Peptide-based probes play prominent roles in biomedical research due to their promising properties such as high biocompatibility,fast excretion, favorable pharmacokinetics as well as easy and robust preparation. Considering the translation of imaging probes into clinical applications, peptide-based probes remain to be the most desirable and optimal candidates. This review summarized the development of peptide-based probes with promising imaging modalities and highlighted the successful applications for in vivo biomedical imaging.

Purpose Compared to pixelated crystal detectors,monolithic crystal detectors present more advantages.Without reflective layer between crystal pixel,monolithic crystal detectors provide higher detection efficiency while being cheaper.Another important feature is that monolithic crystal detectors can provide depth of interaction(DOI)information,which can improve image quality in nuclear imaging systems.In this article,we aim to study a new positioning scheme of this kind of detector in order to push its applications in nuclear imaging systems.Methods Fan-beam collimation and convolutional neural network are combined to reconstruct interaction points in crystal.Firstly,we built a monolithic detector model based on Geant4 and validated this positioning scheme with simulation data.Then,we constructed a monolithic GAGG(Ce)detector and evaluated its performance with experimental data.Results With proposed positioning scheme,in experiment,we obtained~1.7 mm FWHM resolution on average in x-/ydirection,while~2.5 mm FWHM resolution on average in DOI direction for a 33×33×10 mm3 monolithic GAGG(Ce)crystal coupled with 8×8 SiPM array.Conclusion Through simulations and experiments,we validated our positioning scheme,i.e.,fan-beam collimation and convolutional neural network.Convolutional neural network can reconstruct 3D positions of gamma-rays interaction points in a high resolution,and fan-beam collimation can provide high calibration efficiency;with combination of them,we established a high position resolution and high calibration efficiency positioning scheme for monolithic crystal detector.

BACKGROUND： Acupuncture can improve motor function in patients with cerebral infarction, and activate brain glucose metabolism in relevant brain areas. However, the association between encephalic region activation and acupuncture, as well as the clinical significance of activation remain unclear. OBJECTIVE： Through the use of positron emission tomography-computed tomography （PET-CT）, acute cerebral infarction patients were analyzed for global cerebral metabolism, cerebral infarction focus, peripheral edema, and pyramidal tract pathway changes, which were directly related to clinical symptoms. The influence of resuscitating acupuncture on cerebral glucose metabolism was analyzed in patients with acute cerebral infarction in basal ganglia. DESIGN, TIME AND SETTNG： Randomized, controlled, clinical trials were performed from March 2007 to October 2008 at the PET-CT Center of the General Hospital of Tianjin Medical University, China. PARTICIPANTS： Twelve patients with acute basal ganglia infarction were recruited from the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine, the Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine, Tianjin Chinese Medicine Hospital, and Affiliated Hospital of Medical College of Chinese People＇s Armed Police Force. METHODS： The cerebral infarcted patients were randomly assigned to acupuncture and control groups. In addition to routine treatment, the acupuncture group was treated by acupuncture at the main acupoints for resuscitation [Neiguan （PC 6）, Renzhong （DU 26）, and Sanyinjiao （SP 6）], while the control group received routine treatment. MAIN OUTCOME MEASURES： Before and after treatment, patients with acute cerebral infarction were evaluated for global brain, cerebral infarction focus, and surrounding edema and glucose metabolism in encephalic region of pyramidal tract conduction by 18-labeled fluorodeoxyglucose for PET-CT imaging. BACKGROUND： It is widely known that fluonno-18-fluorodeoxyglucose positron emission tomography （18F-FDG PET） is commonly used to evaluate and diagnose epilepsy; however, whether it is beneficial to understand functional metabolism of brain cells so as to reflect injured site and degree of brain cells or not should be studled further. OBJECTIVE： To evaluate the correlation between glucose metabolism and clinical typing as well as the correlation between active function of brain cells and degree of brain injury among children with cerebral palsy with 18F-FDG PET and MRI and compare the results of them. DESIGN： Case analysis SETTING： Department of Pediatrics, People＇s Hospital of Guangdong Province PARTICIPANTS： A total of 31 children with cerebral palsy were selected from Out-patient Clinic and In-patient Department of People＇s Hospital of Guangdong Province from July 2001 to August 2004. Based on clinical criteria of cerebral palsy, patients were classified into spasm （n =10）, gradual movement （n =4）, mixed type （n =13） and ataxia （n =4）. There were 18 boys and 13 gids aged from 10 months to 4 years. All of them were met the diagnostic cnteda of cerebral palsy and all parents of them were told the facts. Exclusion criteria： Patients who had cerebral palsy caused by genetic metabolism disease were excluded. METHODS：（1） All children accepted MRI examination after hospitalization with Philips Acs NT 15T superconducting magnetic resonance scanner. （2） All children were fasted for 4 hours. And then, PET image of brain was collected based on T＋EID type. If obvious hypermetabolism or hypometabolism region successively occurred on two layers, the image was regarded as abnormality. （3） Different correlations of various abnormal groups of MRI and vanous types of cerebral palsy with PET image were compared and analyzed with Erusal-Willas rank sum test. Pancreatic neuroendocrine neoplasms(panNEN)are a heterogeneous group of tumors with differing pathological,genetic,and clinical features.Based on clinical findings,they may be categorized into functioning and nonfunctioning tumors.Adoption of the 2017 World Health Organization classification system,particularly its differentiation between grade 3,well-differentiated pancreatic neuroendocrine tumors(panNET)and grade 3,poorly-differentiated pancreatic neuroendocrine carcinomas(panNEC)has emphasized the role imaging plays in characterizing these lesions.Endoscopic ultrasound can help obtain biopsy specimen and assess tumor margins and local spread.Enhancement patterns on computed tomography(CT)and magnetic resonance imaging(MRI)may be used to classify panNEN.Contrast enhanced MRI and diffusion-weighted imaging have been reported to be useful for characterization of panNEN and quantifying metastatic burden.Current and emerging radiotracers have broadened the utility of functional imaging in evaluating panNEN.Fluorine-18 fluorodeoxyglucose positron emission tomography(PET)/CT and somatostatin receptor imaging such as Gallium-681,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid–octreotate PET/CT may be useful for improved identification of panNEN in comparison to anatomic modalities.These new techniques can also play a direct role in optimizing the selection of treatment for individuals and predicting tumor response based on somatostatin receptor expression.In addition,emerging methods of radiomics such as texture analysis may be a potential tool for staging and outcome prediction in panNEN,however further investigation is required before clinical implementation.

In this paper a fully Bayesian PET reconstruction method is presented for combining a segmented anatomical membrane a priori. The prior distributions are based on the fact that the radiopharmaceutical activity is similar throughout each region and the anatomical information is obtained from other imaging modalities such as CT or MRI. The prior parameters in prior distribution are considered drawn from hyperpriors for fully Bayesian reconstruction. Dynamic Markov chain Monte Carlo methods are used on the Hoffman brain phantom to gain estimates of the posterior mean. The reconstruction result is compared to those obtained by ML, MAP. Our results showed that the segmented anatomical membrane a priori exhibit improved the noise and resolution properties.

Role of nuclear cardiology for guiding device therapy in patients with heart failure

Heart failure is a dynamic condition with high morbidity and mortality and its prognosis should be reassessed frequently, particularly in patients for whom critical treatment decisions may depend on the results of prognostication. In patients with heart failure, nuclear cardiology techniques are useful to establish the etiology and the severity of the disease, while fewer studies have explored the potential capability of nuclear cardiology to guide cardiac resynchronization therapy(CRT) and to select patients for implantable cardioverter defibrillators(ICD). Left ventricular synchrony may be assessed by radionuclide angiography or gated singlephoton emission computed tomography myocardial perfusion scintigraphy. These modalities have shown promise as predictors of CRT outcome using phase analysis. Combined assessment of myocardial viability and left ventricular dyssynchrony is feasible using positron emission tomography and could improve conventional response prediction criteria for CRT. Preliminary data also exists on integrated positron emission tomography/computed tomography approach for assessing myocardial viability, identifying the location of biventricular pacemaker leads, and obtaining left ventricular functional data, including contractile phase analysis. Finally, cardiac imaging with autonomic radiotracers may be useful in predicting CRT response and for identifying patients at risk for sudden cardiac death, therefore potentially offering a way to select patients for both CRT and ICD therapy. Prospective trials where imaging is combined with image-test driven therapy are needed to better define the role of nuclear cardiology for guiding device therapy in patients with heart failure.

Rheumatoid arthritis(RA)is an autoimmune disease,which is associated with systemic and chronic inflammation of the joints,resulting in synovitis and pannus formation.For several decades,the assessment of RA has been limited to conventional radiography,assisting in the diagnosis and monitoring of disease.Nevertheless,conventional radiography has poor sensitivity in the detection of the inflammatory process that happens in the initial stages of RA.In the past years,new drugs that significantly decrease the progression of RA have allowed a more efficient treatment.Nuclear Medicine provides functional assessment of physiological processes and therefore has significant potential for timely diagnosis and adequate follow-up of RA.Several single photon emission computed tomography(SPECT)and positron emission tomography(PET)radiopharmaceuti-cals have been developed and applied in this field.The use of hybrid imaging,which permits computed tomography(CT)and nuclear medicine data to be acquired and fused,has increased even more the diagnostic accuracy of Nuclear Medicine by providing anatomical localization in SPECT/CT and PET/CT studies.More recently,fusion of PET with magnetic resonance imaging(PET/MRI)was introduced in some centers and demonstrated great potential.In this article,we will review studies that have been published using Nuclear Medicine for RA and examine key topics in the area.

Introduction: [18F]-fluoro-methylcholine (FCH) PET/CT and MRI with diffusion-weighted MRI (DW-MRI) have insufficient performance in lymph node staging of primary prostate cancer by themselves, but the combination may perform better. We aim to prospectively determine the diagnostic performance of combined FCH PET and MRI for lymph node staging. Methods: This was a single site study of diagnostic accuracy in a well-defined group of 21 consecutive high-risk primary prostate cancer patients (>30% chance of lymph node metastases) in a large community hospital. We performed FCH PET/CT and MRI with DW-MRI prior to endoscopic extended pelvic lymph node dissection (EPLND). PET was fused and interpreted together with various MRI image sets (T1, T2, DWIBS) and was only scored positive when a lymph node seen on MRI coincided with increased focal FCH uptake on PET. Findings were compared with detailed histological evaluation, on a per-patient and per-region level. We calculated sensitivity, specificity, positive and negative predictive value of combined PET-MRI. Results: 14 out of 21 patients had metastatic lymph nodes with 37 out of 164 evaluable regions harboring metastases. On a per-patient analysis, PET-MRI had a sensitivity/specificity of 79/100% with a PPV/NPV of 100/77%. On a per-region analysis (n = 164) these figure were 65/99% and 96/91%, respectively. Conclusions: Combined DW-MRI and FCH PET/CT has a very high positive predictive value in high risk prostate cancer patients. If confirmed in larger series a positive combined scan may safely allow cancellation of surgical staging in selected patients, depending on local protocols in N1 M0 patients.
