OBJECTIVE To examine the therapeutic effects and toxicity of high-dose-folinic acid plus a 5-fluorouracil(5-FU)bolus and continuous infusion with 5-FU combined with locally produced oxaliplatin(L-HOP)in treating advanced gastric carcinoma patients. METHODS Sixty-five patients with advanced gastric carcinoma were treated with high-dose-folinic acid plus a 5-FU bolus and a 48-h continuous infusion of 5-FU combined with *** effects of treatment and toxicity were observed. RESULTS There were 4 complete responses,26 partial responses, 30 with no change and 5 with progressive *** overall effective response rate was 46.2%(30/65).The median duration was 7 months,with the main side effects including nausea and vomiting,peripheral phlebitis,alopecia,leukopenia,dental ulcers, peripheral neuritis and *** the side effects were tolerated and minimal. CONCLUSION The results showed that high-dose folinic acid plus a 5-FU bolus and continuous infusion of 5-FU combined with oxaliplatin appears to be a safe and effective therapy for patients with advanced gastric *** therapeutic regimen is of value for these patients.

OBJECTIVES:To analyze the distribution characteristics of Traditional Chinese Medicine(TCM)syndromes in patients with oxaliplatin-induced peripheral neuropathy(OIPN)and observe the clinical efficacy of Bushen Yiqi formula(补肾益气方,BSYQF)in treating patients with ***:A total of 89 patients with OIPN were enrolled in this *** TCM syndrome characteristics were investigated by frequency analysis methodology after collecting and analyzing the TCM syndrome elements of the patients with the OIPN TCM syndrome element ***,62 cases of cold-dampness obstruction syndrome and kidney-Qi deficiency and cold syndrome were selected and randomly divided into the control group(n=31)and the treatment group(n=31).The patients in the treatment group were treated with modified BSYQF,while those in the control group were treated with mecobalamin tablets for 3 *** Levi sensory neurotoxicity score and the neuro-electrophysiological changes were observed before and after the treatment in both ***:The distribution of TCM syndrome types in 89 patients with OIPN were in order of kidney-Qi deficiency and cold syndrome(44 cases),cold-dampness obstruction syndrome(18 cases),Yin deficiency of liver and kidney syndrome(11 cases),blood stasis obstruction syndrome(7 cases),and dampness-heat obstruction syndrome(5 cases).Improvement in Levi sensory neurotoxicity score:After 3-week treatment,the total effective rate in the treatment group was higher than that in the control group(P<0.05).The subgroup analysis showed that the total effective rate in the treatment group of patients with kidney-Qi deficiency and cold syndrome was higher than that in the control group before and after treatment(P<0.05).Improvement in nerve conduction velocity:The sensory nerve conduction velocity of bilateral ulnar nerves improved in the control group after treatment compared with that before treatment(P<0.05).The sensory and motor nerve conduction velocities of the bilateral ulnar and

oxaliplatin is a novel platinum compound with clinical activity against several solid tumors. It has been associated with sensory neuropathies and, rarely, a neuromyotonia-like hyperexcitability syndrome;a form of peripheral neuropathy manifested by tremor and twitching activity of muscles. We present a case of hyperexcitability syndrome developed during the treatment of stage III appendiceal adenocarcinoma with oxaliplatin-containing regimen and was successfully treated with Pregabalin.

Objective:The aim of this study was to investigate the synergistic effects of oxaliplatin and teniposide on proliferation and apoptosis of gastric cancer cell line ***:MTT assay was carried to examine the inhibition rate of oxaliplatin and teniposide on gastric cancer cell line BGC-823 with various concentrations separately and *** apoptosis rate of BGC-823 cells under the treatment of oxaliplatin or/and teniposide was examined by flow *** expression level of livin, an apoptosis-associated protein, was explored by western ***:oxaliplatin or teniposide could remarkably inhibit the BGC-823 gastric cancer cell growth with a dose-response manner, separately and *** inhibition rate of oxaliplatin combined with teniposide on BGC-823 cells was higher than that of single oxaliplatin or single teniposide(P oxaliplatin for 12 h, 24 h and 48 h were 6.13%, 13.86% and 21.48%, respectively, while which of teniposide were 4.60%, 10.72%, 17.07%.But when the two medicines were carried associatively, the apoptosis rates for 12 h, 24 h and 48 h were 11.73%, 24.14% and 44.75%, *** blot showed that the expression level of livin was more down-regulated when cells were treated by oxaliplatin + teniposide than by oxaliplatin ***:The combination of oxaliplatin and teniposide can exert a synergistic effect on gastric cancer cell BGC-823.

A phase II trial of oxaliplatin plus S-1 as a first-fine chemotherapy for patients with advanced gastric cancer

Background Palliative chemotherapy has been shown to have a survival benefit for patients with recurrent or metastatic gastric cancer. We conducted a Phase II trial to determine the efficacy and safety of S-1 plus oxaliplatin （SOX regimen） as first-line chemotherapy for patients with unresectable locally advanced or metastatic gastric cancer. Methods Eligible patients had measurable lesions and no previous history of chemotherapy （except adjuvant chemotherapy）. oxaliplatin was administered intravenously at a dose of 130 mg/m2 on day 1. S-1 was administered orally in doses of 80, 100, or 120 mg/d according to body surface areas of 〈1.25 m2, 1.25-1.5 m2, or 〉1.5 m2 respectively; the total dose was divided into two daily doses on days 1-14. Treatments were repeated every 3 weeks until disease progression or intolerable toxicity occurred. Results Forty-three patients were enrolled in the study. All were assessable for efficacy and adverse events. The objective response and disease control rates were 55.8% and 76.7% respectively. The median follow-up time was 16.5 months. The median progression-free survival time was 7 months （95% CI, 5.8-8.2 months） and the median overall survival time was 16.5 months （95% CI, 9.7-23.3 months）. The one-year survival rate was 54.2%. Major adverse reactions were grade 3/4 neutropenia （9.3%） and thrombocytopenia （20.9%）. Conclusion The SOX regimen with oxaliplatin at a dose of 130 mg/m2 was found to be effective and safe as a first-line chemotherapy in Chinese patients with advanced gastric cancer.

Background To compare clinical efficacy and toxicity of irinotecan combined with 5-fluorouracil and leucovorin with those of oxatiplatin combined with 5-fluorouracil and leucovorin as first-line therapy for advanced colorectal cancer. Methods Literature search was performed by keywords ＂irinotecan＂, ＂oxaliplatin＂ and ＂colorectal cancer＂ on all randomized controlled trails reported on irinotecan versus oxaliplatin combined with 5-fluorouracil and leucovorin as first-line therapy for advanced colorectal cancer in MEDLINE, OVID, Springer, Cochrane Controlled Trials Register （CCTR） and CBMdisc （Chinese Biology and Medicine disc） before January 2010. Two authors drew the details of trial design, characteristics of patients, outcomes, and toxicity from the studies included. Data analysis was performed by RevMan 4.2. Results According to the screening criteria, 7 clinical studies with 2095 participants of advanced colorectal cancer were included in this meta analysis. The baseline characteristics of irinotecan group were similar to those of oxaliplatin group. The response rate of oxaliplatin group was higher than that of irinotecan group （relative risk （RR）=0.82, 95% confidence interval （95%C/）（0.70, 0.96）, P=0.01）, and the median overall survival of oxaliplatin group was longer by 2.04 months than that of irinotecan group （95%C/（-3.54, -0.54）, P=0.008）. In the comparison of grade 3-4 toxicity between the two groups, the incidences of nausea, emesis, diarrhoea and alopecia in irinotecan group were higher than those in oxaliplatin group （RR=1.94, 95%C/（1.22, 3.09）, P=0.005; 1.71, 95%C/（1.34, 2.18）, P 〈0.001; 14.56, 95%C/（4.11, 51.66）, P 〈0.0001）, respectively. However, the incidence of neurotoxicity, neutropenia and thrombocytopenia in irinotecan group were lower than those in oxaliplatin group （RR=0.06, 95%CI（0.03, 0.14）, P 〈0.00001; 0.70, 95%CI （0.55, 0.91）, P=0.006; 0.18, 95%CI（0.05, 0.61）, P=0.006）, respectively. Objective: To evaluate the activity and safety of combination chemotherapy with gemcitabine plus oxaliplatin (GEMOX regimen) in patients of advanced pancreatic carcinoma. Methods: 30 patients with advanced pancreatic cancer were enrolled into this study. All patients received gemcitabine 1000 mg/m2, given by 30-minute intravenous infusion, on days 1 and 8 of each 21-day cycle. oxaliplatin 100 mg/m2 was administered as a 2 h infusion on day 1 of each 21 day. Clinical outcomes for patients treated with two cycles of chemotherapy were evaluated according to WHO criteria. Results: All 30 patients were eligible for effectiveness and safety analysis. Objective response rate was approximately 20.0%. Clinical benefit response (CBR) was a composite of assessment of pain, performance status and body weight. The pain relief rate, improve-ment rate of performance status and body weight were 53.3%, 46.7% and 36.7%, respectively. The main adverse effects were bone marrow depression, peripheral nerve toxicity and gastrointestinal reaction. There was no treatment-related death during the chemotherapy. Conclusion: The high response rate with low toxicity observed in this study suggests that GEMOX regimen may be an effective alternative curative treatment for patients with advanced pancreatic carcinoma and can be used more extensively in clinical practice.

Objective： The aim of this study was to evaluate the efficacy and safety profile of DeFazio （S-l） combined with oxaliplatin against unresectable advanced or metastatic gastric cancer. Methods： oxaliplatin was given intravenously at 130 mg/m2 for 2 h on dl and S-1 was administered bid. at 80 mg/m2/day on d1-14 followed by a 7-day rest during the 3-week schedule. Results： All 62 patients were assessed for efficacy and adverse events. The response and disease control rates were 47.3% and 80.8%, respectively. The median time to progression was 7.8 months, and the median overall survival was 11.6 months. The grade 3/4 adverse events were hematological toxicities, including neutropenia （11.3%）, thrombocytopenia （9.7%） and gastrointestinal reactions （6.5%）. Conclusion： The SOX regimen （oxaliplatin, 130 mg/m2 d l; S-1, 80 mg/m2/day, bid. d1-14, q3w） provide a favorable efficacy and safety profile in patients with advanced gastric cancer.

Objective： To observe the efficacy and tolerability of continuously infusing 5-fluorouracil （5-FU） / folic acid combined with oxaliplatin （L-OHP/5-FU/LV regimen） as first line treatment in advanced colorectal cancer. Methods： 23 patients of advanced colorectal cancer were treated with 5-FU 500 mg/d, civ, d 1-d5, d8-d12, leucovorin 100 mg/d, iv gtt, d1, d8, folic acid tablet 60 mg/d, po, d2-d5, d9-d12, and oxaliplatin 65 mg/（m^2·d）, iv gtt, dl, d8, repeated every 21 days （one cycle）. The effect was evaluated after two cycles. Results： Complete response in 2 cases and partial response in 10 cases were observed with an overall response rate of 47.18%. Adverse effects were mainly grade 1-2, including nausea, vomiting, diarrhea, dental ulcer, peripheral neuritis and myelosuppression. Conclusion： L-OHP/5-FU/LV regimen is an effective and better tolerated alternative treatment in advanced colorectal cancer and yields promising clinical application.

Secondary malignant neoplasms are important late complications after chemotherapy and/or radiotherapy in cancer patients. Therapy-related malignancies include acute leukemia, myelodysplastic syndrome （MDS）, lymphoma, and solid tumors. However, secondary acute lymphoid leukemia or chronic myeloid leukemia （CML） is rarely reported. Here, we present a patient with P190BCR-ABL-positive CML following S-I plus oxaliplatin therapy for gastric adenocarcinoma. The patient has given written informed consent for the use of his medical data.
