Determining oxygenation targets in acute respiratory distress syndrome(ARDS)remains a challenge.Althoughoxygenation targets have been used since ARDS was first described,they have not been investigated in detail.However,recent retrospective and prospective trials have evaluated the optimal oxygenation threshold in patientsadmitted to the general intensive care unit.In view of the lack of prospective data,clinicians continue to relyon data from the few available trials to identify the optimal oxygenation strategy.Assessment of the cost-benefitratio of the fraction of inspired oxygen(FiO_(2))to the partial pressure of oxygen in the arterial blood(PaO_(2))is an additional challenge.A high FiO_(2) has been found to be responsible for respiratory failure and deaths innumerous animal models.Low and high PaO_(2) values have also been demonstrated to be potential risk factors inexperimental and clinical situations.The findings from this literature review suggest that PaO_(2) values rangingbetween 80 mmHg and 90 mmHg are acceptable in patients with ARDS.The costs of rescue maneuvers needed toreach these targets have been discussed.Several recent papers have highlighted the risk of disagreement betweenarterial oxygen saturation(SaO_(2))and peripheral oxygen saturation(SpO_(2))values.In order to avoid discrepanciesand hidden hypoxemia,SpO_(2) readings need to be compared with those of SaO_(2).Higher SpO_(2) values may beneeded to achieve the recommended PaO_(2) and SaO_(2) values.

Tumor oxygenation level has been regarded as an attractive target to elevate the efficiency of photodynamic therapy(PDT).Cyanobacterial photosynthesis-mediated reversal of tumor hypoxia could enable an oxygen-boosted PDT,but is limited by scant penetration depth and efficiency of external light.Herein,aiming at the dual purposes of reducing biological toxicity induced by long-term light irradiation and alleviating hypoxia,we here introduce a novel-designed CaAl2O4:Eu,Nd blue persistent luminescence material(PLM)as the in vivo light source after pre-excited in vitro.The ingenious construction of blue-emitting PLM with“optical battery”characteristics activates cyanobacterial cells and verteporfin simultaneously,which performs the successive oxygen supply and singlet oxygen generation without the long-term external excitation,resulting in the modulated tumor hypoxic microenvironment and enhanced photodynamic tumor proliferation inhibition efficiency.Both in vitro cellular assessment and in vivo tumor evaluation results affirm the advantages of self-produced oxygen PDT system and evidence the notable antineoplastic outcome.This work develops an irradiation-free photosynthetic bacteria-based PDT platform for the optimization of both oxygen production capacity and light utilization efficiency in cancer treatment,which is expected to promote the clinical progress of microbial-based photonic therapy.

Dear Editor,Soldiers or people in battlefield settings are frequently injured with acute trauma, resulting in respiratory depression, hypoxia and associated morbidity and mortality. Traumatic brain injury(TBI) is with as much as 80%–90% of mild to moderate in combat casualties and contributed significantly to morbidity and mortality in battlefield settings. Correction of hypoxia at as capable as possible is expected to reduce or minimize the morbidity/mortality due to acute brain injury. An easy, quick and safe setup to promote oxygenation/ventilation of injured military personnel or peoples by non-medical personnel in battlefield settings immediately after acute injury before transfer to hospital is expected to reduce the morbidity/mortality due to injury mediated respiratory depression.

Oxygen reserve index,a new method of monitoring oxygenation status:what do we need to know?

The oxygen reserve index(ORI)is a new technology that provides real-time,non-invasive,and continuous monitoring of patients’oxygenation status.This review aimed to discuss its clinical utility,prospect and limitations.A systematic literature search of PubMed,MEDLINE,Google Scholar,and ScienceDirect was performed with the keywords of“oxygen reserve index,”“ORI,”“oxygenation,”“pulse oximetry,”“monitoring,”and“hyperoxia.”Original articles,reviews,case reports,and other relevant articles were reviewed.All articles on ORI were selected.ORI can provide an early warning before saturation begins to decrease and expands the ability to monitor the human body’s oxygenation status noninvasively and continuously with the combination of pulse oximetry so as to avoid unnecessary hyperoxia or unanticipated hypoxia.Although the technology is so new that it is rarely known and has not been applied to routine practices in hospitals,it shows good prospects for critical care,oxygen therapy,and intraoperative monitoring.

A smart MnO_(2)-doped graphene oxide nanosheet for enhanced chemo-photodynamic combinatorial therapy via simultaneous oxygenation and glutathione depletion

The combination of chemotherapy and photodynamic therapy provides a promising approach for enhanced tumor eradication by overcoming the limitations of each individual therapeutic modality.However,tumor is pathologically featured with extreme hypoxia together with the adaptable overexpression of anti-oxidants,such as glutathione(GSH),which greatly restricts the therapeutic efficiency.Here,a combinatorial strategy was designed to simultaneously relieve tumor hypoxia by self-oxygenation and reduce intracellular GSH level to sensitize chemo-photodynamic therapy.In our system,a novel multifunctional nanosystem based on MnO_(2)-doped graphene oxide(GO)was developed to co-load cisplatin(Cis Pt)and a photosensitizer(Ce6).With Mn O_(2)doping,the nanosystem was equipped with intelligent functionalities:(1)catalyzes the decomposition of H_(2)O_(2)into oxygen to relieve the tumor hypoxia;(2)depletes GSH level in tumor cells,and(3)concomitantly generates Mn^(2+)to proceed Fenton-like reaction,all of which contribute to the enhanced anti-tumor efficacy.Meanwhile,the surface hyaluronic acid(HA)modification could facilitate the targeted delivery of the nanosystem into tumor cells,thereby resulting in amplified cellular toxicity,as well as tumor growth inhibition in nude mice model.This work sheds a new light on the development of intelligent nanosystems for synergistic combination therapy via regulating tumor microenvironment.

Destratification and oxygenation efficiency of a water-lifting aerator system in a deep reservoir:Implications for optimal operation

Thermal stratification is a common phenomenon in lakes and reservoirs and has a significant influence on water quality dynamics. Heihe Reservoir is a canyon-shaped reservoir in Shaanxi Province with strong thermal stratification. Therefore, eight water-lifting aerators （WLAs） were installed in this reservoir, which could overcome thermal stratification and increase oxygenation with gas flows between 20 and 50 m3/hr, and oxygenate the hypolimnion with gas flows less than 20 m3/hr. To examine the destratification efficiency of the WLA system, we used a three- dimensional hydrodynamic module based on MIKE 3 to simulate the thermal structure of Heihe Reservoir and compared the simulations with measured data. Results showed that operation of the WLA system promoted water mixing and effectively oxygenated the hypolimnion. Through the established energy utilization assessment method, the energy utilization efficiency of the WLA system was between 5.36% and 7.30%, indicating the capability of the technique for destratification in such a large reservoir. When the surface water temperature dropped to the theoretical mixed water temperature calculated by the energy utilization assessment method, reducing gas flow could save energy. This would prevent anaerobic conditions from occurring in the bottom water and maintain good water quality in Heihe Reservoir.

Emotion processing in Parkinson's disease: a blood oxygenation level-dependent functional magnetic resonance imaging study

Parkinson's disease is a neurodegenerative disorder caused by loss of dopamine neurons in the substantia nigra pars compacta. Tremor, rigidity, and bradykinesia are the major symptoms of the disease. These motor impairments are often accompanied by affective and emotional dysfunctions which have been largely studied over the last decade. The aim of this study was to investigate emotional processing organization in the brain of patients with Parkinson's disease and to explore whether there are differences between recognition of different types of emotions in Parkinson's disease. We examined 18 patients with Parkinson's disease(8 men, 10 women) with no history of neurological or psychiatric comorbidities. All these patients underwent identical brain blood oxygenation level-dependent functional magnetic resonance imaging for emotion evaluation. Blood oxygenation level-dependent functional magnetic resonance imaging results revealed that the occipito-temporal cortices, insula, orbitofrontal cortex, basal ganglia, and parietal cortex which are involved in emotion processing, were activated during the functional control. Additionally, positive emotions activate larger volumes of the same anatomical entities than neutral and negative emotions. Results also revealed that Parkinson's disease associated with emotional disorders are increasingly recognized as disabling as classic motor symptoms. These findings help clinical physicians to recognize the emotional dysfunction of patients with Parkinson's disease.

Background:Although the use of extra-corporeal membrane oxygenation (ECMO) has been rapidly increasing,the benefit of ECMO in patients with acute respiratory distress syndrome (ARDS) remains unclear.Our objective was to investigate the effect of venovenous ECMO (VV-ECMO) on adult patients with severe ARDS.Methods:We conducted a multi-center,retrospective,cohort study in the intensive care units (ICUs) of six teaching hospitals between January 2013 and December 2018.Patients with severe ARDS who received W-ECMO support were included.The detailed demographic data and physiologic data were used to match ARDS patients without ECMO.The primary endpoint was the 28-day mortality.Results:Ninety-nine patients with severe ARDS supported by VV-ECMO and 72 patients without ECMO were included in this study.The acute physiology and chronic health evaluation Ⅱ score was 23.1 ± 6.3 in the ECMO group and 24.8 ± 8.5 in the control group (P =0.1195).The sequential organ failure assessment score was 12.8 ± 3.4 in the ECMO group and 13.7 ± 3.5 in the control group (P =0.0848).The 28-day mortality of patients with ECMO support was 39.4%,and that of the control group was 55.6%.The survival analysis curve showed that the 28-day mortality in the ECMO group was significantly lower than that in the control group (P =0.0097).Multivariate Cox regression analysis showed that the independent predictors of the 28-day mortality were the requirement of vasopressors before ECMO (hazard ratio [HR]:1.006;95% confidence interval [CI]:1.001-1.013;P =0.030) and duration of mechanical ventilation before ECMO (HR:3.299;95% CI:1.264-8.609;P =0.034).Conclusions:This study showed that ECMO improved the survival of patients with severe ARDS.The duration of mechanical ventilation and the requirement of vasopressors before ECMO might be associated with an increased risk of death.

Unexpected complication during extracorporeal membrane oxygenation support: Ventilator associated systemic air embolism

Systemic air embolism through a bronchovenous fistu-la(BVF) has been described in patients undergoing positive-pressure ventilation. However, no report has mentioned the potential risks of systemic air embolism through a BVF in patients undergoing extracorporeal membrane oxygenation(ECMO). Positive-pressure ventilation and ECMO support in patients with lung injury can increase the risk of systemic air embolism through a BVF. Increased alveolar pressure, decreased pulmonary venous pressure, and anticoagulation are thought to be the factors that contribute to this complication. Here, we present a case of systemic air embolism in a patient with ECMO and mechanical ventilator support.

Proton beam therapy in apneic oxygenation treatment of an unresectable hepatocellular carcinoma: A case report and review of literature

Presented here is the clinical course of a 63-yearold patient with a central, large and unresectable hepatocellular carcinoma(HCC) with liver metastases and tumor invasion of the portal and hepatic veins. After the tumor had been diagnosed, the patient was immediately treated with proton beam therapy(PBT), at a total dose of 60 Gy(relative biological effectiveness) in 20 fractions administered within 4 wk. To manage the respiratory movements, at the Rinecker Proton Therapy Center, apneic oxygenation was given daily, under general anesthesia. The patient tolerated both the PBT and general anesthesia very well, and did now show any signs of acute or late toxicity. The treatment was followed by constant reductions in the tumor marker alpha-fetoprotein and the cholestatic parameters gamma-glutamyltransferase and alkaline phosphatase. The patient commenced an adjuvant treatment with sorafenib, given at 6-wk intervals, after the PBT. Follow-up with regular magnetic resonance imaging has continued for 40 mo so far, demonstrating remarkable shrinkage of the HCC(maximal diameter dropping from approximately 13 cm to 2 cm). To date, the patient remains free of tumor recurrence. PBT served as a safe and effective treatment method for an unresectable HCC with vascular invasion.
