Acupuncture Treatment for Post-Stroke Dysphagia: An Update Meta-Analysis of randomized controlled trials

Objective： To explore the effectiveness and safety of acupuncture in patients with post-stroke dysphagia by an update meta-analysis. Methods： Potentially eligible RCTs aimed to evaluate the effects of acupuncture vs. non-acupuncture treatments, such as rehabilitation training or routine medication on the swallowing difficulty after stroke were searched from PubMed, Cochrane Library, China National Knowledge Infrastructure, and other database from the earliest record to June 2016. Patient demographics, regimens for acupuncture, type of controls, methods of randomization, and measurements of the clinical symptoms of dysphagia were retrieved. The relative risk （RR） and 95% confidence interval （CI） of effective rate of dysphagia was calculated after intervention performed following admission. Subgroup analyses and a meta- regression analysis were performed to describe the heterogeneity. Results： Twenty-nine RCTs comprising 2,190 patients were included. The included studies had a medium quality grade based on the Consolidated Standards of Reporting trials （CONSORT） and Standards for Reporting Interventions in Clinical trials of Acupuncture （STRICTA） checklist. Acupuncture therapy provided a higher effective rate compared with non- acupuncture treatments [RR=1.33, 95% confidence interval （CI）, 1.25 to 1.43]. Subgroup and meta-regression analyses suggested that acupuncture intensity and measurement method were main sources of heterogeneity and resulted in a significant difference for pooled effect size. No severe adverse events were documented in these RCTs. Conclusions： Our meta-analysis provides a new evidence supporting the efficacy and safety of acupuncture in treatment to post-stroke dysphagia in short-term compared with rehabilitation or medication. More high-quality and large-scale research studies are needed.

Myelotomy promotes locomotor recovery in rats subjected to spinal cord injury： a meta-analysis of six randomized controlled trials

OBJECTIVE： To investigate the effects of myelotomy on locomotor recovery in rats subjected to spinal cord injury. DATA SOURCES： Electronic databases including Pub Med, Science Citation Index, Cochrane Library, China National Knowledge Infrastructure, Chinese Journals Full-text Database, China Biology Medicine disc, and Wanfang Database were searched to retrieve related studies published before September 2017. The Me SH terms（the Medical Subject Headings） such as ＂myelotomy＂, ＂spinal cord injuries＂, ＂rats＂, ＂randomized controlled trial＂ and all related entry terms were searched. DATA SELECTION： randomized controlled trials using myelotomy for the treatment of acute spinal cord injury in rats were included. Basso, Beattie, and Bresnahan scores were adopted as the evaluation method. Rev Man Software（version 5.3） was used for data processing. The χ^2 and I^2 tests were used to assess heterogeneity. Using a random-effects model, a subgroup analysis was conducted to analyze the source of the heterogeneity. OUTCOME MEASURES： Basso, Beattie, and Bresnahan scores were observed 1–6 weeks after spinal cord ***： Six animal trials were included, using a total of 143 lab rats. The included trials were divided into two subgroups by injury degrees（moderate or severe）. The pooled results showed that, 1–6 weeks after spinal cord injury, the overall Basso, Beattie, and Bresnahan score was significantly higher in the myelotomy group than in the contusion group（weighted mean difference（WMD） = 0.60; 95% confidence interval（CI）： 0.23–0.97; P = 0.001; WMD = 2.10; 95% CI： 1.56–2.64; P 〈 0.001; WMD = 2.65; 95% CI： 1.73–3.57; P 〈 0.001; WMD = 1.66; 95% CI： 0.80–2.52; P 〈 0.001; WMD = 2.09; 95% CI： 0.92–3.26, P 〈 0.001; WMD = 2.25; 95% CI： 1.06–3.44, P 〈 0.001）. The overall heterogeneity was high（I^2 = 85%; I^2 = 95%; I^2 = 94%; I^2 = 88%; I^2 = 91%; I^2 = 89%）. Impact Evaluation of CONSORT and STRICTA Guidelines on Reporting Quality for randomized controlled trials of Acupuncture Conducted in China

Objective： To evaluate and compare the reports＇ qualities of acupuncture randomized controlled trials （RCTs） conducted in China before and after the implementation of two guidelines, i.e., the Consolidated Standards of Reporting trials （CONSORT） statement and the Standards for Reporting Interventions in controlled trials of Acupuncture （STRICTA）. Methods： Proportions of studies that reported CONSORT and STRICTA items were compared for the years before and after implementation of these two guidelines. In addition, the total score of each item＇s reporting were calculated and reported differences during different date ranges were compared. Results： For CONSORT items （maximum score 8）, there was evidence of a slight improvement in reporting between 1994-1995 and 1999-2000 combined （2.5 ± 0.6） and 2004-2006 and 2009-2010 combined （3.0 ± 0.9; difference 0.4, 95% confidence interval, 0.3 to 0.6, P〈0.01）. For STRICTA items （maximum score 17）, there was evidence that a slight improvement in reporting between 1994-1995 and 1999-2000 combined （8.6±2.1） and 2004-2005 and 2009-2010 combined （10.1 ±1.8; difference 1.5, 95% confidence interval, 1.1 to 1.9, P〈0.01）. Conclusion： Quality of reporting for RCTs of acupuncture treatment conducted in China have generally improved since the introduction of the STRICTA and CONSORT guidelines.

Ginkgo Leaf Extract and Dipyridamole Injection as Adjuvant Treatment for Angina Pectoris:A Meta-Analysis of 41 randomized controlled trials

Objective： To provide information about the effectiveness and safety of Ginkgo Leaf Extract and Dipyridamole Injection （GD） as one adjuvant therapy for treating angina pectoris （AP） and to evaluate the relevant randomized controlled trials （RCTs） with meta-analysis. Methods： RCTs concerning AP treated by GD were searched in China Biology Medicine Disc （SinoMed）, PubMed, the China National Knowledge Infrastructure Database （CNKI）, the Chinese Scientific Journals Database （VIP）, Wanfang Database, Embase, and the Cochrane Library, from inception to February, 2017. The Cochrane Risk Assessment Tool was adopted to assess the methodological quality of the RCTs. The Review Manager 5.3 software was utilized to conduct the meta-analysis. Results： A total of 41 RCTs involving 4,462 patients were included in the meta-analysis. The results indicated that the combined use of GD and Western medicine （WM） against AP was associated with a higher total effective rate [risk ratio （RR）=1.25, 95% confidence interval （CI）： 1.21-1.29, P〈0.01], total effective rate of electrocardiogram （RR=1.29, 95% CI： 1.21-1.36, P〈0.01）. Additional, GD combined with WM could decrease the level of plasma viscosity [mean difference （MD）=-0.56, 95% CI： -0,81 to -0.30, P〈0.01], flbrinogen [MD=-1.02, 95% CI： -1.50 to -0.54, P〈0.01], whole blood low shear viscosity [MD=-2.27, 95% CI： -3.04 to -1.49, P〈0.01], and whole blood high shear viscosity （MD=-0.90, 95% CI： 1.37 to -0.44, P〈0.01）. Conclusions： Comparing with receiving WM only, the combine use of GD and WM was associated with a better curative effect for patients with AP. Nevertheless, limited by the methodological quality of included RCTs more large-sample, multi-center RCTs were needed to confirm our findings and provide further evidence for the clinical utility of GD.

Acupuncture for cerebral palsy: a meta-analysis of randomized controlled trials

OBJECTIVE： To evaluate the efficacy and safety of acupuncture therapy for children with cerebral palsy. DATA SOURCES： We conducted electronic searches of PUBMED（1950/2017）, EMBASE（1974/2017）, Science Direct（1986/2017）, Academic Source Premier（1887/2017）, the Cochrane Library（Issue 4, April 2017）, Science Citation Index Expanded（1900/2017）, China National Knowledge Infrastructure（1915/2017）, China Biological Medicine（1990/2017-04）, Wan Fang（1980/2017）, VIP（1989/2017）, and Chinese Science Citation Database（1989/2017）. DATA SELECTION： We included randomized controlled trials that aimed to compare the effect of acupuncture plus rehabilitation training versus rehabilitation training alone. Data about functional motor abilities, daily activity/social participation, effective rate, intellectual development, and adverse effects were included. We used Revman 5.2 software for statistical analysis. OUTCOME MEASURES： The primary outcomes included functional motor abilities, daily activity, and effective rate. The secondary outcomes included intellectual development and adverse ***： Twenty-one studies with a total of 1718 participants met the inclusion criteria. The effect size of gross motor function（SMD = 0.64, 95% CI： 0.52 to 0.76, P 〈 0.00001; I^2 = 0%, P = 0.69; in 13 studies with 1144 patients） and the total effective rate（RR = 1.28, 95% CI： 1.20 to 1.37, P 〈 0.00001; I^2 = 18%, P = 0.27; in 12 studies with 1106 patients） suggested that acupuncture plus rehabilitation produced a significant improvement in gross motor function and a high total effective rate. Objective： To evaluate the efficacy of oral Chinese medicine （CM） in comparison with donepezil, a cholinesterase inhibitor （ChEI）, for the treatment of Alzheimer＇s disease （AD）. Methods： randomized controlled trials （RCTs） have been searched, and the effect of CM compared with donepezil in AD has been investigated. An electronic search of MEDLINE, Excerpta Medica Database （EMBASE）, Cochrane Library, Chinese Biological Medicine Database （CBMdisc）, and China National Knowledge Infrastructure （CNKI） to identify articles in English and Chinese from the inception of the database until October 18, 2015. A modified Jadad score （7-points） to judge the methodological quality of studies, comprehensive meta-analysis was performed with Cochrane Collaboration Revman 5.3. Dichotomous data were analyzed by relative risk （RR） with a 95% confidence interval （CI）, while continuous variables were analyzed by using mean differences （MD） with 95% CI for effect size. Results： Six studies involving 596 AD patients through Jadad assessment with low bias were included in the meta-analysis. No significant difference was observed in cognitive improvement and daily abilities of patients using the Mini Mental State Examination （MMSE）（MD： 0.69, 95% CI： -0.17 to 1.56） and Activities of Daily Living （ADL） scale （MD： 0.94, 95% CI： -1.54 to 3.43）. There were no significant differences in status of illness or MD for mUd-moderate AD patients at 24 weeks （MD： 0.62, 95% CI： -2.99 to 4.23） and 48 weeks （MD： -0.73, 95% CI： -5.02 to 3.56）. Severe AD patients were also assessed at 24 weeks （MD： 3.13, 95% CI： -6.92 to 13.18） and 48 weeks （MD： 4.23, 95% CI： -6.38 to 14.84）. Furthermore, compared with donepezil, Xin （Heart）-regulating CM and Shen （Kidney）-tonifying groups were observed （MD： -1.50, 95% CI： -3.08 to 0.08; MD： -1.92, 95% CI： -3.50 to -0.33; respectively）. CM had fewer side effects in AD patients. Conclusion： Compared with donepezil, oral CM showed no significant difference in effectiveness in AD

This invited paper reviews recent initiatives in teaching about randomized field trials at the graduate level in education research. Reports in a book on randomized trials by Mosteller and Boruch are used as a benchmark. The initiatives have been driven heavily by recent governmental emphasis on ＂evidence based policy＂ in education, criminology, welfare and other sectors. This policy has been backed, in the US at least, by substantial investments in mounting randomized controlled trials to evaluate curriculum packages and programs in grades K-12, crime prevention programs, welfare, and other work. Accelerated growth in randomized trials has entailed cross discipline pre-doctoral and post-doctoral institutes and graduate education programs aimed at enhancing the quality in trial design, execution, and analysis of results. Specific illustrations of graduate education in this area of statistics are taken mainly from experience at the University of Pennsylvania＇s Graduate School of Education and selected other institutions.
