BACKGROUND: In adult-to-adult living donor liver transplantation (LDLT), the use of a right lobe graft without the middle hepatic vein (MHV) can cause hepatic congestion and disturbance of venous drainage. To solve this problem, we successfully used cadaveric venous allografts preserved in 4 ℃ University of Wisconsin (UW) solution within 10 days as interposition veins for drainage of the paramedian portion of the right lobe in adult LDLT. METHODS: From June 2007 to January 2008, 11 adult LDLT patients received modified right liver grafts. The major MHV tributaries (greater than 5 mm in diameter) of 9 cases were preserved and reconstructed using cadaveric interposition vein allografts that had been stored for 1 to 10 days in 4 ℃ UW solution. The regeneration of the paramedian sector of the grafts and the patency of the interposition vein allografts were examined by Doppler ultrasonography after the operation. RESULTS: MHV tributaries were reconstructed in 9 recipients. Only 1 recipient died of renal failure and severe pulmonary infection on day 9 after transplantation without any hemiliver venous outflow obstruction. The other 8 recipients achieved long-term survival with a median follow-up of 30 months. The cumulative patency rates of the 8 recipients were 63.63% (7/11), 45.45% (5/11), 45.45% (5/11) and 36.36% (4/11) at 3, 6, 12 and 24 months, respectively. Regeneration of the paramedian sectors was ***: The cadaveric venous allograft preserved in 4 ℃ UW solution within 10 days serves as a useful alternative for interposition veins in facilitating implantation of a right lobe graft and guarantees outflow of the MHV.

reconstruction OF TEMPERATURE SERIES OF NORTH CHINA FROM 1380s TO 1980s

The decade mean temperature anomaly series was reconstructed on the basis of histori-cal documents recorded in substantive local gazetteers. Firstly, the severity of the cold weath-er events such as snowing and severe frost, which were frequently described in gazetteers,was estimated by using modern instrumental temperature observations. Secondly, the assort-ed cold events were classified into three classes according to the severity. the severity indexof them being --0.5, -- 1.0 and --2.0 respectively. The severity index was found fot eachseason since 1380 AD while there were somc descriptions of cold events. Finally, the re-gression coefficients between the decade sum of severity index and decade mean seasonal tem-perature anomalies was calculated. It shows that the severity index of--1 is equal to --0.2℃of the temperature anomaly. Then the decade mean seasonal temperature anomaly serieswas reconstructed from the 1380s (1380--1389 AD) to the 1980s. The series shows that therewere two cold periods; one between the 1550s and 1690s, another during the 1800s--1860s. Thelowest decade mean temperature anomaly reacked -- 1.8℃ in winter and varicd from 1--1.1to --1.4℃ for other seasons. In spring and summer series, the first cold period was mani-fested well. In autumn tbe second cold period was prominent. But in winter both the firstand the second cold periods were clearly observed. The timing of the cold periods found inthe present paper was earlier by 70 and 40 years respectively than that found by Chu Kochen(1973), which was mainly based on winter temperature documents in South China. It sug-gests that the timing of the cold period in the Litt1e Ice Age varied from place to placeeven within the Chinese territory.

reconstruction of complex orbital fracture with titanium implants

AIM: To evaluate the effectiveness and safety of complex orbital fracture reconstruction with titanium implants. METHODS: A retrospective review of 46 patients treated with complex orbital fractures reconstruction using titanium implants from January 2005 to December 2008 was conducted. The following data were recorded: age, gender, mechanism of injury, preoperative and postoperative orbital CT, visual acuity, diplopia, ocular motility and Hertel exophthalmometer. RESULTS: The most common cause was motor vehicle accident (47.8%), followed by industrial injury (30.4%). All patients had improved appearance after operation and CT scan at one week after operation showed the fracture defects of orbit and neighboring areas had been reconstructed. Forty-six cases had various degrees of enophthalmos before operation. Among them, 32 cases were completely corrected, 11 cases improved obviously and 3 cases had no improvement after operation. Thirty-six patients with visual acuity =20/60 revealed diplopia of various degrees, including 26 patients had diplopia in right ahead and/or reading positions. At the sixth month after operation, diplopia disappeared in five patients, 7 patients still had diplopia in right ahead and/or reading positions, 14 patients had diplopia in positions rather than right ahead and reading positions (20 degrees). All patients had various degrees of ocular motility disorders before operation. At the sixth month after operation, eyeball movement disorder disappeared in 9 patients, 31 patients showed improvement and 6 patients had no improvement. Complications of implant infection, rejection and displacement were not reported after operation. CONCLUSION: The application of titanium implants in the repair of complex orbital fractures greatly improves the appearance and functional results, which is a favorable material for plastic surgery of complex orbital fracture.

reconstruction of impact force of mechanical press in time domain

To overcome the difficulty in directly measuring the impact force of a mechanical press, the inverse theory is employed to reconstruct the impact force from the corresponding response data in time domain. The nature of ill-posedness of impact force reconstruction is explored through singular value decomposition （SVD） and the Tikhonov regularization is utilized to deal with the ill-posedness, in which the optimal parameter is chosen in light of the L-curve criterion and the generalized cross- validation （GCV）. The experimentally measured strain responses of upper and lower dies of the press are chosen as source data for impact force reconstruction, and the corresponding numerical results are compared with the experimental measurements, which verifies the effectiveness of the reconstruction method.

The pore structural characteristics have been the key to the studies on the mechanisms of fluids flow in porous media. With the development of experimental technology, the modern high-resolution equipments are capable of capturing pore structure images with a resolution of microns. But so far only 3D volume data of millimeter-scale rock samples can be obtained losslessly. It is necessary to explore the way of virtually reconstructing larger volume digital samples of porous media with the representative structural characteristics of the pore space. This paper proposes a reconstruction method of porous media using the structural characteristics captured by the data templates of multiple-point geostatistics. In this method, the probability of each structural characteristic of a pore space is acquired first, and then these characteristics are reproduced according to the probabilities to present the real structural characteristics in the reconstructed images. Our experimental results have shown that: (i) the deviation of LBM computed permeability respectively on the virtually reconstructed sandstone and the original sample is less than 1.2%; (ii) the reconstructed sandstone and the original sample have similar structural characteristics demonstrated by the variogram curves.

Generally, a signal cannot be uniquely specified only from its FT magnitude unless there is some additional information, e.g. it is already known that the signal is the minimum or maximum phase, its FT sign information is available and partial sample points of the signal are given.

reconstruction of post-traumatic upper extremity soft tissue defects with pedicled flaps: An algorithmic approach to clinical decision making

Purpose:Pedicled flaps are still the workhorse flaps for reconstruction of upper limb soft tissue defects in many centers across the world. They are lifeboat options for coverage in vessel deplete wounds. In spite of their popularity existing algorithms are limited to a particular region of upper limb;a general algorithm involving entire upper limb which helps in clinical decision making is lacking. We attempt to propose one for the day to day clinical ***:A retrospective analysis of patients who underwent pedicled flaps for coverage of post-traumatic upper extremity (arm, elbow, forearm, wrist & hand) soft tissue defects within the period of January 2016 to October 2017 was performed. Patients were divided into groups according to the anatomical location of the defects. The flaps performed for different anatomical regions were enlisted. Demographic data and complications were recorded. An algorithm was proposed based on our experience, with a particular emphasis made to approach to clinical decision ***:Two hundred and twelve patients were included in the study. Mean age was 27.3 years (range: 1-80 years), 180 were male, and 32 were female. Overall flap success rate was 98%, the following complications were noted marginal flap necrosis requiring no additional procedure other than local wound care in 32 patients (15%), partial flap necrosis requiring flap advancement or extra flap in 15 patients (7%), surgical site infection in 11 patients (5%), flap dehiscence requiring re-suturing in 5 patients (2.4%), total flap necrosis 4 patients (2%).Conclusion:The proposed algorithm allows a reliable and consistent method for addressing diverse soft tissue defects in the upper limb with high success rate.
