Short-term therapeutic outcomes of robotic-assisted laparoscopic radical prostatectomy for oligometastatic prostate cancer:a propensity score matching study

Background:The role of local treatment in oligometastatic prostate cancer(PCa)is gaining interest with the oligometastases hypothesis proposed and the improvement of various surgical methods and techniques.This study aimed to compare the short-term therapeutic outcomes of robotic-assisted laparoscopic radical prostatectomy(RALP)for oligometastatic prostate cancer(OPC)vs.localized PCa using propensity score matching.Methods:Totally 508 consecutive patients underwent RALP as a first-line treatment.The patients were divided into two groups according to oligometastatic state:the OPC group(n=41)or the localized PCa group(n=467).Oligometastatic disease was defined as the presence of two or fewer suspicious lesions.The association between the oligometastatic state and therapeutic outcomes of RALP was evaluated,including biochemical recurrence(BCR)and overall survival(OS).A Cox proportional hazards model was used to assess the possible risk factors for BCR.Results:Totally 41 pairs of patients were matched.The median operative time,the median blood loss,the overall positive surgical margin rate,the median post-operative hospital stays,and the post-operative urinary continence recovery rate between the two groups showed no statistical significance.The 4-year BCR survival rates of the OPC group and localized PCa group were 56.7%and 60.8%,respectively,without a significant difference(P=0.804).The 5-year OS rates were 96.3%and 100%,respectively(P=0.326).Additionally,the results of Cox regression showed that oligometastatic state was not an independent risk factor for BCR(P=0.682).Conclusions:Our findings supported the safety and effectiveness of RALP in OPC.Additionally,oligometastatic state and sites did not have an adverse effect on BCR independently.

Recent observational studies showed that breast-conserving surgery(BCS) resulted in superior survival compared to mastectomy in breast cancer patients. This study compared the clinical outcomes of BCS and mastectomy using propensity score(PS)matching analysis, which had advantages over conventional methods in reducing bias. Nonmetastatic breast cancer patients who underwent BCS and mastectomy were matched 1:1 based on their PS. We used the Kaplan-Meier method and Cox-regression model to estimate the treatment effects. A total of 2,866 patients with a median follow-up time of 67 months were included in the original study population. Although the mastectomy cohort(N=1,219) had more advanced disease compared to the BCS cohort(N=1,647), LRFS was similar between the two groups(93.8% vs. 92.4%, P>0.05). BCS(vs. mastectomy) was associated with improved DFS(73.8% vs. 58.7%, P0.05) or CSS(87.5% vs. 84.9%, P>0.05). We found that PS methods reduce bias when estimating treatment effects using observational data. BCS and mastectomy show equivalent outcomes in nonmetastatic breast cancer patients.

Developing coal burst propensity index method for Australian coal mines

Coal burst is the violent failure of overstressed coal, and it is often accompanied by sound, coal ejection and seismic events. It is subsequently recognized as a serious safety risk of Australia after double fatalities coal burst happened at Austar Coal Mine. Considering the increasing trend of coal burst severity and frequency with mining depth, it is an urgent task to develop the coal burst risk assessment methods for Australia underground coal mines. Coal burst propensity index method is a widely used method of burst risk evaluation of coal as it is summed up from the coal burst research and practice of many countries.This paper presents the experimental and theoretical research of coal burst propensity index method for coal burst risk assessment in Australia. The definition of four indexes including elastic strain energy index(W_(ET)), bursting energy index(K_E), dynamic failure time(DT) and uniaxial compression strength(RC)is introduced in the first part. Then, the standard laboratory test process and test parameter of coal burst propensity index is presented. DT test is conducted with 0.3 mm/min displacement control loading rate while other test is with 0.5 mm/min. Besides, modified data processing and risk classification method of test are proposed. Differentiate analysis of stress-strain curve is adopted in the data processing of DT and KEindex. A four level risk classification form of burst risk is recommended for Australian underground coal mines. Finally, two likely improvement methods of W_(ET) test, including volumetric strain indicator method and theoretical calculation method, are discussed.

Comparison of diagnostic efficacy between transrectal and transperineal prostate biopsy:a propensity score-matched study

This study compared the diagnostic efficacy of transrectal ultrasound(TRUS)-guided prostate biopsy(TRBx)and transperineal prostate biopsy(TPBx)in patients with suspected prostate cancer(PCa).We enrolled 2962 men who underwent transrectal(n=1216)or transperineal(n=1746)systematic 12-core prostate biopsy.Clinical data including age,prostate-specific antigen(PSA)level,and prostate volume(PV)were recorded.To minimize confounding,we performed propensity score-matching analysis?We measured and compared PCa detection rates between TRBx and TPBx,which were stratified by clinical characteristics and Gleason scores.The effects of clinical characteristics on PCa detection rate were assessed by logistic regression.For all patients,TPBx detected a higher proportion of clinically significant PCa(Ppropensity score-matching analysis showed that the detection rates in TRBx were higher than those in TPBx for patients aged≥80 years(80.4%vs 56.5%,P=0.004)and with PSA level 20.1-100.0 ng ml^-1(80.8%vs 69.1%,P=0.040).In conclusion,TPBx was associated with a higher detection rate of clinically significant PCa than TRBx was;however,because of the high detection rate at certain ages and PSA levels,biopsy approaches should be optimized according to patents'clinical characteristics.

BACKGROUND Robotic surgery has been considered to be significantly better than laparoscopic surgery for complicated procedures.AIM To explore the short-term effect of robotic and laparoscopic spleen-preserving splenic hilar lymphadenectomy(SPSHL)for advanced gastric cancer(GC)by Huang’s three-step maneuver.METHODS A total of 643 patients who underwent SPSHL were recruited from April 2012 to July 2017,including 35 patients who underwent robotic SPSHL(RSPSHL)and 608 who underwent laparoscopic SPSHL(LSPSHL).One-to-four propensity score matching was used to analyze the differences in clinical data between patients who underwent robotic SPSHL and those who underwent laparoscopic SPSHL.RESULTS In all,175 patients were matched,including 35 patients who underwent RSPSHL and 140 who underwent LSPSHL.After matching,there were no significant differences detected in the baseline characteristics between the two groups.Significant differences in total operative time,estimated blood loss(EBL),splenic hilar blood loss(SHBL),splenic hilar dissection time(SHDT),and splenic trunk dissection time were evident between these groups(P<0.05).Furthermore,no significant differences were observed between the two groups in the overall noncompliance rate of lymph node(LN)dissection(62.9%vs 60%,P=0.757),number of retrieved No.10 LNs(3.1±1.4 vs 3.3±2.5,P=0.650),total number of examined LNs(37.8±13.1 vs 40.6±13.6,P=0.274),and postoperative complications(14.3%vs 17.9%,P=0.616).A stratified analysis that divided the patients receiving RSPSHL into an early group(EG)and a late group(LG)revealed that the LG experienced obvious improvements in SHDT and length of stay compared with the EG(P<0.05).Logistic regression showed that robotic surgery was a significantly protective factor against both SHBL and SHDT(P<0.05).CONCLUSION RSPSHL is safe and feasible,especially after overcoming the early learning curve,as this procedure results in a radical curative effect equivalent to that of LSPSHL.

First-line pemetrexed-platinum doublet chemotherapy with or without bevacizumab in non-squamous non-small cell lung cancer: A real-world propensity score-matched study in China

Objective: To evaluate the efficacy and safety profile of first-line bevacizumab(Bev)-containing pemetrexedplatinum chemotherapy in a real-world Chinese cohort with advanced non-squamous non-small cell lung cancer(NS-NSCLC).Methods: A total of 415 eligible patients with NS-NSCLC who received first-line pemetrexed-platinum chemotherapy at National Cancer Center/National Clinical Research Center for Cancer/Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College between February 2010 and September 2017 were reviewed retrospectively: 309 Bev(-) and 106 Bev(+) cases. Bev was administered at 7.5 mg/kg every 3 weeks in the Bev(+) group. To reduce the risk of a selection bias, a propensity score-matching(PSM) was conducted and 105 pairs of Bev(-) and Bev(+) cases were identified.Results: The median duration of follow-up was 15.8 months. The median progression-free survival(PFS) was prolonged significantly in the Bev(+) group than in the Bev(-) group in overall(9.8 vs. 7.8 months, P=0.006) and PSM pairs(9.8 vs. 6.6 months, P<0.001). Moreover, patients receiving maintenance therapy with pemetrexed plus Bev had longer PFS than those interrupted after induction chemotherapy, or those receiving mono-maintenance with pemetrexed(12.3 vs. 4.8 vs. 8.6 months;P<0.001). Multivariate analyses revealed Bev to be one of the favorable prognostic factors for PFS, along with the predictor of maintenance therapy.Conclusions: First-line induction and maintenance therapy with Bev(7.5 mg/kg every 3 weeks) combined with pemetrexed-platinum chemotherapy was efficacious and superior to non-Bev chemotherapy in Chinese patients with advanced NS-NSCLC.

Offspring of aged mothers of rotifer Brachionus calyciflorus shows lower sexual propensity than their elder siblings under crowded conditions

Maternal eff ects are one of the most interesting topics in evolutionary ecology as they can aff ect the rate of evolution and population dynamics by phenotypic manipulation of off spring related to fi tness. At present, studies examining the interaction between maternal environment eff ects and maternal age (birth order) eff ects are scarce. We designed an experiment to reveal whether environmentally induced maternal manipulation works equally on off spring across birth orders in rotifer Brachionus calycifl orus. In the experiment, newborn amictic mothers (F0) were cultured parallelly in low (LD) and high (HD) population density. Off spring (F1) of young (YF0) and old (OF0) mothers in both LD and HD groups were cultured under a crowded condition and the mixis ratios of both F1 and F2 were examined. Results show that F1 from HD-YF0 had a signifi cantly higher mixis ratio and those amictic ones produced daughters (F2 ) with higher mixis ratio in response to crowding than those from HD-OF0. In contrast, no such diff erences among F1 siblings were found in the LD-F0 group. Therefore, the eff ect of maternal age on the sexual propensity of the off spring can be aff ected independently by the maternal population density.

为了合理地解释集合中单个事件的客观概率，波普尔在对频率理论的修改基础上提出了概率的倾向性解释。在波普尔看来，哥本哈根诠释对量子理论的解释过于主观主义，为了维护科学实在论，必须破除量子理论的神秘性。于是，波普尔以概率的倾向性解释对量子理论做了科学哲学意义的实在论诠释。然而，波普尔对量子理论的实在论诠释存在诸多问题，它既混淆了量子力学与统计力学，夸大了其与哥本哈根学派之间的对立，同时也并没有解决所谓的单称概率问题，甚至波普尔还误解了量子理论的哥本哈根诠释。究其原因，这既与波普尔对量子理论的解释立场过于狭隘有关，也与他对量子理论的物理内涵理解不够准确有关，还与他将布尔逻辑规则应用于量子理论的非布尔逻辑空间有关，且最后一点是其中重要原因。

AIM:To compare the prognoses of hepatocellular carcinoma(HCC)patients that underwent anatomic liver resection(AR)or non-anatomic liver resection(NAR)using propensity score-matched populations.METHODS:Between January 2002 and December2010,268 consecutive HCC patients,including 110 and158 patients that underwent AR and NAR,respectively,were retrospectively enrolled in this study.Forty-four patients from each group were selected and matched using logistic multivariate analysis followed by propensity score analysis.RESULTS:In the whole analysis set,the histological background of the liver,liver function,and tumor marker levels differed significantly among the groups.Although the overall survival(OS)and recurrence-free survival rates of the two groups did not differ significantly in the whole analysis set,the OS of the AR group was significantly longer than that of the NAR group after propensity matching(76.2±6.3 mo vs 58.9±6.3mo;P=0.0039).Although AR(HR=0.456,P=0.039)was found to be a prognostic factor in the univariate analysis,only vascular invasion(HR=0.228,P=0.002)and the hepatocyte growth factor level(HR=52.366,P=0.035)were subsequently found to be independent prognostic factors.CONCLUSION:AR conveys a survival advantage over NAR in specific subpopulations of HCC patients with tumors of less than 5 cm in diameter,single tumor,and good liver function.

AIM:To investigate the outcomes of pancreas-sparing duodenectomy(PSD)with regional lymph node dissection vs pancreaticoduodenectomy(PD).METHODS:Between August 2001 and June 2014,228 patients with early-stage ampullary carcinoma(Amp Ca)underwent surgical treatment(PD,n=159;PSD with regional lymph node dissection,n=69).The patients were divided into two groups:the PD group and the PSD group.propensity scoring methods were used to select patients with similar disease statuses.A total of 138 matched cases,with 69 patients in each group,were included in the final analysis.RESULTS:The median operative time was shorter among the patients in the PSD group(435 min)compared with those in the PD group(481 min,P=0.048).The median blood loss in the PSD group was significantly less than that in the PD group.The median length of hospital stay was shorter for patients in the PSD group vs the PD group.The incidence of pancreatic fistula was higher among patients in the PD group vs the PSD group.The 1-,3-,and 5-year overall survival and disease-free survival rates for patients in the PSD group were 83%,70%,44%and 73%,61%,39%,respectively,and these values were not different than compared with those in the PD group(P=0.625).CONCLUSION:PSD with regional lymph node dissection presents an acceptable morbidity in addition to its advantages over PD.PSD may be a safe and feasible alternative to PD in the treatment of earlystage Amp Ca.
