periodontal disease is a high prevalent disease.In the United States 47.2% of adults ≥ 30 years old have been diagnosed with some type of periodontitis.Longitudinal studies have demonstrated a two-way relationship between diabetes and periodontitis,with more severe periodontal tissue destruction in diabetic patients and poorer glycemic control in diabetic subjects with periodontal disease.periodontal treatment can be successful in diabetic patients.Short term effects of periodontal treatment are similar in diabetic patients and healthy population but,more recurrence of periodontal disease can be expected in no well controlled diabetic individuals.However,effects of periodontitis and its treatment on diabetes metabolic control are not clearly defined and results of the studies remain controversial.

The purpose of this article was to increase the knowledge about oral manifestations and complications associated with diabetes mellitus.An overview was performed on Google,especially in recent reliable papers in relation to diabetes mellitus and its oral manifestations(keywords were“diabetes mellitus”,“oral manifestations”,and“oral complications”).Data were collected and the results were declared.Diabetes mellitus is one of the most common chronic disorders characterized by hyperglycemia.This disease can have many complications in various regions of the body,including the oral cavity.The important oral manifestations and complications related to diabetes include xerostomia,dental caries,gingivitis,periodontal disease,increased tendency to oral infections,burning mouth,taste disturbance,and poor wound healing.Oral complications in diabetic patients are considered major complications and can affect patients’quality of life.There is evidence that chronic oral complications in these patients have negative effects on blood glucose control,so prevention and management of the oral complications are important.

This study was carried out to compare individuals diagnosed with atherosclerosis and periodontal periodontitis based on the degree of change in the human beta-defensins(HBD)HBD-2,HBD-3,and calprotectin.Atherosclerosis is the most frequently observed cardiovascular disease.Dental and periodontal infections are known to provide a considerable basis for atheroma plaque formation.The study group consists of a total number of 40 subjects,with 20 patients diagnosed with atherosclerosis and chronic periodontitis and 20 systemically healthy patients diagnosed with chronic periodontitis.Clinical periodontal and blood parameters and HBD-2,HBD-3,and calprotectin biomarkers in the gingival crevicular fluid were measured.In both groups,following clinical periodontal treatment,a statistically significant decrease in white blood cells(WBC),low-density lipoproteins(LDL),fibrinogen,creatinine,and platelets(PLT),a statistically significant increase in high-density lipoproteins(HDL)in blood samples,statistically meaningful decrease in HBD-2,HBD-3,and calprotectin in the gingival crevicular fluid were achieved.Blood values and HBD-2,HBD-3,calprotectin amounts in the gingival crevicular fluid were increased significantly in the test group compared to the control group.A positive correlation was observed between decreases in HBD-2,HBD-3,calprotectin,and clinical periodontal indices.Regression in systemic inflammation was observed after clinical periodontal treatment.It is concluded that nonsurgical periodontal treatment of chronic periodontitis positively affects atherosclerosis prognosis.

AIM:To evaluate the influence of oral Helicobacter pylori(H.pylori)on the success of eradication therapy against gastric H.pylori.METHODS:A total of 391 patients with dyspepsia were examined for H.pylori using the saliva H.pylori antigen test(HPS),13C-urea breath test(UBT),gastroscopy,and gastric mucosal histopathological detection.Another 40 volunteers without discomfort were subjected to HPS and13C-UBT,and served as the control group.The 233 patients who were13C-UBT+were enrolled in this study and divided into 4 groups.Patients who were HPS-and13C-UBT+(n=53)received triple therapy alone.Those who were both HPS+and13CUBT+(n=180)were randomly divided into 3 groups:(1)the O+G+t group which received triple therapy alone(n=53);(2)the O+G+tm group which received both triple therapy and mouthrinse treatment(n=65);and(3)the O+G+tmp group which received triple therapy,mouthrinse,and periodontal treatment(n=62).The HPS and13C-UBT were continued for 4 wk after completion of treatment,and the eradication rate of gastric H.pylori and the prevalence of oral H.pylori in the 4 groups were then compared.RESULTS:The eradication rates of gastric H.pylori in the O-G+t group,the O+G+tm group,and the O+G+tmp group were 93.3%,90.0%,and 94.7%respectively;all of these rates were higher than that of the O+G+t group(78.4%)[O-G+t group vs O+G+t group(P=0.039);O+G+tm group vs O+G+t group(P=0.092);O+G+tmp group vs O+G+t group(P=0.012);O+G+tm group vs O-G+t group(P=0.546);O+G+tmp group vs O-G+t group(P=0.765);O+G+tm group vs O+G+tmp group(P=0.924)].The eradication of gastric H.pylori was significantly improved using the combination of triple therapy,mouthrinse,and periodontal treatment.The eradication rates of gastric H.pylori in the peptic ulcer group,chronic atrophic gastritis group and control group were higher than in the duodenitis group and the superficial gastritis group.The prevalence rates of oral H.pylori in the O-G+t group,O+G+t group,O+G+tm group and O+G+tmp group following treatment were 0%,76.5%,53.3%,and 50.9%,respectively[O-G

Objective:To investigate the TNF-α、IL-6、IL-1βeffect of omeprazole combined with amoxicillin on cytokines in gingival crevicular fluid of patients with periodontal disease.Methods:102 patients with periodontal disease who were treated in our hospital from June 2016 to March 2018 were selected.Randomly divided into two groups,the control group was treated with omeprazole alone,and the research group was treated with omeprazole combined with amoxicillin.The levels of inflammatory cytokines(TNF-α、IL-6、IL-1β),therapeutic effects and adverse reactions to drugs were compared between the two groups.Results:the levels of inflammatory factors(TNF-α、IL-6、IL-1β)in the study group were lower than those in the control group(Pperiodontal disease patients achieved ideal therapeutic efficacy.

The use of elastics to close a diastema or correct tooth malpositions can create unintended consequences if not properly controlled. The American Association of Orthodontists recently issued a consumer alert, warning of "a substantial risk for irreparable damage" from a new trend called "do-it-yourself" orthodontics, consisting of patients autonomously using elastics to correct tooth position. The elastics can work their way below the gums and around the roots of the teeth, causing damage to the periodontium and even resulting in tooth loss. The cost of implants to replace these teeth would well exceed the cost of proper orthodontic care. This damage could also occur in a dental office, when a general dentist tries to perform a simplified orthodontic correction of a minor tooth malposition. The present case report describes a case of tooth loss caused by a displaced intraoral elastic, which occurred during a simple preprosthetic orthodontic treatment.

Microarray Analysis of the Effects of Amelogenin on U937 Monocytic Cells

periodontal diseases are chronic inflammation caused by particular types of bacteria and have been recognized as a cause of tooth loss in adults. These bacteria which invade periodontal tissue are phagocytosed mainly by monocytes and macrophages in this immune response, and will be presented to lymphocytes. Recently, therapies for regenerating periodontal tissues have been used extensively to treat periodontal disease, and in particular, enamel matrix derivative (EMD) is commonly used for such therapies in Japan. Amelogenin is a type of the extracellular matrix protein that accounts for 90% of the constituents of EMD. In this study, we carried out a detailed microarray analysis in order to evaluate a gene group involved in amelogenin stimuli in the human monocytic cell line U-937. Microarray analysis revealed that statistically significant changes were apparent in 273 genes (163 up-regulated and 110 down-regulated) subsequent to 4 h of amelogenin stimulation. The most highly enriched categories included “cell cycle”, “DNA replication”, and “DNA repair” in up-regulated annotation terms. On the other hand, “type I diabetes mellitus”, “allograft rejection”, and “graft versus host disease” were observed in down-regulated annotation terms. Specifically, the gene expression of major to compatibility complex (MHC) class I/II and CD80/86 was impaired in U937 cells after stimulation with amelogenin. In addition, the results of heat-map showed that the gene expression of inflammatory cytokine such as tumor necrosis factor (TFN), interleukin-18 (IL-18), and CXCL16 was markedly decreased after stimulation of monocytes with amelogenin. In conclusion, the findings of our study showed that by inducing monocyte growth through the suppression of the antigen-presenting ability of U937 cells, amelogenin may affect the immune responses of periodontal tissues originating from monocytes. Porphyromonas gingivalis, is the most prominent member of the bacteria flora associated with pathogenesis of periodontitis, a chronic inflammatory disease resulting in tooth loss. The extent of oral mucosal reaction to P. gingivalis invasion relays heavily on Toll-like receptors (TLRs) that recognize structurally common motifs of pathogens and initiate antibacterial responses. Among the virulence factors of P. gingivalis implicated in TLRs activation and triggering inflammatory responses leading to the development of periodontitis is the bacterium cell-wall lipopolysaccharide (LPS). The engagement by the LPS of oral mucosal TLR4 leads to initiation of signaling events characterized by the activation of mitogen-activated protein kinase (MAPK) and IκB-kinase complex (IKK) cascades, induction of phosphoinositide-specific phospholipase C (PLC)/protein kinase C (PKC)/PI3K pathway, up-regulation in TGF-α ectodomain shedding and EGFR transactivation, and the amplification of proinflammatory signals by spleen tyrosine kinase (Syk). These events, in turn, exert their control over transcription factors implicated in the induction of the expression of cyclooxygenase-2 (COX-2) and inducible nitric oxide synthase (iNOS) genes that lead to up-regulation in the inflammatory mediators, PGE2 and NO. The systems involved in transcription factors activation, furthermore, remain under additional regulatory control through S-nitrosylation. Moreover, the LPS-induced TLR4 activation provides a docking site for Syk, the activation of which leads to amplification of the inflammatory signals by affecting transcription factors activation and their assembly to transcriptional complexes. Interestingly, the extent of oral mucosal inflammatory response to P. gingivalis remains under modulatory influence by two biologically active peptide hormones, leptin and ghrelin.
