Objective:To investigate the mechanistic basis for the attenuation of bone degeneration by edible bird's nest(EBN)in ovariectomized rats.Methods:Forty-two female Sprage-Dawley rats were randomized into 7 groups(6 in each group).The ovariectomized(OVX)and OVX+6%,3%,and 1.5%EBN and OVX+estrogen groups were given standard rat chow alone,standard rat chow+6%,3%,and 1.5%EBN,or standard rat chow+estrogen therapy(0.2mg/kg per day),respectively.The sham-operation group was surgically opened without removing the ovaries.The control group did not have any surgical intervention.After 12 weeks of intervention,blood samples were taken for serum estrogen,osteocalcin,and osteoprotegerin,as well as the measurement of magnesium,calcium abd zinc concentrations.While femurs were removed from the surrounding muscles to measure bone mass density using the X-ray edge detection technique,then collected for histology and estrogen receptor(ER)immunohistochemistry.Results:Ovariectomy altered serum estrogen levels resulting in increased food intake and weight gain,while estrogen and EBN supplementation attenuated these changes.Ovariectomy also reduced bone ER expression and density,and the production of osteopcalcin and osteorotegerin,which are important pro-osteoplastic hormones that promote bone mineraliztion and density.Conversely,estrogen and EBN increased serum estrogen levels leading to increased bone ER expression,pro-osteoplastic hormone production and bone density(all P<0.05).Conclusion:EBN could be used as a safe alternative to hormone replacement therapys for managing menopausal complications like bone degeneration.

Efficacy evaluation of standardized Rheum rhaponticum root extract(ERr 731®)on symptoms of menopause:A systematic review and meta-analysis study

menopause is characterized by various physical,mental and emotional symptoms.ERr 731®is a standardized extract from Rheum rhaponticum root and has been clinically studied for its role in reducing menopausal symptoms.The current systematic review and meta-analysis aimed to evaluate the efficacy of ERr 731®supplementation in alleviating the severity of menopausal symptoms.In this review,we searched across three online databases up to March 2023,evaluated the quality of the included studies by the Physiotherapy Evidence Database scale,and assessed the risk of bias by the Cochrane Risk of Bias tool.We then performed a metaanalysis using RevMan software to estimate the pooled mean difference(MD).The study protocol was registered in the Prospective Register of Systematic Reviews(CRD42023416808).After screening and evaluation,we included four high-quality studies(a total of 390 participants;the ERr 731®group:193 participants;the control group:197 participants)in the meta-analysis.The results showed that ERr 731®supplementation significantly reduced the menopause Rating Scale score(MD:–15.12;Pmenopause symptoms.Potential bias and high heterogeneity in the results warrant further clinical studies.

Women gradually lose bone from the age of〜35 years,but around menopause,the rate of bone loss escalates due to increasing bone resorption and decreasing bone formation levels,rendering these individuals more prone to developing osteoporosis.The increased osteoclast activity has been linked to a reduced estrogen level and other hormonal changes.However,it is unclear whether intrinsic changes in osteoclast precursors around menopause can also explain the increased osteoclast activity.Therefore,we set up a protocol in which CD14f blood monocytes were isolated from 49 female donors(40-66 years old).Cells were differentiated into osteoclasts,and data on differentiation and resorption activity were collected.Using multiple linear regression analyses combining in vitro and in vivo data,we found the following:(1)age and menopausal status correlate with aggressive osteoclastic bone resorption in vitro;(2)the type I procollagen N-terminal propeptide level in vivo inversely correlates with osteoclast resorption activity in vitro;(3)the protein level of mature cathepsin K in osteoclasts in vitro increases with age and menopause;and(4)the promoter of the gene encoding the dendritic cell-specific transmembrane protein is less methylated with age.We conclude that monocytes are"reprogrammed"in vivo,allowing them to"remember"age,the menopausal status,and the bone formation status in vitro,resulting in more aggressive osteoclasts.Our discovery suggests that this may be mediated through DNA methylation.We suggest that this may have clinical implications and could contribute to understanding individual differences in age-and menopause-induced bone loss.

Effect of Jianpi Bushen formula on aromatase-inhibitor-associated bone loss after menopause

OBJECTIVE: To investigate the effect of Jianpi Bushen(JPBS) formula on aromatase inhibitor(AI)-associated bone loss after menopause.METHODS: Six-month-old female rats were randomly divided into 6 groups: a sham group, an ovariectomized(OVX) group, an OVX treated with exemestane and 3 OVX groups each treated with a different dose of JPBS formula. Bone mineral density(BMD) at the lumbar vertebrae, histology, bone markers and serum levels of estrogen were assessed. Furthermore, a cohort study was conducted in 130 postmenopausal women with breast cancer that had undergone treatment with AIs. The subjects were given JPBS + caltrate D or caltrate D only, administered orally. BMD at the lumbar vertebrae and femoral neck and bone markers were evaluated in both control and herbal treatment groups at baseline and 12 months.RESULTS: Experimental results indicated that a high dose of JPBS significantly increased the trabecular bone area percentage(Tb.Ar %) and broadened the trabecular thickness(Tb.Th). The JPBS formula enriched the carboxyterrninal propeptide of type ipmcollagen and increased serum estrogen level significantly. The clinical investigation revealed that bone loss was decreased in the group treated with JPBS vs control(BMD T score at lumbar vertebrae, 3.9% increased vs 14.58% decreased, respectively, P = 0.004 and BMD T score on femoral neck, 1.8% decreased vs 22.45% decreased, respectively, P = 0.008). Besides, JPBS formula elevated Nmiddle osteocalcin and decreased type Ⅰ collagen cross-linked C-terminal telopeptide.CONCLUSION: JPBS formula prevented aromatase-inhibitor-associated bone loss after menopause by inhibiting bone resorption and promoting bone formation.

A severe impairment of cognitive function characterizes dementia.Mild cognitive impairment represents a transition between normal cognition and dementia.The frequency of cognitive changes is higher in women than in men.Based on this fact,hormonal factors likely contribute to cognitive decline.In this sense,cognitive complaints are more common near menopause,a phase marked by a decrease in hormone levels,especially estrogen.Additionally,a tendency toward worsened cognitive performance has been reported in women during menopause.Vasomotor symptoms(hot flashes,sweating,and dizziness),vaginal dryness,irritability and forgetfulness are common and associated with a progressive decrease in ovarian function and a subsequent reduction in the serum estrogen concentration.Hormone therapy(HT),based on estrogen with or without progestogen,is the treatment of choice to relieve menopausal symptoms.The studies conducted to date have reported conflicting results regarding the effects of HT on cognition.This article reviews the main aspects of menopause and cognition,including the neuroprotective role of estrogen and the relationship between menopausal symptoms and cognitive function.We present and discuss the findings of the central observational and interventional studies on HT and cognition.

Liver disease in menopause

There are numerous physiologic and biochemical changes in menopause that can affect the function of the liver and mediate the development of liver disease. menopause represents a state of growing estrogen deficiency, and this loss of estrogen in the setting of physiologic aging increases the likelihood of mitochondrial dysfunction, cellular senescence, declining immune responses to injury, and disarray in the balance between antioxidant formation and oxidative stress. The sum effect of these changes can contribute to increased susceptibility to development of significant liver pathology, particularly nonalcoholic fatty liver disease and hepatocellular carcinoma, as well as accelerated progression of fibrosis in liver diseases, as has been particularly demonstrated in hepatitis C virus liver disease. Recognition of the unique nature of these mediating factors should raise suspicion for liver disease in perimenopausal and menopausal women and offer an opportunity for implementation of aggressive treatment measures so as to avoid progression of liver disease to cirrhosis, liver cancer and liver failure.

menopause in German and Chinese Women——An Analysis of Symptoms,TCM-diagnosis and Hormone Status

Objective： To investigate menopause-related symptoms, traditional Chinese Medicine （TCM）-diagnosis and hormone status of two comparable urban samples of menopausal women, one in Essen （Germany） and the other in Shanghai （China）. Methods： Patients suffering from menopausesyndrome were recruited from the TCM-outpatient clinic of the University of Duisburg-Essen in Germany （35 subjects） and from the Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine in China （35 subjects）. The Kupperman-lndex for tracing menopausal symptoms was applied. The complete TCM-diagnosis was carried out by the same investigator in China as well as in Germany. Testosterone and estrogen blood samples were collected once from every woman. Results： There were significant differences in specific symptoms of the Kupperman-lndex, such as a higher prevalence of formication and depression in German menopausal women; whereas Chinese menopausal women suffered significantly more from vertigo, headache and paraesthesia symptoms. Concerning TCM- diagnosis, Shen （肾）-yang deficiency was diagnosed in 51.43% of the German women in contrast to 5.71 % of the Chinese women; 17.14 % of the German women were diagnosed as having Shen-yin deficiency compared to 74.29 % of the Chinese women. The German women showed significantly lower mean hormone levels for testosterone compared to the Chinese women （P≤0.0005）. Conclusions： German and Chinese menopausal women do not show different prevalence but have different patterns of menopausal symptoms. Furthermore, from a TCM point of view, German women suffer more from Shen-yang deficiency whereas Chinese women suffer more from Shen-yin deficiency syndrome. These results are supported by significantly lower levels of testosterone in German women compared to Chinese women, which, in TCM, is a characteristic of yang deficiency.

Royal jelly for genitourinary syndrome of menopause:A randomized controlled trial

Objective:Genitourinary syndrome of menopause is a progressive condition characterized by a decrease in estrogen,which causes bothersome genital symptoms.The purpose of the study was to determine the effect of royal jelly on the genitourinary syndrome of menopause.Materials and methods:A randomized controlled trial was carried out from November 2018 to June 2019 in Bandar Abbas,Iran.The trial was registered in the Iranian Registry of Clinical Trials(1RCT20181107041585N1)with the main objective of determining if royal jelly could reduce the genitourinary syndrome of menopausal women.Eligible women were randomly assigned to receive either daily 1g of oral royal jelly or placebo for 8 weeks.The urogenital subscale of the Menopausal Rating Scale was used to determine genitourinary syndrome.Independent samples t-test was used for inter-group comparisons and paired samples t-test for pre-and post-treatment comparisons.Results:There were no differences in the severity of sexual problems,bladder complications,or vaginal dryness between groups before intervention.Although the intervention group's bladder complications improved slightly after eight weeks of royal jelly treatment compared to the control group(p?0.04),there were no significant changes in vaginal dryness,sexual problems,or total urogenital score.The within-group changes(before and after treatment)also showed no differences in urogenital symptoms.Conclusions:A daily dose of 1g royal jelly taken orally for 8 weeks did not alleviate menopausal genitourinary syndrome.No serious side effects were observed.To make more reliable decisions about the use and safety of royal jelly in the future,different doses of royal jelly and longer trials are required.

Background Objectives:menopause is a critical stage in women’s life,and both menopausal and postmenopausal women are at risk of developing hypertension.However,the association between menopause and hypertension lacks sufficient study.This study aimed to investigate the relationship between menopause and hypertension in women.Methods:The participant inclusion criteria were age≥18 and being female,while the exclusion criteria were having no record of menopause and being unable to determine the presence of hypertension.Data from 1,884Chinese women from the China Health and Retirement Longitudinal Study were included for analysis,and the association between menopause and hypertension in women was determined by conducting multivariate logistic regression and excluding potential confounding factors by subgroup analysis.Results:The prevalence of hypertension in menopausal women was overall higher than in non-menopausal women(39.3%vs 23.6%),and the difference was statistically significant(Pmenopause is associated with hypertension(OR:1.6595%CI:1.22-2.21 Pmenopause.In particular,the risk of hypertension in women with middle and early menopause should be of concern.

Objective:This study explored the relationship between menopause and metabolic syndrome(MetS),stroke,hyperlipidemia,diabetes and hypertension.Study design:This cross-sectional study surveyed 440 women in Yuci,China in 2012.Main outcome measures:MetS,diabetes,hypertension,hyperlipidemia,stroke,and behavioral and demographic variables.Results:The prevalence of MetS in this study was 40.28%to 49.66%(p=0.065)among preand post-menopausal women,respectively,after adjusting for age.Conclusions:The prevalence of diabetes,stroke,hypertension,and hyperlipidemia was higher among post-than pre-menopausal women.Health screenings for women in China should consider the increased risk for metabolic disorders during the postmenopausal stage of life.
