Poly(ADP-ribose)polymerase(PARP)inhibitors are one of the most exciting classes of targeted therapy agents for cancers with homologous recombination(HR)deficiency.However,many patients without apparent HR defects also respond well to PARP inhibitors/cisplatin.The biomarker responsible for this mechanism remains unclear.Here,we identified a set of ribosomal genes that predict response to PARP inhibitors/cisplatin in HR-proficient patients.PARP inhibitor/cisplatin selectively eliminates cells with high expression of the eight genes in the identified panel via DNA damage(ATM)signaling-induced pro-apoptotic ribosomal stress,which along with ATM signaling-induced pro-survival HR repair constitutes a new model to balance the cell fate in response to DNA damage.Therefore,the combined examination of the gene panel along with HR status would allow for more precise predictions of clinical response to PARP inhibitor/cisplatin.The gene panel as an independent biomarker was validated by multiple published clinical datasets,as well as by an ovarian cancer organoids library we established.More importantly,its predictive value was further verified in a cohort of PARP inhibitor-treated ovarian cancer patients with both RNA-seq and WGS data.Furthermore,we identified several marketed drugs capable of upregulating the expression of the genes in the panel without causing HR deficiency in PARP inhibitor/cisplatin-resistant cell lines.These drugs enhance PARP inhibitor/cisplatin sensitivity in both intrinsically resistant organoids and cell lines with acquired resistance.Together,our study identifies a marker gene panel for HR-proficient patients and reveals a broader application of PARP inhibitor/cisplatin in cancer therapy.

BACKGROUND Gastric cancer is a common malignant tumor.Early detection and diagnosis are crucial for the prevention and treatment of gastric cancer.AIM To develop a blood index panel that may improve the diagnostic value for discriminating gastric cancer and gastric polyps.METHODS Thirteen tumor-related detection indices,38 clinical biochemical indices and 10 cytokine indices were examined in 139 gastric cancer patients and 40 gastric polyp patients to build the model.An additional 68 gastric cancer patients and 22 gastric polyp patients were enrolled for validation.After area under the curve evaluation and univariate and multivariate analyses.RESULTS Five tumor-related detection indices,12 clinical biochemical indices and 1 cytokine index showed significant differences between the gastric cancer and gastric polyp groups.Carbohydrate antigen(CA)724,phosphorus(P)and ischemia-modified albumin(IMA)were included in the blood index panel,and the area under the curve(AUC)of the index panel was 0.829(0.754,0.905).After validation,the AUC was 0.811(0.700,0.923).Compared to the conventional index CA724,the blood index panel showed significantly increased diagnostic value.CONCLUSION We developed an index model that included CA724,P and IMA to discriminate the gastric cancer and gastric polyp groups,which may be a potential diagnostic method for clinical practice.

Clinical management and associated costs for moderate and severe Alzheimer’s disease in urban China: a Delphi panel study

Background:Healthcare resource utilisation for Alzheimer’s disease(AD)in China is not well understood.This Delphi panel study aimed to describe the clinical management pathways for moderate and severe AD patients in urban China and to define the amount and cost of healthcare resources used.Methods:A panel of 11 experts was recruited from urban China to participate in two rounds of preparatory interviews.In the first round,9 physicians specialised in dementia gave a qualitative description of the clinical management of AD patients.In the second round,2 hospital administrators were asked about the cost of AD management and care.Results from the interviews were discussed by the experts in a Delphi panel meeting,where consensus was reached on quantitative aspects of AD management,including the rate of healthcare resource utilisation,the respective unit costs and caregiving time.Results:Interviewees reported that mild AD is under-recognised in China;most patients are diagnosed with moderate to severe AD.Loss of independence and agitation/aggression are the main drivers for healthcare resource utilisation and contribute to a heavier caregiver burden.It was estimated that 70%moderate AD patients are independent/non-aggressive at the time of diagnosis,15%are independent/aggressive,10%are dependent/non-aggressive,and 5%are dependent/aggressive.Dependent/aggressive AD patients are more likely to be hospitalised(70–90%)than accepted in a nursing home(0–20%),while the opposite is true for dependent/non-aggressive patients(5–35%for hospitalisation vs.80%for nursing home).Independent AD patients require 1–3 hours/day of caregiver time,while dependent patients can require up to 12–15 hours/day.Experts agreed that AD complicates the management of age-related comorbidities,found in 70–80%of all AD patients,increasing the frequency and cost of hospitalisation.Conclusions:The Delphi panel approach was an efficient method of gathering data about the amount of healthcare resources used and associated costs for moderate and severe AD

Establishment of a Stable PrP^(Sc) panel from Brain Tissues of Experimental Hamsters with Scrapie Strain 263K

Objective To establish a stable PrP^Sc panel from brain tissues of experimental hamsters infected with scrapie agent 263K for evaluating diagnostic techniques of human and animals＇ prion diseases. Methods Thirty brain tissue samples from hamsters intracerebrally infected with scrapie strain 263K and another 30 samples from normal hamsters were selected to prepare 10%, 1%, and 0.5% brain homogenates, which were aliquoted into stocks. PrP^Sc in each brain homogenate was determined by proteinase K digestions followed by Western blot assay and partially by immunohistochemistry. Stability and glycoforms of PrP^Sc were repeatedly detected by prp^SC-specific Western blots in half a year and 3 years later. Results PrPsc signals were observed in all 10% brain homogenates of infected hamsters. Twenty out of 30 stocks and 19 out of 30 stocks were PrPsc positive in 1% and 0.5% brain homogenatesof infected hamsters, respectively. TWenty-seven out of 30 stocks presented three positive bands in 10% brain homogenates, whereas none of 1% and 0.5% homogenates contained 3 bands. The detection of prpSc-specific signals stored in half a year and 3 years later demonstrated that the ratio of PrPsc positive samples and glycoforms was almost unchanged. All normal hamsters＇ brain homogenates were PrP^Sc negative. Conclusion A PrP^Sc panel of prion disease can be established, which displays reliably stable prp^Sc-specific signals and glycoforms.

Prediction of airway resistance in panel cave mines using a discrete and continuum model

The configuration of an airway(or production drift)in panel cave mines is different from the typical(straight)mine airway designs.The drawpoints are connected to the airway(cross-cuts),which allows airflow from the cave into the airway or air loss from the airway into the cave due to the ventilation approach and cave porosity.These affect airflow in the production drifts,but it is difficult to investigate these conditions from field or laboratory scaled studies.Therefore,this study develops discrete and continuum computational fluid dynamics(CFD)models to study the effects of the ventilation approach and cave porosity on the airway resistance.Our findings show that:with active undercut ventilation,a unique resistance model is required for the airway in panel cave mines;and an increase in cave porosity decreases the drift’s resistance.These findings provide essential tools for a panel cave ventilation design.

Analyzing economic feasibility for investing in nursing care: Evidence from panel data analysis in 35 OECD countries

Objective:To analyze economic feasibility for investing in nursing care.Method:The number of practicing nurses'density per 1000 population as a proxy for nursing staff and Gross Domestic Product(GDP)per capita(current US$)were collected in 35 member countries of Organization for Economic Co-operation and Development(OECD)over 2000-2016 period.The statistical technique of panel data analysis including unit root test,cointegration analysis,Granger causality test,dynamic long-run model analysis and error correction model were applied to measure economic impact of nursing-related services.Results:There was a committed bilateral relationship between nurse-staffing level and GDP with longrun magnitudes of 1.39 and 0A1 for GDP-lead-nurse and nurse-lead-GDP directions in OECD countries,respectively.Moreover,the highest long-run magnitudes of the effect nursing staff has on increasing GDP per capita were calculated in Finland(2.07),Sweden(1.92),Estonia(1.68),Poland(1.52),Czech Republic(1.48),Norway(1.47)and Canada(1.24).Conclusion:Our findings verify that although the dependency of nursing characteristics to GDP per capita is higher than the reliance of GDP to number of nurses'density per 1000 population,investing in nursing care is economically feasible in OECD countries i.***.nursing is not only a financial burden(or cost)on health care systems,but also an economic stimulus in OECD countries.Hence,we alert governments and policy makers about the risk of underestimating the economic impacts of nurses on economic systems of OECD countries.

Nonlinear bending analysis of a 3D braided composite cylindrical panel subjected to transverse loads in thermal environments

The aim of this study is to investigate nonlinear bending for a 3-Dimensional（3D）braided composite cylindrical panel which has transverse loads on its finite length. By refining a micro-macro-mechanical model, the 3D braided composite can be treated as a representative average cell system. The geometric structural properties of its components deeply depend on their positions in the section of the cylindrical panel. The embedded elastic medium of the panel can be described by a Pasternak elastic foundation. Via using the shell theory of the von Ka′rma′nDonnell type of kinematic nonlinearity, governing equations can be established to get higherorder shear deformation. The mixed Galerkin-perturbation method is applied to get the nonlinear bending behavior of the 3D braided cylindrical panel with a simply supported boundary condition.Based on the analysis of the braided composite cylindrical panel with variable initial stress, geometric parameter, fiber volume fraction, and elastic foundation, serial numerical illustrations are archived to represent the appropriate nonlinear bending responses.

Synchronization of home departure and arrival times in dual earner households with children:panel regression model of time gaps

Organizing schedules and allocating time to different activities is always a challenge in dual-earner households,especially when they have children.Parents may need to link their schedule to those of their children to allow them escorting their children to school or to take care or be with their children at home.This paper reports the results of an analysis of the degree of synchronization of home departure and arrival times in dual earner households with children,where the degree of synchronization is defined as the gap between departure and arrival times of a parent and child.Using activity-travel diary data of different household members,a random parameters regression model is estimated to examine differences in time gaps in home departure and arrival times between parents and children as a function of gender,day of the week,age of the youngest child,and other socio-demographic characteristics.The results of the analysis provide insight into factors influencing the degree of synchronization and coordination of double activity-travel scheduling decisions in households with children.Findings indicate that gender,number of children in the household,age of the youngest child,travel within or outside peak hours,day of the week,transport mode used for the work commute and household income level significantly affect time gaps,especially arrival time gaps.
