Benign prostatic hyperplasia(BPH)occurs commonly in older males,with its prevalence increasing linearly with age,reaching approximately 50%and 80%in males aged 60 and 90 years,respectively.[1]Histologically,BPH is characterized by smooth muscle and epithelial cell proliferation within the prostatic transitional zone.Subsequent anatomical compression of the urethra causes increased bladder outlet resistance and detrusor muscle irritability,usually resulting in lower urinary tract symptoms(LUTS),which can be bothersome and make patients seek medical care.

We evaluated whether the prostate-specific antigen (PSA)mass or free PSA (fPSA)mass (i.e.,absolute amount of total circulating PSA or fPSA protein,respectively),versus serum PSA or fPSA concentration,improves the accuracy of predicting the total prostate volume (TPV)in relation to obesity.Among men whose multicore (≥12)transrectal prostate biopsy was negative,586 who had a PSA of <10 ng ml^-1 and underwent the fPSA test prior to biopsy were enrolled.The PSA mass or fPSA mass (pg)was calculated by multiplying the serum level by plasma volume.At each TPV cut-off point (30 ml,40 ml,and 50 ml),the areas under the receiver operating characteristics curve (AUCs)of each variable were compared in obesity-based subgroups.AUCs of fPSA and fPSA mass for predicting TPV were significantly larger than those for PSA and PSA mass by 8.7%-12.1%at all cut-off points. Subgroup analyses based on obesity showed that,although PSA mass and fPSA mass enhanced accuracy by 4%(P =0.031)and 1.8%(P =0.003),respectively,for determining TPVs of ≥30 ml and ≥50 ml in obese and overweight men,they did not improve the accuracy in most other combinations of the degrees of obesity with TPV cut-off points.Thus,compared with serum PSA or fPSA,the absolute amount of PSA or fPSA protein mass improved the accuracy of predicting TPV in obese men very minimally and only for certain TPV cut-off points.Hence,these indicators may not provide clinically meaningful improvement in predicting TPV in obese men.

One of the major concerns for the prescription of medications containing testosterone(T)in the aging men with T deficiency(TD)is prostate disorders(such as benign prostatic hyperplasia,BPH)and their related symptoms(lower urinary tract symptoms,LUTS).This is because prostate is an androgen-responsive gland;however,the androgen dependence of prostate growth,well demonstrated in earlier phases of life,has no clear evidence in late adulthood and senescence.In fact,after the age of 40 years,BPH becomes increasingly more prevalent.On the other hand,in men older than 40 year,a progressive decline in T is observed.These epidemiological data are a starting point for questioning a putative detrimental role of T on the prostate health in the ageing man.

Dear Editor, We describe a rare case of prostatic stromal tumor of uncertain malignant potential （STUMP） presenting as a huge bladder mass. Based on the previous literature, prostatic STUMP has a low incidence of morbidity. To the best of our knowledge, this is the first report of prostatic STUMP presenting as a huge bladder mass.

prostatic small cell carcinoma (PSCC) is a distinct clinical phenotype of prostate cancer. Although rare, this phenotype is highly aggressive with very high mortality. Due to this, it is imperative for clinicians to be aware of it, diagnose it early and treat it appropriately. In this article we discuss the current literature, outline a plan for its diagnosis and management, and highlight latest research on this topic.

目的:探讨多西紫杉醇联合曲格列酮(troglitazone,TGZ)对人前列腺癌裸鼠移植瘤的抑制作用,并初步探索其作用机制。方法:建立人前列腺癌裸鼠移植瘤模型,观察多西紫杉醇联合TGZ对移植瘤生长的抑制作用,FCM法检测移植瘤细胞的凋亡率,免疫组织化学法检测移植瘤组织中bcl-2和bax蛋白的表达情况。结果:与对照组比较,多西紫杉醇联合TGZ可明显抑制人前列腺癌裸鼠移植瘤的生长,肿瘤体积和质量明显下降,细胞凋亡率明显上升;免疫组织化学法检测结果显示,移植瘤组织中bax蛋白的表达明显增强,bcl-2蛋白的表达明显减弱。结论:多西紫杉醇联合TGZ对前列腺癌有一定的抑制作用,其作用机制可能是通过上调促凋亡蛋白bax的表达及下调抗凋亡蛋白bcl-2的表达,导致肿瘤细胞加速凋亡而实现的。

Objective:To study the correlation of urodynamic characteristics with insulin resistance and serum damage media in patients with diabetes and benign prostatic hyperplasia (BPH). Methods:45 patients with type 2 diabetes mellitus and BPH treated in our hospital between May 2014 and August 2016 were selected as DM+BPH group, 58 patients with BPH alone were selected as BPH group, and 50 healthy volunteers were selected as control group. Urodynamic tester was used to measure the maximum flow rate (MFR), postvoid residual (PVR) and detrusor pressure at maximum flow rate (Pdet), and serum was collected to determine insulin resistance indexes and oxidative stress indexes. Results:MFR and Pdet of DM+BPH group were significantly lower than those of control group (P<0.05) while PVR was significantly higher than that of control group (P<0.05);MFR of BPH group was significantly lower than that of control group (P<0.05) while PVR and Pdet were significantly higher than those of control group (P<0.05);MFR and Pdet of DM+BPH group were significantly lower than those of BPH group (P<0.05) while PVR was significantly higher than that of BPH group (P<0.05);insulin secretion index (HOMA-β), insulin sensitive index (ISI) as well as serum manganese superoxide dismutase (MnSOD), copper-zinc superoxide dismutase (CuZnSOD) and glutathione peroxidase (GPx) levels of DM+BPH group and BPH group were significantly lower than those of control group (P<0.05) while insulin resistance index (HOMA-IR) as well as serum thioredoxin (Trx) and thioredoxin-interacting protein (TXNIP) levels was significantly higher than those of control group (P<0.05);HOMA-β, ISI as well as serum MnSOD, CuZnSOD and GPx levels of DM+BPH group were significantly lower than those of BPH group (P<0.05), positively correlated with MFR and Pdet, and negatively correlated with MFR, and HOMA-IR as well as serum Trx and TXNIP levels was significantly higher than those of BPH group (P<0.05), negatively correlated with MFR and Pdet, and positively correlated with MFR.
