Biochemical or clinical changes of hyperandrogenism are important elements of polycystic ovary syndrome （PCOS）. There is currently no consensus on the definition and diagnostic criteria of hyperandrogenism in PCOS. The aim of this study was to investigate the complex symptoms of hyperandrogenic disorders and the correlations between metabolism and hyperandrogenism in patients with PCOS from an outpatient reproductive medicine clinic in China. We conducted a case control study of 125 PCOS patients and 130 controls to evaluate differences in body mass index （BMI）, total testosterone （TT）, modified Ferriman-Gallwey hirsutism score, sex hormone binding globulin （SHBG）, homeostasis model assessment-estimated insulin resistance （HOMA-IR） and free androgen index （FAI） between PCOS patients and controls and subgroups of PCOS. The prevalence of acne and hirsutism did not differ significantly between the hyperandrogenic and non-hyperandrogenic subgroup. Patients with signs of hyper- androgenism had significantly higher BMI （P 〈 0.05）, but differences in TT, SHBG, FAI and waist/hip ratio were insignificant. The odds ratio of overweight was calculated for all PCOS patients. Our results suggest that PCOS patients with high BMI tend to have functional disorders of androgen excess; therefore, BMI may be a strong pre-dictor of hyperandrogenism in PCOS.

polycystic ovary syndrome(PCOS)often coexists with a wide spectrum of dysglycemic conditions,ranging from impaired glucose tolerance to type 2 diabetes mellitus(T2D),which occur to a greater extent compared to healthy body mass index-matched women.This concurrence of disorders is mainly attributed to common pathogenetic pathways linking the two entities,such as insulin resistance.However,due to methodological flaws in the available studies and the multifaceted nature of the syndrome,there has been substantial controversy as to the exact association between T2D and PCOS which has not yet been elucidated.The aim of this review is to present the best available evidence regarding the epidemiology of dysglycemia in PCOS,the unique pathophysiological mechanisms underlying the progression of dysglycemia,the most appropriate methods for assessing glycemic status and the risk factors for T2D development in this population,as well as T2D risk after transition to menopause.Proposals for application of a holistic approach to enable optimal management of T2D risk in PCOS are also provided.Specifically,adoption of a healthy lifestyle with adherence to improved dietary patterns,such the Mediterranean diet,avoidance of consumption of endocrine-disrupting foods and beverages,regular exercise,and the effect of certain medications,such as metformin and glucagon-like peptide 1 receptor agonists,are discussed.Furthermore,the maintenance of a healthy weight is highlighted as a key factor in achievement of a significant reduction of T2D risk in women with PCOS.

Background: As one of the most common endocrinal disorders for women at childbearing age, the diagnostic criteria of polycystic ovary syndrome (PCOS) have been defined differently among different international health organizations. Phenotypic heterogeneity of PCOS also brings about difficulties for its diagnosis and management assessment. Therefore, more efficient biomarkers representing the progression of PCOS are expected to be integrated into the monitoring of management process using metabolomic approaches.Methods: In this prospective randomized controlled trial, 117 PCOS patients were enrolled from December 2016 to September 2017. Classical diagnostic parameters, blood glucose, and metabolome were measured in these patients before and at 2 months and 3 months of different medical interventions. The receiver operating characteristic (ROC) curves were built based on multivariate statistical analysis using data at baseline and 3 months’ management, and combinational biomarkers with appreciable sensitivity and specificity were selected, which then validated with data collected at 2 months.Results: A set of metabolites including glutamic acid, aspartic acid, 1-methylnicotinamide, acetylcarnitine, glycerophosphocholine, and oleamide were filtered out with high performance in representing the improvement through 3-month management of PCOS with high sensitivity and specificity in ROC analysis and validation with other two groups showed an appreciable area under the curve over 0.96.Conclusions: The six metabolites were representative of the remission of PCOS through medical intervention, making them a set of potential biomarkers for assessing the outcome of PCOS management.Trial Registration: ClinicalTrials.gov, NCT03264638.

Background:polycystic ovary syndrome(PCOS)is the primary cause of anovulatory infertility,bringing serious harm to women’s physical and mental health.Acupuncture may be an effective treatment for PCOS.However,systematic reviews(SRs)on the efficacy and safety of acupuncture for PCOS have reported inconsistent results,and the quality of these studies has not been adequately assessed.Objective:To summarize and evaluate the current evidence on the efficacy and safety of acupuncture for PCOS,as well as to assess the quality and risks of bias of the available SRs.Search strategy:Nine electronic databases(Cochrane Library,MEDLINE,Embase,PsycINFO,CINAHL,Chinese National Knowledge Infrastructure,Wanfang Data,Chongqing VIP Chinese Science and Technology Periodical Database,and China Biology Medicine disc)were searched from their establishment to July 27,2022.Based on the principle of combining subject words with text words,the search strategy was constructed around search terms for“acupuncture,”“polycystic ovary syndrome,”and“systematic review.”Inclusion criteria:SRs of randomized controlled trials that explored the efficacy and(or)safety of acupuncture for treating patients with PCOS were included.Data extraction and analysis:Two authors independently extracted study data according to a predesigned form.Tools for evaluating the methodological quality,risk of bias,reporting quality,and confidence in study outcomes,including A Measurement Tool to Assess Systematic Reviews 2(AMSTAR 2),Risk of Bias in Systematic Reviews(ROBIS),Preferred Reporting Items for Systematic Reviews and Metaanalyses for Acupuncture(PRISMA-A),and the Grading of Recommendations Assessment,Development and Evaluation(GRADE),were used to score the included SRs.Results:A total of 885 studies were retrieved,and 11 eligible SRs were finally included in this review.The methodological quality of 2 SRs(18.18%)was low,while the other 9 SRs(81.82%)were scored as extremely low.Four SRs(36.36%)were considered to be of low risk of bias.As for reporting quality,the r

Objective:polycystic ovary syndrome(PCOS)is an endocrine disorder with diverse clinical manifestations that often occurs in women of childbearing age.However,its molecular pathogenesis remains unclear,and this study aimed to identify miRNA targets in PCOS through text mining and database analysis.Methods:First,three different sets of text mining genes(TMGs)associated with"polycystic ovary syndrome","obesity/adiposis",and"anovulation"keywords were retrieved from the GenCLiP3 database,and overlapping genes were selected.Second,Gene ontology annotation and biological pathway enrichment analyses of these overlapping TMGs were performed,followed by protein-protein interaction(PPI)network analysis.Third,genes in the gene module clustered in the PPI were selected to predict potential miRNAs for PCOS via miRNA-mRNA analysis.Results:A total of 4291 TMGs related to three different keywords were obtained through text mining;72 intersect TMGs were retained among the three gene sets,and 62 TMGs participated in the establishment of the PPI network,of which 18 were aggregated in the gene module.Finally,11 miRNAs that simultaneously bound to two TMGs(IGF1,ESR1,MAPK1,NAMPT,PIK3CA,and SERPINE1)could be prioritized as targets to study PCOS.Conclusion(s):The discovery of 11 miRNAs(miR-301a-3p,miR-301b-3p,miR-3666,miR-454-3p,miR-130a-3p,miR-130b-3p,miR-4295,miR-190a-3p,miR-5011-5p,miR-548c-3p,and miR-4799-5p)and 6 TMGs,which are associated with the HIF-1 signaling pathway(P=4.799E-08),could be used as potential targets for PCOS.

Background Epilepsy comorbidities adversely affect the quality of life of patients.Women with epilepsy are at a high risk of comorbid endocrine disorders.Among them,the polycystic ovary syndrome(PCOS)has a threefold higher prevalence in women with epilepsy than in healthy women and is the main cause of infertility among the patients.Clinically,women with epilepsy show heterogeneity in the susceptibility to PCOS.This heterogeneity may be associated with genetic factor.Methods To test this,we retrospectively collected clinical data from 45 female patients with epilepsy and divided them into three groups according to their susceptibility to PCOS.Groups A and B represented a high susceptibility to PCOS.Patients in Group A were diagnosed with PCOS before their first seizure,while patients in Group B were diagnosed with PCOS after a short period of monotherapy with a low dose of antiseizure medication(ASM)following the diagnosis of epilepsy.Patients in Group C did not develop PCOS despite a prolonged treatment with high-dose ASM.We compared the clinical data and genetic profiles among the three groups.Results We found a clear trend of impaired metabolism in Group B patients and this may be associated with highfrequency mutations in MYO10 and ADGRL3.Conclusions Our study suggests that women with epilepsy are heterogeneous in the susceptibility to PCOS and this is associated with mutations in specific genes.Therefore,genetic screening should be conducted to screen for women with epilepsy who are more likely to have comorbid PCOS,so that they can receive targeted interventions at an early stage to reduce the risk.

Objective:To examine the association between sleep characteristics and night shift work and the risk of polycystic ovary syndrome(PCOS)in Chinese women and to investigate their relationship with infertility in PCOS.Methods:From March 21,2021 to April 31,2021,3927 Chinese women with or without PCOS were recruited online.All participants completed WeChat-based electronic questionnaires.Sleep characteristics were measured using the Pittsburgh sleep quality index.Results:A total of 2871 women were included in the final analysis.Sleep duration(odds ratio[OR],0.857;95%confidence interval[CI],0.763-0.963),sleep midpoint(OR,1.142;95%CI,1.049-1.244),sleep disturbance(OR,1.320;95%CI,0.957-1.146),daytime dysfunction(OR,1.136;95%CI,1.030-1.253),and night shift work(OR,1.628;95%CI,1.264-2.097)were associated with a higher risk of PCOS.After adjusting for confounders,including age,body mass index,smoking status,and coffee and tea drinking status,sleep disturbance(OR,1.314;95%CI,1.111-1.555),daytime dysfunction(OR,1.143;95%CI,1.034-1.264),and night shift work(OR,1.800;95%CI,1.388-2.333)remained associated.In addition,sleep disturbance(OR,1.887;95%CI,1.400-2.542)and subjective sleep quality(OR,1.299;95%CI,1.037-1.627)were associated with infertility in women with PCOS,and sleep disturbance(OR,1.750;95%CI,1.281-2.390)remained significant after adjusting for confounders.Conclusions:Sleep disturbance,daytime dysfunction,and night shift work are significantly associated with PCOS.Screening for sleep disturbances and providing appropriate treatment could be potential strategies for managing PCOS and its long-term complications.

polycystic ovary syndrome (PCOS) is a heterogeneous reproductive disease that can cause infertility. The Hippo signaling pathway, a network highly conserved throughout evolution, maintains the balance between follicle proliferation and dormancy. Dynamic changes in primordial follicles cannot occur without the participation of biological signals and mechanical force;however, little is known about the mechanism by which biomechanical signaling triggers PCOS, especially in the context of primordial follicle development. To investigate the contribution of mechanical stress and the Hippo signaling pathway to the onset of PCOS, we searched the literature via the PubMed database, and inclusion and exclusion criteria were established to ensure the rigor of this research. We eventually included 54 publications in which Hippo signaling and mechanical force were suggested to play a vital role in the development of primordial follicles as well as elucidate the pathogenesis of PCOS. The Hippo pathway modulating follicle growth can be perturbed via extracellular mechanical stress caused by the stiff ovarian cortical environment in PCOS. Clinical intervention targeting the Hippo pathway can alter the activity of core Hippo members, such as the Yes-associated protein/transcriptional co-activator PDZ-binding motif complex. In some patients with PCOS, follicle overactivation can be attributed to the dysfunction of Hippo signal transduction. PCOS, a condition with various patterns, cannot be accurately explained by a single, specific mechanism. The present review identifies potential targets and therapeutic strategies for PCOS.
