Auxiliary partial liver transplantation for acute liver failure using "high risk" grafts: Case report

Acute liver failure(ALF) is a reversible disorder that is associated with an abrupt loss of hepatic mass, rapidly progressive encephalopathy and devastating complications. Despite its high mortality, an emergency liver transplantation nowadays forms an integral part in ALF management and has substantially improved the outcomes of ALF. Here, we report the case of a 32-year-old female patient who was admitted with grade Ⅳ hepatic encephalopathy(coma) following drug-induced ALF. We performed an emergency auxiliary partial orthotopic liver transplantation with a "high risk" graft(liver macrovesicular steatosis approximately 40%) from a living donor. The patient was discharged on postoperative day 57 with normal liver function. Weaning from immunosuppression was achieved 9 mo after transplantation. A follow-up using CT scan showed a remarkable increase in native liver volume and gradual loss of the graft. More than 6 years after the transplantation, the female now has a 4-year-old child and has returned to work full-time without any neurological sequelae.

Retrograde eclogite and garnet amphibolite of the Lüliangshan unit of the Shenglikou area,North Qaidam,were studied with emphasis on rutile and titanite.A special focus is on the formation of rutile and its corona of titanite(Ttn1)in retrograde eclogite and on coarse-grained titanite(Ttn2)from the garnet amphibolite.Using zirconium(Zr)-in-rutile and Zr-in-titanite thermometers,the temperatures estimated for the formation of an early generation of rutile are 823–884℃at 2.5–2.8 GPa,while 812–894℃at 1.3–1.5 GPa are derived for the formation of coronitic Ttn1 in the retrograde eclogite.Therefore,isothermal decompression must have occurred during exhumation,which also has triggered the partial melting of the retrograde eclogite.Ttn2 of the garnet amphibolite has high REE contents and high Th/U ratios,indicating that it is newly grown from a Ti,Ca,and LREE enriched anatectic melt derived from the partial melting of retrograde eclogite.LA-ICP MS U-Pb dating yields a lower intercept age of 423±4 Ma for Ttn2,which is consistent with the granulite-facies metamorphic age of the retrograde eclogite.Moreover,a temperature of 781–823℃at 1.0–1.2 GPa is obtained for Ttn2,which fits the P-T conditions of the HP granulite-facies metamorphic stage(P=1.07–1.24 GPa and T=774–814℃),and documents that the crystallization of the melt occurred at the granulite-facies stage at 423 Ma.The high amount of REE of the garnet amphibolite is a consequence of the formation of Ttn2 from the melt.The contents and ratios of Zr and Hf in rutile and Ttn2 differ from those in the garnet amphibolite,and the whole rock Zr/Hf ratios of retrograde eclogite and garnet amphibolite are both higher than the respective ratios in rutile and Ttn2,suggesting that rutile and titanite cannot be the major carriers of Zr and Hf accounting for the high whole rock Zr/Hf ratios.

Background:To assess efficacy and safety of oxcarbazepine (OXC) oral suspension in pediatric patients aged 2-16 years with partial seizures (PS) and/or generalized tonic-clonic seizures (GTCS) in real-world clinical practice in China.Methods:This 26-week,single arm,multicenter and observational study recruited patients aged 2-16 years with PS or GTCS suitable for OXC oral suspension treatment.Enrolled patients received OXC oral suspension treatment for 26 weeks.Primary endpoints included mean seizure frequency at the end of the treatment and mean seizure frequency reduction at the end of the treatment vs.baseline.Secondary efficacy-related endpoints and safety parameters were also assessed.Results:Nine hundred and eighty-seven pediatric patients were enrolled and 912 (92.4%) completed the study.The mean seizure frequencies at baseline and the end of week 26 were 13.40±64.92 and 1.62±19.47 times/month,respectively.The mean seizure frequency reduction was 10.03±63.67 times/month and the mean seizure frequency reduction percentage was 90.02%±5127.0% (P<0.0001).After 26 weeks of treatment,82.36%,7.24% and 3.86% of the patients became controlled,significantly improved and improved,respectively.Adverse events (AEs) were reported in 74 (7.65%) patients.Rash was the most common AE.The efficacy of OXC was not affected by seizure types,age or gender.Conclusion:This study confirms the efficacy and good safety profile of OXC oral suspension in Chinese pediatric patients aged 2-16 years with PS and/or GTCS.

OBJECTIVE: To evaluate the effect of ZiYin Xiehuo granules(ZYXH) and Zishen Qinggan granules(ZSQG) on partial precocious puberty(PPP).METHODS: This was a multicenter, randomized, single-blind, positive-controlled trial. A total of 143 pa-tients were assigned to either the ZYXH group or the ZSQG group using a random number table. The ZYXH group received ZYXH three times daily for 6 months and the ZSQG group received ZSQG three times daily for 6 months. Mammary nucleus diameter; the results of uterus, ovarian, and maximum follicle measures; and Chinese medicine symptom pattern scores were compared at baseline and after 3 months and 6 months of treatment.RESULTS: After 3 months' treatment, there were no significant differences between the two groups in mammary nucleus index changes(left 3.4 ± 3.1 vs3.5 ± 3.1, P = 0.790; right 3.0 ± 2.9 vs 3.6 ± 3.0, P =0.719). The uterine volume in the ZYXH group was smaller than that in the ZSQG group(2.1 ± 1.6 vs2.6 ± 2.2, P = 0.006). There were no significant between-group differences in ovarian volume and maximum follicular diameter on either side(ovarian volume: left 1.2 ± 0.7 vs 1.3 ± 0.6, P = 0.8; right1.2 ± 0.7 vs 1.4 ± 1.1, P = 0.984; maximum follicular diameter: left 3.9 ± 1.7 vs 3.5 ± 2.2, P = 0.158; right3.5 ± 1.7 vs 3.9 ± 2.1, P = 0.314).CONCLUSION: ZYXH granules and ZSQG granules both affected the size of the mammary nucleus in girls with PPP, and improved Chinese medicine symptom patterns. ZYXH granules showed slight advantages over ZSQG granules in terms of the decrease in the size of the uterus, ovaries, and ovarian follicles.

Rapid formation of autotrophic partial denitrification(APD)granules is of practical interest to start up an expanded granular sludge bed reactor for wastewater treatment.This study demonstrates that methanogenic granules can be easily acclimated into autotrophic partial denitrification granules in one day,with the ability to remove 82%of 2.7 kg-S/(m^3·d)sulfide into S^0 and to convert 97%of 0.9 kg-N/(m^3·d)nitrate into nitrite,which can provide a promising feedstock for anaerobic ammonia oxidation process.Arcobacter sp.is essential for S^0 accumulation.Under high loadings,the abundance of Arcobacter sp.decreased,while on the contrary the abundance of unclassified_p_Firmicutes increased,leading to the deterioration of autotrophic partial denitrification performance.The granules performance could be recovered by adopting the strategies of properly reducing the influent loadings.

Accelerated partial breast irradiation(APBI) focuses higher doses of radiation during a shorter interval to the lumpectomy cavity, in the setting of breast conserving therapy for early stage breast cancer. The utilization of APBI has increased in the past decade because of the shorter treatment schedule and a growing body of outcome data showing positive cosmetic outcomes and high local control rates in selected patients undergoing breast conserving therapy. Technological advances in various APBI modalities, including intracavitary and interstitial brachytherapy, intraoperative radiation therapy, and external beam radiation therapy, have made APBI more accessible in the community. Results of early APBI trials served as the basis for the current consensus guidelines, and multiple prospective randomized clinical trials are currently ongoing. The pending long term results of these trials will help us identify optimal candidates that can benefit from ABPI. Here we provide an overview of the clinical and cosmetic outcomes of various APBI techniques and review the current guidelines for selecting suitable breast cancer patients. We also discuss the impact of APBI on the economics of cancer care and patient reported quality of life.

A multivariate partial least squares approach to joint association analysis for multiple correlated traits

Many complex traits are highly correlated rather than independent. By taking the correlation structure of multiple traits into account, joint association analyses can achieve both higher statistical power and more accurate estimation. To develop a statistical approach to joint association analysis that includes allele detection and genetic effect estimation, we combined multivariate partial least squares regression with variable selection strategies and selected the optimal model using the Bayesian Information Criterion(BIC). We then performed extensive simulations under varying heritabilities and sample sizes to compare the performance achieved using our method with those obtained by single-trait multilocus methods. Joint association analysis has measurable advantages over single-trait methods, as it exhibits superior gene detection power, especially for pleiotropic genes. Sample size, heritability,polymorphic information content(PIC), and magnitude of gene effects influence the statistical power, accuracy and precision of effect estimation by the joint association analysis.

We present a female patient with preterm labor, severe viral hepatitis B of acute phase, hepatic encephalopathy stage Ⅲ and coma.After delivery, the illness was exacerbated and the patient presented with clinical signs of vital organ dysfunctions such as acute respiratory distress syndrome, cerebral edema and hypoxemia that needed mechanical ventilation.Emergency liver transplantation was recommended after multidisciplinary panel consultations.The donor, her mother, consented to donate her right liver.Auxiliary partial orthotopic living donor liver transplantion(APOLDLT) was performed.After operation, the patient was on triple medication of tacrolimus plus mofetil mycophenolate and prednisone for immunosuppression.The combination of antihepatitis B virus(HBV) immunoglobulin and entecavir was initiated for anti-HBV therapy.Both the patient and the donor recovered well without any complications.The patient was followed up regularly.Her liver function, clinical signs and symptoms improved significantly.Until now, the recipient has been living for more than 78 mo free of any complications.The APOLDLT is a life-saving modality for rescuing patients with high-risk acute liver failure following HBV infection without available donor and hence is recommended under standardized antiviral therapy coverage as stated above.

A New Fractional Projective Riccati Equation Method for Solving Fractional partial Differential Equations

In this paper, a new fractional projective Riccati equation method is proposed to establish exact solutions for fractional partial differential equations in the sense of modified Riemann–Liouville derivative. This method can be seen as the fractional version of the known projective Riccati equation method. For illustrating the validity of this method,we apply this method to solve the space-time fractional Whitham–Broer–Kaup(WBK) equations and the nonlinear fractional Sharma–Tasso–Olever(STO) equation, and as a result, some new exact solutions for them are obtained.
