Prevalence of unsuspected thyroid nodules in adults on contrast enhanced 16-and 64-MDCT of the chest

AIM: To determine the prevalence of unsuspected thyroid nodules on contrast enhanced 16and 64-modified discrete cosine transform (MDCT) of the chest, in a population of adult outpatients imaged for indications other than thyroid disease. METHODS: This retrospective study involved review of intravascular contrast-enhanced MDCT scans of the chest from 3077 consecutive adult outpatients, to identify unsuspected thyroid nodules. Exclusion criteria included history of thyroid cancer, known thyroid nodules or thyroid disease and risk factors for thyroid cancer, as evidenced by their medical records. One of 9 radiologists recorded number of nodules, location and bidirectional measurement of largest nodule, as well as amount of thyroid visualized on the chest computed tomography (CT). Presence of nodule was correlated with age, gender, race and percentage of thyroid imaged. RESULTS: A total of 2510 (2510/3077 or 81.6%) study subjects were included in the data analysis; among them,one or more nodules were identified in 629 subjects (629/2510 or 25.1%), with 242 (242/629 or 38.5%) having multiple nodules. Patients with nodule(s) were significantly older than those without (64 ± 13 years vs 58 ± 14 years, P nodule(s) (373/1222 or 30.5% vs 256/1288 or 19.9%, P nodules (167/373 or 44.8%) compared to men (75/256 or 29.3%, P nodules (427/629 or 67.9%) were less than 1 cm. CONCLUSION: This retrospective review revealed a prevalence of 25.1% for unsuspected thyroid nodules on contrast-enhanced chest CT.

Goldenhar Syndrome with Occipital Meningoencephalocele: A Rare Case

A 7-year-old male patient reported in our neurosurgery OPD with chief complaints of swelling in occipital region. The swelling was gradually increasing in size as per history given by patient’s attendant. Antenatal and birth history was uneventful. Developmental milestones were normal. Ear examination showed left sided pre auricular tag. Hearing was normal. Eye examination showed right eye limbal dermoid. Limbs examination showed syndactyly of 5th finger. Cardiovascular, respiratory and oral examination was normal. His height and weight were within normal limit. Chest X-ray was normal. Skeletal survey did not show any vertebral deformities. The karyotype was normal. The MRI brain showed a defect of size 1.3 cm in the occipital region through which cerebellum was seen herniating. The mass was surrounded by hypodense collection with multiple thickened septae within suggestive of CSF collection with thickened meninges. Patient underwent surgery and cystic portion was removed, with preservation of the occipital and cerebellar parenchyma and closure of defect was done. Patient responded well that there were no post operative complications and child was discharged on the 10th postoperative day. Hence, based on clinical and radiographic findings, a diagnosis of Goldenhar syndrome was made.

Early Detection of Thyroid Gland Disorders for Students in the Faculty of Applied Medical Sciences

In this study 70 male students were participated to determine the thyroid disorder through ultrasonography. Thyroid scan was done in 70 students prospectively with no indicative of thyroid disease (age of 19 - 23 yrs). Thyroid scan for students who participated the study took place in the department of Faculty of Applied Medical Sciences, King Abdulaziz University by using an IU 22 Philips ultrasound machine with a 5 - 12 MHz linear transducer. Among the total number of the subjects, 26% was found with abnormal ultrasound findings, 17% of them with cystic nodule, while solid and mixed nodule represented 4% for each. The high rate of abnormal findings of thyroid gland in the study suggested that screening using ultrasound scan was useful in detecting early thyroid disorders.

Thyroid tumors include those that originate from follicular cells and those that arise from parafollicular cells (C cells). Differentiated thyroid cancer, which originates from follicular cells, includes papillary carcinoma, follicular carcinoma, oncocytic cell carcinoma (Hürthle), poorly differentiated carcinoma, and anaplastic carcinoma. The incidence of thyroid cancer has been increasing significantly, with an estimated incidence in the United States of America of 53,990 cases by the year 2018. This neoplasm is listed as the most common endocrine tumor and represents approximately 3% of all malignant tumors in humans, with 75% of cases occurring in women, and two-thirds of cases occurring in people under 55 years. The increase in the prevalence/incidence of low-risk thyroid cancer over the last 10 to 20 years has required a re-appraisal of the standard one-size-fits-all approach to differentiated thyroid cancer. This adaptation to a more individualized management of the patient with thyroid cancer has led to a much more risk-adapted approach to the diagnosis, initial therapy, adjuvant therapy, and follow-up of patients with differentiated thyroid cancer. This paper with review the current understanding of the clinical presentation, diagnostic workup, and management of thyroid cancer centered on evidence-based and personalized medicine.

Measurement of shear wave velocities coupled with an evaluation of elasticity using ARFI elastography in diagnosis of papillary thyroid carcinoma

This study aimed to evaluate the diagnostic value of measuring the shear wave velocities (Vs) of thyroid nodules in diagnosis of the thyroid carcinoma. Using Virtual Touch Tissue Quantification (VTTQ) of acoustic radiation force impulse (ARFI) elastography (ACUSON S2000®, Siemens Medical Solutions, Mountain View, CA, USA), we measured the Vs of thyroid nodules in 39 nodules from 34 patients (four males and 30 females) before surgery. Elasticity itself was also estimated by VTTI (Virtual Touch Tissue Imaging) using ARFI. The average Vs of normal thyroid tissue was 2.24 ± 0.68 m/s. Twenty-three out of 39 nodules showed an average 2.52 ± 1.33 m/s. However, 16 other nodules showed “*** m/s”. These 16 nodules showed either “black” or “honeycomb” patterns of elasticity in VTTI. This value suggested that the Vs of these nodules were too fast or heterogenous to measure by this device. After the surgery, the pathologies of these nodules were revealed to be papillary carcinoma (21 cases), adenomatous goiter (11), follicular carcinoma (4), follicular adenoma (2) and follicular lymphoma (1). Five papillary carcinomas were found within adenomatous goiters. nodules from four follicular carcinomas showed average Vs of 1.92 ± 0.48 m/s, while two follicular adenomas and follicular lymphoma showed 2.19 ± 0.06 and 2.34 m/s respectively. nodules of adenomatous goiter showed 2.14 ± 0.60 m/s. On the other hand, nodules from five out of 21 papillary carcinomas showed average Vs of 4.00 ± 2.37 m/s. In the nodules of 16 papillary carcinomas, Vs were measured as “*** m/s” and this value was only observed in nodules of papillary carcinoma. These data reflected high and/or heterogenous elasticity of papillary carcinoma, in other words, the tissue of papillary carcinoma was hard and/or heterogenous. It was notable that the Vs value of “*** m/s” strongly suggested papillary carcinoma.

Objective:To investigate the clinical significance of airway hyperresponsiveness, eosinophil (Eos) percentage, levels of eosinophil cationic protein (ECP) and interleukin-5 (IL-5) in induced sputum in elderly patients with late onset asthma (LOA)during the remission period. Methods: Fifty-eight elderly patients with LOA during the remission period were selected. Bronchial challenge test was performed and the percentage of Eos and levels of ECP and IL-5 in induced sputum were measured by Wrights′ stain, immuno-CAP system and ELISA, respectively. Twenty-five elderly patients with LOA during their exacerbation period and 15 healthy elderly subjects were selected as control. Results:In fifty-eight LOA patients during their remission period, 49 cases (84.5%) showed positive bronchial challenge test and 9 cases(15.5 %) negative. The percentage of Eos, levels of ECP and IL-5[(9.3± 3.1)%], [(144.2±68.1) μg/L] and [(20.3±6.3) μg/L], in induced sputum in patients with LOA during their remission period were lower than those[ (21.1±9.8)%, (399.3±172.5) μg/L] and[(50.6±10.7) μg/L, respectively]in LOA group during the exacerbation period, and higher than those[(1.3±0.7)%,(48. 7±21.9) μg/L and (10.3±3.2) μg/L]in healthy group(all P<0.01). The percentage of Eos, levels of ECP and IL-5 in induced sputum were all higher [(12.7±3.6)% ]vs [(3.1±1. 3)%], [(171.3±69.8) μg/L] vs [(52.7±21.1) μg/L] and [(23.6±8.5) μg/L]vs[(13.7±4.1) μg/L] (all P<0.01) in positive group than negative group of bronchial challenge test in LOA patients during remission period.
