BACKGROUND： Kawasaki disease （KD） is a common cause of acquired heart disease in children. Recent studies have focused on the biochemical markers of the myocardium, their high sensitivity and specificity and significance in the diagnosis of KD. This study aimed to determine the serum level of brain natriuretic peptide （BNP） and its relation with the heart function of children with KD and to explore its clinical value in diagnosis of ***： Forty-three KD children, aged from 5 months to 8 years （mean 2.3±0.6 years ）, were admitted to Qingdao Children＇s Hospital from February 2007 to April 2009. Among them 27 were male, and 16 female. The 43 patients served as a KD group. Patients with myocarditis, cardiomyopathy, congenital heart disease and other primary heart diseases were excluded. Thirty healthy children, aged from 3 months to 15 years （mean 2.5±0.8 years） or 17 males and 13 females served as a control group. There were no significant differences in age and gender between the two groups （P〉0.05）. In the KD group, ELISAwas used to measure the levels of serum BNP in acute and convalescent stages; and in the control group, the levels of serum BNP were measured once randomly. Left ventricular ejection fraction （LVEF）, left ventricular shorten fraction （LVSF）, cardiac index （CI） and left ventricular inflow velocity through the mitral annulus （including E-velocity and A-velocity） were measured by two- dimensional echocardiography in the acute and convalescent stages in the KD group. All data were expressed as mean±SD. The methods of analysis included Student＇s t test and the linear regression analysis test. Objective:To investigate the relationship between the level of serum B-type natriuretic peptide(BNP) and right ventricular infarction in patient s with acute inferior myocardial infarction(AIMI).Method:The serum BNP level was measured in 213 consecutive patient s with AIMI who were admitted in the coronary care unit(CCU) and in the normal control with 98 cases from October 2006 to May *** patients were divided into five groups in accordance with the coronary angiographic findings and electrocardiogram(ECG):Acute inferior myocardial infarction group;acute inferior myocardial infarction and right ventricular infarction group;control group;left anterior circumflex(LXC) group;proximal,middle and distal segment of right coronary artery(RCA)*** patient s were performed directly percutaneous coronary intervention(PCI) within 24 hours after the onset of *** incidence of major adverse cardiac events(MACE),including arrhythmia, heart failure,cardiac shock and mortality,had been observed during hospital,30 days and 3 ***:BNP level in the acute inferior myocardial infarction and right ventricular infarction group was significantly higher than that in acute inferior myocardial infarction group(P<0101).The level in the proximal2mid segment of RCA group was higher than that in the LXC group(P<0101).Additionally,logistic regression analysis showed that the level of BNP was an independent predictor of MACE in the 30 days and 3 months in acute inferior myocardial infarction patient s (r=0.701 0,95%CI<0.01 - 0.615,P<0101).Conclusion:We demonstrated the level of BNP in patients with acute inferior myocardial infarction or/ and right ventricular infarction,and BNP could be a good predictor for patients with acute inferior myocardial infarction and right ventricular infarction.

Background To the effect of percutaneous coronary intervention （PCI） on plasma level of N-terminal pro-B-type natriuretic peptide （NT-proBNP） in patients with coronary heart disease （CHD） and normal left ventricular function. Methods One hundred and five patients with CHD and normal ventricular function were enrolled. Blood samples for assessment of NT-proBNP and cTn-T were collected before and after PCI. Results The mean left ventricular ejection fraction was 60.3 ± 5.3%. After revascularization, the level of lgNT-proBNP was signifi-cantly reduced （2.40 ± 0.44 vs 2.23± 0.43, P 〈 0.001）. Subgroup analysis showed that the level of lgNT-proB-NP ws consistently decreased in different clinical classifications （stable angina： 45, unstable angina： 31 and acute myocardial infarction： 29） and target-vessel revascularization （left anterior descending artery： 30, left cir-cumflex artery： 26 and right coronary artery： 49）, and in 99 patients without elevation of post-procedural cTn- T, but it showed a trend of non-significant increase in 6 patients with elevated cTn-T. Conclusions Our study demonstrates that successful PCI reduces plasma NT-proBNP concentration in patients with CHD and normal ven-tricular function. This implicates that the impact of PCI should be considered in the interpretation of NT-proBNP change in clinical practice, and further studies are necessary to investigate the direct and/or indirect effect of myocardial ischemia on BNP/NT-proBNP.

Takotsubo cardiomyopathy(TC) is characterized by reversible ventricular dysfunction, not limited to the distribution of an epicardial coronary artery. A disease primarily afflicting post-menopausal women, it is frequently mistaken for acute anterior wall myocardial infarction. Alternatively called Stress Cardiomyopathy, physical or emotional triggers are identified in only three fourths of TC patients. Long considered a benign condition, recent findings suggest poor short term prognosis similar to acute coronary syndrome(ACS). Despite the widely recognized pathophysiological role of catecholamine excess, its diagnostic role is uncertain. TC is suspected based on typical wall motion abnormalities in ventriculogram or echocardiogram. Several additional electrocardiographic, laboratory and imaging parameters have been studied with the goal of clinical diagnosis of TC. While several clinical clues differentiate it from ACS, a clinical diagnosis is often elusive leading to avoidable cardiac catheterizations. natriuretic peptides(NPs), a family of peptide hormones released primarily in response to myocardial stretch, play a significant role in pathophysiology, diagnosis as well as treatment of congestive heart failure. TC with its prominent ventricular dysfunction is associated with a significant elevation of NPs. NPs are elevated in ACS as well but the degree of elevation is typically lesser than in TC. Markers of myocardial injury such as troponin are usually elevated to a higher degree in ACS than in TC. This differential elevation of NPs and markers of myocardial injury may play a role in early clinical recognition of TC.
