Background Contralateral subdural effusion（SDE） is usually considered as an uncommon complication after decompressive craniectomy（DC）. This complication may need more aggressive treatment because of its tendency to cause midline shift and neurologic deterioration. We present 2 cases of SDE following DC and give an overview of the English literature pertaining to this disease. Materials and method We described 2 new cases of SDE following DC and performed a search of English literature in PubMed and Google Scholar. Keywords used were as follows（single word or combination）: "traumatic subdural hygroma", "subdural hygroma" AND "decompressive craniectomy". Only patients with contralateral SDE following DC and contained adequate follow-up information pertinent to the analysis were included. Result The first patient presented with contralateral SDE twelve days after right fronto-temporo-parietal DC and evacuation of the SDH. He was managed conservatively. Six days later, his consciousness deteriorated and CT showed progressive enlargement of the SDE, with midline structures deviating. Ventriculoperitoneal（VP） shunt procedure was performed and his clinical presentation improved gradually. The other patient presented with contralateral SDE thirteen days after right fronto-temporo-parietal DC and evacuation of the SDH. Burr-hole with a closed drainage system was performed to drain the left SDE. The SDE decreased significantly. Repeat CT scan showed reaccumulation of the left SDE three days after removal of the drainage system. VP shunt procedure was performed and the patient recovered well. A total of 63 cases of SDE after DC were found in the literature. Including out two patients, a total of 65 patients were analyzed. 22 symptomatic cases were managed conservatively, and 86.4% achieved good outcome（3 cases were deteriorated and converted to surgery）. A total of 46 patients were converted to surgery. 29 cases were underwent burr hole drainage to evacuate the SDE, and 58.6%

Current approaches to the management of patients with cirrhotic ascites

This review describes current approaches to the management of patients with cirrhotic ascites in relation to the severity of its clinical manifestations. The PubMed database, the Google Scholar retrieval system, the Cochrane Database of Systematic Reviews, and the reference lists from related articles were used to search for relevant publications. Articles corresponding to the aim of the review were selected for 1991-2018 using the keywords:“liver cirrhosis,”“portal hypertension,”“ascites,”“pathogenesis,”“diagnostics,” and “treatment.” Uncomplicated and refractory ascites in patients with cirrhosis were the inclusion criteria. The literature analysis has shown that despite the achievements of modern hepatology, the presence of ascites is associated with poor prognosis and high mortality. The key to successful management of patients with ascites may be the stratification of the risk of an adverse outcome and personalized therapy. Pathogenetically based approach to the choice of pharmacotherapy and optimization of minimally invasive methods of treatment may improve the quality of life and increase the survival rate of this category of patients.

It has been demonstrated that the lateral geniculate nucleus (LGN) of the cat possesses two types of inhibitory circuits: feedforward and recurrent inhibitory circuits. Braking distance prediction for vehicle consist in low-speed on-sight operation:a Monte Carlo approach

The first and last mile of a railway journey, in both freight and transit applications, constitutes a high effort and is either non-productive(e.g. in the case of depot operations) or highly inefficient(e.g. in industrial railways). These parts are typically managed on-sight, i.e. with no signalling and train protection systems ensuring the freedom of movement. This is possible due to the rather short braking distances of individual vehicles and shunting consists. The present article analyses the braking behaviour of such shunting units. For this purpose, a dedicated model is developed. It is calibrated on published results of brake tests and validated against a high-definition model for lowspeed applications. Based on this model, multiple simulations are executed to obtain a Monte Carlo simulation of the resulting braking distances. Based on the distribution properties and established safety levels, the risk of exceeding certain braking distances is evaluated and maximum braking distances are derived. Together with certain parameters of the system, these can serve in the design and safety assessment of driver assistance systems and automation of these processes.

To restrict indication for stenting of the inferior vena cava and liver transplantation in patients with Budd-Chiari syndrome

The Budd-Chiari syndrome(BCS)used to be a very rare disorder and lacked a means of ***,various shuntings and radical procedures have emerged in the last three decades with quite encouraging *** minimally invasive therapies,such as stenting of the inferior vena cava(IVC),are simple and easy to ***,it has been realized that the IVC stent may compromise the hepatic veins and cause catastrophic consequences,thus the indication for IVC stenting should be reevaluated and especially not to be ***,it should be applied by percutaneous transangiography(PTA)only,which may be repeated when *** BCS in China predominantly belongs to the IVC type rather than the intrahepatic type in Western;the outcome from its conventional therapy,at least for the moment,is better than that of liver *** is thus suggested that,before liver transplantation is decided,the conventional means for managing BCS be *** final suggestion is to restrict the indication both for stenting of the IVC and liver *** this paper,ten examples are given with figures.

Combined splenocaval or mesocaval C shunt and portoazygous devascularization in the treatment of portal hypertension: analysis of 150 cases

BACKGROUND: Portal hypertension is a common disease and its major surgical therapeutic approaches include devascularization and shunting. This study was undertaken to investigate the effects of combined splenocaval or mesocaval C shunt and portoazygous devascularization (combined procedures) on portal hypertension. METHODS: The clinical data of 150 patients with portal hypertension who had undergone combined procedures at the First Affiliated Hospital of Zhengzhou University from May1990 to May 2003 were analyzed retrospectively. RESULTS: The mean free portal pressure (FPP) was 25.6±1.83 mmHg, 18.0±2.07 mmHg and 18.4±2.19 mmHg before operation, after splenectomy plus splenocaval or mesocaval C shunt, and combined procedures, respectively. There was no operative death in all patients. The 1-7 year follow-up of 100 patients showed rebleeding in 3 patients, encephalopathy in 4, thrombosis of artificial vascular graft in 3, and dying from liver failure in 2. CONCLUSIONS: The combined procedures can not only decrease portal pressure but also preserve hepatic blood flow to some extent. It may be one of the best choices for treating portal hypertension in China.

目的特发性常压脑积水(Idiopathic normal pressure cephalus,iNPH)是痴呆的唯一可逆性原因,但许多患者由于诊断不足而未能及时接受分流手术。本研究旨在确定手术时间点与iNPH患者术后持续改善之间的关系。方法根据患者发病至手术时间分为早期手术组(0～3个月)和晚期手术组(> 3个月)。患者来自中国不同省份的3家综合医院。主要终点包括特发性常压脑积水分级量表(INPHGS)和改良Rankin量表(mRS)的改善。结果共纳入120例患者。平均年龄为62.76±12.27岁,平均症状持续时间为22.00±37.99个月,平均INPHGS为7.15±1.759(认知:2.71±0.91;步态:2.47±0.95;尿:1.96±0.88),平均mRS为2.45±0.77。早期手术组患者自出院至随访1年症状持续改善。在mRS改善方面,术后时间点及随访1年,早期手术组优于晚期手术组(p <0.05)。结论这项多中心研究的结果表明,早期分流可促进iNPH患者的持续改善。
