Socio-Demographic and Maternal Determinants of Birth Preparedness among Pregnant Women Seeking skilled Birth Services at a Rural General Hospital, Mid Western Uganda

Background: Although the practice of preparing for childbirth among pregnant women is associated with 24.0% and 53.0% reduction in neonatal and maternal mortality respectively in low income countries, it remains inadequately practiced in Uganda. This study sought to assess socio-demographic and maternal determinants of birth preparedness among pregnant women at one of the rural hospitals in Uganda. Methods: This was a descriptive cross sectional study conducted among 332 pregnant women in first stage of normal labour between April and May 2017 at Kagadi hospital, mid western Uganda. In order to understand socio-demographic and maternal determinants of birth preparedness, we considered overall level of birth preparedness as our dependent variable and determined it using five elements. Data were collected using a researcher administered questionnaire. We used descriptive statistics to understand characteristics of respondents and multivariable logistic regression model to determine factors associated with birth preparedness. Results: Only 94 (28.3%) of the 332 pregnant women who were included in the study were prepared for childbirth. We found that male partners having secondary level of education (aOR 1.86, CI 1.107 - 3.243, p 0.02), discussing birth plan with spouse (aOR 1.97, CI 1.06 - 3.490, p 0.03), monthly income of at least eighty three dollars (aOR 2.98, CI 1.847 - 5.981, p 0.002), being escorted by the spouse during ANC visit (aOR 1.73, CI 1.010 - 2.964, p 0.04), intention to conceive (aOR 1.99, CI 1.087 - 3.65, p 0.026) and being health educated about birth preparedness during antenatal visit (aOR 1.90, CI 1.006 - 3.59, p 0.048) were associated with birth preparedness. Conclusion and Recommendations: We observed that the level of birth preparedness among respondents was associated with many socio-demographic and maternal factors. With the rapid development of knowledge economy, a number of important shifts are emerging in urbanization pattern across the world. Traditional urbanization theory has become hard to interpret these changes on a global scale, and 'innovation' is a core concept to explain the new changes of the urbanization dynamics. As one of the important contents of urban geography, urbanization dynamic needs to turn from the general population flows between rural and urban areas into emphasizing high skilled migrants flow among cities and regions research. Against this background, we propose a conception of innovation-based urbanization. Using this concept, this study analyzes the spatial distribution characteristics of high skilled migrants and cultural diversity on urban innovation in China, based on the data of the sixth census of 270 cities at the prefecture level or above in China. This study measures the extent to which highly skilled migrants and cultural diversity increase urban innovation, by using spatial Durbin method to construct urban innovation regression model, to support the concept of innovation-based urbanization. The result shows that: first of all, the concept of innovation-based urbanization conforms to the development of knowledge economy, which emphasizes the migration process of highly skilled labor to cities. It helps to promote the changes in urban functions and landscapes and the expansion of urban knowledge activities, which undelines new dynamics of urban development, innovative landscape. Secondly, innovation urbanization based on highly skilled migrants flow is an important driving force for the development of Chinese cities, especially for eastern coastal cities and capital cities. Thirdly, the scale of highly skilled migrants flow and the level of urban cultural diversity in China both have been demonstrated of having positive effect on urban innovation output. Birth Preparedness and Complication Readiness among Pregnant Women Attending Antenatal Classes at Primary Health Center in Ibadan, Nigeria

Maternal mortality has been reported to be a challenge globally, with the highest maternal mortality in Africa. However, the first target for the third Sustainable Development Goal (SDG) is to reduce the global maternal mortality ratio to less than 70 per 100,000 live births by 2030. To achieve this goal, pregnant women’s knowledge of birth preparedness and complication readiness (BPCR) is a key. Therefore, this study set out to assess knowledge of birth preparedness and complication readiness among pregnant women attending antenatal classes. This pilot study adopted a descriptive cross-sectional study design using quantitative approach. Structured questionnaire was used to generate data from 46 respondents after obtaining ethical approval for the study. All data collected were adequate for data analysis. The mean age is 27.71 years with a standard deviation of 5.85, the average booking time was 4.69 months. Among the respondents who had given birth before, the average number of children was two children (1.8571). After categorizing the knowledge scores, 52.2% of the women have good knowledge of obstetric danger signs, 20 (43.5%) of the women have poor knowledge of BPCR and 32 (69.6%) of the respondents have good knowledge of skilled birth attendants. In conclusion, there is a need to formulate policies and strategies that will help to improve pregnant women’s knowledge of BPCR if the first target of the third SDG will be achieved.

Physical assessment ability of nurses and midwives on maternal care in Mongolia

This study examined the physical assessment ability of Mongolian nurses and midwives engaged in maternal care and the factors that could influence this ability. We found that nurses and midwives from prefectural hospitals had higher scores than those from national and district (soum) hospitals. The latter group could not perform more than half of the 28 tested procedures on their own, even though most had over 10 years’ experience. Therefore, to decrease the Mongolian maternal mortality ratio, we argue that in-service training for nurses and midwives in both the capital city and peripheral areas is needed.

作为衔接基础教育和高等教育人才培养的重要环节，高考改革牵动着教育改革的全局。长期单一模式的高考与追求学历的路径相互作用，彼此助长，双轨制高考改革是社会发展和工程人才培养的共同需要。笔者认为，双轨制高考改革意味着重新建轨，轨道需要科学规划，改革过程中需注意双轨的前后联接与互通、对运行进行监督反馈，发挥高校的主导作用。改革要突破几个难点：充分认识办学定位的复杂性，不囿于职高与普高、高职与普本的形式分轨；畅通人才培养及流动通道，避免分数决定轨道的陈旧模式；合理设计改革方案，兼顾公平与效率；转变人才观，建立改革的社会支持系统。双轨制高考改革意味着重新建轨，需要科学规划轨道，改革过程中应注意双轨的前后联接与互通，对运行进行监督、反馈，发挥高校的主导作用。
