Oncocytic metaplasia is a well-established pathological entity that may occur in many organs;including salivary glands, thyroid, parathyroid and kidney. The occurrence of oncocytic metaplasia in the nasopharynx is uncommon and metaplasia with melanin pigmentation is extremely rare, no more than twenty cases having been reported to date.

A tumor-like presentation of systemic amyloidosis in the nasopharynx is exceedingly rare. A male of 52-year-old with a recurrent mass in the nasopharynx underwent surgical resection 2 years ago. Immunohistochemical examination revealed M protein band and kappa immunoglobulin light chain. The sections of the biopsy of the mass in the nasopharynx and anterior abdominal wall were stained with Congo red and examined with microscopy. Amyloid deposits were found. It was diagnosed as systemic amyloidoma. Colchicine, melphalan and prednisone were administered. The mass decreased its size in the 8 months of follow-up. This case suggests that a solitary tumor in the nasopharynx might be an early sign of systemic amyloidosis because of the slow progression of the disease. The existence of a solitary amyloidoma in any location of the body should lead the doctor to look for other symptoms and signs of systemic amyloidosis.

Primary salivary gland-type carcinoma of the nasopharynx is a rare malignancy with aggressive clinical behavior and poor prognosis. Few cases have been reported in the literature. We report the case of a salivary gland-type carcinoma of the nasopharynx diagnosed in a 70-year-old woman managed in department of radiotherapy at Hassan II University Hospital in Morocco. The patient received a total of 70 Gy of radiations, which were divided into 2 Gy per session during 7 weeks. Follow-up was performed regularly, including nasal endoscopic examination and nasopharyngeal CT. Evolution was marked by disease control after 12 months of follow-up. The aim of this work is to show the rarity of this entity, describe clinical, radiological and histological features and finally discuss the treatment management.

Background: Tuberculosis (TB) of the nose and nasopharynx are rare localizations for tuberculosis to manifest. Aim: We hope to present a case of tuberculosis of the nasopharynx and its rare presentation. Case presentation: This patient presented with a long standing history of headache and neck pain prior to being seen. Imaging showed a mass at the nasopharynx and a biopsy con-firmed tuberculosis. Patient was initiated on anti-tuberculosis therapy with improvement in her symptoms on follow up. Conclusion: A high index of suspicion is imperative for early diagnosis.

The Epstein-Barr virus (EBV) is consistently associated with nasopharyngeal non-keratinizing carcinoma (NKC). 1 Zong et ala reported that keratinizing squamous cell carcinoma (KSCC) is also closely related to EBV infection. The question of how many of the EBV infection rates are related to the adenosquamous carcinoma/mucoepidermoid carcinomas (ASC/MEC)and adenocarcinoma (AC) needs to be answered. Cheung et al3 reported that 92% (34/37) of nasopharyngeal carcinoma (NPCs), most of them might be NKCs harbored EBV with 30 bp-deleted LMP1 gene (delLMP1) in Hong Kong. Is this also the situation in Guangzhou? Are there any differences in the frequency of del-LMP1 EBV detected among four histological types of NPC? What is the role of del-LMP1 EBV playing in nasopharyngeal carcinogenesis? And this study aims at solving these issues.

Nasopharyngeal carcinoma (NPC) is a common cancer in Southern China and Southeast *** disease is a poorlydifferentiated carcinoma without effective cure,and the mechanism underlying its development remains largely *** several factors identified in NPC aetiology in recent years,Epstein-Barr virus (EBV) infection has emerged to bemost *** almost all NPC cells,EBV uses several intracellular mechanisms to cause oncogenic evolution of theinfected *** such mechanism by which EBV infection induces cellular immortalization is believed to be throughthe activation of telomerase,an enzyme that is normally repressed but becomes activated during cancer *** show that greater than 85% of primary NPC display high telomerase activity by mechanisms involving EBVinfection,consistent with the notion that EBV is commonly involved in inducing cell *** recently,different EBV proteins have been shown to activate or inhibit the human telomerase reverse transcriptase gene,bymodulating intracellular signalling *** findings suggest a new model with a number of challenges towardsour understanding,molecular targeting and therapeutic intervention in NPC.

OBJECTIVE: To observe the biological changes of primary human nasopharyngeal epithelial cells in the early stage of immortalization. METHODS: The morphological changes of nasopharyngeal epithelial cells were observed by phase contrast microscopy, and the activity profile of senescence-associated beta-galactosidase (SA-beta-Gal) was detected by SA-beta-Gal staining. The expression of p16(INK4a) protein was tested by immunochemical assay, and the life span in vitro of nasopharyngeal epithelial cells was calculated as population doublings. In addition, the expression of Epstein-Barr (EB) virus latent membrane protein 1 (LMP1) was also detected by immunofluorescence staining. RESULTS: Morphologically, cells treated with EB virus and 12-o-tetradecanoyl-phorbol-13-acetate (TPA) formed multi-layer foci, and their cellular life span in vitro was extended (about 155 days of culture). A low percentage of cells (about 4.8%) expressed SA-beta-Gal activity at late primary culture, and did not always express p16(INK4a) protein in the progression of culture. CONCLUSIONS: Nasopharyngeal epithelial cells treated with EB virus in cooperation with TPA can pass through the stage of senescence and enter the early stage of immortalization. Some changes of phenotype occur in these *** results provide data for further studying the mechanism of immortalization and the establishment of a human nasopharyngeal epithelial cell line.

Background: Streptococcus pneumoniae (pneumococcus) is one of the most frequent causes of bacterial infection in children and is a leading cause of otitis, sinusitis, pneumonia, and meningitis worldwide. Nasopharyngeal colonization is a risk factor for pneumococcal disease, a leading cause of complications and death in infants. HIV-infected persons are at high risk of invasive pneumococcal disease. Method: Nasopharyngeal swabs were collected from 296 HIV infected children below five years recruited from Gertrude’s Children hospital and Nazareth Hospital Nairobi, Kenya. The nasopharyngeal swabs were processed to isolate S. pneumoniae, which were serotyped and tested for drug susceptibility. Results: The carriage prevalence of S. pneumoniae in the study was 30.4% while the isolated serotypes were (in order of decreasing frequency): 35B, 19F, 3, 13, 15A, 11A, 16F, 7C and 23A. Most of the serotypes were resistant to the commonly used antibiotics but all were susceptible to vancomycin and chloramphenicol. Conclusion: Carriage prevalence of nasopharyngeal S. pneumonia in HIV infected children was lower than that of similar prevalence studies in children. Most of the S. pneumoniae isolates were however non pneumococcal vaccine isolates.

In Japan, the heptavalent pneumococcal conjugate vaccine (PCV7) became available in February 2010 and was subsidized by the national funding system from May 2011 in Okinawa, after which it was incorporated into the national immunization practice (NIP) in April 2013 using a 3 + 1 schedule for all infants. We conducted an annual survey in 2012 to determine the effect of PCV7 on nasopharyngeal colonization by pneumococcal serotypes and to analyze the risk factors for colonization in infants. Nasopharyngeal swabs for pneumococcal isolation and serotyping were obtained from infant 2 to 22 months of age before and after PCV7 immunization among 4 clinics in Okinawa, Japan. Between January 2012 and December 2012, nasopharyngeal swabs for bacterial cultures were obtained among 782 infants aged 2 to 22 months old and demographic data was obtained among 725 participant infants. Among the 725 evaluable infants, 193 pneumococcal strains were detected in 180 infants for an overall nasopharyngeal carriage of 24.8%. The main capsular serotypes isolated were 6C (16.1%), 19A (12.4%) and 15B (9.8%). Carriage of PCV7 serotypes accounted for 21.8% (42/193). The result of multivariate data analysis showed the pneumococcal carriage rate of non-PCV7 serotypes was significantly (P < 0.001) high in infant with siblings and daycare attendance. On the other hand, the result of multivariate data analysis showed that carriage rate of PCV7 serotype had only significantly high risk in infant with siblings and did not have a significant risk dependent on age and daycare attendance. Carriage PCV7 serotypes increased in the presence of other siblings, while PCV7 vaccination was shown to eliminate daycare attendance as a risk. The results of this study demonstrates that PCV7 vaccination decrease the overall nasopharyngeal carriage of PCV7 serotypes in vaccinated children including children at risk such as children attending day-care centers.
