BACKGROUND spontaneous gastric hematoma is an exceedingly rare condition characterized by the accumulation of blood within the gastric wall without any apparent iatrogenic or traumatic cause.Coagulopathies are the most frequent cause of gastric he-matomas.However,other causes include amyloidosis,pancreatitis,visceral vas-cular aneurysms,endoscopy complications and others.The pathophysiology of spontaneous gastric hematoma is not completely understood.However,it is postulated that it is caused by disruption of submucosal vessels that leads to dissection of the muscularis layer and formation of false lumen.The rarity of this condition increases the challenge of diagnosis,and there is no standard treatment protocol.CASE SUMMARY We present the case of a spontaneous gastric hematoma in a 22-year-old male.He presented to our emergency department complaining of pain in the left flank area lasting for 2 wk.There was no history of trauma,antico-agulant medications or endoscopy procedures.His hemoglobin and hematocrit levels were slightly lower than normal.Multi-slice computed tomography,ultrasound and endoscopy confirmed a gastric intramural hematoma.We recommended conservative treatment because there was no hemodynamic instability nor significant bleeding.The patient responded well,and there were no unexpected events.At the 3-mo follow-up,the ultrasound exa-mination revealed complete regression of the hematoma.CONCLUSION After reviewing the literature and our experience,we recommend that more of these cases should be treated conservatively.The tendency to treat these cases with potentially burdensome procedures such as total or subtotal gastrectomy should be significantly reduced.

To editor:spontaneous spinal epidural hematoma(SSEH)is uncom-mon,especially during pregnancy.However,SSEH is a seriousemergency that must be managed immediately to prevent per-manent neurological deficit of the mother and stillbirth.Theetiology and pathogenesis of SEEH remain unknown,al-though multiple theories and precipitating factors are de-scribed in the literature.In this report,we describe the caseof a pregnant female who developed SSEH.

spontaneous combustion of coals and coal-shales

spontaneous combustion of coal is a well-known phenomena around the globe. Apart from the coal itself,burning coal-shales is becoming a problem in the South African coal mines. Serious incidents of spontaneous combustion have been reported as a result of self-heating of reactive coal-shales. The intrinsic properties and spontaneous combustion tests of 28 selected coal and coal-shale samples were conducted and a relationship between the two has been established. Intrinsic properties were obtained by using the proximate and ultimate analysis, and spontaneous combustion liability tests results were obtained by using the Wits-Ehac and Wits-CT indices. The experimental results show that intrinsic properties of these materials complement to the spontaneous combustion liability tests results. Comparative analyses of intrinsic properties and spontaneous combustion characteristics indicate similarities between the mechanism of coal oxidation and that of the oxidative processes undergone by coal-shales. For the tested samples, coal samples have a higher intrinsic spontaneous combustion reactivity rating than the coal-shales.Furthermore, an increase in carbon, moisture, hydrogen, volatile matter, nitrogen and a decrease in ash content indicate an increased proneness to self-heating. The concentration of pyrite found in the coal-shales accelerates self-heating. The event of spontaneous combustion can occur if coal-shales absorb sufficient oxygen when subjected to atmospheric conditions.

spontaneous combustion influenced by surface methane drainage and its prediction by rescaled range analysis

This study established numerical modeling using COMSOLTMto examine the influence of horizontal location and drainage ability of surface borehole on spontaneous combustion in longwall working face gob. Rescaled Range Analysis(R/S analysis) was employed to investigate the chaos characteristic of N_2/O_2 ratio from a surface borehole in 10416 working face gob, Yangliu Colliery, China. The simulation results show that there is always a circular ‘‘dissipation zone" around the drainage borehole and an elliptic ‘‘spontaneous combustion zone" in deep gob. Little influence was found on spontaneous combustion zone on the intake side of the gob but the width of spontaneous combustion zone in middle gob is enlarged, while the depth of spontaneous combustion zone near the return side is reduced. The R/S analysis indicates that the influence of surface borehole on spontaneous combustion can be divided into two stages by the chaos feature of N_2/O_2: safety drainage stage and spontaneous combustion initiating stage. It can be concluded that the methane drainage from gob through surface borehole can intervene in the distribution of spontaneous combustion zone in gob and the chaos feature of N_2/O_2 from surface borehole can effectively reflect coal spontaneous combustion condition in gob.

spontaneous retroperitoneal hemorrhage after hemodialysis involving anticoagulant agents

In this paper, we described the symptoms and treatment of a patient with diabetic nephropathy accom- panied by spontaneous retroperitoneal hemorrhage after hemodialysis. An elderly female patient with diabetic neph- ropathy presented with severe pain, numbness, and an increasing swelling in the left hip and left thigh after six ses- sions of hemodialysis involving the use of an antiplatelet drug and an anticoagulant agent. Her hemoglobin decreased to 46 g/L. An abdominal ultrasound showed a hematoma in the left retroperitoneal space, and computed tomography (CT) findings revealed a 6 cm×8 cm×10 cm hematoma in the left psoas muscle. After aggressive supportive therapy [the administration of packed red blood cell transfusion, carbazochrome sodium sulfonate injection, and continuous venovenous hemofiltration (CVVH)], the patient's vital signs stabilized and her hemoglobin increased to 86 g/L. Repeat CT showed that the hematoma had been partially absorbed after two weeks. Eventually, the patient was discharged with stable vital signs. Physicians should be aware of the possibility of spontaneous retroperitoneal hemorrhage, particularly in patients with diabetic nephropathy undergoing hemodialysis involving the use of anticoagulant agents.

BACKGROUND Complications associated with upper gastrointestinal(UGI)endoscopy are uncommon,and rarely involve those of cardiovascular nature.We report herein a unique case of spontaneous superior mesenteric artery dissection(SMAD)after UGI pandenoscopy.CASE SUMMARY A 45-year-old man who had previously undergone UGI panendoscopy and colonoscopy during a voluntary health check-up at our facility was admitted to the emergency room(ER)at the same facility due to persistent epigastric pain with radiation to the back.At the ER,the patient did not present notable abnormalities upon physical,conscious,or laboratory examinations apart from mild tenderness in the epigastric abdomen.Acute abdominal aortic dissection was suspected,and abdominal contrast-enhanced computed tomography confirmed SMAD.He was then transferred to the cardiovascular ward and treated conservatively with fasting,prostaglandin E1,and aspirin.The patient recovered and returned home soon after,and was symptom-free 6 months after discharge from the facility.CONCLUSION SMAD after UGI panendoscopic procedure is a previously unreported complication.Awareness of this complication and associated sequela is warranted.

spontaneous low-temperature crystallization of α-FAPbI3 for highly efficient perovskite solar cells

Formamidinium lead triiodide(HC(NH2)2PbI3 or FAPbI3)is a promising light absorber for high-efficiency perovskite solar cells because of its superior light absorption range and thermal stability to CH3NH3PbI3(MAPbI3).Unfortunately,it is difficult to fabricate high-quality FAPbI3 thin films to surpass the MAPbI3-based cells due to easily forming unwanted but more stable yellow d-phase and thus requiring high annealing-temperature for wanted photovoltaic-active black a-phase.Herein,we reported a novel low-temperature fabrication of highly crystallized a-FAPbI3 film exhibiting uniaxial-oriented nature with large grain sizes up to 2 lm.First-principles energetic calculations predicted that this novel deposition should be ascribed to the formation of a high-energy metastable two-dimensional(2D)intermediate of MAFAPbI3 Cl followed by a spontaneous conversion to a-FAPbI3.The ions exchange reaction during this MAFAPbI3 Cl-FAPbI3 conversion account for the perovskite film uniaxial-oriented grown along the(111)direction.This large-grain and uniaxial-oriented grown a-FAPbI3 based solar cells exhibited an efficiency up to 20.4%accompanying with low density-voltage(J-V)hysteresis and high stability.

spontaneous multivessel coronary artery spasm diagnosed with intravascular ultrasound imaging:A case report

BACKGROUND Coronary artery spasm is a major cause of myocardial ischemia.Although coronary artery spasm has been known for a long time,its mechanism has not yet been identified.Many clinicians,especially young clinicians pay less attention to coronary artery spasm,which may lead to some patients not being appropriately diagnosed and treated in time.We report a patient with spontaneous multivessel coronary artery spasm for more than 30 years diagnosed with intravascular ultrasound(IVUS)imaging.CASE SUMMARY A 66-year-old Chinese male patient had chest squeezing at rest for more than 30 years.He had a history of cigarette smoking for more than 40 years and hypertension for 10 years.Before presenting at our institution,the patient had undergone coronary angiography 4 times and percutaneous transluminal coronary angioplasty procedures twice at other hospitals without a diagnosis of coronary artery spasm.However,his chest symptoms worsened.spontaneous multivessel coronary artery spasm occurred during IVUS without provocation testing,and the IVUS image was recorded.Thus,the diagnosis of multifocal spontaneous coronary artery spasm was confirmed.The patient was placed on oral diltiazem,isosorbide mononitrate,and nicorandil to suppress coronary artery spasms.All medications were given at the maximum dosages tolerated by the patient.He was discharged after 5 d without complications.During the six-month follow-up period,the patient was symptom-free.CONCLUSION Coronary artery spasm is still prevalent in Eastern countries.It is essential for clinicians to be aware of coronary artery spasm,which may be hard to detect and can be lethal,in order to diagnose and treat patients appropriately.

spontaneous rupture of hepatic metastasis from a thymoma: A case report

Bleeding resulting from spontaneous rupture of the liver is an infrequent but potentially life threatening complication that may be associated with an underlying liver disease. A hepatocellular carcinoma or hepatic adenoma is frequently reported is such cases. However, hemoperitoneum resulting from a hepatic metastatic thymoma is extremely rare. Here, we present a case of a 62-year-old man with hypovolemic shock induced by ruptured hepatic metastasis from a thymoma. At the first hospital admission, the patient had a 45-mm anterior mediastinal mass that was eventually diagnosed as a type A thymoma. The mass was excised, and the patient was disease-free for 6 years. He experienced sudden-onset right upper quadrant pain and was again admitted to our hospital. We noted large hemoperitoneum with a 10-cm encapsulated mass in S5/8 and a 2.3-cm nodular lesion in the right upper quadrant of the abdomen. He was diagnosed with hepatic metastasis from the thymoma, and he underwent chemotherapy and surgical excision.

spontaneous Emission of a Two-Level Static Atom Coupling with Electromagnetic Vacuum Fluctuations Outside a High-Dimensional Einstein Gauss-Bonnet Black Hole

Using the generalized formalism of Dalibard, Dupont-Roc and Cohen-Tannoudji we investigate the spontaneous excitation of a static atom interacting with electromagnetic vacuum fluctuations outside an Einstein Gauss-Bonnet black hole in d-dimensions. It shows that spontaneous excitation does not occur in a Boulware vacuum, while exists in an Unruh vacuum and Hattie-Hawking vacuum. As to the total rate of change of the atomic energy, it does not receive the contribution from the coupling constant of the Gauss-Bonnet term at spatial infinity, only the dimensional parameter has the contribution to it. Near the event horizon, both the coupling constant and the dimension p contribute to the total rate of change of the atomic energy in all three kinds of vacuum. We discuss the contribution of the coupling constant and dimensional factor to the results in three different kinds of spacetime lastly.
