Long-term outcome of percutaneous or surgical revascularization with and without prior stroke in patients with threevessel disease

OBJECTIVE To determine whether high-risk patients with three-vessel disease(TVD)with and without prior stroke preferen-tially benefit from three strategies[percutaneous coronary intervention(PCI),coronary artery bypass grafting(CABG)and medical therapy(MT)].METHODS A total of 8943 patients with TVD were included in the study.Patients enrolled were stratified into two categories according to the presence or absence of prior stroke history.The primary endpoint was all-cause death.Secondary endpoints in-cluded stroke and major adverse cardiac and cerebrovascular event(MACCE),a composite of death,myocardial infarction(MI),unplanned revascularization and stroke.RESULTS prior stroke was present in 888 patients(9.9%).These patients were older and had higher rates of comorbidities.Du-ring a median follow-up of 7.5 years,patients with prior stroke were strongly associated with increased risks of all-cause death,cardiac death,stroke and MACCE,even after adjusting for confounding variables and results been consistent across either treat-ment subgroup(PCI,CABG and MT)(all adjusted Pprior stroke his-tory and treatment strategies.Revascularization strategy(PCI or CABG)was associated with a lower incidence of all-cause death and MACCE compared with MT alone,and favorable rates of MACCE,MI and unplanned revascularization in the CABG group compared with the PCI group,but with similar rate of all-cause death regardless of prior stroke history.The prevalence of stroke was significantly higher after CABG when compared with PCI or MT in no prior stroke patients[hazard ratio(HR)=1.429,95%CI:1.132-1.805 for CABG vs.MT;HR=1.703,95%CI:1.371-2.116 for CABG vs.PCI].CONCLUSIONS Patients with TVD and prior stroke have poor clinical outcomes.It is essential to balance benefit and risk when determining the optimal treatment strategy for TVD with and without prior stroke.

In Indonesia,undervirilisation in 46,XY males is the most common form of difference of sex development(DSD).This can include hypospadias(misplacement of the urethra),micropenis,bifid scrotum,and undescended testis[1].Undervirilisation or 46,XY DSD can be associated with a number of congenital syndromes,including Smith-Lemli-Opitz Syndrome(OMIM 602858),caused by an inborn error of cholesterol synthesis,and characterised by growth delay,intellectual disability,microcephaly,distinctive facial features,cleft palate,limb anomalies,and hypospadias[2]or Opitz syndrome(also known as Opitz G/BBB syndrome).Opitz syndrome can be caused by variants in the X-linked midline 1(MID1)gene(Type I)or in an autosomal dominant manner by monoallelic variants in sperm antigen with calponin homology and coiled-coil domains 1-like(SPECC1L)on chromosome 22q11.2(Type II)[3].Opitz syndrome is characterised by hypospadias,hypertelorism,cleft lip/palate,and heart defects[4].The prevalence of X-linked Opitz syndrome is estimated to be from 1 in 50000 to 1 in 100000 males[5].Recognition of a syndrome informs appropriate clinical management and patient care.Therefore,although these syndromes are rare,hypospadias may be diagnosed before the emergence of other comorbidities meaning that it is crucial for clinicians to perform a thorough clinical evaluation with syndromic causes in mind.

Effect of prior martensite on bainite transformation and microstructure of high-carbon nano-bainitic steel

Bainite transformation kinetics,microstructure,and mechanical properties were comparably investigated by X-ray diffraction,scanning electron microscopy,electron backscatter diffraction,transmission electron microscopy observation,and tensile and impact tests for two treatments:with and without quenching before isothermal bainite transformation at 300°C.Both the incubation time and growth stage of bainite transformation are accelerated by the introduction of prior martensite,especially the former.In addition,the bainite microstructure around the prior martensite is refined and shows a similar orientation with the martensite due to the strain field caused by the existence of prior martensite.The finer bainite microstructure and prior martensite improve the strength and hardness of the high-carbon nano-bainitic steel.Although the presence of prior martensite and low misorientation relationship between the martensite and the adjacent bainite microstructure are prejudicial to impact toughness,the nanoscale bainite microstructure can inhibit the crack propagation and then improve the impact toughness of the high-carbon nano-bainitic steel.

Working temperature,sensitivity,and selectivity are some of the characteristics of the applied gas sensors.How to design and fabricate an ideal gas sensor working at room temperature is still challenging and attracting lots of interest.Two-dimensional(2D)materials with ultra-thin structure have been demonstrated as a family of ideal candidates to achieve this goal.Among them,Ti_(3)C_(2)T_(x) MXene,a kind of layered sheet synthesized by selectively etching MAX phases materials,shows remarkable potential to be the sensitive materials solely or in a composite.However,their designing rules are still lacking critical thinking from the viewpoint of the intrinsic property of Ti_(3)C_(2)T_(x) MXene based materials.In this article,two critical features,i.e.,the thickness of the sensitive materials,and the scope of the analytes,are elaborated towards Ti_(3)C_(2)T_(x) MXene based gas sensors after characterizing the performance of sensing reducing gases(NH3 and CO)and oxidizing gas(NO2).First,the thinner the Ti_(3)C_(2)T_(x) MXene sensitive layer,the better the sensitivity.Second,the Ti_(3)C_(2)T_(x) MXene based gas sensor is not suitable for strong and moderate oxidation gas due to its ease of oxidation.These two rules are demonstrated,and could be considered with priority both in the future researches and practical applications.

Image Blind Deblurring Using an Adaptive Patch prior

Image blind deblurring uses an estimated blur kernel to obtain an optimal restored original image with sharp features from a degraded image with blur and noise artifacts. This method, however, functions on the premise that the kernel is estimated accurately. In this work, we propose an adaptive patch prior for improving the accuracy of kernel estimation. Our proposed prior is based on local patch statistics and can rebuild low-level features,such as edges, corners, and junctions, to guide edge and texture sharpening for blur estimation. Our prior is a nonparametric model, and its adaptive computation relies on internal patch information. Moreover, heuristic filters and external image knowledge are not used in our prior. Our method for the reconstruction of salient step edges in a blurry patch can reduce noise and over-sharpening artifacts. Experiments on two popular datasets and natural images demonstrate that the kernel estimation performance of our method is superior to that of other state-of-the-art methods.

Single Image Dehazing with V-transform and Dark Channel prior

Single image dehazing algorithm based on the dark channel prior may cause block effect and color distortion.To improve these limitations,this paper proposes a single image dehazing algorithm based on the V-transform and the dark channel prior,in which a hazy RGB image is converted into the HSI color space,and each component H,I and S is processed separately.The hue component H remains unchanged,the saturation component S is stretched after being denoised by a median filter.In the procession of intensity component,a quad-tree algorithm is presented to estimate the atmospheric light,the dark channel prior and the V-transform are used to estimate the transmission map.To reduce the computational complexity,the intensity component I is decomposed by the V-transformfirst,coarse transmission map is then estimated by applying the dark channel prior on the low frequency reconstruction image,and the guided filter is finally employed to refine the coarse transmission map.For images with sky regions,the haze removal effectiveness can be greatly improved by just increasing the minimum value of the transmission map.The proposed algorithm has low time complexity and performs well on a wide variety of images.The recovered images have more nature color and less color distortion compared with some state-of-the-art methods.

Effect of prior Cold Deformation on the Stability of Retained Austenite in GCr15 Bearing Steel

In this work, the effect of prior cold deformation on the stability of retained austenite in GCr15 bearing steel was investigated after quenching and tempering treatment. The thermal stability was evaluated by calculating thermal activation energy for decomposition of retained austenite using differential scanning calorimeter. The mechanical stability was investigated according to the strain-induced martensitic transformation behavior of retained austenite under the standard compression testing. It is found that the prior cold deformation not only accelerates the carbide dissolution during the austenitization process but also contributes to the carbon partitioning in the tempering stage due to the higher density of phase boundaries, which results in the improvement of the thermal stability of retained austenite. Due to the enhanced carbide dissolution, the higher carbon content in the prior austenite will intensify the isotropic strain of martensitic transformation. As a consequence, the film-like retained austenite is likely to form under a higher hydrostatic pressure and thus shows a higher mechanical stability. Additionally, it is noteworthy that the benefits of the prior cold deformation to the stability of retained austenite would be saturated when the cold deformation degree is larger than 40%.

Could Gravitons from a prior Universe Survive a (LQG Inspired) “Quantum Bounce” to Re-Appear in Our Present Universe?

We ask the question if a formula for entropy, as given by with a usual value ascribed of initial entropy of the onset of inflation can allow an order of magnitude resolution of the question of if there could be a survival of a graviton from a prior to the present universe, using typical Planckian peak temperature values of . We obtain values consistent with up to 1038 gravitons contributing to an energy value of if we assume a relic energy contribution based upon each graviton initially exhibiting a frequency spike of 1010 Hz. The value of is picked from looking at the aftermath of what happens if there exists a quantum bounce with a peak density value of [1] in a regime of LQG bounce regime radii of the order of magnitude of meters. The author, in making estimates specifically avoids using ,?by setting the chemical potential for ultra high temperatures for reasons which will be brought up in the conclusion.

Neonatal Infection among Women with prior Premature Labor

Objectives: Since studies demonstrate that neonates born to mothers having been cared for premature labor will suffer from congenital neonatal sepsis, we aimed to evaluate the prevalence and main risk factors of neonatal infection among mothers having experienced a prior premature labor. Methods: This was a cross sectional study carried out from January 1st throughout 31st December, 2013 at the university clinics of Kinshasa. It concerned all delivered women at term having been cared for premature labor prior to giving birth a live newborn. Maternal variables of interest were: parity, gestation, age, intrapartum fever, malaria, urogenital infection during the last 2 weeks before delivery (UGI), premature rupture of membranes (PROM), cervical cerclage, meconium-stained amniotic fluid (MSAF) and the way of delivery. For neonates attention was paid on gestational age, birth weight, admission at neonatal intensive care unit (ANICU) and infection as stated within three days after birth. T-test and Chi-square were used where appropriate. Logistic analysis was used to determine the risk for maternal variables to induce neonatal infection (OR and CI), the significance stated at p prior cerclage and MSAF were significantly more frequent in couples with neonatal infection. Prematurity, birth weight <2500 g and UGI were found to enhance the risk by 3 to 4 times. Conclusion: The prevalence of neonatal infection was very high. Prematurity, birth weight <2500 g and maternal UGI were found to enhance the risk by 3 to 4 times.
