BACKGROUND System based practice(SBP) milestones require trainees to effectively navigate the larger health care system for optimal patient care. In gastroenterology training programs, the assessment of SBP is difficult due to high volume, high acuity inpatient care, as well as inconsistent direct supervision. Nevertheless,structured assessment is required for training programs. We hypothesized that objective structured clinical examination(OSCE) would be an effective tool for assessment of *** To develop a novel method for SBP milestone assessment of gastroenterology fellows using the *** For this observational study, we created 4 OSCE stations: Counseling an impaired colleague, handoff after overnight call, a feeding tube placement discussion, and giving feedback to a medical student on a progress note. Twentysix first year fellows from 7 programs participated. All fellows encountered identical case presentations. Checklists were completed by trained standardized patients who interacted with each fellow participant. A report with individual and composite scores was generated and forwarded to program directors to utilize in formative assessment. Fellows also received immediate feedback from a faculty observer and completed a post-session program evaluation *** Survey response rate was 100%. The average composite score across SBP milestones for all cases were 6.22(SBP1), 4.34(SBP2), 3.35(SBP3), and 6.42(SBP4)out of 9. The lowest composite score was in SBP 3, which asks fellows to advocate for cost effective care. This highest score was in patient care 2, which asks fellows to develop comprehensive management plans. Discrepancies were identified between the fellows’ perceived performance in their self-assessments and Standardized Patient checklist evaluations for each case. Prototyping is one of the core activities of User-Centered Design (UCD) processes and an integral component of Human-Computer Interaction (HCI) research. For many years, prototyping was synonym of paper-based mockups and only more recently we can say that dedicated tools for supporting prototyping activities really reach the market. In this paper, we propose to analyze the evolution of prototyping tools for supporting the development process of interactive systems. For that, this paper presents a review of the literature. We analyze the tools proposed by academic community as a proof of concepts and/or support to research activities. Moreover, we also analyze prototyping tools that are available in the market. We report our observation in terms of features that appear over time and constitute milestones for understating the evolution of concerns related to the development and use of prototyping tools. This survey covers publications published since 1988 in some of the main HCI conferences and 118 commercial tools available on the web. The results enable a brief comparison of characteristics present in both academic and commercial tools, how they have evolved, and what are the gaps that can provide insights for future research and development.

The United States medical education system has undergone dramatic changes over the past 350 years. The system has evolved from an individualized apprenticeship format into a standardized, nationally accredited and outcomes based system of training. As residencies in specialty care developed so did the recognized need for a robust primary care system to meet the national healthcare demand; thus leading to the creation of the American Board of Family Practice. Medical education in the US operates on a continuum from college to medical school to residency training to continuing lifelong *** of these components are overseen by nationally recognized accrediting bodies that are designed to ensure the public that physicians are competent and qualified to deliver safe and effective patient care. The most recent trend has been led by the Accreditation Council for Graduate Medical Education(ACGME) through its Next Accreditation System(NAS). The NAS focuses on continuous quality improvement in residency training programs, achievement of observable milestones by individual residents, and sponsoring institutional oversight of the learning environment. These NAS concepts are also expanding into medical school education and continuing medical education areas. These constructs have been incorporated into the newest requirements for Family Medicine training also with a special emphasis on ambulatory care, faculty development, scholarly activity, and defined program outcomes. The evolution of the US medical education system has resulted in a focus on outcomes,national standards, national accreditation, and competent physicians to provide safe and effective are.
