Protocol of Jidong Women Health Cohort Study: Rationale, Design, and Baseline Characteristics

Objective The Jidong Women Health Cohort Study is a prospective cohort study on female-specific characteristics and risks of chronic diseases in Chinese women and focuses on the potential association between menopause and risks of cardiovascular disease(CVD). Methods The study includes 4,179 female participants with an age of older than 18 years from Caofeidian district, Tangshan city, northern China. Baseline information on female-specific characteristics and potential cardiovascular risk factors was collected and all the participants underwent a physical examination with blood samples collected in 2013. To establish a better risk assessment tool of female CVD, updated information from questionnaire investigation, physical examinations and occurrence of outcome events will be collected through a longitudinal follow-up annually up to the year 2024. Results At baseline, Mean age of the participants was 42.3 ± 12.8 years. Reproduction occurred in 2,948 participants(70.5%), menopausal transition in 173(4.3%), and postmenopause in 1,058(25.3%). The incidence of arterial hypertension, dyslipidemia, and diabetes showed significant difference across different groups stratified by Stage of Reproductive Aging Workshop(STRAW) system(P < 0.05). Conclusion The Jidong Women Health Cohort Study will contribute to the scientific evidence on association between female-specific characteristics and cardiovascular risks, and will also be helpful to provide a new path for early detection and prevention of CVD.

Royal jelly for genitourinary syndrome of menopause:A randomized controlled trial

Objective:Genitourinary syndrome of menopause is a progressive condition characterized by a decrease in estrogen,which causes bothersome genital symptoms.The purpose of the study was to determine the effect of royal jelly on the genitourinary syndrome of menopause.Materials and methods:A randomized controlled trial was carried out from November 2018 to June 2019 in Bandar Abbas,Iran.The trial was registered in the Iranian Registry of Clinical Trials(1RCT20181107041585N1)with the main objective of determining if royal jelly could reduce the genitourinary syndrome of menopausal women.Eligible women were randomly assigned to receive either daily 1g of oral royal jelly or placebo for 8 weeks.The urogenital subscale of the Menopausal Rating Scale was used to determine genitourinary syndrome.Independent samples t-test was used for inter-group comparisons and paired samples t-test for pre-and post-treatment comparisons.Results:There were no differences in the severity of sexual problems,bladder complications,or vaginal dryness between groups before intervention.Although the intervention group's bladder complications improved slightly after eight weeks of royal jelly treatment compared to the control group(p?0.04),there were no significant changes in vaginal dryness,sexual problems,or total urogenital score.The within-group changes(before and after treatment)also showed no differences in urogenital symptoms.Conclusions:A daily dose of 1g royal jelly taken orally for 8 weeks did not alleviate menopausal genitourinary syndrome.No serious side effects were observed.To make more reliable decisions about the use and safety of royal jelly in the future,different doses of royal jelly and longer trials are required.

Combined treatment with valproic acid and estrogen has neuroprotective effects in ovariectomized mice with Alzheimer’s disease

Postmenopausal women with Alzheimer’s disease exhibit dramatically reduced sensitivity to estrogen replacement therapy,which is though to be related to an estrogen receptor(ER)α/ERβratio imbalance arising from a significantly decreased level of ERs of the brain.The aim of our study was to investigate whether valproic acid(VPA)can enhance the beneficial effects of estrogen on cognitive function through restoration of ERαand ERβexpression in the brain.We removed the ovaries of female APP/PS1 mice to simulate the low estrogen levels present in postmenopausal women and then administered VPA(30 mg/kg,intraperitoneal injection,once daily),17β-estradiol(E2)(2.4μg,intraperitoneal injection,once daily),liquiritigenin(LG)(50μg/kg,intragastric infusion,once daily),VPA+E2,or VPA+LG for 4 successive weeks.Compared with treatment with a single drug,treatment with VPA+E2 or VPA+LG significantly increased the level of glycogen synthase kinase 3β,increased the expression of estrogen receptorα,reduced the expression of small ubiquitin-like modifiers,and increased the level of estrogen receptorβ.This resulted in enhanced sensitivity to estrogen therapy,reduced amyloidβaggregation,reduced abnormal phosphorylation of the tau protein,reduced neuronal loss,increased dendritic spine and postsynaptic density,and significantly alleviated memory loss and learning impairment in Alzheimer’s disease.This study was approved by the Chongqing Medical University Animal Protection and Ethics Committee,China on March 6,2013.

Objective:This study explored the relationship between menopause and metabolic syndrome(MetS),stroke,hyperlipidemia,diabetes and hypertension.Study design:This cross-sectional study surveyed 440 women in Yuci,China in 2012.Main outcome measures:MetS,diabetes,hypertension,hyperlipidemia,stroke,and behavioral and demographic variables.Results:The prevalence of MetS in this study was 40.28%to 49.66%(p=0.065)among preand post-menopausal women,respectively,after adjusting for age.Conclusions:The prevalence of diabetes,stroke,hypertension,and hyperlipidemia was higher among post-than pre-menopausal women.Health screenings for women in China should consider the increased risk for metabolic disorders during the postmenopausal stage of life.

Risk of venous thromboembolic disease in postmenopausal women taking oral or transdermal hormone replacement therapy

Objective: The influence of hormone replacement therapy (HRT) on hemostasis processes depends on the type of hormone,the combination of doses,the time of taking HRT,and the route of administration (oral,transdermal,implanted).The aim of the current study was to assess some parameters of coagulation,especially tissue factor pathway inhibitor (TFPI) and tissue factor (TF) in postmenopausal women using oral or transdermal HRT.Methods: The study was conducted on 76 healthy women,including 46 women aged 44–58 years who were taking oral (26) or transdermal (20) HRT,and 30 women aged 44–54 years who did not take HRT as the control group.Plasma concentrations of TF,TFPI,thrombin-antithrombin complex (TAT),and D-dimer were performed by enzyme-linked immunosorbent assay (ELISA).Moreover,the concentration of fibrinogen and activity of protein C were measured by chromogenic and chronometric methods.Results: We observed a significantly higher concentration of TF and a significantly lower concentration of TFPI in women taking oral and transdermal HRT in comparison with the control group.We also found a significantly lower concentration of fibrinogen in women taking oral HRT vs.the control group.Moreover,no statistically significant changes in concentrations of TAT and D-dimer,or activity of protein C were noted.Conclusions: In this study,the occurrence of an increased TF concentration simultaneously with a decreased concentration of TFPI in women taking HRT indicates hypercoagulability.No significant modification of TAT or D-dimer occurred,and thus there may not be increased risk of thrombosis.

Menopausal depression:comparison of hormone replacement therapy and hormone replacement therapy plus fluoxetine

BACKGROUND: To compare the efficacy and safety of hormone replacement therapy (HRT) combined with fluoxetine, with HRT alone, in post-menopausal women suffering from depression. METHODS: A randomized, open-label, parallel trial was applied. HRT was administered to all patients for 2 cycles, with 14 days of estrogen therapy and 14 days of estrogen plus progesterone. Patients who were randomly assigned to the HRT plus fluoxetine group were given fluoxetine in combination with HRT. Hamilton Depression Rating Scale (HAMD), Kupperman Menopausal Index (KMI), and Clinical Global Impressions scale were used to measure the efficacy. RESULTS: One hundred and twenty-three post-menopausal patients with depression were enrolled in the study. Among them, 120 had at least one post-treatment visit and entered into the statistical analysis. The mean total HAMD scores were significantly lower, and the percentages of HAMD score reductions were higher in the HRT plus fluoxetine Group compared with the HRT Group, after at least 3 weeks of treatment, with an average difference of 5 points at the endpoint. The Clinical Global Impression-Severity and Clinical Global Impression-Improvement scores were significantly different in the 2 groups, in favor of the combination therapy. The mean total KMI was significantly lower in the Combination Group compared with the HRT Group, after at least 6 weeks of treatment, with an average 4.5-point difference between the groups. No statistically significant differences were found in most of the adverse events reported in the Combination Group compared with the HRT group, with the exception of 3 symptoms, i.e., dry mouth, loss of appetite, and abdominal distention. They were mild to moderate in severity. Two patients in the HRT group, but none in the combination group, dropped out due to adverse events. CONCLUSION: HRT plus fluoxetine therapy was effective in the treatment of menopausal depression with a satisfactory safety profile.

Effect of Guizhi Gancao Longgu Muli Tang on sleep disturbances in menopausal women

OBJECTIVE: To evaluate the effect of Guizhi Gancao Longgu Muli Tang(GGLMT), a decoction prepared with herbal medicine of Traditional Chinese Medicine, on sleep disturbances in women with menopause.METHODS: Totally 162 participants were recruited for the treatment of sleep difficulty from February,2012 to December, 2014. Decoction of 200 mL was taken by every participant twice daily in half an hour after lunch and dinner during two weeks.Sleep quality was assessed by Pittsburg sleep quali-ty index(PSQI) and menopausal symptoms and quality of life were evaluated by the menopause rating scale(MRS) and the Chinese version of World Health Organization quality of life-BREF at the final fellow-up in the fourth weekend after beginning.RESULTS: The average scores of PSQI had reduced from(13.82 ± 4.97) to(8.14 ± 3.19), 95% CI(- 4.87,- 3.05) after 2-week GGLMT treatment in the fourth week. GGLMT improved symptoms in patients with more severe conditions(MRS ≥ 16).Three adverse drug reaction, mouth ulcer, constipation, and folliculitis, might be related with GGLMT and disappeared after withdrawals of the treatment.CONCLUSION: For menopausal women suffering from chronic sleep disturbances, our findings suggest that two weeks treatment of GGLMT was safe and effective.

Hemorrhagic Bartholin’s cyst in a woman using anti-platelet medication:A case report and review of the literature

BACKGROUND We report the case of a postmenopausal female with a hemorrhagic Bartholin’s cyst who has been using an antiplatelet medication.CASE SUMMARY A postmenopausal woman,84 years of age,had a medical history of hypertension,diabetes mellitus,coronary artery disease(three-vessel disease),chronic kidney disease(stage 3),and dementia.The patient has been taking clopidogrel,an antiplatelet medication,for several years.She presented at our outpatient clinic complaining of painful swelling over her left vulva for several days.A Bartholin’s cyst over the left vulva was suspected,and the patient underwent marsupialization under local anesthesia,which was well-tolerated.During the incision procedure,bright-red blood with some blood clots was discharged,and a hemorrhagic Bartholin’s cyst was observed.There was no recurrence of the hemorrhagic Bartholin’s cyst during the 6-mo subsequent follow-up period.CONCLUSION Hemorrhagic Bartholin’s cysts rarely occur.We report the case of a postmenopausal female with a hemorrhagic Bartholin’s cyst who had been on antiplatelets and was successfully treated with marsupialization.No recurrence was noted during the 6-mo follow-up period.Older females taking antiplatelets should be cautious of bleeding when presenting with a Bartholin’s cyst.

Efficacy and safety of Cimicifuga foetida extract on menopausal syndrome in Chinese women

Background It is now recognized that Cimicifuga foetida extract is effective in alleviating menopausal symptoms. But the durations reported were usually short. This paper compares the clinical effects of different regimens of three-month course on climacteric symptoms in Chinese women, so as to evaluate the efficacy and safety of Cimicifuga foefida extract. Methods This was a prospective, randomized trial. Ninety-six early menopausal women were recruited and randomly assigned into 3 groups to take different kinds of medicine for 3 months; participants were given Cimicifuga foefida extract daily in group A （n=32）, given estradiol valerate and progesterone capsule cycle sequentially in group B （n=32）, and given estradiol valerate and medroxyprogesterone acetate cycle sequentially in group C （n=32）. The questionnaires of Kupperman menopause index, menopause-Specific Quality of Life, and Hospital Anxiety and Depression Scale were finished before and after the treatment. The status of vaginal bleeding and breast tenderness was recorded every day. Results Eighty-nine participants （89/96, 92.7%） completed the treatment. Kupperman menopause index decreased after taking the medicine for 3 months in each group （with all P〈0.001）, but the score after the treatment was higher in group A than in the other 2 groups. Except for the score of sexual domain in group A （P=0.103）, the scores of all domains of the menopause-Specific Quality of Life decreased significantly after the treatment in all groups （with all P〈-0.01）. Score of anxity decreased significantly in group A （P=0.0l 5） and B （P=0.003）. Incidence of breast tenderness was 12.9% （4/31） in groupA, 36.7% （11/30） in group B, and 14.3% （4/28） in group C. Incidence of vaginal bleeding was 6.5% （2/31）in group A, 26.7% （8/30） in group B, and 82.1% （23/28）in group C. Conclusions Cimicifuga foeticla extract is effective and safe in the treatment of menopausal syndrome. A comparison of two different dosages of conjugated equine estrogen in continuous combined hormone replacement therapy with progestin

To investigate the effects of two different dosages of conjugated equine estrogen (CEE) on preventing bone loss and relieving the symptoms of menopausal syndrome in women at an early stage of menopause Methods A total of 236 postmenopausal women were randomly allocated to one of the following groups: Group A: 0 625 mg CEE+2 mg medroxyprogesterone acetate (MPA)+1 tab Caltrate-D per day; Group B: 0 3 mg CEE+2 mg MPA+1 tab Caltrate-D per day; Group C: 1 tab Caltrate-D per day as the control group The study was continued for 2 years The following parameters were monitored: ① L2-4 bone mineral density (BMD) (measured with dual energy X-ray absorptiometry (DEX)), ② menopausal syndr ome improvement (assessed by comparing Kupperman scores), ③ vaginal bleeding rate, and the thickness of the endometrium and breast in each group Results Overall, 213 cases (90%) completed the 1-year study and 176 cases (75%) completed the 2-year study The percentage changes in L2-4 BMD at the 12th and 24th month in Group A were +2 3% and +3 7%, respectively, with the posttreatment values being significantly higher than pretreatment values (P0 05) in Group B And that of Group C were -0 4% at 12t h month and -1 6% at 24th month (P>0 05) L2-4 BMD in both Group A and B w as significantly higher than that in Group C at 12th and 24th month (A vs C, P<0 001; B vs C, P<0 05) Kupperman Sco res were significantly reduced after 1, 3, 6 ,12 and 24 months in all 3 groups when compared with baseline (P<0 001) Scores in Group A and Group B were significantly lower than that in Group C (P<0 001) However, the vaginal bleeding rates in Group A were significantly higher than that in Group B or in Group C There was no atypical hyperplasia of endometrium in the 3 groups by the end of the study One p atient in Group A developed superficial thrombophlebitis by the end of 12th month Conclusion Continuous combination
