Background:Sulfur mustard(SM)is a chemical warfare vesicant that severely injures exposed eyes,lungs,and skin.Mechlorethamine hydrochloride(NM)is widely used as an SM surrogate.This study aimed to develop a depilatory double-disc(DDD)NM skin burn model for investigating vesicant pharmacotherapy countermeasures.Methods:Hair removal method(clipping only versus clipping followed by a depilatory),the effect of acetone in the vesicant administration vehicle,NM dose(0.5-20μmol),vehicle volume(5-20μl),and time course(0.5-21 days)were investigated using male and female CD-1 mice.Edema,an indicator of burn response,was assessed by biopsy skin weight.The ideal NM dose to induce partial-thickness burns was assessed by edema and histopathologic evaluation.The optimized DDD model was validated using an established reagent,NDH-4338,a cyclooxygenase,inducible nitric oxide synthase,and acetylcholinesterase inhibitor prodrug.Results:Clipping/depilatory resulted in a 5-fold higher skin edematous response and was highly reproducible(18-fold lower%CV)compared to clipping alone.Acetone did not affect edema formation.Peak edema occurred 24-48 h after NM administra-tion using optimized dosing methods and volume.Ideal partial-thickness burns were achieved with 5μmol of NM and responded to treatment with NDH-4338.No dif-ferences in burn edematous responses were observed between males and females.Conclusion:A highly reproducible and sensitive partial-thickness skin burn model was developed for assessing vesicant pharmacotherapy countermeasures.This model pro-vides clinically relevant wound severity and eliminates the need for organic solvents that induce changes to the skin barrier function.

BACKGROUND The association of Sjögren's syndrome(SS)and lymphoma is similar.Mucosaassociated lymphoid tissue(MALT)or extranodal marginal zone B-cell lymphoma was the most common lymphomatous histology in SS patients.MALT in SS patients is frequently located in the parotid gland,while MALT lymphoma of the skin with SS is an exceedingly rare entity that needs to be recognized.CASE SUMMARY A 60-year-old woman presented with a 3-year history of progressive dry mouth associated with a 1-year history of enlarging cutaneous nodules.Physical examination revealed two hard subcutaneous nodules on her right lower leg.The results of Schirmer’s test were positive,despite the absence of dry eyes.Labial salivary gland biopsy revealed lymphocytic infiltration and chronic inflammation with a focus score of 2.The patient was diagnosed with SS.She underwent resection of one cutaneous nodule,and histopathological analysis identified the nodule as MALT lymphoma.Her dry mouth symptoms improved,and the nodules decreased after 6 mo of treatment with hydroxychloroquine sulfate and chemotherapy(thalidomide,cyclophosphamide,and dexamethasone).CONCLUSION Lymphoma is a severe complication of SS,shown by the reported unique case of cutaneous MALT lymphoma with SS.

To the Editor:Laboratory diagnostic methods of bullous pemphigoid(BP)include direct immunofluorescence(DIF)microscopy,BP180 NC16A enzyme-linked immunosorbent assays(ELISA),and indirect immunofluorescence using salt-split skin(ssIIF)in our daily clinical practice.Immunofluorescence(IF)studies are important parts of the laboratory workup for BP.DIF is regarded as the gold standard for BP diagnosis;however,it is not specific and can also be found in other acquired autoimmune subepidermal blistering dermatosis.ssIIF was found to have a specificity of 100%for BP diagnosis and in some guidelines,it is recommended to perform ssIIF in every patient with clinically suspected BP.[1]To date,it is still not defined whether ssIIF can be used as a routine diagnosis method of BP.The BP180 NC16A ELISA is a practical and reliable diagnostic test for BP;however,the false-positive results of BP180 NCA6A ELISA were recently reported in a wide range of dermatoses,which might cause its uncertainty in the diagnosis of BP.Thus,we conducted a single center retrospective study to compare diagnostic values of these tests for BP from Hospital for skin Diseases,Chinese Academy of Medical Sciences between January 2014 and January 2019.It was approved by the local medical ethical committee(No.2017-KY-022)and consent was obtained from patients prior to serum collection.

skin plays a pivotal role in the human body’s interaction with the external environment,enabling detection and response to sensory stimuli^([1,2]).However,skin damage can impede the sensorimotor system,with severe cases often requiring amputation and resulting in the inability to perform basic movements^([3]).While prosthetic limbs can aid in certain movement processes to a limited extent,they also possess constraints in terms of perception and flexibility.

The transdermal drug delivery(TDD) shows considerable advantages over other administration pathways.However, conventional enhancing permeation methods face a series of challenges owing to barrier function provided by the skin, of which enhancing abilities either are so strong that it results in toxicity and irritation, or too weak to achieve desirable therapeutical effects. To address these issues, it is an urgent need to develop a novel method to overcome the limitations of current measures. Fortunately, in the preceding decades, ionic liquids(ILs) have been extensively studied and increasingly applied in pharmaceutical drug delivery due to their unique physicochemical and biological properties. What is more, tunability of structure resolves the challenges in processing active pharmaceutical ingredient(API) formulation, such as polymorphism and poor solubility of drugs. Thus, the presence of ILs provides an ample design space for the transdermal drug delivery system(TDDS). This review discusses the shortcomings of conventional enhancing permeation methods and introduces the application of ILs in transdermal delivery from three aspects: i) ILs are applied as enhancers to weaken the barrier function of the stratum corneum(SC). ii) As counterions, ILs are combined with API to modify the physicochemical properties of drugs. iii) ILs assist in the design of transdermal preparation for perfecting formulation. This review comprehensively introduces the major breakthroughs made in the applications of ILs, which can serve as guidance to provide novel ideas for formulation scientists who hit the bottleneck in the development of TDD.

Wounds in diabetic patients,especially diabetic foot ulcers,are more difficult to heal compared with normal wounds and can easily deteriorate,leading to amputation.Common treatments cannot heal diabetic wounds or control their many complications.Growth factors are found to play important roles in regulating complex diabetic wound healing.Different growth factors such as transforming growth factor beta 1,insulin-like growth factor,and vascular endothelial growth factor play different roles in diabetic wound healing.This implies that a therapeutic modality modulating different growth factors to suit wound healing can significantly improve the treatment of diabetic wounds.Further,some current treatments have been shown to promote the healing of diabetic wounds by modulating specific growth factors.The purpose of this study was to discuss the role played by each growth factor in therapeutic approaches so as to stimulate further therapeutic thinking.

Transdermal drug delivery plays a significant part in the drug delivery system when compared to other routes of drug administration.The function of the stratum corneum(SC)is a barrier.Recently,numerous methods have been thrived to improve the perforation of drugs across the skin.The most effective method is to use enhancers since these agents enhance skin permeability.Natural penetration enhancers like essential oils demonstrate higher enhancement activity and are more widely accepted than synthetic penetration enhancers.High potential in the expansion and interaction with the SC intercellular lipids has led to an increasing interest in these oils as penetration enhancers.This article gives an overview of a few essential oils,including their mode of action and important parameters for permeation improvement.The present work can provide essential oils as alternative enhancers,and this could be useful in transdermal administration.

Several studies demonstrated the favorable effects of platelet rich plasma(PRP)on the skin and promoted its wide use in clinical practice.The growth factors stored in platelet alfa-granules allow for the tissue regeneration and the main fields of application of PRP in current clinical practice are the cartilage and musculoskeletal defects,osteoarthritis and other bone disorders,chronic and difficult to heal wounds,and aesthetic procedures.The relevant number of different PRP preparation protocols may explain the inconsistency of the different clinical outcomes reported in the literature.Despite the technological advances in PRP preparation,the objective assessment of the clinical efficacy of PRP from the literature reports still is difficult due to the low homogeneity of the samples in terms of both inclusion criteria and size.Therefore,it might be useful to establish standardized and reproducible experimental models to confirm and objectively measure the effectiveness of the available clinical results.Many experimental investigations have been carried out to objectively assess the effectiveness of PRP and platelet gel on several tissues.As far as the skin is concerned,the studies carried out to date are limited to fibroblasts in in-vitro culture models or to collagen,vascular supply,epithelium,and hair follicle in in-vivo models.The skin,however,is a very complex organ,where different cell lines coexist and feature complex mutual interaction.A model that combines the advantages of both in-vitro and in-vivo cultures is the ex-vivo model.The demonstration of the platelet derived growth factors effects through the ex-vivo human full-thickness skin culture model is a keystone to support the evidence of the PRP effectiveness,as it represents an objective,fast,reproducible,and ethical investigational method.
