features,patients usually attend the hospital with late stage disease.CASE SUMMARY A 54-years-old female presented with left flank pain and an abdominal mass for 6 mo.Imaging studies revealed that the left kidney was enlarged and massive hydronephrosis was present.A stone was seen in the ureteropelvic junction.The patient subsequently underwent left radical nephrectomy,and histopathological examination of the mass revealed a poorly differentiated renal SCC with sarcomatoid differentiation.After primary surgery,the patient received four cycles of tirelizumab.Four months later,the patient developed adrenal,lymph,and uterine appendage metastases.CONCLUSION SCC of the kidney has a poor prognosis,and should be considered in patients with a renal mass,long-standing urinary calculi and massive hydronephrosis.

BACKGROUND Pulmonary Langerhans cell histiocytosis(PLCH)is a rare cystic lung disease usually affecting young adults.It is predicted that PLCH is a lung tumor precursor associated with dysfunction of the myeloid dendritic cells in the lung.CASE SUMMARY A 70-year-old male patient presented with chronic cough and sputum.He had symptoms for 5 years and described shortness of breath on exertion for the previous 3 years.He had a 60 packs/year smoking history.Computerized tomography of the thorax revealed an 11-mm nodule in the right lung lower lobe superior segment and a 7-mm nodule in the right lung lower lobe poster basal segment.Those two nodules were resected by means of right thoracoscopic surgery.Pathological evaluation revealed a squamous cell carcinoma and PLCH.CONCLUSION Coexistent squamous cell carcinoma and PLCH suggest possible association between PLCH and lung cancer.
