Analgesic Effects of Two Types of Spinal Manipulation in Acute Lumbar radiculopathy Model Rats

Objective:To compare the analgesic effects of two types of spinal manipulation(SM)in acute lumbar radiculopathy(ALR)model rats induced by self-transplantation of autologous nucleus pulposus(ANP),and clarify the therapeutic ***:Totally 108 male Sprague-Dawley rats were randomly divided into 6 groups by a random number table(18 rats in each group),including a blank group with no interference,a sham operation group with a surgery by making a local soft tissue incision on the left side of L5-6 vertebral segment,a model group with ALR of L5 extraforaminal nerve by ANP self-transplantation without other interference,a sham manipulation(SMA)group with simulating physical rotation,as well as a mobilization(MOB)group with simulating low-velocity and variable-amplitudle rotation and a manipulation(MAN)group with simulating high-velocity and low-amplitude *** interventions in SMA,MOB,and MAN groups started 1 day after modeling followed by another 5 treatments at days 3,5,8,10 and *** in the other3 groups did not receive any special *** pain tests of 50%mechanical pain withdrawal threshold(50%PWT)and paw withdrawal latency(PWL)were conducted 1 day before operation followed by another 10 tests on days 1-7,10,12 and *** expression of nitric oxide synthase(NOS)was investigated on days 5 and 12 after ***:After 3 experimental SM interventions,50%PWT and PWL were higher in the MAN group than the SMA group on days 6 and 7,and higher on days 10,12 and14 postoperatively(P<0.05 or P<0.01),while the same indices were significantly higher in the MOB group than MAN group on days 1-4(P<0.05 or P<0.01).The expression of NOS was lower in the MAN and MOB groups than SMA group on day 12 postoperatively(P<0.01).Conclusions:Both manipulation and mobilization produced better results than sham interference in relieving pain by reducing neuroinflammation *** the early period,compared with manipulation,mobilization

OBJECTIVE： To ascertain if it is effective to use extradural injections and manual repositioning of the spine to treat megalgia caused by cervical spondy- Iotic radiculopathy （CSR）. METHODS： Patients with megalgia caused by CSR were divided into a treatment group （n=46） treated by extradural injection and manual repositioning of the spine and a control group （n=46） treated by a conventional method. RESULTS： The prevalence of cure was 58.69% and the total prevalence of effective cure in the treatment group was 97.83% and was 23.91% and 78.26%, respectively, in the control group： this difference between the two groups was significant （P〈 0.05）. CONCLUSION： Treatment of megalgia caused by CSR by extradural injections and manual repositioning of the spine has a good curative effect with rapid analgesia and short therapeutic course.

Background and Aims: Low back pain (LBP) is considered one of the most common health conditions in the world right now, and it affects many individuals throughout different stages of their lives. Chronic LBP (CLBP) was estimated to be between 5% and 10%, defined as LBP that lasts for 12 weeks. The most common causes of CLBP with radiculopathy are lumbar disc prolapse (LDP) and degenerative facet osteoarthropathy (DFO);the aim of this study is to investigate the efficacy of ultrasound (US) guided, fluoroscopy (FL) guided, Caudal Epidural Steroid Injection (CESI), lumbar epidural steroid injections (LESI), and blinding lumbosacral steroid injections (LSPSI) in patients with CLBP with radiculopathy. Patients and Methods: This is a randomized prospective study that was conducted at the department of rheumatology at Al Azhar University Hospital in Egypt between November 2020 and August 2021. A total of 100 patients with refractory CLBP with radiculopathy were enrolled in the study. Consequently, they were divided into 2 groups: the first consisted of fifty patients with CLBP and radiculopathy caused by LDP, as determined by lumbosacral magnetic resonance imaging (MRI), and the second group consisted of fifty patients with refractory low back pain and radiculopathy caused by DFO, as determined by lumbosacral plain x-rays and lumbosacral MRI. The following procedures were performed: US-guided CESI, FL-guided CESI, FL-guided LESI, US-guided LESI, and blinding LSPSI. Results: In the LDP group, there is a statistically significant difference between considered spinal nerve roots as regards Visual Analogue Scale (VAS) (at 2 months). Likewise, a statistically significant difference was found between blinding LSPSI and US-Guided LESI with respect to VAS (baseline) and VAS (2 months) (P-value = 0.018 and 0.003, respectively). Objective:To observe the clinical efficacy of warm needling moxibustion plus Mulligan dynamic joint mobilization for cervical ***:A total of 60 cases with cervical radiculopathy were randomly divided into group A,group B and group C by random digital table,20 cases in each *** patients in three groups were treated with similar warm needling *** A only received warm needling moxibustion;group B was treated with warm needling moxibustion plus Mulligan dynamic joint mobilization;group C was treated with warm needling moxibustion plus cervical *** the treatment and after 12 sessions of treatment,the patients in the three groups were assessed for score of pain and range of motion(ROM).Results:By 12 sessions of the treatments,the scores of pain were obviously decreased in the three groups,with statistical significances(all P〈0.05),and the pain scores of group B and group C were significantly different from the score of group A(both P〈0.05).ROM of the neck was increased than before the treatment in the three groups,with statistical significances(all P〈0.05),and the ROM of the neck of group B was significantly different from those of group A and group C(both P〈0.05).Conclusion:Warm needling moxibustion plus Mulligan dynamic joint mobilization can effectively improve the neck ROM and relieve pain in patients with cervical radiculopathy.
