BACKGROUND Benign symmetric lipomatosis(BsL)was first described by Brodie in 1846 and defined as madelung’s disease by madelung in *** present,about 400 cases have been reported *** these cases,surgical resection remains the recommended *** we report a case of neck BsL with concomitant thick fatty deposit in the inguinal region,which concealed the signs of a right incarcerated femoral *** sUMMARY A 69-year-old male patient was admitted to our hospital with“abdominal pain,abdominal distension,nausea-vomiting and difficult defecation for half a month”.Moreover,he had a mass in the right inguinal region for more than 10 *** egg-sized neck mass also developed 15 years ago and had developed into a full neck enlargement 1 year *** addition,the patient had a history of heavy alcohol consumption for more than 40 *** the aid of computerized tomography scan,the patient was diagnosed with BsL and a low intestinal mechanical obstruction caused by a right inguinal incarcerated *** general anesthesia,right inguinal incarcerated femoral hernia loosening and tension-free hernia repair was ***,this patient did not receive BsL *** a 1-year follow-up,no recurrence of the right inguinal femoral hernia was ***,no increase in fat accumulation was found in the neck or other *** secretive intraperitoneal fat increase may be difficult to detect,but a conservative treatment strategy can be adopted as long as it does not significantly affect the quality-of-life.

A 56-year-old man with a long history of alcoholism and a palpable raised mass about 23 cm×12 cm in size on his neck was diagnosed with madelung’s disease(MD);the patient also had *** day 1,he underwent neck lipoma tissue resection and suddenly had dyspnea 9 h after the operation,which was followed by cardiac *** heart rate resuscitation,he developed a heartbeat,but he remained in a coma and died 9 days after the *** obvious pathological tracheal compression from a hematoma was observed while evaluating the patient’s neck on *** microscopy,the patient had an extensive pulmonary fat embolism,partial alveolar septal rupture and hemorrhage,and fat particles,and there was extensive infiltration of macrophages,which had phagocytosed fat particles,in the alveolar *** was also evidence of pulmonary infection and hypoxic-ischemic *** cause of death was postoperative pulmonary fat embolism resulting in pneumonia and central nervous system *** is the first report of a patient with MD who died of a fatal pulmonary fat embolism after neck lipoma tissue *** experts and clinicians should pay sufficient attention to this case.
