Stage Ⅱ surgical resection of hepatocellular carcinoma after TAE:a report of 38 cases

AIM To evaluate the curative effect of stage Ⅱ surgical resection of hepatocellular carcinoma after TAE. METHODS Thirty eight patients with unresectable hepatocellular carcinoma were treated by transcatheter arterial embolization (TAE). When the sizes of tumors were markedly reduced after TAE, stage Ⅱ surgical resections were performed. RESULTS Before TAE, the diameters of tumors were 12 84cm±4 87cm ( ±s ), but reduced to 5 12cm±1 82cm ( ±s ) after TAE ( P surgery, 26 patients were alive, with the longest survival of 96 months, twelve died and 10 had tumor recurrence. CONCLUSION Patients in moderate and advanced stages of hepatocellular carcinoma after TAE should be treated surgically, but the indication must be controlled strictly.

AIM To compare the validity of radiotherapy and surgery for operable esophageal carcinoma in 180 patients with pathologically proved esophageal carcinoma who had been accepted for surgery, but for various reasons, were given radical radiation therapy. METHODS The reasons of abandoning surgery in the patients were: poor cardiac function (21), poor pulmonary function (36), poor general condition (9), senility (aged 69～81 years), 32 and refusal by the patient (82). They were treated either by the isocenter technic alone or anteroposterior plus isocenter irradiation to a total dose of 50～70 Gy/5～7wk. RESULTS The 1 , 3 and 5 year survival rates were 64%, 34% and 23%. The 3 and 5 year survival rates showed that lesions in the upper third did better than those in the middle and lower thirds ( P surgery. The effect for lesions in the middle third was slightly inferior, and that for the lower third was even poorer than that of surgery. CONCLUSION The result of radiation alone for the operable esophageal carcinoma was similar to that of surgery.

Study on the causes of local recurrence of rectal cancer after curative resection: analysis of 213 cases

AIM To study the local recurrent rate and the causes of rectal cancer after surgery. METHODS The clinicopathological data of 213 rectal cancer patients and the follow up information were analyzed. The overall recurrent rate and the recurrent rates from different surgical appreaches were calculated. The main causes of recurrence were investigated. RESULTS Among the 213 cases, 73 (34 27%) had local recurrence. The recurrent time ranged from 3 months to 62 months after the first operation. Most of the recurrence ( 65/73 , 89 04%) occurred within 3 years after operation. CONCLUSION Local recurrence had no significant correlation with surgical methods or pathological types, but closely related to Dukes′ stages, location of primary tumors and the length of the distal rectum resected. Early resection and a wide tumor free resection margin are key factors to prevent local recurrence.

AIM To discuss the effect of surgical procedures on the prognosis of patients of bile duct cancer and their *** A retrospective analysis was made for 52 cases of hepatoportal bile duct cancer treated from January 1991 to December 1996. All the cases were classified according to the modified Bismuth-Corlettle system and received appropriate operation. Therapeutic effects were evaluated on the basis of their survival rates, jaundice elimination, comfort index, operative mortality and *** Seventeen cases received surgical resection (32.7%). The survival rate was 71.4%, 35.7% and 10.4% for one, two and three years respectively, and was 30%, 16.8% and 0% for those with drainage (P＜0.05). The mortality rate was 6.0% for the drainage group and 5.9% for the resection group (P＞0.05). Of the 17 resected patients, 8 (47.1%) had curative resection and 9 (52.9%) noncurative resection. Their mean survival time was 21.1 months and 7.5 months respectively (P＜0.05).CONCLUSION Proper surgical procedure should be used on the basis of the local and general conditions of the patients, and aggressive resection with or without liver resection is a valid procedure for the treatment of hepatoportal bile duct cancer and can significantly improve the prognosis of patients.
