Application of immunosuppressant facilitates the therapy of optic neuritis combined with Sj（o）gren＇s syndrome

Background Optic neuritis （ON） is often the first symptom of multiple sclerosis （MS） and neuromyelitis optica （NMO） while there has been very little research reported on ON combined with Sj（o）gren＇s syndrome （SS）.The aim of this study is to provide different treatments and services for and NMO patients combined with SS.Methods Twenty-seven patients with ON combined SS were divided into two groups：corticosteroid group （C group,methylprednisolone sodium succinate,14 patients） and corticosteroid＋ immunosuppressant group （C＋I group,leflunomide,13 patients）.ON relapse times in 1 year after treatment,number of patients who relapsed to NMO/MS in 1 years,visual acuity and retina nerve fiber layer （RNFL） thickness were measured.Mann Whitney-Wilcoxon test was used to compare continuous variables and Chi-square test or Fisher＇s exact test was to compare proportions.Results ON combined with SS patients had higher incidence rates in middle-aged women who have binocular damage and heavier visual function damage or when there is an easy relapse,and the patients are often hormone dependent.The patients are more likely anti-aquaporin-4 IgG seropositive （70.4％）.They are liable to form a centrocecal scotoma and tubular vision.The times of relapse decreased in patients who used immunosuppressant,and a significant difference was found between immunosuppressant and non-immunosuppressant groups in visual acuity recovery during 6-month followup period （P ＜0.05）; however,the RNFL thickness at the four quadrants was not significantly different.Conclusions The effect of immunosuppressant plus corticosteroid on the early onset of ON combined with SS was to provide ON remedy and to prevent recurrence in clinics.This study provides a significant reference for the prevention and treatment of ON on the basis of immunosuppressant and corticosteroid.

Reinforced concrete structures experience alteration and degradation during their lifetime due to the corrosion of the steel reinforcements.Prevention of the steel corrosion is indispensable to avoid structural degradation.In this paper,a preventive numerical approach of corrosion of steel reinforcements is presented.An in-house program which is part of a developed software called REHA is used in the present work.The corrosion initiation time due to carbonatation and penetration of chloride ions is studied.The model is applied on a case study which concerns a reinforced concrete T-beam of a bridge.The results revealed that the penetration of chloride ions represents the unfavorable case,which leads to rapid corrosion of the steel reinforcement,and the environmental conditions do not have high influence on the crack opening width in the service and spalling phases.The objective of the present model is to act as a decision aid for including the problem of corrosion cracking of steel reinforcements when planning strategies for rehabilitation and maintenance of existing structures or when dimensioning elements of new structures.

Malaria is one of the six major seriousepidemic diseases prevalent in tropical andsubtropical areas.Beginning from 1967,specialinstititutions,organized by professional researchworkers,were established for searching antima-larial drugs through research work in chemistryand traditional Chinese medicine.As early as in770-221 BC,the“Recipes for Treating 52 Kindsof Diseases”,a textual relic unearthed inMawangdui,described the application of artemisiaannue as an antimalarial remedy.In 341,Dr.Ge Hong of the Iin Dynasty recorded,in his“Handbook of Prescriptions for EmergencyTreatment”,artemisia annua as an ideal anti-malarial drug.

Interfacing DC sources to load/power grid requires DC converters that produce minimum level of current ripples. This is to limit the losses and hence increase the life span of these sources. This article proposes a simple inter-leaved boost converter that interfaces PhotoVoltaic (PV) module into a common DC-link. The article also addresses the faulty mode operation of the proposed circuit while advising the appropriate remedy actions. A MATLAB and Simulink dynamic platform are used to simulate the transient performance of the proposed converter. The results revealed the effectiveness and the viability of the proposed converter in reducing the ripples in the PV current without employing bulky input inductors or increasing the switching frequency.
