Based on the clinical effect of the treatment on 546 Pediatric Shi Ji fever cases, the thesis tries to explore the effectiveness of Traditional Chinese Medicine(TCM) treatment on Pediatric Shi Ji and the relationship between Pediatric Shi Ji and fever. The methodology applied is a retrospective analysis on the clinical curative effect of TCM treatment on Shi Ji fever cases in our hospital from January 2008 to December 2012. And the results show that a total effective rate of 96.3% could be guaranteed through either oral Chinese Medicinal Herbs, Chinese Medicine Enema, Massage Therapy, or navel administration with TCM. The thesis holds that Pediatric Shi Ji may cause fever, which could be cured simply by applying TCM treatment (promoting digestion to eliminate stagnation) with less or no use of antibiotics.

Results are presented of a retrospective audit of admissions to the Intensive Care Unit (ICU) of Kamuzu Central Hospital in Lilongwe, Malawi, which is a tertiary referral facility. The audit was conducted for a period of one year spanning from January to December, 2012. The objectives of the audit were to: describe the profile of admissions and treatment outcomes of the admissions and identify main causes of mortality in ICU of the facility. The admission book and patients’ records were reviewed retrospectively guided by a data extraction form which was specifically designed for this study. The extracted data included age, sex, referring unit, diagnosis, treatment outcome and length of stay. The data were analyzed using STATA version 10.0. A total of 253 patients were admitted to the ICU over the one year period of study. About a third of the patients (33.6% n = 85) were admitted due to postoperative surgery. There were 154 deaths representing an overall mortality of 60.9%. Sepsis was the commonest cause of death and accounted for 39.6%, n = 61 of the deaths. Younger age of less than 40 years and increased patients’ length of stay in the unit were significantly and positively associated with mortality (P < 0.05). The high mortality rates among patients admitted to ICU reflects numerous challenges at various levels of critical care service delivery in the country. There is therefore a need to strengthen critical care services to improve treatment outcomes for patients admitted to ICU of the facility.

Induction of Labor with PGE2 after One Previous Cesarean Section: 18 Years Experience in a University Hospital

Objective: To review the outcome of prostaglandin induction of labor in term pregnant women with previous one ce-sarean section compared to those without previous Cesarean section. Design: 18 years retrospective review of hospital records and case note review of index cases. Setting: University hospital. Population: Three hundred and twenty two women who had their labor induced with prostaglandin E2. One hundred and sixty one women had one previous Ce-sarean section. Methods: This study was conducted at King Fahad University Hospital, University of Dammam. It is a tertiary referral center with approximately 2300 births per year. We searched the hospital's records of deliveries from January 1992 to December 2009 and reviewed all indications and outcomes of prostaglandin induction of labor in women with one previous Cesarean section. The control group was composed of women who had their labor induced with prostaglandin but without previous Cesarean section. Main outcome measures: Labor outcome and uterine rup-ture Results: Three hundred and twenty two women were included. All received prostaglandin E2 for induction of labor. One hundred and sixty one women had one previous Cesarean section (study group) and the rest had no previous Ce-sarean section (control group). There was no difference in the rate of vaginal delivery between study and control group, 68.3% and 79.5% (p value 0.3), respectively. The rate of uterine rupture was 30 times higher in study group (2.5% Vs 0.033%). Conclusion: In women with one previous Cesarean section, induction of labor with prostaglandin leads to comparable rate of vaginal delivery similar to those without prior Cesarean section but with relatively high risk of uter-ine rupture.

Chin J Nephrol 2007;23(4):209-203. Objective To explore the clinicopathological characteristics of acute kidney injury (AKI) of systemic sclerosis (SSc). Methods A retrospective study was performed on 11 SSc patients with AKI. The clinical data were analyzed and the patients were divided into antineutrophil cytoplasmic antibodies (ANCA) negative group(n=9) and ANCA positive(n=2) group. Results In the ANCA negative group, 2 cases were without malignant hypertension, 1 was acute tubular necrosis(ATN) caused by herbs, 6 were scleroderma renal crisis(SRC), including 4 with renal biopsy, indicating hypertrophic arterial media, edema, thickened intima, onion-skin lesion in interlobular arteries and afferent arterioles, as well as ischemic lesion in glomeruli. In the MPO-ANCA positive group, 1 was crescentic glomerulonephritis. Malignant hypertension was not noticed. All patients were given steroid, 8 of them received CTX in addition. Nine patients received dialysis, and 8 cases progressed to permanent hemodialysis. Six cases with SRC were given high dose ACEI and / or ARB. Six patients resulted in early death. Conclusion Scleroderma renal crisis and ANCA associated vasculitis may cause AKI in SSc patients. Patients with positive ANCA differ from those with negative ANCA in terms of clinical manifestation, pathology and treatment. Survival and prognosis of SSc patient were bad. High dose corticosteroids increases the risk of scleroderma renal crisis, so it is thereby recommended that the dose above 15 rog/day should be avoided if possible.

Objective:To compare the sex ratio,age distribution,severity of the illness,comorbidities,and the treatment of the patients with obstructive sleep apnea syndrome in China and Germany.Methods:A retrospective study of the data was conducted,and a total of 200 patients diagnosed with obstructive sleep apnea syndrome in Germany and China from March 2013 to October 2015 were analyzed.The sex ratio,age distribution,severity of the illness,comorbidities and treatment methods were analyzed.Results:There was no significant difference in the severity of disease,sex ratio,and age distribution between the patients with obstructive sleep apnea syndrome in China and Germany(P>0.05).In terms of comorbidities,the combination of moderate and severe patients in the German group,compared with the moderate and severe patients in the Chinese group,except for cerebrovascular disease,the comorbidities were less than those in the Chinese group(P<0.05),the difference was statistically significant.In terms of treatment,the patients in the German group were compared with the patients in the Chinese group with light,moderate,and severe patients.Except for the German patients who received oral appliances and surgical treatment,more patients were treated than Chinese patients(P<0.05),the difference is statistically significant.Conclusion:The two country(Germany and China)are basically the same in the diagnosis and treatment of obstructive sleep apnea syndrome,but Germany pays more attention to details and individualization in treatment.

Fecal microbiota transplant(FMT)has quickly become popular in research not only for recurrent Clostridioides difficile infections but for other chronic conditions as well.Recent,small dermatologic studies have reported improvements in inflammatory skin conditions in individuals treated with FMT,but larger studies are needed to clarify this possible relationship between the skin and the gut microbiome.We conducted a single-center,retrospective chart review to assess changes in acne,dermatitis herpetiformis and/or celiac disease,eczema,and psoriasis.Due to the retrospective nature of this study and the limitations of the current electronic medical record,we were unable to adequately assess cases of these diseases in relation to FMT.However,this study informs us that improvements in retrospective data are needed to formally evaluate this possible association.The better,but more cumbersome,study design would be a prospective,observational study.We encourage others to pursue further interdepartmental research on the influence of the gut microbiome on inflammatory skin diseases.
