Reliability and concurrent validity of postural asymmetry measurement in adolescent idiopathic scoliosis

AIM To investigate the reliability and concurrent validity of the Baseline~? Body Level/Scoliosis meter for adolescent idiopathic scoliosis postural assessment in three anatomical planes.METHODS This is an observational reliability and concurrent validity study of adolescent referrals to the Orthopaedic department for scoliosis screening at KarolinskaUniversity Hospital, Stockholm, Sweden between MarchMay 2012. A total of 31 adolescents with idiopathic scoliosis(13.6 ± 0.6 years old) of mild-moderate curvatures(25°± 12°) were consecutively recruited. Measurement of cervical, thoracic and lumbar curvatures, pelvic and shoulder tilt, and axial thoracic rotation(ATR) were performed by two trained physiotherapists in one day. The intraclass correlation coefficient(ICC) was used to determine the inter-examiner reliability(ICC2,1) and the intra-rater reliability(ICC3,3) of the Baseline~? Body Level/Scoliosis meter. Spearman's correlation analyses were used to estimate concurrent validity between the Baseline~? Body Level/Scoliosis meter and Gold Standard Cobb angles from radiographs and the Orthopaedic Systems Inc. Scoliometer.RESULTS There was excellent reliability between examiners for thoracic kyphosis(ICC2,1 = 0.94), ATR(ICC2,1 = 0.92) and lumbar lordosis(ICC2,1 = 0.79). There was adequate reliability between examiners for cervical lordosis(ICC2,1 = 0.51), however poor reliability for pelvic and shoulder tilt. Both devices were reproducible in the measurement of ATR when repeated by one examiner(ICC3,3 0.98-1.00). The device had a good correlation with the Scoliometer(rho = 0.78). When compared with Cobb angle from radiographs, there was a moderate correlation for ATR(rho = 0.627).CONCLUSION The Baseline~? Body Level/Scoliosis meter provides reliable transverse and sagittal cervical, thoracic and lumbar measurements and valid transverse plan measurements of mild-moderate scoliosis deformity.

Background and purpose:Complementary therapies,such as yoga,have been proposed to address gait and balance problems in Parkinson's disease(PD).However,the effects of yoga on gait and static balance have not been studied systematically in people with PD(PWP).Here we evaluated the effects of a 12-week long Hatha yoga intervention on biomechanical parameters of gait and posture in PWP.Methods:We employed a pilot randomized controlled trial design with two groups of mild-to-moderate PWP(immediate treatment,waitlist control;N?10 each;Mean Hoehn and Yahr score=2 for each group).Baseline Unified Parkinson's Disease Rating Scale(UPDRS)motor scores,and gait and postural kinematics including postural sway path length,cadence,walking speed,and turning time were obtained.The immediate treatment group received a 60-min Hatha yoga training twice a week for 12 weeks,while the waitlisted control group received no training.After 12 weeks,gait and postural kinematics were assessed(post-test for treatment group and second-baseline for waitlist group).Then,the waitlist group received the same yoga training and was evaluated post-training.Results:After Hatha yoga training,UPDRS motor scores improved with an 8-point mean decrease which is considered as a moderate clinically important change for mild-moderate PD.Sway path length during stance decreased significantly(mean reduction:-34.4%).No significant between-group differences or improvements in gait kinematics were observed.Conclusion:This study showed that a 12-week Hatha yoga training can improve static balance in PWP.We found no evidence that it systematically improves gait performance in PWP.

The Way We Stand: A Sequential Case Study on Foot Angle

This study quantifies the significance of foot angle as it relates to common biometric data to assess its use in the clinical realm. There are two objectives to this study;first to determine the relationship foot angle has with ten surveyed biometric data points (age, weight, height, biological makeup, shoe size, orthotic use, physical activity level, competitive sports history, lower extremity injury history, and reason for visiting doctor), and second to determine the average left and right foot angles of the population studied. The duration of this sequential case study with a sample size of 73 was December 9, 2021, to April 7, 2022. Participants were fully informed of the measurement steps, the purpose of the survey, and the goal of the project. Participants were asked to walk down the hallway as a distraction to collect the most accurate data on the participants’ stance. This study was performed at the Arizona Institute of Motion, a clinical office and private practice. Participants had the option to decline participation in the study. Correlational calculations and linear regressions were performed to support or reject the existence of a relationship between foot angle and the biometric data. It was found that the average left foot angle was 26.35 degrees, and the average right foot angle was 26.54 degrees. It was found that there was a strong positive correlation between left and right foot angles, and a weak positive relationship with both foot angles and weight, height, and shoe size. The optimized linear regression model had an adjusted r2 value of 0.549 for left foot angle against right foot angle, shoe size, and height, and an adjusted r2 value of 0.522 for right foot angle against left foot angle, shoe size, and height. Foot angle had no other relationships with the other biometric data and had limited quantitative significance. Noise-enhanced dynamic single leg balance in subjects with functional ankle instability

Background:Stochastic resonance stimulation(SRS) transmits subsensory electrical Gaussian white noise into the body to enhance sensorimotor function.This therapy has improved static single leg balance in subjects with functional ankle instability.However,the effect of this stimulation on dynamic single leg balance is not known.Improvements in dynamic single leg balance with SRS may have implications For enhancing functional rehabilitation for ankle instability.Thus,the purpose of this study was to determine the effects of SRS on dynamic single leg balance in subjects with functional ankle instability. Methods:This study was an experimental research design and data were collected in a sports medicine research laboratory.Twelve subjects with functional ankle instability(69±15 kg;173±10 cm;21±2 years) reported a history of ankle sprains and instability at the ankle with physical activity.A single leg jump-landing test was used to assess dynamic balance.Subjects were required to jump between 50% and 55% of the maximal vertical jump height,land on a single leg atop a force plate,and stabilize as quickly as possible.Jump-landing tests were performed with and without SRS.Three trials were performed for each treatment condition(SRS and control).A randomized block design was used to determine test order.Anterior/posterior and medial/lateral time-to-stabilization were computed to assess dynamic balance.Lesser time indicated better stability.One-tailed paired samples t tests were used for analysis(α≤0.05). Results:SRS improved anterior/posterior time-to-stabilization(stochastic resonance = 1.32±0.31 s,control = 1.74±0.80 s,p = 0.03),but did not enhance medial/lateral time-to-stabilization(stochastic resonance = 1.95±0.40 s,control = 1.92±0.48 s,p = 0.07). Conclusion:Clinicians might use SRS to facilitate balance improvements with sagittal plane dynamic single leg balance exercises that patients may not be able to perform otherwise.

The influences of different design factors,as well as dummy posture,on an occupants' knee slider compression,were studied in this paper.Based on the vehicle geometry data,the simulation model,including both the multi-rigid-body and finite element(FE)part,was built up and validated with China New Car Assessment Program(C-NCAP)full impact to ensure the accuracy of the model.By adjusting the design parameters and the posture of the femur and lower leg,different factors affecting the passengers' knee slider compression were evaluated,with the help of MAthematical DYnamic MOdel(MADYMO)simulations.The study indicated that the leg posture,the stiffness of the IP and angles of the carpet have significant effects on the knee slider compression in this case.By decreasing the angle between the femur and lower leg from 133° to 124°,the maximum knee slider compression was decreased by 17.3% and by scaling the IP stiffness from 1 to 0.7,it could be decreased by 18.6%.Also,decreasing the angles of the carpet from 28° to 37°can help reduce the knee slider compression by 18.3%.

The computer aided process planning will certainly play a significant role in the success of enterprise information- ization.3-dimensional design will promote Tri-dimensional process planning.This article analysis nowadays situation and prob- lems of assembly process planning,gives a 3-dimensional computer aided process planning system(3D-VAPP),and researches on the product information extraction,assembly sequence and path planning in visual interactive assembly process design,dynamic emulation of assembly and process verification,assembly animation outputs and automatic exploding view generation,interactive craft filling and craft knowledge management,etc.It also gives a multi-layer collision detect and multi-perspective automatic camera switching algorithm.Experiments were done to ualidate the feasibility of such technology and algorithm,which established the foundation of tri-dimensional computer aided process planning.

posture adjustment of open-type hard rock tunnel boring machine(TBM) can be achieved by properly adjusting the hydraulic pressure of gripper cylinder and torque cylinders. However, the time-varying inhomogeneous load acting on tunneling face of TBM and complex stratum working condition can cause the trajectory deviation. In this paper,the position and posture rectification kinematics and dynamics models of TBM have been established in order to track the trajectory. Moreover, there are uncertain parameters and uncertain loads from complex working conditions in the dynamic model. An indirect adaptive robust control strategy is applied to achieve precise position and posture trajectory tracking control. Simulation results show when the position deviation only occurs in Y-axis and the current orientation is parallel with the designed axis, the deviation can be corrected by controlling the pressure of gripper cylinder and the actual trajectory meets the designed axis when TBM is pushed forward 0.14 m in X-axis. If the deviation only occurs in Z-axis, then the deviation can be corrected by controlling torque cylinders. If the position deviation occurs both in Y-axis and Z-axis at the same time, the pressure of gripper cylinder and torque cylinders should be controlled at the same time to rectify the deviation. Simulation results are shown to illustrate the e ectiveness and robustness of the proposed controller. This research proposes an indirect adaptive robust controller that can track the planned tracking trajectory smoothly and rapidly.
